What: USMC HMLA-469 Bell AH-1W Super Cobra Where: Yuma, AZ When: Feb 22, 2012, 8 pm Who: 2 aboard, 2 fatalities Why: The helicopter was engaged in night flight exercises on February 22nd near Yuma, AZ when it collided with another flight. Service members with the 3rd Marine Aircraft Wing were involved.
The crash happened west of the Chocolate Mountains in California.
The names of the two fatalities have not been released.
What: Policía Federal Rockwell-Gulfstream 695 Jetprop Commander 980 Where: El Refugio en el Municipio de Acatic, Mexico When: Feb 21, 2012, 21:00 pm Who: 3 aboard, 3 fatalities Why: In the town of Refugio Padrone, the Jetprop Commander 980 crashed. The names of the three victims have not been released.
State Unit of Civil Protection and Fire Jalisco (UEPCBJ), the Attorney General of the State (PGJE) are investigating.
What: Beechcraft 35-A33 Debonair Where: Rio Vista California When: Feb 19 2012 Who: 2 aboard Why: After the helicopter and plane bumped over Rio Vista, the pilot had to make a hard landing short of Byron Airport, 8 miles south of Rio Vista Municipal Airport. Apparently the plane struck the chopper’s skids. The 1961 six-seat Beechcraft plane involved in the collision is registered to Ronald A. Gawer of Brentwood in Contra Costa County.
When the Solano Fire crews arrived on the scene, they found the pilot with minor injuries, and the passenger was unhurt.
What: United Airways ATR en route from Dhaka to Chittagong Where: Hazrat Shah Amanat International Airport When: Feb 20, 2012 8:20pm Who: 50 aboard Why: The pilot was en route to Chittagong when the plane developed a problem with one of the engines. On advice from ATC, the pilot was able to land safely ten minutes early.
Examination has not uncovered the engine problem.
A replacement jet has been provided for the return flight.
What: Cessna 414A Chancellor En route from Texas to Hayden. Where: Yampa Valley Regional Airport, Hayden, Colorado When: Feb 19 2012 3:30 p.m. Who: 6 aboard, 2 fatalities Why: Before flying to Colorado, the plane left Corpus Christi International Airport at 9 a.m. and stopped in Dalhart, Texas. By the time they reached Colorado, there was a heavy snow storm. The flight was coming in to the airport, and with zero visibility crashed 30 yards short of the airport.
Of the six aboard, two—a man and a woman—were killed on impact and four were injured. The plane crashed 300 yards from the airport’s fire station but did not catch afire. The four injured were conscious when they were taken to Yampa Valley Medical Center in Steamboat Springs. They had been going to Steamboat Springs for a ski vacation.
The name on the plane’s registration is Scott A. Humpal of Woodsboro Texas. The pilot and Humpal’s wife Gabby were the victims of the crash. Humpal and his three children survived the crash.
Eleven year old Sara, the youngest daughter, has a broken back. She is scheduled for surgery Monday. Scott has a punctured lung.
What: Air Via Airbus A320-200 en route from Bourgas to Dublin Where: Dusseldorf When: Feb 17th 2012 Who: 3 crew Why: While en route on the ferry flight, the left engine gauge indicated low oil, signifying an oil leak.
Pilots diverted to Dusseldorf where they made a safe landing.
What: American Airlines Boeing 757-200 en route from Miami to New York Where: New York When: Feb 17th 2012 Why: On approach to New York, the plane developed problems with the left engine and pilots shut down the engine.
They made a safe landing with emergency services on stand-by.
What: Norte Táxi Aéreo Beechcraft 95-B55 Baron en route from Cametá to Baião Where: Cametá, PA Brazil When: Feb 16 2012, 10:00 am Who: 4 aboard, 4 fatalities Why: The Taxi service plane crashed not long after takeoff, killing the pilot, copilot and two security guards who worked transporting money for a bank. A fire ensued after the crash, and rescuers found the bodies were charred.
The crash occurred a kilometer from the runway. The airport fire department team responded. Seripa (regional research and prevention of Aviation Accidents) is investigating.
What: Iberia Airbus A319-100 en route from Madrid to Vienna Where: Vienna When: Feb 15th 2012 Why: On approach to Vienna, the plane was struck by lightning.
Pilots landed the plane safely and handed off to maintenance where it will be inspected. Another plane was called in to handle the return flight.
