Where: crashed on Mount Hamilton near San Jose When: Saturday 04/18/2009 at 0705 Who: Pilot and passenger Why: The plane at an elevation of about 400 feet on Saturday flew into Mount Hamilton. The pilot and passenger were killed. The reason for the accident has not been determined. The Citabria was registered to Jeff Blanton, of Sacramento, and owned by Blanton and David Hartley.

What: Aerocaribbean ATR-72-500 en route from Santiago to Havana Where: Guasimal, Sancti Spiritus Cuba When: Nov 4 2010 Who: with 61 passengers and 7 crew Why: The last flight to leave Santiago before the airport closed due to tropical storm Tomas disappeared from radio contact at 5:42 pm while over Guasimal not far from the Zaza Reservoir. The crew made an emergency call at 5:42. 40 cubans were aboard and 28 non-cubans (9 Argentinian, 7 Mexican, 3 Dutch, 1 French, 2 German, 2 Austrian, 1 Italian, 1 Spanish, 1 Venezuelan, and 1 Japanese).
A hospital worker said “The plane made several abrupt movements before crashing to the ground.” Rescuers hiked in through thick vegetation (using bulldozers to get through the vegetation to find the plane shattered and in flames and no survivors.
Tropical storm Tomas developed into a hurricane. Cuba declared a state of alert.
The ongoing investigation has not ruled that weather is the definitive factor because the storm had not reached Cuba at the time of the crash.
Cuban state airline Cubana de Aviacion owns Aerocaribbean. The flight normally flies twice a week from Port-au-Prince, Haiti, to Santiago de Cuba to Havana

What: Belgian Air Force General Dynamics F-16AM Fighting Falcon en route from Kleine-Brogel air base Where: Belgium When: August 17, 2012, 3:01 p.m Who: no fatalities Why: About a mile from the Kleine- Brogel air base, the pilot of an F-16 fighter jet ejected after the jet suffered a bird strike. The accident occurred near the town of Peer. The pilot who has 500 hours was on a training flight, and after he ejected, ended up in a tree.
The plane has been written off as damaged beyond repair. Residents of Peer were told to keep their windows and doors closed because the plane’s tank contained “hydrazine,” a flammable and toxic. News reports now say that the tank was recovered intact.

What: Mi-2 Plus ambulance helicopter operated by Polish Medical Air Rescue (registration SP-SXF) en route to the scene of an accident near the southwestern town of Budziszow Where: Jarostów Poland When: 17-FEB-2009 7:50am Who: Pilot and medical assistant were killed, while the doctor was taken to a Wroclaw hospital for treatment. Why: A rescue team of the pilot, a doctor and a medical assistant were flying a rescue mission to a traffic accident at theA-4 freeway to assist a pregnant woman who had crashed in (car) traffic in foggy conditions. The helicopter hit the ground 20 km from their destination and 50 km west of Wroclaw. Visibility was poor and it was snowing.The cause of the helicopter crash is under investigation.

What: Inaer/Helland Helicopter en route to Juclar Where: Estany de Juclá – Canillo, Andorra When: June 15, 2011 Who: 6 aboard, 5 fatalities, 1 serious injury Why: At 10:30 am, a flight transporting equipment and supplies to Juclar crashed in a mountainous area near the Andorran town of Canillo. Four bodies were immediately being recovered; one was missing, having been ejected from the chopper. One individual survived with serious burns on 70% of his body, and was evacuated to Barcelona by chopper to the Hospital Vall d’Hebrón de Barcelona..
The helicopter is reported to be of French origin and registered in Spain. Forensic and technical experts are on scene; and the investigation will include Spanish officials, and representatives of the French helicopter buildern.
A field hospital has been set up to care for the victims’ families whose names were not released but were all reported to be “members of the crew.”
Inaer, the company which owns the helicopter, stated the accident is due to bad weather and low visibility due to foggy conditions.
Although the original report lists one as missing, the body of the missing person was found.

A small plane crashed in Riegelwood, North Carolina, on the night of September 11.
The Cessna 172 was heading from Bolton to Wallace when it went down. North Carolina Wing Civil Air Patrol dispatched two aircrafts that found the wreckage at 1:15 a.m. on September 12.
The pilot, who was the only one aboard, was killed in the crash. He was identified as Gene Pierce, 76, of Duplin County.
The cause of crash is not clear at the moment.

Overland Airways flight 1186 swerved off the runway at Ilorin Airport in Kwara State of Nigeria on November 29.
The ATR 72 aircraft, which departed from the Nnamdi Azikiwe Airport in Abuja, veered off the runway after it landed at Ilorin Airport at about 4:45 p.m.
Authorities confirmed that all 59 passengers and 4 crew members remained unhurt.
The Nigerian Civil Aviation Authority is investigating.
