Why: Summary: On October 31 1996, about 0545 UTC a Fokker 100, registration PT-MRK, operating as TAM Airlines flight. 402, a shuttle between Sao Paulo and Rio de Janeiro, Brazil, crashed shortly after takeoff from the Congonhas Airport, Sao Paulo, Brazil. The aircraft was destroyed; 89 passengers,6 crew and 2 on the ground died. 3 passengers were Americans .Witnesses reported the aircraft failed to gain altitude after take-off, collided with a tall building and crashed into residential area 1 mile from the end of the departure runway. Pilot error, unfamiliarity with the craft was judged to be partially responsible. 4 Killed as Small Plane Crashed in Chatsworth, Georgia
A small plane crashed on Piney Hill Road in Chatsworth, Georgia, on July 1st.
The incident happened when the twin-engine Piper PA-23 plane was carrying four people.
Authorities said all four occupants of the plane were killed in the crash.
The incident is being investigated.
2nd Alaska Crash: Piper Strands 5 on Glacier
What: PA-32 sight-seeing in Alaska
Where: Knik Glacier, about 113km northeast of Anchorage
When: Sunday August 8, 2010 1 pm
Who: pilot, Donald Erbey, 49 and 4 friends
Why: Erbey had taken Texas friends sightseeing in his small family Piper 32. When he realized he was flying into bad weather, he turned to head back, “hit a downdraft” and crashed in a snowbank, leaving them stranded since Sunday on Knik Glacier, Alaska.Monday evening, highly trained pararescuers—Maj. Jesse Peterson, Master Sgt. Al Lankford, Tech. Sgt. Chris Uriarte and Tech. Sgt. Angel Santana—braved a 4 mile trek to walk in food and supplies. The plane’s locator beacon was operable.
Helicopters with additional supplies and support are in the area but weather conditions have not permitted drops or a rescue. They have adequate resources to wait till weather clears enough for helicopters to land as hiking out is a last resort.

Gyro Crashes in German Field
What: Fliegmituns.de AutoGyro Europe MTOsport
Where: near Breitscheid Airfield, Germany
When: July 15, 2011 at 15:10
Who: 2 fatalities
Why: On approach to Breitscheid, the Gyrocopter went into a tailspin, fell like a stone, and crashed in a field. The 59 year old pilot and the 26 year old passenger (who had received the flight as a birthday gift from his girlfriend) both died on the scene, 150 meters from the runway.The pilot had been an Air Force transport pilot, and worked for the Defense Department, and an Airbus captain.
Both the pilot and the passenger are being autopsied in order to exclude heart attack or illness as a cause for the accident.
