Proximity Blamed on Tenerife South ATC

On November 12, 2011, the Iberia Airbus A321 had been approaching Tenerife South airport at high speed just before it landed on the same runway that a Jet2 Boeing 757-200 was departing did so because of a premature take-off clearance to the 757.

The 757 was airborne over the runway as the A321 touched down at an unexpectedly fast approach. Although the A321’s airspeed had decreased constantly through the descent, its ground speed was 210kt. Expected airspeed is in the 180-203kt range.

The planes were separated by at least 4,100ft.

The incident has been attributed to the tower controller’s “improper handling” of the take-off clearance to the 757.

    EasyJet Falling Pieces

On August 12, an easyJet Airbus A320-214 en route from Milan-Malpensa Airport to Lisbon took off okay, but pieces of the plane were falling.

The control tower spotted the debris on the runway.

After losing parts of the engine cowling, pilots made a safe, if dramatic landing back at the Milan-Malpensa. None of the 174 passengers and crew sustained injury, though most of them went through an emotional roller coaster, especially when a passenger screamed out that “A piece of the wing has gone flying.

Pilots circled for twenty minutes before landing; then after landing safely, when pilots put engines into reverse, more stuff fell off the engine. Although fire engines were on standby, standby is all they had to do. Passengers and crew exited via the staircase.

In a genius of understatement, Easyjet confirmed “flight EZY2715 from Milan Malpensa Airport to Lisbon on 12 August returned from airborne shortly after take off due to a technical issue with one of the engines.”

Passengers were provided an alternative flight.

    In George’s Point of View

I don’t make a lot of comments, due to my copious free time, but I can’t resist. In the meantime while waiting for the updates, we are considering shipping easyJet some Superglue, or maybe a certified mechanic or two. In the words of mechanics everywhere, lefty loosie, righty tighty. I am no mechanic, but this smells to me like…dare I say it…

    Maintenance! Maintenance! Maintenance!

Not to make light of the situation though–airline safety is no joke. We expect there will be public updates about the investigation into this incident.

    UPS Crash in Birmingham Alabama

A UPS Airbus A300F4-622R cargo jet en route from Louisville to Birmingham International Airport was on final approach to runway 1-8 when it crashed in Birmingham. The two pilots aboard sent no warning message, and died on impact.

There is no indication yet if something in the cargo contributed to the crash, or if there was mechanical failure or some other factor involved.

Visibility was 10 miles and there was a 700 ft cloud ceiling.

Virgin and Jetstar Collide in Australia

A Virgin Australia Boeing 737-800 scheduled from Melbourne to Maroochydore had a ground accident on the tarmac.

The plane collided with a Jetstar AirbusA320. None of the 175 passengers on the Virgin flight were injured but both planes sustained damage.

The Boeing’s left winglet was damaged.

The Virgin Australia plane was reversing.

The Jetstar sustained damage to the tail. It had just returned from Sydney, and was stationary during the collision.

An investigation is ongoing. Damages of about three million are estimated to each craft.

    June 30, Yemenia Flight 626

We remember on June 2009 when 152 people died aboard a Yemenia Airlines Airbus A310-300.

    Yemenia Flight 626.

    We remember.

The years pass and the headlines fade, but the families will never forget the loss of their loved ones, and neither should we.

The pilot was about to land on an airstrip in the Comores Islands when ATC lost contact.

The plane and all its passengers crashed into the ocean.

12-year old Bahia Bakari survived the crash, rescued after thirteen hours in the ocean, clinging to debris. She was and is a miracle.

The final report on Flight 626 concluded the crash was due to the crew’s inappropriate flight control inputs that led to an aerodynamic stall.

Crews inappropriate flight control inputs means pilot error. In this case, the pilot error was exacerbated by a nineteen year old plane in dubious condition, bad weather conditions (winds gusting to 64 km/h 40 mph; 35 kn) and a primitive airport.

Crashes like Yemenia should function like red lights. The only way they make sense is if when they happen, we stop and consider those who were lost, and investigate what went wrong. On behalf of safety, the aviation industry should mandate to use every atom of what is discovered to prevent future accidents. The industry in general, and the airlines in specific should make changes that will help prevent future accidents.

Pilot error means airlines like Yemenia are still having problems training pilots. Still problems in the cockpit. Sadly, conditions still exist for more accidents like this in Yemenia’s future.

    Commercial Plane Comparison

    June 17. 2013
While the upbeat “video below” compares basic features, the real world of aviation is focusing on a comparison of the problems noted.

    • Airbus (350) had a “blockage in the drilling station and the knock-on effect hampering wing production” and problems with suppliers of A350 structural components. A350 also face the looming challenge of obtaining certification
    • Boeing (787) which already graduated to the market, has demonstrated issues with 787’s lithium-ion batteries which use 10 times more power during initial start-up than other Boeings. In-flight monitoring made a demonstrable difference in reliability. A recent report estimates that 29 flights had been “saved” because of the 787’s extensive monitoring system

    Mechanics Forgot is no excuse

Remember that British Airways Airbus A319 event we talked about the other day? The one where (oops) the engine cover fell off of the engine on that Oslo flight?

