    Petrobra Oil Field Chopper Crashes off Coastal Brazil

    What: Senior Táxi Aéreo Agusta Westland AW139 from oil platform to Rio de Janeiro
    Where: in the Campos Basin, Rio de Janeiro

    Why: On Friday afternoon, the chopper charted to Brazilian oil major Petrobras set out from the PetrobrasP-65 platform en route to Macaé.

    While miles from shore, the crew declared emergency, and went down 100 km off the coast of Rio de Janeiro. Contact with the chopper was lost at 17:00 local time.

    A Bandeirante Patrol plane and two search Brazilian military aircraft, the 4th Squadron of the 7th Aviation Group, a H-34 Super Puma helicopter, and the 3rd Squadron of the 8th Aviation Group initiated a search for the downed Senior Táxi helicopter. In all, there were five aircraft, a helicopter and four ships, including a Guajará Navy patrol vessel and a Lynx helicopter on standby in the area.

    After two days of searching in rough seas, three bodies were found on the ocean floor 100 meters down and 62 miles from the coast. One body is still somewhere in the ocean.

    The fatalities are listed as Rommel Oliveira Garcia, pilot, Lauro Pinto Haytzann, co-pilot, both employees of Senior Air Taxis, and passengers Ricardo Leal de Oliveira from the company Engevix, and Joao Carlos Pereira da Silva, the Brasitest inspector.

    Tam Air Crash Lawsuit

    A complaint filed by his family on behalf of Ricardo Tazoe, in the U.S. District Court for the Southern District of Florida, also names European jet manufacturer Airbus, the Goodrich Corp, and International Aero Engines (IAE) as defendants. The Miami resident died in an Airbus crash in Sao Paulo.

    198 other people who were killed when the Tam Airbus A320 skidded off a rainy runway at Sao Paulo’s Congonhas Airport.

    TAM is charged with negligence. The plane should have been grounded because one of its thrust reversers was not working. Defendants include Goodrich, the manufacturer of the aircraft’s braking system and IAE, which assembled the plane’s engine.

    “From the evidence gathered so far, it’s clear that TAM knew there were problems with the aircraft,” Steven C. Marks, an attorney at Miami law firm Podhurst Orseck, said in the statement. “Had (the thrust reverser) been operational, it may have prevented this accident.”

    The lawsuit seeks unspecified financial damages.

    IAE is United Technologies Corp. Pratt & Whitney, Britain’s Rolls-Royce Plc, Japan’s Aero Engines Corp. and Germany’s MTU Aero Engones.

    The firm that filed the suit is also representing several families in a suit concerning an earlier crash in the Amazon this year.

    Two planes collide over Westlake Village


    A Cessna 172 and a Cessna 172RG Cutlass collided over Westlake Village eight miles east northeast of Ventura.

    The Cutlass, registered to Ameriflyers of Florida LLC made an emergency landing on the Westlake Golf Course, Westlake Village, California. Three aboard sustained minor injuries in a belly landing at 2:15 p.m. near the third hole. They were hospitalized with minor injuries.

    The second plane crashed in the Santa Monica Mountains, sparking a brush fire and a search and rescue effort. The pilot was found under the wreckage.

    One of the planes had left Santa Monica Airport on an engine test flight and was heading east when it collided with the other.

    Plane Crashes Kansasville Garage

    What: Beech A36 Bonanza en route from Chicago Executive Airport to Minocqua
    Where: Eagle lake Kansasville, Racine County, WI
    When: Sept 9, 2012
    Who: 1 fatality
    Why: The pilot who had been en route from Chicago Executive Airport to Minocqua crashed into a Kansasville Wisconsin garage but avoided hitting any of the neighborhood houses in Eagle Lake. the plane, a garage, and two cars were destroyed; and two homes sustained damage. Debris and plane parts were scattered over the immediate area.

    Witnesses say that when the plane hit, it shook the neighborhood.

    The FAA and Racine County Sheriff’s Office are investigating.

    Small plane crash in Brazil


    An Embraer EMB-201A Ipanema was en route from Paracatu MG to Goinia when the pilot had to make a forced landing in a field. The plane was commonly used as a cropduster.

    The plane had had a fuel supply problem before but apparently this time the problem was in the fuel pump/power system.

    The pilot was uninjured but the plane sustained some damage. There was no fire, but firefighters were on hand, and there was a fuel leak.

    Centro de Investigação e Prevenção de Acidentes Aeronáuticos (Cenipa) will be investigating.

    Fatal Crash during Secunda Airshow

    A Red Bull Extra 300 aircraft crashed on October 12 during a Secunda airshow in Mpumalanga.

    The engine appeared to stall when the pilot came out of a dive. The plane leveled out, made it to the ground and skidded seventy meters. When the plane caught fire, the pilot, Glen Dell, had to be helped from the cockpit. Reports indicated that he was airlifted and hospitalized at Mediclinic with burns and other injuries then transferred to Glenwood hospital/Netcare Sunninghill Hospital for burn care, but succumbed to the injuries.

    The plane was a Extra 330LC, which is a Lycoming AEIO-580 powered two-seat competition aircraft, a type of Aerobatic monoplane, designed in 1987 by Walter Extra, an award-winning German aerobatic pilot and built by Extra Flugzeugbau.

    Dell had 25,000 hours flying experience on 250 aircraft, held a zero foot aerobatic display waiver, which is awarded to pilots with proven competency and reputation reflecting display pilot expertise. In 2004, Glen finished first overall at the 6th Advanced World Aerobatic Championships, becoming the first South African to win. He had competed several times in the Red Bull Air Race but had announced he was returning to South African Airlines.

    The Civil Aviation Authority is investigating the cause of the crash.
