Why: The single-engine Cessna 177 was en route to St. Thomas when it crashed Thursday and burst into flames. The pilot had reported problems after takeoff and was returning to the airport.

Mayaguana Medical Evacuation Crash Kills 3 in Vehicle
On April 4, 2013 about 12:30 am, a Leair Charter Service Limited Cessna 402C was landing in the process of a medical evacuation and collided with a parked truck being used to illuminate the runway.The pilot and passenger in the plane survived.
The vehicle caught on fire, killing three occupants. The fatalities are named in the video.
Additionally, the video calls for the need for lights on the runway.

Tragic aviation accident in Mayaguana

In the early hours of this morning I was informed of an aviation accident in Mayaguana resulting in the tragic death of 3 people. I am advised that all 3 persons are residents of the island of Mayaguana. A preliminary report from the Department of Civil Aviation says that on 4th April, 2013 at approximately 12:30 a.m. a Cessna C402 owned by Leair Charter Service Limited with a crew of 2 on board was making a landing at night for an emergency evacuation flight with two vehicles strategically placed to outline the useable portion of the runway. It is reported that the aircraft made contact with one of the vehicles resulting in the deaths of the 3 passengers in that vehicle.An investigative team comprising the Royal Bahamas Police Force and the Air Accident Investigation and Prevention Unit of the Department of Civil Aviation will fly into Mayaguana at 5 a.m. this morning.
I wish to assure that a full investigation will be carried out by the Department of Civil Aviation into the causes and circumstances surrounding this terrible event. I will report further as information permits. Out of respect for the families, the names of the persons killed will not be disclosed until an official release is made by the Royal Bahamas Police Force.
I extend my heartfelt condolences to the families of these beloved persons and to the wider community of Mayaguana who share in this loss.

NTSB to Hold Investigative Hearing Into August 2013 UPS A300 crash in Birmingham, Ala.Jan. 30, 2014
WASHINGTON – The National Transportation Safety Board has scheduled an investigative hearing on February 20 into the crash of a UPS Airbus A300-600 on approach to Birmingham-Shuttlesworth International Airport in Birmingham, Ala., on Aug. 14, 2013.
The two flight crew members were killed and the airplane was destroyed when it impacted the ground less than a mile short of Runway 18. The cargo flight had originated from Louisville, Ky. Runway 18 was being used because the main runway at the airport was closed for repairs at the time of the airplane’s arrival.
The one-day hearing will examine:
• Execution of non-precision approaches, including initial and recurrent training, adherence to standard operating procedures, and proficiency
• Human factors issues associated with effective crew coordination and resource management applicable to this accident, including decision-making, communication, fatigue and fitness for duty, as well as monitoring and cross-checking, policies, standard operating procedures, guidance, and training provided to UPS crewmembers.
• Dispatch procedures, including the training, evaluation, roles and responsibilities of UPS dispatchers and the limitations of dispatch-related software.
The investigation is ongoing and this hearing will develop additional facts to support the investigation. The hearing will be held in the NTSB Board Room and Conference Center in Washington, D.C. A detailed agenda and a list of attendees will be forthcoming.
Parties to the hearing will include the Federal Aviation Administration, UPS, Airbus, the Independent Pilots Association and the Transport Workers Union. The accredited representative from the French Bureau d’Enquêtes et d’Analyses pour la sécurité de l’aviation civile (BEA) will participate on the technical panels.
The determination of the probable cause of the crash will be released when the investigation is complete. Just prior to the start of the hearing, the public docket will be opened. Who: 2 aboard, 2 fatalities
Why: A Cessna that crashed in Blackburn Canyon, Tehachapi, California killed both people aboard the flight and ignited a fire that caused the evacuation of hundreds of homes south of Bakersfield. Two were confirmed dead in the crash, and it has not (yet) been confirmed if there were more than 2 aboard.As of Monday, the number of evacuations had grown from 100 to 650 to about 1,500 homes homes. The threatened area was the mountains around the crash and a seven square mile burn area, all threatened by a raging forest fire. Firefighters concentrated on creating a fire-break.
A DC-10 water tanker has been part of the firefighting team, including 600 firefighters and more than a dozen aircraft.
Tehachapi, with a population of 8,000, has some renown as an area with wind farms. Smaller fires spread to San Bernardino National Forest and rural areas of San Bernardino County.
The fire has been 95% contained.
