    ICAO Spotlighting Indonesian Air Safety


    Possibly, the problems in Indonesian aviation are about to be addressed. At least they’re doing something. “They” is the ICAO. Canada is lending Indonesia a helping hand.

    There’s a new office in the International Civil Aviation Organization (ICAO) in Montreal. Indonesia has set up office. The human resource department has signed an ICAO agreement: ICAO is posting a couple of experts, but the big thing is the $896,000 to upgrade Curug aviation systems. There are to be four points of concentration: safety oversight, civil aviation team support, training improvements, and assistance to the ICAO reps.

    Delta Offers Winter Weather Travel Assistance to Customers in Atlanta

    ATLANTA, Jan. 7, 2011 — Delta Air Lines is offering customers whose flight plans may be affected by winter weather expected in Atlanta beginning Sunday night the ability to make one-time changes to their travel schedules without fees. Delta’s weather advisory encourages customers to consider postponing or re-routing their travel to avoid possible inconvenience from expected flight delays.

    Customers booked on Delta-ticketed flights to, from or through Atlanta Jan. 9-11 may immediately rebook for travel with the same origin and destination before or after their original travel dates as long as new flights are ticketed and rescheduled with travel beginning by Jan. 13, 2011.

    Flight delays are expected at Hartsfield-Jackson Atlanta International Airport as a result of winter weather, and Delta will proactively reduce flight schedules to minimize delays.

    Delta encourages customers to make changes and manage their travel at delta.com. All customers traveling in impacted markets should check their flight status at delta.com before arriving at the airport.

    Changes to origin or destination may result in a fare increase, and any fare difference between the original ticket and the new ticket will be collected at the time of rebooking. Customers whose flights are cancelled may request refunds.

    FAA Delays Closure of 149 Air Traffic towers until June 15

    WASHINGTON – The U.S. Department of Transportation’s Federal Aviation Administration (FAA) announced today that it will delay the closures of all 149 federal contract air traffic control towers until June 15. Last month, the FAA announced it would eliminate funding for these towers as part of the agency’s required $637 million budget cuts under sequestration.

    This additional time will allow the agency to attempt to resolve multiple legal challenges to the closure decisions. As part of the tower closure implementation process, the agency continues to consult with airports and operators and review appropriate risk mitigations. Extending the transition deadline will give the FAA and airports more time to execute the changes to the National Airspace System.
    “This has been a complex process and we need to get this right,” said U.S. Transportation Secretary Ray LaHood. “Safety is our top priority. We will use this additional time to make sure communities and pilots understand the changes at their local airports.”

    As of today, approximately 50 airport authorities and other stakeholders have indicated they may join the FAA’s non-Federal Contract Tower program and fund the tower operations themselves. This additional time will allow the FAA to help facilitate that transition.

    “We will continue our outreach to the user community to answer any questions and address their concerns about these tower closures,” said FAA Administrator Michael Huerta.

    On March 22, the FAA announced that it would stop federal funding for 149 contract towers across the country. A phased, four-week closure process was scheduled to begin this Sunday, April 7. That phased closure process will no longer occur. Instead, the FAA will stop funding all 149 towers on June 15 and will close the facilities unless the airports decide to continue operations as a nonfederal contract tower.

    IMPROPER CONTRACTOR ACTIONS AND INSUFFICENT FEDERAL OVERSIGHT LED TO 2008 FATAL FIREFIGHTING HELICOPTER CRASH NEAR WEAVERVILLE, CALIFORNIA, NTSB SAYS

    FOR IMMEDIATE RELEASE:
    December 7, 2010

    The National Transportation Safety Board today determined that a series of improper actions by the contractor and insufficient oversight by the U.S. Forest Service (USFS) and the Federal Aviation Administration (FAA) led to the August 5, 2008, fatal crash of a Sikorsky S-61N helicopter near Weaverville, California. The contractor’s actions included the intentional alteration of weight documents and performance charts and the use of unapproved performance calculations.

    Contributing to the accident was the failure of flight crewmembers to address issues related to operating the helicopter at its maximum performance capability. Contributing to the fatalities and survivors’ injuries were the immediate and intense fire that resulted from fuel spillage from the fuel tanks that were not crash resistant, the separation from the floor of the cabin seats that were not crash resistant, and the use of an inappropriate mechanism on the cabin seat restraints. The pilot-in- command, the safety crewmember, and seven firefighters were fatally injured; the copilot and three firefighters were seriously injured.

    On August 5, 2008, a Sikorsky S-61N helicopter (N612AZ), which was being operated by the USFS as a public flight to transport firefighters battling forest fires, impacted trees and terrain during the initial climb after takeoff at a location about 6,000 feet above sea level in mountainous terrain near Weaverville. The USFS had contracted with Carson Helicopters, Inc. (CHI) of Grants Pass, Oregon, for the services of the helicopter, which was registered to CHI and leased to Carson Helicopter Services, Inc. (CHSI), also of Grants Pass.