What: Cessna 172 Where: North Bend, Washington When: Feb 15, 2012 Who: 3 fatalities Why: Around 2 a.m. a King County resident heard the sputtering engine of a plane in distress, and called in a 911. Deputies on foot patrol heard the sound of a crash in the same area. The FAA picked up the emergency beacon (transponder). Emergency teams were using portable emergency locator receivers to follow the beacon to find the crash site. At 4:15 a.m., the King County’s Guardian One helicopter crew with night vision goggles assisted in discovering the fuselage from a single-engine Cessna 172 near a sheer face of Little Si mountain. The bodies of three individuals were found by teams of rescue hikers traveling about a mile by foot from the local base of operations.
Two men and a woman were found. Federal Aviation Administration spokesman Mike Fergus said the flight was not in contact with ATC.
Two of the three people killed in the small plane crash on Mount Si were swim coaches in King County. 31-year-old “Coach Seth” Dawson’s memorial was held Thursday night. Federal Way’s Decatur High School swim coach, Rob Hill was also killed in the crash, as well as Liz Redling. Hill was pursuing his commercial pilot’s license.
There may have been ground fog at the time of the crash.
What: Thomas Cook Boeing 757-300 en route from Glasgow to Antalya Where: London When: Feb 15th 2012 Why: The flight had taken off normally from Glasgow, but while en route, the weather radar system failed.
The crew of flight MT-3614 notified London and were able to divert to Gatwick where they made a safe landing.
One might say it was an overabundance of caution because weather radar might not be considered essential–but considering the distance covered and potential conditions en route and on landing in Antalya, it seems their caution was, in my opinion at least, justified.
What: CSA Czech Airlines Avion de Transport Regional ATR-42-500 en route from Warsaw to Prague Where: Prague When: Feb 15th 2012 Who: 1 fatality Why: After a normal flight, on approach to Prague, the first officer informed ATC that the captain was incapacitated. The f.o. landed the plane.
Emergency services responded, but the captain was pronounced dead. He as 58 years old.
What: American Airlines McDonnell Douglas MD-82 en route from Detroit to Dallas Ft. Worth Where: Chicago When: Feb 13th 2012 Why: While en route, the plane developed a problem with cabin pressure.
When the pressure could not be controlled, the pilots descended until pressure adjusted, then diverted to Chicago where they made a safe landing.
The passengers were booked on to alternative flights. The plane is being examined.
What: TAM Linhas Aereas Airbus A320-200 en route from Montevideo to Sao Paulo Where: Porto Alegre When: Feb 11th 2012 Who: 1 unruly passenger Why: A Tam airbus was en route when a passenger intruded into the cockpit. The pilot yelled for help, and the plane veered sharply to the right. The cockpit door was open. Some reports of the incident mention that the passenger had a TAM badge. (Some Uraguan accounts say the individual was flying in the cockpit on a jumpseat.)
Passengers talked about the incident feeling like a September 911 film.
The pilot diverted to Porto Alegre and made a safe landing. A number of passengers and possibly the two flight attendants were injured in the altercation. The passenger was taken into police custody, and he was incarcerated in the psychiatric ward of a local hospital for having suffered a “psychotic episode.”
The flight reached Sao Paulo nearly three hours late.
What: US Airways/PSA CRJ2 Charlotte to Charleston,WV Where: Charlotte-Douglas International airport. When: Feb 12, 2012 Who: 44 passengers 3 crew Why: A flight from Charlotte to Atlanta had just taken off when the plane began making unidentified noises. In about an hour after take-off, pilots returned to the airport to make a safe landing with emergency services on standby.
What: CSA Czech Airlines Boeing 737-500 en route from Prague to Skopje Where: Skopje When: Feb 11th 2012 Why: The plane developed a flaps problem on first approach to Skopje, which was aborted, but they made a successful second approach. The pilots burned off fuel and made a fast landing, but did not drive off the pavement.
What: American Airlines Boeing 767 Where: New York When: 02/08/2012 16:15 Who: no fatalities Why: The flight was en route when crew reported a fire in engine one.
Pilots shut down the engine and made a safe landing. Passengers were provided alternative flights.
What: Cessna T182T Turbo Skylane Where: WEST LEBANON, N.H. When: Feb 9, 2012, 3:45 p.m Who: pilot, 1 fatality Why: The Cessna T182T Turbo Skylane took off from Lebanon New Hampshire Municipal airport returning the plane from repairs but developed problems. The pilot, Paul Schlieben, of Peterborough, N.H requested a turnback to the airport.
