    Gulfstream International Airlines Fined

    FAA Proposes $1.3 Million Civil Penalty for Gulfstream International Airlines

    WASHINGTON, D.C. — The Federal Aviation Administration (FAA) has proposed a $1.3 million civil penalty from Florida-based Gulfstream International Airlines, Inc. for violations of the Federal Aviation Regulations.

    The alleged violations include improper scheduling of flight crew duty time, and the installation of unapproved air conditioner compressors and improperly maintained vent blowers on the airline’s fleet of 27 BE-1900-D aircraft.

    An FAA review of the airline’s electronic record-keeping system for tracking crew duty and rest time revealed that Gulfstream International did not accurately input the proper data from its manually generated hard-copy aircraft logbook records into the electronic system. The discrepancies resulted in scheduling crew members in excess of daily and weekly flight time limitations.

    During a June 2008 inspection, the FAA determined that the airline had installed unapproved automotive air conditioner compressors on its aircraft between September 2006 and May 2008. Following the FAA inspection, the airline grounded all of the affected aircraft and replaced the units with approved aircraft air conditioner compressors.

    In the course of a July 2008 inspection of Gulfstream International avionics and component shops in Fort Lauderdale, the FAA discovered that the airline had installed improperly maintained vent blowers on six planes between January 2008 and June 2008. Following that inspection, the airline replaced the blowers with properly maintained units.

    Gulfstream International Airlines has 30 days from the receipt of the civil penalty letter to respond to the FAA.

    Gulfstream is a former employer of the Captain Marvin Renslow of the ill-fated Continental Connection 3407 flight. If Gulfstream had kept proper records, trained and maintained according to accepted protocol, the Buffalo NY tragedy may never have happened.

    March 2007: Yogyakarta Crash Landing

    This is the sad story of Marwoto Komar, a pilot with 22 years of flight experience.

    In March 2007, he piloted a Boeing 737-400 with 140 passengers on board which crashed on landing at the Yogyakarta airport. The plane came in for landing too high and too fast, and although the co-pilot called for a go-around, he landed on that pass anyway. The Boeing overran the runway and caught fire. Twenty-one people died in that crash.

    The* IFALPA position is that criminalisation of individuals does not improve air safety.

    In July 2008 he was arrested, tried and found guilty of negligence.

    An Indonesian court sentenced Komar to two years imprisonment for his role in the crash.

    Flight Management Computing Systems; Navigation Database Updates – Data and Procedural Exclusions

    Introduction
    This Special Airworthiness Information Bulletin is written to inform and remind owners and operators about changes contained in manufacturers’ navigation database updates. The cyclical updates may exclude certain navigation data including approach procedures, which makes this information unavailable for selection on the aircraft flight management system or navigation equipment.

    The Federal Aviation Administration (FAA) has determined that the airworthiness concern is not an unsafe condition that would warrant airworthiness directive (AD) action under Title 14 of the Code of Federal Regulations (14 CFR) part 39. The FAA continues to monitor these issues with the original equipment manufacturers (OEMs).

    Background
    The FAA occasionally has received inquiries regarding missing data within the aircraft’s flight management system or navigation unit. In many scenarios, a pilot has attempted to select or load a particular approach, but is unable to do so and must determine an alternative in flight. This has often led to confusion and questions about how or why the data appears to be missing.

    From time-to-time, avionics manufacturers may exclude procedures from the aviation database. The causes driving such removals vary, and while some data may be reinstated one cycle later, other data may remain excluded for longer periods of time.
    It is important to note that each avionics manufacturer processes the aeronautical data differently, and procedures excluded on one aircraft or system may not be excluded on another. Avionics manufacturers make data exclusion information available to their customers for consideration before use of affected data in flight operations. Exclusions can frequently be found on the avionics manufacturer’s internet website or may be published in another format as part of the new database cycle.

    Recommendations
    The FAA recommends that pilots do the following:
    1) Become familiar with the most appropriate location where they can find information regarding exclusions from the database that can affect their flight operations.

    2) Exercise diligence in their preflight planning by staying informed about data exclusions pertaining to the type of avionics they utilize.

    Mining Company Plane Crash

    What: Australian Mining Company Sundance Resources Charter Aéro-Service CASA C-212 Aviocar en route from Yaoundé-Nsimalen International Airport at Yaounde, to Yangadou
    Where: 6.3 miles from Cameroon / Congo border
    When: June 19 2010
    Who: 11 passengers 2 crew, (six Australians, two French, one American and two British citizens) all fatalities
    Why: En route to visit an iron ore mining site, the helicopter crashed in the Congolese jungle; it was located Monday afternoon. Those lost include Australian billionaire, Ken Talbot, and company chairman, Geoff Wedlock.

    Flight Attendant Shakedown at OK Corral (Read on a Stolen iPad)

    Flight attendants made the news this weekend with one flight attendant bringing her gun to work even though it was not Bring Your GUn to Work day. (Note: It is NEVER bring your gun to work day for a flight attendant, even if you have a valid Chester County permit to carry a concealed weapon as Republic Airlines FA Jaclyn Luby did.) The .38 caliber Smith and Wesson Airweight was in her purse. After it was confiscated when it showed up at checkpoint, a police officer was attempting to unload it and discharged it into the wall. Oops. I guess the wall was judged guilty, and it didn’t away. Bad wall. The Flight Attendant is being charged with disorderly conduct, and the police officer is on desk duty.

    There’s also Horizon Air flight attendant Wendy Ronelle Dye who said that a passenger brought her an iPad he found on a seat, and she never used it, honest, even though the owner used an ap to track it to her house, and police say they found some of her personal information on it.
