    Petrobra Oil Field Chopper Crashes off Coastal Brazil

    What: Senior Táxi Aéreo Agusta Westland AW139 from oil platform to Rio de Janeiro
    Where: in the Campos Basin, Rio de Janeiro
    When: August 19, 2011, 17:15
    Who: 4 aboard, 4 fatalities
    Why: On Friday afternoon, the chopper charted to Brazilian oil major Petrobras set out from the PetrobrasP-65 platform en route to Macaé.

    While miles from shore, the crew declared emergency, and went down 100 km off the coast of Rio de Janeiro. Contact with the chopper was lost at 17:00 local time.

    A Bandeirante Patrol plane and two search Brazilian military aircraft, the 4th Squadron of the 7th Aviation Group, a H-34 Super Puma helicopter, and the 3rd Squadron of the 8th Aviation Group initiated a search for the downed Senior Táxi helicopter. In all, there were five aircraft, a helicopter and four ships, including a Guajará Navy patrol vessel and a Lynx helicopter on standby in the area.

    After two days of searching in rough seas, three bodies were found on the ocean floor 100 meters down and 62 miles from the coast. One body is still somewhere in the ocean.

    The fatalities are listed as Rommel Oliveira Garcia, pilot, Lauro Pinto Haytzann, co-pilot, both employees of Senior Air Taxis, and passengers Ricardo Leal de Oliveira from the company Engevix, and Joao Carlos Pereira da Silva, the Brasitest inspector.

    Climber Rescue in Japan Goes Wrong

    What: Gifu Prefecture Bell 412 EP Ambulance/Rescue helicopter en route from Gifu Airbase.
    Where: Takayama, about 240 kilometers west of Tokyo.
    When: Sept 11 2009, 3:30pm (0630 GMT
    Who: four people (5 crew on board but at least one was on the mountain in the process of the rescue)
    Why: During an emergency rescue of a climber who collapsed with cardiopulmonary arrest while mountain climbing, the helicopter’s tail rotor hit the mountain; and it fell 400 meters into a ravine. 5 crew were originally on board, 2 crew off from the helicopter rappelling down.

    The climber and the three aboard the helicopter were pronounced dead at the scene.

    Please excuse the bad translation below but it is pretty much indecipherable unless we get names:

    Disaster Asakura Hitoshi County (57) (Kakamigahara, Gifu Prefecture), Miyoshi Hideho mechanic (47) (same), vice captain Gotou Atsushi (34) (Kasamatsu-cho, Hyogo Prefecture ) confirmed dead.

    Brownsville Medical Helicopter crash


    What: Hospital Wing Eurocopter AS.350B3 Ecureuil helicopter en route from Jackson-Madison County General Hospital to Brownsville
    Where: 100 yards off Springfield School Road, about four miles east of downtown Brownsville Tennessee
    When: MARCH 25, 2010
    Who: 3 on board (no patients)
    Why: Dispatchers lost contact with the helicopter crew before 6 a.m. A lightning strike and a fireball was seen near the field where the wreckage was found, a location that was found by GPS.

    Four Killed After Small Plane Crashes in Hawaii

    A small plane crashed in Waianae mountains on Oahu Island, Hawaii, on July 28th.

    The single-engine plane went down while it was performing a sunset flight from Daniel K. Inouye International Airport.

    All four people aboard were killed in the crash. They were identified as Gerrit Evensen, his girlfriend Heather Riley, and their friends Dean Hutton and Alexis Aaron.

    The incident is being investigated.

    Crop Duster Plane Crashes in Colorado; Pilot Killed

    A crop duster plane crashed in the San Luis Valley, near Alamosa, Colorado, on August 19th.

    The plane hit the fields of barley and afterwards it burst into flames.

    The pilot lost his life at the scene.

    The cause of the crash remains under investigation.

    Rescue Helocopter Crashes during Maintenance Flight

    A Metro Aviation / Life Air Rescue Eurocopter EC-135P-1 was on a test flight with the pilot and two mechanic aboard when it crashed in a field behind Metro Aviation. All three aboard were injured, and hospitalized at Willis-Knighton Hospital.

    There was fuel on the ground, so emergency responders followed hazmat protocol. The accident was close to Shreveport Downtown Airport in a field at Hawn and North Thomas Avenue.

    The leased EC135 experienced mechanical problems and was in for maintenance..

    The affected helicopter has no history of problems, and has current certification. The cause of the incident has not yet been determined or released.
