Pilot and Three Passengers Walk away from Frankenmuth Crash
What: Kitze Aviation Inc. Piper PA-28-180 Cherokee G Where: Frankenmuth, MI When: May 20, 2012, 3:20 pm Who: 4 aboard Why: A 25 year old pilot Thomas Wolfe successfully landed the Piper in Zehnder Field after losing power.
The pilot and one passenger got medical treatment after landing. Two others were transported to Flint’s Hurley Medical Center for treatment.
The pilot, Thomas Wolf, 56 year old Gerald Wolfe and 57 year old Frank Tima, are from Livonia. 26-year-old Laura Richardson is from Royal Oak.
The accident is under investigation.

What: ACS Helicopter LLC Bell 407 helicopter Where: Mount Pocono When: Oct 11, 2012 Who: Two fatalities, one injury Why: The Tuesday wreck piloted by a full time American Airlines pilot (part time with ACS Helicopter LLC) was discovered at 2:30 a.m. wednesday. The helicopter encountered bad weather as it passed through northern Monroe County. Pocono Mountains Municipal Airport is not equipped for receiving distress calls.
The helicopter was found on its right side, leaking fluids, with broken rotor and tail.
Fatalities of the crash were Pilot William Ellsworth, 52, of Califon, N.J., and passenger Tighe Sullivan, 51, of Darien, Conn.

A helicopter that went down in the Prospect Creek drainage outside of Thompson Falls ten miles up Prospect Creek road on July 27, 2013, suffered one fatality and two injured who were transported to the hospital.
Two Bear Air 2 assisted in finding the downed helicopter.
Search and Rescue, Forest Service and fire personnel responded to the scene. An air evacuation helicopter call was cancelled.

What: Piper PA-31-350 Navajo en route from Moorabbin Aerodrome, east of Melbourne to Port Macquarie on the NSW mid-north coast. The plane had stopped at Bathurst to re-fuel. Where: Bathurst Airport When: 8.30pm (AEDT) Friday November 7, 2008 Who: Pilot Timothy O’Brien, 51, pilot’s girlfriend Jackie, 40; Senior Constable Erin Condon, 40, and her nine-year-old son Matthew Why: crashed shortly after takeoff from Bathurst. The plane exploded when it hit the ground and caused fires in the immediate area. The pilot tried to return to the airport but crashed 3 km shy. The families are in mourning.

A small plane crash-landed in a field near Vernon airport in British Columbia, Canada, on December 23.
Authorities said the plane experienced engine failure shortly after it took off from Vernon airport.
According to Vernon’s fire captain Chris Robinson, “The pilot landed in the field just next to the airport. There was just a bit of damage to the plane.”
There were two people aboard including the pilot and his passenger; both of them suffered minor injuries.

What: Southwest Airlines Boeing 737-3H4 en route from Phoenix to Sacramento Where: Yuma International Airport, AZ When: 01 APR 2011 Who: 118 aboard Why: A hole burst through the top of this Boeing while en route. The plane suffered decompression, descended to a safe level (when an attendant was injured) and made an emergency landing in Yuma.
The hole has opened a witch hunt on aging Boeings in search of the kind of metal fatigue that would result in a hole during operations now that the aluminum skin has been known to separate at the lap joints, where panels are spliced together. Although the incident did not result in death, it is being taken as a “warning.” 175 Boeing 737 are slated for examination, and maintenance programs (Southwest, FAA, Boeing) are being re-evaluated to take this into account.
Discussion of lap joint and the 3 rows of rivets. The skin separated at the lower rivet line. Pre-existing fatigue existed on the fracture surface. (Multi site damage.) The decompression happened 18.5 minutes after take-off, and no pre-indications. The flight crew immediately declared emergency.

What: Private homebuilt kit plane that resembled a Cessna. Where: Covington Field in Spencer on a private air strip in Henry County just west of Blue Ridge Airport, near Virginia 58 and the Patrick County line. When: Friday afternoon 2:30 p.m Who: Pilot Jerome Tuttle, 71, of Stoneville, N.C. was taken to the Martinsville Hospital and died of his injuries. Why: The pilot was attempting to land on the private air strip when the plane crashed.
