What: Sikorsky S-76C helicopter, operated by PHI Inc Where: Louisiana marshland – Bayou Penchant area about 100 miles from New Orleans in southwestern Terrebonne Parish When: Sunday Jan. 4, 2009 Who: eight fatalities, one survivor. The helicoptor was carrying two pilots and seven passengers The dead were identified as Allen Boudreaux, of Ama, La.; Andrew Moricio and Ezequiel Cantu of Morgan City, La.; Randy Tarpley of Jonesville, La.; Jorey A. Rivero, of Bridge City, La.; Charles W. Nelson of Pensacola, Fla.; Thomas E. Ballenger of Eufaula, Ala. and Vyarl W. Martin of Hurst, Texas. The injured survivor is Steven Yeltin was medevacted; he is listed as being in critical condition and is awaiting to be transferred to Oschner Medical Center from the Leonard J. Chabert Medical Center in Houma. Mike Moreno, CEO of the Moreno Group along with Dynamic Industries, employed five of the victims. Why: The helicopter is believed to have crashed about seven minutes after it took off at 3 p.m, A distress signal was received just after 3. The NTSB investigation is well underway and the black box has already been recovered.

What: Cessna 150 owned by Jet Air Inc Where: Crashed on to Edingale Drive, Iowa City near Iowa City Municipal Airport When: Oct. 6, 2008 Who: James N. Meade, 64, of Iowa City Why: The FAA’s preliminary report points to engine failure as a possible cause. After the crash where the plane ended up nose down in the street, Meade was taken to University Hospital.
This is the second crash within the month on Iowa City streets, in fact less than a mile from the June 3 crash on Sunset Street and Willow Creek Court that killed a 2-year-old girl, injured the pilot and one passenger and one passenger Sydney Blanton–died.

While testifying at the coroner’s inquest before Lagos State Coroner Mr. Oyetade Komolafe, Tony Usidamen of Dana Air revealed that the Dana plane (registration number 5N-RAM) that crashed in Lagos in June 2012 had its engine replaced after a bird strike while outbound from Lagos on April 19, 2010.
Records indicate 5N-RAM also had hydraulic leaks on May 23 at Murtala Mohammed Airport.
On the June 3rd, the plane was making it’s third round trip and crashed close to it’s destination airport.
The Accident Investigation Bureau (AIB) investigator is Mr Emmanuel Dialla. His preliminary report said that the first sign of trouble came 17 minutes after the plane was airborne at 2.58pm, following a “non-normal” engine condition. According to the CVR at 1543:27 hours (3.43pm), the Captain informed the FO, “we just lost everything, we lost an engine. I lost both engines”…
On March 3, 2013, at 17:13 Eurocopter EC-120 Colibri crashed in Horný ?epe?, Sere? district in Slovakia after hitting a cable that was stretched between trees.
The helicopter landed in the Váh river.
Three wounded individuals were transported to University Hospital in Trnava where they were treated for hypothermia, head injuries, lacerations and abrasions.
The thirteen year old son of the pilot was trapped in the helicopter and died at the scene.
The injured parties include a twenty-one year old woman and a 42 year old man. They are in stable condition. The pilot is 40 years old. He is being investigated. Alcohol may have been a factor.
Part of the helicopter was retrieved by firemen initially, and the rest recovered later and loaded on to an armed forces flatbed, with the assistance of divers and military technology.
All of those aboard were Slovakian.

The pilot had been flying to his veterinary clinic and made no mayday call prior to the crash. Visibility may have been a factor as the weather was overcast.
Crash crews rapelled from the helicopter due to the inaccessibility of the crash site.
