    Just Released: Investigation Boeing 787 engine failure

    WASHINGTON – The National Transportation Safety Board is investigating an engine failure that occurred on a Boeing 787 Dreamliner during a taxi test in Charleston, South Carolina.

    On Saturday, July 28, 2012, Boeing and General Electric, the engine manufacturer, notified the NTSB that a Boeing 787 experienced an engine failure during a pre-delivery taxi test. As a result of the failure, it has been reported that the engine left debris on the active runway at Charleston International Airport and caused a brush fire. There were no passengers aboard the aircraft nor were there any fatalities or injuries.

    Upon the notification of the incident, the NTSB sent an aviation investigator with extensive expertise in aircraft powerplants, to the scene on Sunday to gather information to better understand the circumstances of the event.

    As a result, the NTSB decided to conduct an investigation and has named David Helson as the Investigator in Charge. In the next few days, an NTSB aircraft powerplants expert and a metallurgist from the NTSB Materials Lab will travel to a General Electric facility in Cincinnati, OH, to lead and coordinate the disassembly and examination of the engine in question.

    The Federal Aviation Administration, Boeing and General Electric are parties to the investigation.

    NTSB PROVIDES INVESTIGATIVE UPDATE AND PHOTO OPPORTUNITY OF LAB WORK RELATED TO BOEING 787 BATTERY FIRE IN BOSTON


    January 23, 2013
    WASHINGTON – Tomorrow, National Transportation Safety Board Chairman Deborah A.P. Hersman will provide an update on the NTSB’s investigation of the Jan. 7 fire aboard a Japan Airlines (JAL) Boeing 787 at Logan International Airport in Boston.

    Chairman Hersman will brief credentialed members of the news media on the progress of the investigation, after which she and senior staff will take questions. Those unable to attend in person can call in to a teleconference line.

    Reporters will then be invited to view the materials laboratory where the battery is being examined. Photography will be allowed but no “stand-ups” or taped reporting from the lab will be permitted.

    For planning purposes, RSVP is required for those planning to either attend in person or call in.

    The following factual information has been developed about the battery: It consists of eight cells of 3.7 volts each. All eight cells had varying degrees of thermal damage. Six of eight cells have been CT scanned and have been disassembled to expose their electrodes. All electrode windings in the battery are in the process of being photo-documented and are undergoing microscopic examination. In the coming days, the remaining two cells will undergo the same examination. Additional information will be provided tomorrow.

    Ask NTSB Why United Boeing Battery overheat was a non-reportable incident?

    What is significant here is that no report was filed on the United Boeing 787-800 (Washington Dulles,DC-Paris Charles de Gaulle) that received a main battery overheat indication while landing in Paris on
    Nov 12th 2017. The battery was venting fluid via from the forward vent relief system. The NTSB reported to Aviation Herald that the occurrence was rated a non-reportable incident. No investigation was opened. 14 other SDR reports were filed).

    NTSB to Present Findings of Safety Study


    The National Transportation Safety Board will hold a Board meeting on May 22, 2012, to consider the findings of a draft safety study on experimental amateur-built (E-AB) aircraft.

    Of the approximately 224,000 general aviation (GA) aircraft in the U.S., about 33,000 of them are classified as E-AB. This includes a wide variety of aircraft, which can be built from a prefabricated kit, existing plans, or a builder’s unique design. Unfortunately, this group of aircraft has, for several years, experienced accident rates greater than those of other comparable segments of GA.

    The NTSB, with the assistance and input from the Experimental Aircraft Association (EAA) and its many members, undertook the study to identify areas that could be addressed to improve the safety record of this growing and innovative segment of GA.

    NTSB Denies Petition on 1996 Crash of TWA Flight 800


    The National Transportation Safety Board today denied a petition for reconsideration of its findings in the investigation of the 1996 crash of TWA flight 800.

    The petition was filed in June 2013 by a group called The TWA 800 Project. Petitioners claimed a “detonation or high-velocity explosion” caused the crash.

    “Our investigations are never ‘closed’,” said Acting Chairman Christopher A. Hart. “We always remain open to the presentation of new evidence.”

    Before responding to the petition, NTSB staff met with the petitioners’ representatives and listened to an eyewitness who described what he saw on the night of the accident. After a thorough review of all the information provided by the petitioners, the NTSB denied the petition in its entirety because the evidence and analysis presented did not show the original findings were incorrect.

  • NTSB SAFETY RECOMMENDATION: 14 Code of Federal Regulations Part 91


    The National Transportation Safety Board makes the following recommendation to the Aircraft Owners and Pilots Association:

    Educate pilots of 14 Code of Federal Regulations Part 91 flight operations about the benefits of notifying passengers about the location and operation of survival and emergency communication equipment on board their airplanes.
