    Jetstar Airbus Pilot Catches Hydraulic Leak in Time

    When: 9.30am.
    Who: 179 people
    Why: When the pilot identified a leak in the hydraulics, he made several loops of Cairns airport before undertaking a “standard air return.” After an interim of 3 and a half hours, the plane resumed the flight.

    What: American Eagle Embraer ERJ-140 en route from Lindbergh Field San Diego to Los Angeles
    What: LAN Airlines Boeing 767-300 from from Lima Peru to Los Angeles, CA
    Where: LAX
    When: Jan 19th 2010
    Why: At the time of the American Eagle’s arrival to LAX, both jets were flying at the same altitude. The danger was not collision. The danger was wake turbulence. Required separation behind the Boeing is 5 nautical miles.

    Trailing behind an aircraft, wake turbulence is made up of multiple force drafts including wingtip vortices and jetwash. Jetwash is jet engine gas output which is turbulent but of short term but wingtip vortices can remain for up to three minutes.

    Picture, if you will, invisible speed bumps made of wind that could knock your car off the road trailing the car in front of you. If this were a factor with cars, tailgating would be a completely different thing.

    A cockpit voice recorder of the pilots responses will clearly indicate if the plane in the rear of the situation runs into the leading aircraft’s wake. What officials are questioning here are the actions and responses of LAX Air Traffic Control, which placed these two jets close enough to be endangered.

    On January 19, maybe Air Traffic Control error put the Eagle jet less than three miles from the 767, but the pilot managed to stay out of the other jet’s wake. LAX denies this is a case of inexperience and maybe they are correct, because the worst case scenario crash did not happen. Maybe it would have happened if the jet following were flying at lower altitude.

    What matters is that the flight landed safely and whether it was ATC or the pilot, someone did something right because both flights made it to the ground safely.

    Brazil Regulator Proposes Restrictions

    SAO PAULO, Brazil-Brazil’s civil aviation regulator proposed restricting flights at the nation’s busiest airport on Thursday, hoping to improve safety at the site of the nation’s deadliest plane crash.

    The rules proposed by the Civil Aviation Authority would restrict destinations to a 1,000-kilometer (620-mile) radius of the Congonhas airport and would bar connecting flights from the airport.

    The Defense Ministry will now assess the report, and Defense Minister Nelson Jobim announced separately that authorities will decide within one month the precise location for construction of a third runway at Sao Paulo’s Guarulhos airport.
    Guarulhos handled mostly international flights before the July 17 crash at Congonhas, but many domestic flights were transferred there after the crash, prompting officials to revive a long dormant plan for the extra runway.

    As many as 5,000 families living near Guarulhos may have to be relocated to make way for the runway, depending on the location selected, Brazil’s Agencia Estado news service said.

    The proposals grow out of a review of operations at Congonhas following the crash in which a TAM airliner ran off the runway and slammed into a building two months ago, killing 199 people.

    Officials have not determined a cause, but many experts have said that Congonhas’ main runway is too short and that the airport _ Brazil’s busiest _ handles far too many flights.

    The 1,000-kilometer (620-mile) limit would let airlines fly to key destinations such as Brasilia, Rio de Janeiro and cities in southern Brazil, but not to tourist destinations in the northeast.

    TAM Linhas Aeras SA and Gol Linhas Aereas Inteligentes _ Brazil’s two largest airlines _ previously used Congonhas as a major hub, but are now reorganizing operations.

    The regulators also proposed limiting the number of passengers aboard flights, and restricting takeoffs and landings to 33 per hour at Congonhas, cutting overall passenger capacity at the airport to 4,700 per hour from 5,100 per hour.
    Jobim, whose ministry oversees Brazilian civil aviation, earlier announced plans to for an escape area at the end of the runway and limits on operations in wet conditions.

    Pilot Fatigue

    We passengers have a problem. We don’t want our pilots falling asleep on the job. Behind that locked cockpit door, there’s a real danger that our pilots might well be dozing off. Like Howard Beale, in the movie NETWORK, we’ve gotta say, “I’m a human being, goddammit! My life has value! … I’m as mad as hell, and I’m not going to take this anymore!!”

    Apparently aviation authorities are taking notice of the sleepy pilot phenomenon.