    “The probable cause of this accident had to do with Carson’s actions and the oversight entities’ inactions,” said NTSB Chairman Deborah A.P. Hersman. “Carson engaged in a bargain that violated the trust of their crewmembers, the firefighters that they carried onboard, and the aviation industry. But the FAA and the Forest Service did not hold up their end of the deal to oversee Carson’s actions. Public aircraft have been made the orphans of the aviation
    industry. It’s now time for the FAA and other government agencies to step up and take responsibility.”

    In order to prevent similar accidents and to improve the survivability of such accidents when they do occur, the NTSB issued 11 new recommendations to the FAA and reiterated one from 2006. Ten recommendations were issued to the USFS.

    Recommendations to the FAA include oversight of 14 Code of Federal Regulations Part 135 operators with aircraft that can operate part of the time as public aircraft and part of the time as civil, clarification of oversight responsibilities for public aircraft, accuracy of hover performance charts, pilot performance, fuel tank crashworthiness, and occupant protection.

    To the USFS, the NTSB recommended the development of mission-specific operating standards for firefighter transport operations, a requirement that its contractors adhere to these standards, and the creation of an oversight program that can monitor and ensure contractor compliance with all standards and requirements. Other issue areas for the USFS recommendations included pilot training, occupant protection, weather instrumentation, and onboard recorders.

    Define Irrational

    Dear George,

    Irrational could be changing, now, what was had claimed before.

    Like the NTSB saying in the 1989 NTSB Sioux City Final Report, the ‘probable cause’ to their references in their earlier National Airlines Final Report. That National Report found no probable cause but suggested ‘not understood vibrations’. That Report also drew in three earlier fan explosions as references and citing these as so similar, they were used for “comparison purposes”. That Report also expressly ruled out engine over speed as “pure conjecture”.

    But now, in the Sioux City Report, and where the probable cause was a metallurgical defect, the NTSB listed just the National accident, and relabeled the National’s probable cause to be “engine over speed”. Daring not to also re-label those three other references, they now were simply left out. Presto ! – by doing so, the NTSB could now present the Sioux City accident as a ‘once-off’ (only happened once) when – in fact – it was 5th fan disk uncontained explosion (National, and the three others there having ‘no probable cause’ were now not relevant, separated and with no connection to Sioux City).#Irrational could be claiming as factual what scientific testing had ruled out.

    In the NTSB’s ValuJet 592 Report, the NTSB claims “one or more” of the oxygen canisters carried in the cargo hold self-ignited and flooded the compartment with oxygen and thus started, and made that fire possible. They didn’t test their theories of self-ignition and the quantities of oxygen released, they had the FAA Technical Center do it. Contrary to all of this, the Tech Center tests said that, even if self-ignited, the canisters heat was only 400 degrees (525 degrees is needed to ignite the packing materials said to have ignited) and that the actual oxygen released is ‘insignificant’ (“local hot spot”). Lest they discredit the NTSB theory, neither of these FAA findings found their way into the NTSB’s Report. The Report also ignored another letter buried in the Docket, a oxygen manufacture’s statement that such self-ignition was unheard of, and “highly unlikely”, and thus the NTSB’s probable cause holds. Lost too was that four months after the loss of ValuJet, the aircraft manufacturer (Douglas) issued a warning to remove the fuselage insulation blankets (keeps the cabin areas warm) because of two earlier Douglas product (MD-88, MD-11) fires and that because of this, the Douglas method for fire testing was now deemed invalid. Four years later the FAA issued an airworthiness directive (AD) saying to check for burning below the cargo area (electric heaters for the static pressure sensors) and then issued yet another saying what Douglas said four year earlier – to remove the (acoustic) insulation blankets because of in-service fires and that the earlier flammability tests were invalid. Simply, ValuJet flight 592 was the precursor and warning to the coming loss of Swissair flight 111 in 1998 but the NTSB’s probable cause with ValuJet served to obscure, or hide, the long-known problems of insulation blankets that failed the flammability tests. Whose tests were these ? – Douglas and Boeing’s – the FAA had taken the manufactures’ word for it all. Now, and years after these two horrific losses, the FAA has just created it’s own flammability standards – ones that none of these two most used insulation blankets could pass. Oh yes ! – the FAA is giving the industry to 2016 to get these flammable materials out because, as they said, ‘of cost considerations to the industry’.

    Irrational could be claiming a scenario as unique when dozens of other reports exist.

    In the NTSB’s Report on the loss of American Airlines flight 587, the NTSB blamed the American Airline’s pilots aggressive use of the rudder pedals in American’s A-300 model aircraft. But the Report doesn’t address 20 other rudder incidents (largely from the SDR database) where the pedals and rudder where reported to be moving on their own (uncommanded rudder). A few reports called the movements ‘sudden’, and another “violent”. The words “violent” seen in the SDR and also in a Aviation Safety Report filed by pilots was missing in the NTSB in their Accident/Incident database report. “Sudden ” and ‘Violent’ described the scene as the American pilots fought with the aircraft for seconds and then losing all control. Worst yet, 3/4th of these other 20 reports weren’t on American Airline or on the A-300s but rather four other carriers including United, Fed Ex, America West, Air Canada and on different models, A-319s and A-320s.