The one where “the coverings broke off and punctured the right engine’s fuel pipe, damaging the aircraft’s systems?” The Air Accidents Investigation Branch the investigation said evealed that the fan cowl doors on both engines were left unlatched during maintenance.”

BA Airways emergency landing with Engine fire

A British Airways Airbus A319-100 was en route from London to Oslo when it developed an engine problem.

Passengers heard a loud explosion, and saw smoke from the right engine. Apparently the engine doors blew off.

During takeoff, the casing of the left engine ‘came away’ and the right engine exploded when the plane made a quick return, landing in London.

Good thing that Emergency services were on standby to extinguish the engine fire.

Passengers and crew made an emergency evacuation via emergency slides.

None of the 5 crew and 75 passengers were injured.

The ABC News video below suggests a birdstrike that is purely speculative

Thomas Cook Unexpected Stop in Bermuda

    photographer Andrea Tabanelli-Lugo Spotter
A Thomas Cook Airbus A330-200 OY-VKF en route from Cancun to Manchester had 338 people aboard when pilots made an emergency diversion to Bermuda due to what one source called a “reported fire in the cabin” at about 8.20pm. Another account recorded the problem as ‘smoke coming from the in-flight entertainment system’.

Pilots made a safe landing with emergency services on standby. Passengers disembarked without injury, and were provided hotel accommodations until their flight out.

Thomas Cook’s official statement is that ‘It was initially reported as a suspected fire in the cabin, but there was no fire.’There was a small amount of smoke detected by a member of the crew in the cabin, but it is thought that was something to do with the in-flight entertainment system.’

Nordwind Commercial Passenger Flight Dodges Missiles over Syria

On April 29, we saw a report that Nordwind passenger airlines dodged missles over Syria.

The Airbus A320-232 was flying from Sharm el Sheikh-Ophira Airport to Kazan Airport (from Sharm el Sheikh, Egypt to Kazan, Russia) was over Syria when two missiles exploded in the air close by.

Interfax.ru reported the crew steered the jet and saved the passengers’ lives. Unidentified forces launched the two ground-to-air missiles which came very close.

    Russian authorities announced the incident.

On April 29, at 0455 (Moscow time), a Northern Wind plane was travelling in Syrian airspace from Sharm el Sheikh to Kazan and the pilot crew has recorded military activities on the ground that could have endangered the plane and 159 passengers.

    Pilots made a safe landing.

On April 28, 2013, a Wizz Air Airbus A320-232 took off from Warsaw then descended rapidly from about 11000ft.

Passengers smelled smoke. A passenger named Barbara was quoted (machine translation) “Zachybota?o plane dropped like a machine. Then I felt a burning smell. Other passengers can feel it. After a while we went to the employee. Were informed that there was a failure

Pilots notified ATC and turned around to return to Chopin Airport. The airport said that Pilots said this was not an emergency landing.

There were 161 passengers aboard. They disembarked without any problem, and were provided a later Wizz flight to London.

    Singapore Airlines Emergency Landing

The Singapore Airlines Airbus A330-300 en route from Singapore Dhaka with 105 passengers and 12 crew aboard diverted to Bangkok when they got an internal flight warning. There were no injuries.

In spite of the cargo fire suppression system, there was a burning smell and a fire.

The plane made a safe landing in Bangkok. The plane taxied to the gate, and passengers disembarked and were provided hotel accommodations, meals and transferred to another airline.

    An investigation is underway.

Singapore Airlines’ spokesman, Nicholas Ionides said that “Flight SQ446 operating between Singapore and Dhaka diverted to Bangkok as a result of indications of smoke from the aircraft’s rear cargo hold.”

When Thai authorities clear the Airbus, it will be flown back to Singapore.

    In George’s Point of View

It has been well documented that Airbus and Boeing are taking different paths regarding their approach on batteries.

But still, it is one of my least favorite things to read (especially while I’m halfway to Singapore) that there has been yet another cargo fire at all, regardless of what kind of plane it is on.

At least I had time to jot down some thoughts.

I hope that the media shines as bright a light on this airbus cargo fire as it has recently on the Dreamliner battery situation. Sure, the Dreamliner is developing new technology and Boeing is responsible for going the distance to keep new developments safe. The Dreamliner battery issue is new. But the fact that Boeing is developing new technologies and new solutions does not provide Airbus and an established plane like a A330-300 a free ticket when it comes to combustion.

Hopefully Airbus is paying attention to all problems and not getting too cocky. It should remember and respect that a plane is still an air-filled bubble hundreds of feet in the air. Until people develop their own wings, random combustion situations like cargo fires on an Airbus A330 are just as potentially hazardous situation–if not more so–than any battery incident that actually occurred on the ground.

Let us please see a fix for this problem.

Air Europa Airbus Runs over Debris.

On April 16, 2013, an Air Europa Airbus A330-202 #EC-JPF was en route from Madrid-Barajas Airport to Caracas-Simón Bolívar International Airport in Venezuela.

However, the flight took off after an Aéromexico tail strike, and ran over fuselage debris which punctured a nose gear tire.

Pilots circled the airport, and returned to Barajas where they made a safe landing at 19:30 with emergency services on standby.
