    What: Bell 206 registered to Hillcrest Aircraft Company en route from Ogallala, Nebraska to Wichita. to Virginia for the U.S. Forest Service.
    Where: Buckskin Road between 19th and 20th Avenues in McPherson County, 40 miles north of Wichita.
    When: 2:45 p.m. Sunday March 8, 2009
    Who: Pilot Roger Hershner was the only one aboard.
    Why: The helicopter crashed after being in route for 2 hours; the crash is under investigation.

    What: Lufthansa A320-200 en route from Munich to Hamburg
    Where: Hamburg
    When: Mar 1 2008
    Who: 132 passengers and 5 crew
    Why: On approach to Hamburg in stormy windy (47 knot wind gusts) weather, the left wing struck the runway on an attempted landing, veered, and had to make another go-round before a final landing.

    What: Aeromexico Connect Embraer ERJ-145 en route from Hermosillo to Tijuana
    Where: 36 passengers and 3 crew
    When: Jan 21st 2010
    Who: 36 passengers 3 crew
    Why: On landing, the Embraer veered off the runway and came to a rest on the ground.

    Several passengers were reported to be in shock, but there were no other reported injuries. The plane’s fuselage incurred damage.

    FAA Press Release: Continental Airlines

    FAA Proposes $230,000 Civil Penalty against Continental Airlines

    FORT WORTH, Texas — The Federal Aviation Administration is proposing a civil penalty of $230,000 against Continental Airlines Inc., of Houston for allegedly operating a Boeing 767 on 22 revenue flights when it was not in compliance with Federal Aviation Regulations.

    The FAA alleges that on Aug. 12, 2008, Continental replaced the nose landing gear wheel and tire assembly on a B-767, but failed to install the required axle washer despite warnings in the maintenance manual and on the tire assembly itself. The warning said failing to install the washer could lead to failure of the wheel bearing.

    FAA inspectors discovered the violation during a records check and noted three identical earlier violations.

    Continental has 30 days from receipt of the civil penalty letter to respond to the agency.

    Emirates Aborts Takeoff

    What: Emirates Boeing 777-200 en route from Dubai to Houston
    Where: Dubai
    When: Aug 8th 2010
    Why: The pilot taking off in the Emirates 777 rejected the high speed take-off, deflating nose gear tires. No injuries were reported.

    A replacement jet was provided.
