    NTSB to Hold Investigative Hearing Into August 2013 UPS A300 crash in Birmingham, Ala.

    WASHINGTON – The National Transportation Safety Board has scheduled an investigative hearing on February 20 into the crash of a UPS Airbus A300-600 on approach to Birmingham-Shuttlesworth International Airport in Birmingham, Ala., on Aug. 14, 2013.
    The two flight crew members were killed and the airplane was destroyed when it impacted the ground less than a mile short of Runway 18. The cargo flight had originated from Louisville, Ky. Runway 18 was being used because the main runway at the airport was closed for repairs at the time of the airplane’s arrival.
    The one-day hearing will examine:
    • Execution of non-precision approaches, including initial and recurrent training, adherence to standard operating procedures, and proficiency
    • Human factors issues associated with effective crew coordination and resource management applicable to this accident, including decision-making, communication, fatigue and fitness for duty, as well as monitoring and cross-checking, policies, standard operating procedures, guidance, and training provided to UPS crewmembers.
    • Dispatch procedures, including the training, evaluation, roles and responsibilities of UPS dispatchers and the limitations of dispatch-related software.
    The investigation is ongoing and this hearing will develop additional facts to support the investigation. The hearing will be held in the NTSB Board Room and Conference Center in Washington, D.C. A detailed agenda and a list of attendees will be forthcoming.
    Parties to the hearing will include the Federal Aviation Administration, UPS, Airbus, the Independent Pilots Association and the Transport Workers Union. The accredited representative from the French Bureau d’Enquêtes et d’Analyses pour la sécurité de l’aviation civile (BEA) will participate on the technical panels.
    The determination of the probable cause of the crash will be released when the investigation is complete. Just prior to the start of the hearing, the public docket will be opened. Included in the docket are photographs, interview transcripts and other documents.

    Delta Air Lines CEO to Chair Air Transport Association Board

    WASHINGTON, Dec. 2, 2010 — The Air Transport Association of America (ATA), the industry trade organization representing the leading U.S. airlines, announced today that Delta Air Lines CEO Richard H. Anderson has been elected chairman of the ATA Board of Directors. Southwest Airlines Chairman of the Board, President and CEO Gary C. Kelly was named vice chairman.

    Anderson, who succeeds United’s Glenn F. Tilton and Kelly both will serve a two-year term.

    “Richard Anderson is a strong leader and a proven chief executive, with the necessary credentials to further propel the mission of ATA as it faces the challenges and priorities in this next Congress,” said ATA President and CEO James C. May. “Richard Anderson, Gary Kelly and incoming ATA President and CEO Nick Calio will complement each other as the association’s board representatives with the administration and the new Congress.”

    “I look forward to working cooperatively with the administration, Congress and regulatory agencies to advance initiatives that support continued improvements in the financial health of the U.S. aviation industry and maintain our focus on safety,” said Anderson. “Airlines are a vital industry in the U.S. and provide 11 million jobs annually.     Richard Anderson became Delta Air Lines CEO in September 2007 and was the architect of the airline’s merger with Northwest Airlines, which closed in October 2008. Prior to joining Delta, Anderson was executive vice president at UnitedHealth Group and served as president of UnitedHealth’s Commercial Markets Group. He also served as CEO of Northwest Airlines from 2001 to 2004.
    Kelly began his career at Southwest Airlines as controller in 1986, moving up to chief financial officer and vice president of finance, then executive vice president and CFO, before being promoted to CEO and vice chairman in July 2004. Gary assumed the role of chairman in May 2008 and president in July 2008. Prior to joining Southwest Airlines in 1986, Gary was a CPA for Arthur Young & Company in Dallas and controller for Systems Center, Inc.

    ATA recently named Nicholas E. Calio president and CEO, effective Jan. 1, 2011. Calio currently leads Global Government Affairs for Citigroup, both in the United States and in the more than 100 countries in which the company does business. Prior to joining Citigroup in 2003, Calio was the assistant to the president for legislative affairs and service as chief liaison to the U.S. Congress for President George W. Bush, the same position he held under President George H.W. Bush.

    3 Lost when Cessna Crashes Mt Si, King County Washington

    Who: 3 fatalities
    Why: Around 2 a.m. a King County resident heard the sputtering engine of a plane in distress, and called in a 911. Deputies on foot patrol heard the sound of a crash in the same area. The FAA picked up the emergency beacon (transponder). Emergency teams were using portable emergency locator receivers to follow the beacon to find the crash site. At 4:15 a.m., the King County’s Guardian One helicopter crew with night vision goggles assisted in discovering the fuselage from a single-engine Cessna 172 near a sheer face of Little Si mountain. The bodies of three individuals were found by teams of rescue hikers traveling about a mile by foot from the local base of operations.