He was returning to the airport when he crashed in the airport safety area impacting beside a runway. West Lebanon firefighters responded to the scene and put out the flames. The pilot did not survive the impact.

What: Maldivian Air Taxi DHC-6 Twin Otter en route from Lily Beach resort to seaplane lagoon Where: seaplane lagoon on Hulhule Island When: Feb 9, 2012, 12.08pm Who: 9 passengers, no fatalities Why: A Maldivian Air Taxi seaplane made a crash landing at Hulhule Island in heavy rain with low visibility.
None of the nine passengers aboard were seriously hurt, but some were in shock after the twenty-five minute flight. The passengers were Maldavian(1) British(2) and Vietnamese (4), were rescued within ten minutes and taken to the CIP ‘Koimala’ Executive Lounge for medical treatment.
What: QANTAS Dash-8 Where: Moranbah Airport When: Feb 9 2012, 12:30 pm Who: 3 crew, no passengers Why: While ferrying the plane, the crew made an emergency landing at Moranbah Airport after circling.
They reported smoke in the cockpit, but on landing, no fire was found. Queensland Fire and rescue crews stood on standby during the landing. The ATSB is investigating.
What: Thomas Cook Airbus A320 en route from Belfast to Tenerife Where: Belfast When: February 7, 2012 Who: 175 aboard Why: After taking off from Belfast, the plane developed a technical problem (problems with the landing gear.) Pilots returned to Belfast. They circled for over an hour. While they were in the air, a police helicopter came up and inspected the plane.
The pilots dumped fuel before they made a safe landing.
Passengers interviewed about the event said they were terrified. Flight attendants advised them to brace themselves, take off their shoes and use the emergency exits.
Emergency services were on standby.
Belfast International Airport said that the airline planes to repair the plane and use it this afternoon to fly out passengers at 3:00 pm. The airport spoke highly of the efficiency and emergency management performed by the staff.
What: Batavia Air Airbus A320-200 en route from Makassar to Jayapura Where: Jayapura When: Feb 7th 2012 Who: 148 passengers and 6 crew Why: On landing in Jayapura, the flight encountered a strong storm.
Landing in rain, the plane overran the runway but did stop on pavement. The plane was towed to the gate where passengers disembarked normally.
No passengers were injured. The plane did not end up mired in soft ground, but abrasions on the plane’s skin, and lights on the runway were reported as the extent of the damage.
What: Privately owned Alpi Aviation Pioneer 300 Where: San Donà di Piave (Venezia When: Feb 5, 2012 Who: 2 aboard, 2 fatalities Why: After taking off from Caposile Airfield, the plane lost altitude and fell vertically. and crashed in a field near San Donà di Piave. Franco Borin, 45 and Simone Conte, 18 died on impact. Conte was flying in order to take photographs.
The judge has ordered autopsies on the victims.
Investigators question the structural integrity of the aircraft.
Police and firefighters of San Donà di Piave responded to the scene.
The Alpi Aviation Pioneer 300 is classified as an ultralight.
Prosecutor Rita Uglolini who is coordinating the investigation has acquired a video which may have documented the fall.
What: ANA All Nippon Airways Airbus A320-200 en route from Osaka Itami to Sendai Where: Sendai When: Feb 5th 2012 Who: 166 aboard Why: The Airbus was landing when a sudden gust of wind caused pilots to abort the landing, and leaving scrapes of white paint on the runway.
While performing the go-around, the tail of the plane struck the runway. Pilots circled and made a second approach, which resulted in a safe landing.
Damage to the underside of the tail was significant, and the bulkhead was also damaged The local weather service did not have any indication of wind, and the investigation is underway.
What: Bankstown Helicopter Pty. Ltd. Robinson R44 Raven II Where: Jaspers Brush Airfield, Berry, NSW, Australia When: Feb 2, 2012, 3:55 pm Who: 2 aboard Why: Taking off, the helicopter was carrying two men scouting locations for a Jervis Bay documentary when the Robinson crashed and caught the field on fire.
Andrew Wight, The fifty year old pilot was from Melbourne, writer-producer of the 3D movie “Sanctum” based on Wight’s near-death experience in an underwater cave.
Mike DeGruy, 60, of Santa Barbara, California, won multiple Emmy and British Academy of Film and Television Arts, or BAFTA, awards for cinematography. He was a specialist in underwater photography.
Four ATSB investigators will be investigating.
A Rural Fire Service (RFS) responded. The cabin of the chopper burned, and sparked a brush fire.
