Thanks to Alexis Stevens of the Atlanta Journal-Constitution, we know that this week Delta :
- landed on the taxi way instead of the runway at Hartsfield-Jackson International Airport.
- flew 150 miles past their destination.
- had a landing gear emergency.
- posted a $161 million third quarter loss.
However, I would like to point out that Delta’s tough week didn’t involve Delta losing a life. However, this week also:
- Divi Divi lost a pilot in the Caribbean
- Victoria Air DC3 lost a whole flight crew in a fiery crash into a warehouse.
- Westjet had a member of the flight crew injured in turbulence
- Indonesian National Transportation Safety Committee released their preliminary report on the Merpati DHC6 plane that flew into a mountain, killing 14
- a part of a Lufthansa’s galley landed on someone unfortunate enough to be in falling distance
And there were the usual bird strikes, unruly passengers, engine failures, cracked windshields, medical emergencies, flat tires, hydraulic failures, random burning smells, electrical fires, engine shut downs and a myriad of other emergencies that somehow managed to be non-catastrophic.
So all in all, a week as usual. Well, maybe a slightly unusual week, what with Northwest overshooting its destination, and Air Asia delivering a baby….
Or maybe in the immortal words of Walter Kornbluth (Eugene Levy, Splash) “Oy, what a week I’m having.”

The next big question is whether or not information is withheld.
That’s what some families are wondering, and that’s part of the reason why they’ve hired lawyers. Plaintiffs have access to more information; and they want to know what reason Air France has for not replacing a known Airbus problem, the pitot probes (airbus speed sensors.)
What has been published is error messages based on inconsistent readings taken by the pitot probes.

While we face a crisis of confidence in the latest evolution of air travel technology, it amazes me how many articles are out there that praise the Airbus. Hull loss statistics and disasters are staring us all in the face. The top criteria of plane design should still be safety.
All of us–passengers and aviation professionals alike– are still subject to the flaws of performance, design, maintenance and safety standards. It’s hard to know where to start.
I’m not even going to talk about fly-by-wire philosophy, even though it has contributed to fatal accidents, like the TAM crash where the fly-by-wire system failed to interpret how to cope with an Airbus’s disabled reverser on a slick runway in less than optimal conditions.
I’m just going to say this because it needs to be said.
Someone needs to stop defending Airbus, and turn an eye to all those hull losses.
And all those deaths.
We have stats that show 5,000+ planes have been manufactured. Thousands of deaths have been attributed to Airbus failure:
A300 – 1449 deaths
A310 678 deaths (plus today’s 146=824)
A320-655 deaths
A330-236 deaths(and this is not a complete list.)
Just go to go to Aviation Safety Net and examine the hull loss statistics of the Airbus.
It’s not just ONE problem. There are a lot of problems, especially if you follow some Airbus history.
The investigation of a 2001 Airbus A300-600 crash led to the discovery that a panel on the tail fin can move beyond its safe limit, which can cause the tail fin to shear off and the plane to crash. Read about Flight 587 and Airbus’s history of non-disclosure of Airbus flaws.
Historically, alleged problems with construction or computer systems have been instrumental in catastrophic vertical stabilizer or tail fin loss. “One of the 24 automatic messages sent from the plane minutes before it disappeared pointed to a problem in the ‘rudder limiter.”
The tail fin fiasco has supposedly been improved upon. It’s now better because it’s plastic. ! ? ! In fact, a large percentage of the Airbus is composed of a carbon-fiber composite, which can be a problem, not only because composite parts can bend or twist unpredictably in flight, but also their tensile strength is questionable, AND so is the length of time they can perform before degrading.
And of course, we’ve all heard about the pitot tubes. While there are other systems out there that seem to be viable, the THALES brand pitot tubes which are known to freeze up, and have not been replaced. Whether flawed readings causes a plane to slow down and fly into a stall or speed up and fly into a dive, either result is catastrophic.
And then, of course, there’s the computer system which may have just gone haywire. As reported by the Northwest Airlines A330 that experienced problems similar to Flight 447, they ran into turbulence. “Their primary and standby airspeed indicators showed the plane had slowed dramatically. Other systems that automatically maintain speed and altitude also disengaged. The master warning and master caution indications flashed on the instrument panel for three minutes until the captain flew out of the weather.”
So now we have a less than perfect aircraft, that is 19 years old, and getting decrepit. The public description of this plane makes it sound unbelievable–like the aviation equivalent of the bus to Cartagena in the movie “Romancing the Stone:” Babies on laps, no seatbelts, seats not even fastened down, and some passengers who fly standing up the whole way.
Is there any surprise when a plane like this (that is banned from French airspace and shunted off to a poor area like Comoros) falls out of the sky and kills all but one passenger? The fact that a plane banned from European airspace is shunted off to serve the inhabitants of an impoverished nation is a failure of the operational practice of the aviation industry. Where are the criteria in place to protect those citizens?
There ought to be a law.