    Two men and a woman were found. Federal Aviation Administration spokesman Mike Fergus said the flight was not in contact with ATC.

    Two of the three people killed in the small plane crash on Mount Si were swim coaches in King County. 31-year-old “Coach Seth” Dawson’s memorial was held Thursday night. Federal Way’s Decatur High School swim coach, Rob Hill was also killed in the crash, as well as Liz Redling. Hill was pursuing his commercial pilot’s license.

    There may have been ground fog at the time of the crash.

    FAA Finalized Fatigue Damage Rule

    Design approval holders must now set a time- or cycle-based limit after which structural damage inspections must take place. The rule specifically addresses widespread fatigue damage (WFD) with the intent of “providing for the establishment of safe operational limits and the maintenance actions necessary to preclude WFD prior to reaching those limits.

    Press Release – FAA Adopts “Comprehensive Solution” to Widespread Fatigue on Aging Aircraft

    WASHINGTON, D.C. – In a continuing effort to address aging aircraft issues, the Federal Aviation Administration (FAA) has finalized a rule designed to protect most of today’s commercial planes and those designed in the future from structural damage as they age.

    The new rule seeks to prevent “widespread fatigue damage” (WFD) by requiring aircraft manufacturers and certification applicants to establish a number of flight cycles or hours a plane can operate and be free from WFD without additional inspections for fatigue. Manufacturers have between 18 and 60 months to comply depending on the particular aircraft type.

    Once manufacturers establish these limits, operators of affected aircraft must incorporate them into their maintenance programs within 30 to 72 months, depending on the model of aircraft. After the limit is in the maintenance program, operators cannot fly the aircraft beyond that point unless the FAA approves an extension of the limit.

    “Safety is our highest priority. This rule provides a comprehensive approach to the problem of widespread fatigue in aging aircraft,” said U.S. Transportation Secretary Ray LaHood. “Requiring carriers to regularly inspect their aircraft for possible fatigue is essential to ensuring the highest levels of safety.”

    “We’ve addressed the problem of aging aircraft with numerous targeted regulations and 100 airworthiness directives over the years,” said FAA Administrator Randy Babbitt. “This rule is a comprehensive solution to ensure the structural safety of today’s airliners and the airplanes of tomorrow.”

    An airplane’s metallic structures are stressed and can develop cracks when they experience repeated loads such as the pressurization and depressurization that happens on every flight. While airlines regularly inspect aircraft for cracks exceeding a certain size, WFD involves aircraft developing numerous tiny cracks, none of which would have raised concerns individually but which together run the risk of joining up and impairing the structural integrity of the plane.

    The new regulation applies to airliners with a takeoff weight of 75,000 lbs. and heavier. It also applies to all transport designs certificated in the future.

    The affected models, totaling 4,198 U.S.-registered airplanes, are listed in the rule.
    The FAA is working closely with the European Aviation Safety Agency (EASA) and other national authorities to harmonize this rule with their regulations as much as possible. EASA is now developing rulemaking to address WFD, and the FAA participates in that process.

    NTSB PROVIDES INVESTIGATIVE UPDATE AND PHOTO OPPORTUNITY OF LAB WORK RELATED TO BOEING 787 BATTERY FIRE IN BOSTON

    WASHINGTON – Tomorrow, National Transportation Safety Board Chairman Deborah A.P. Hersman will provide an update on the NTSB’s investigation of the Jan. 7 fire aboard a Japan Airlines (JAL) Boeing 787 at Logan International Airport in Boston.

    Chairman Hersman will brief credentialed members of the news media on the progress of the investigation, after which she and senior staff will take questions. Those unable to attend in person can call in to a teleconference line.

    Reporters will then be invited to view the materials laboratory where the battery is being examined. Photography will be allowed but no “stand-ups” or taped reporting from the lab will be permitted.

    For planning purposes, RSVP is required for those planning to either attend in person or call in.

    The following factual information has been developed about the battery: It consists of eight cells of 3.7 volts each. All eight cells had varying degrees of thermal damage. Six of eight cells have been CT scanned and have been disassembled to expose their electrodes. All electrode windings in the battery are in the process of being photo-documented and are undergoing microscopic examination. In the coming days, the remaining two cells will undergo the same examination. Additional information will be provided tomorrow.
