    Small Plane Crash Takes Lives of Family, Friend

    What: Cirrus SR22 en route from Kansas City to Springfield
    Where: North of Willard, Missouri
    When: Sept 15, 2012 Midnight
    Who: 5 aboard, 5 fatalities
    Why: The Lambert family plane crashed in a pasture just after midnight Saturday in a field near the intersection of State Highway 123 and Farm Road 81. Victims were reported a family and friend, including John M. Lambert, 44, of Springfield; Grayson Lambert, 16; McKinley Lambert, 15; Joshua Lambert, 10; and Robin E. Melton, 46, of Ozark.

    The family was returning from a Kansas City Royals baseball game when their plane crashed in a farmer’s field, sending out what was described as a “sonic boom” and shooting fireballs into the sky. The plane essentially disintegrated on impact.

    The family in whose farm they landed called 911 just after midnight, but no survivors were found.

    The FAA is investigating.

    Thanksgiving Bonanza Crashes in Orange Grove


    What: single-engine Beechcraft Bonanza
    Where: orange grove a quarter mile from the Porterville Airport.
    When: 9:30 Thursday morning, Thanksgiving day
    Who: The three people–two adults and a child–aboard have minor to serious injuries. They have been identified as pilot John Werle, 49; Kris Goodfellow, 38; and her 8-year-old son, Bennett Watson and the boy’s black Labrador retriever
    Why: Witness heard the plane; heard the plane quit; heard the crash. Possibly Werle had been flying too low on the descent and the plane failed to respond when he tried to pull up higher.

    Small Plane Crash-Lands Near Hayward Executive Airport

    Small planeA small plane crash landed on the Skywest Golf Course in Hayward, California, on the afternoon of February 15.

    According to Hayward Fire Capt. Don Nichelson, the plane departed from Hayward Executive Airport but had to crash-land shortly afterwards, due to power loss.

    The pilot, who was the only one aboard, remained uninjured.

    The incident is being investigated.

    Sukhoi plane crash Caused by Pilot Error

    The National Commission on Safety Transportation announced that the Russian Sukhoi Superjet 100 that crashed into a mountain in Indonesia seven months did so as a result of pilot error. All 45 Aboard were fatalities.

    Indonesian authorities claimed that the plane functioned adequately and there were no malfunctions of safety gear.

    The fact that the pilot digressed from the route, and that the (foreign) pilots were not well aware of the mountainous terrain may be behind part of the decision. As a demonstration flight, the pilots were probably encouraged to make dramatic maneuvers at a terrible price.

    Thirty-eight seconds prior to the impact, cockpit instruments issued a warning saying “pull up, terrain ahead”. Later the warning “avoid terrain” was issued six times, but the instruments were turned off because the crew assumed there was a problem with the database.

    Officials said that “The crew was not aware of the mountainous area surrounding the flight path,”

    The Jakarta radar service was incapable of informing flight crews of minimum safe altitudes.

    The Russian pilots, while experts, were not intimate with the Indonesian topography.
    Six minutes after takeoff, the pilot and co-pilot requested Jakarta ATC for permission to drop from 3,000 meters to 1,800 meters on the scheduled half-hour flight. Six minutes later, the plane struck Mount Salak.

    Pilot Lost off Florida Coast

    What: Tampa Bay Aviation Robinson R22 Helicopter
    Where: Bright Bay Court near Beer Can Island. Apollo Beach Florida
    When: Nov 29, 2012
    Who: 1 fatality
    Why: 400 yards offshore, a helicopter crashed just off of Apollo Beach. The off-duty Tampa police officer who witnessed the crash said that the rotors looked like they came off the top of the chopper. Another witness reported an explosion.

    Divers recovered the body of 60 year-old John Lawrence Ward of Tampa.

    Small Plane Crash in Brazil


    On Nov 8 2013, a privately owned Embraer EMB-711C Corisco en route to São Paulo took off from Cianorte, Paraná (Cianorte). The pilot circled the area several times, lost power, and crashed a thousand feet from the airport on a farm. On impact with the ground, the plane rolled.

    The two people aboard were injured during the impact.

    Pilot Walter Auada Botelho de Morais Toledo and passenger Emanuela Érica Garcia were taken to Hospital São Paulo, Cianorte. She is pregnant, and suffered Traumatic Brain Injury. The pilot broke his foot.

    Walter Auada Botelho de Morais Toledo is a commercial pilot.

    The fire department responded to the scene. The FAB will be investigating the accident.