Technical issues? In this day and time, it seems to me that “technical issues” is an inadequate description of a problem. These have all been lumped at one time under “technical issues”:
- Polish air force Tupolev 154 that crashed during an attempted landing in Smolensk, Russia, on April 10, 2010. 96 killed
- Ethiopian Airlines Boeing 737 that took off in Beirut, caught fire and crashed into the Mediterranean on Jan. 25, 2010. 90 killed
- Yemenia Airways Airbus A310 that crashed into the Indian Ocean on approach to Moroni, on June 30, 2009. 152 killed
- Air France Airbus A330 en route from Rio de Janeiro to Paris that crashed into the Atlantic Ocean on June 1, 2009. 228 killed
- Turkish Airlines Boeing 737 that crashed shy of Amsterdam’s Schiphol Airport runway on Feb. 25, 2009 9 killed (135 aboard)
A cursory announcement of a problem should be specific.
Was it a problem with an indicator? an alarm system? a physical problem? a communication problem? loss of control? overheating? leaks? maintenance?
The only way design issues can be caught ahead of the disaster is if full disclosure of the exact problem is noted publicly, and cross-checked with all other incidents and problem reports. If Air France or Cityjet Avro wants to present details, I would be delighted to present them in this forum here.
Air France—like all carriers—needs to practice full disclosure. If the cause is unknown, then it is multiplicatively more important to disclose the exact issue so that the problem can recorded, cross checked, and be tracked down. Maybe if there were less secrecy and more open communication in the field of aviation safety, the 575 lost in recent crashes would still be here.
Where There’s Smoke, There’s Fire. Where There’s Fire, Will There Be Passengers?

The biggest consequence in this Trent 900 engine brouhaha may be the Rolls Royce reputation.
While the company’s engineering skills have not suddenly degraded overnight, confidence in Rolls Royce certainly has. If, as the Australian Transport Safety Bureau says, the internal fire in the notorious (Qantas owned) Trent 900 engine is the result of an internal engine oil leak at 1,000°C at high pressure, the endangerment of 440 passengers and 26 crew may have been preventible, given that there seems to have been fore-knowlege (by Rolls Royce) of a 1st generation manufacturing defect in a pipe coupling. As the 3rd generation versions of the engine no longer sport the same problem, the issue was recognized prior to the explosion, and dealt with.
The failure to communicate a potential problem to Qantas before the engine fire, and for that matter, to owners of all affected 1st and 2nd generation products (Singapore Airlines and Lufthansa), goes beyond a communications failure and inches toward negligence. And the practical concern now is that irregardless of whether the specific factor was a manufacturing defect or fatigue, how swift and responsible will Rolls Royce be in corrective action, aggressive inspection schedules and reconfiguring a maintenance paradigm that gets A380s back on track?
Time will tell. Also, only time will tell if this engine explosion will be contained within the auspices of Rolls Royce; Qantas, Singapore Airlines and Lufthansa. In fact, Rolls Royce is not alone in its sub-culture of secrecy and non-reportage and disclosure. Will the teetering public confidence in aviation safety will be irrevocably compromised in the days to come?

A ruling by Kentucky Supreme Court regarding the Comair crash that killed 49 people in 2006 says that as part of the county government, Lexington’s Blue Grass Airport cannot be held accountable.
While not the primary factor, poor runway markings, bad signs, and lighting problems contributed to the accident. The crash is being blamed on decisions made by the pilots.
Since the crash, have markings, signs and lighting been improved? If so, then it would seem that any such revisions made to improve conditions stand as testament to acknowledge pre-existing problems.
The sovereign state can do no wrong?

A disaster not in California…
From June 12 to June 15, the Sarychev Peak on the Russian Kuril Islands erupted, causing air traffic disruptions between Northern American and Japan. The ash clouds are reaching “estimated heights of 45,000 feet.” (Danger zone for aviation from from E152 to W155 over a distance of 2000nm and N40 to N48).
The dust clouds are drifting east and dispersing.