    In a September 2009 speech, the Secretary General of the International Civil Aviation Organization, Raymond Benjamin raised the critical issue of pilot fatigue. The ICAO is coming out with new provisions governing flight times, duty times and rest periods for pilots which are applicable now, in November 2009. Fatigue risk management systems to augment new fatigue provisions are in development. I wonder if this includes things like alarms going off at random times during a flight, random check-ins or shrill-voiced stewards armed with tasers knocking on the cockpit door every fifteen minutes asking “Are you guys still awake?”

    Ever since the Colgan Air crash near Buffalo that killed all 50 people onboard, the United States had been developing more stringent standards; and this has been stepped up even more since the recent occurrence of pilots who overshot their destination by 150 miles–although both pilots in this incident claimed to be distracted and not asleep. American regulations currently allow a maximum of 16 hours on duty–but that figure is currently being reassessed by the FAA.

    Canadian pilots are looking toward the Canadian Transportation Safety Board to also up their standards, which they have not yet done, even though the Canadian Transportation Safety Board recently reported about a pilot: “Fatigue likely resulted in a degradation of his concentration” in reference to a 2005 incident where a pilot nodded off and allowed the plane to slow nearly to a stall. Although a dozen Canadian fatigue incidents have been connected to 23 deaths, the Canadian Transportation Safety Board claims that their current regulations are “adequate.” The European Union allows a maximum of 13 hours. Canadian regulations allow a crew to be on duty for 14 hours in 24), night work, irregular start times, routes that cross several time zones and multiple takeoffs and landings.

    Some of the hardest work to deal with is called continuous or stand-up duty, which is when pilots handle a late evening flight, stay overnight and fly the turnaround leg–or the timing of this type of flight which forces irregular daylight hours to be the only time a pilot can sleep. Turnaround times don’t exactly respect human biorhythms.

    Pilots might be dropping off to sleep while they’re flying, but the rest of us look at the statistics and fly white-knuckled with our hearts in our throats. With worries about everything from plane maintenance, to lax air traffic control, from crowded flight corridors to random part failure, the least we passengers can do is bring alarm clocks, our souped-up air horns, and plenty of high-test Starbucks emergency caffeine to keep our pilots awake, alert and on the mark.

    AOPA Places Blame on Holes in System Not Flight Schools

    Update to Should Illegal Aliens Attend Flight School Owned by Illegal Alien?

    The AOPA believes it is a breakdown in government communication that is why foreign students in the United States were able to receive flight training and some even earn their pilot certificate though they had entered the country illegally or had overstayed their authorized period.

    Flight training providers were in compliance, but government was suffering from a lack of communication, in AOPA opinion.

    Only that’s an oxymoron, isn’t it? Government Communication?

    The report says “As of January 2012, inspection results show that the rate of compliance with AFSP requirements increased from 89 percent in fiscal year 2005 to 96 percent in fiscal year 2011,”

    Currently the TSA has limited ability to get information from U.S. Immigration and Customs Enforcement (ICE) to determine whether someone is in the country legally. This is a situation which should obviously be corrected. When this vetting process is transparent, the problem will cease to exist.

    A Word about Safety, Brazil and Towers

    With aviation safety issues buzzing in the US because of the widespread tower closures, I was surprised to find US safety being held as a higher standard in a critique of Brazilian aviation by pilot Antônio Carlos Cruzeta.

    His article at *http://paduim.blogspot.com/2013/02/relato-de-um-piloto-de-linha-aerea.html pillories the conditions of flying in Brazil, even compares the pilot to driving a luxury BMW in the middle of a safari in Africa.

    But I cannot but wonder if even as this pilot pushes for progress in Brazil, we in the US are bound to be falling back. Will it take an aviation disaster here to wake up our government that we need to maintain our current standards of safety?

    A Brazilian pilot can ask that question, and so can we. How can pilots continue to fly millions of passengers millions of flights in state-of-the-art planes when losing so many towers? And now there are lawsuits piling up as localities begin legal battles to keep their towers. Should tower support be withdrawn, leaving pilots to “fly by the seat of their pants?” What do US pilots think of this withdrawal of support? DO pilots consider towers extraneous?

    Three hours or so from home the ride from Rio was unusually turbulent. Though I slept all the way to Houston this time, will I be so confident in the future? I worry for the state aviation safety as thousands of pilots converge flying to and from airports where tower support was once but is no longer.
