    Chopper Pilot Dies Aswering Slave Lake Call

    What: Campbell Helicopters Ltd. Bell 212 Twin Two-Twelve
    Where: Slave Lake, Alberta, Canada
    When: May 20, 2011
    Who: 1 fatality
    Why: The chopper responding to a (wildfire) fire call crashed for unknown reasons 30 meters offshore Slave Lake, Alberta. The forest fire leveled a third of the Slave Lake community. The pilot’s name has not yet been released.

    The STARS Air Ambulance call was cancelled. The pilot died in the crash. Emergency responders were treated for hypothermia.

    Puerto Rican Crash Found

    What: Rockwell International 690B, took off from Tortola in the British Virgin Islands en route to the Puerto Rican capital
    Where: Puerto Rican rainforest fog-shrouded El Yunque mountain
    When: on Wednesday
    Who: Caribbean pilot Ken Webster owner of Websta’s Aviation Services Inc. in St. Croix, U.S. Virgin Islands. ; and two U.S. tourists Tracy Turner and Ken (Kent) Clapp
    Why: During foggy weather, the plane apparently slammed into El Yunque mountain.

    Peoria: 17 Year Old Crashes Piper Cherokee

    What: Piper Cherokee en route from Peoria airport
    Where: four-lane road 10 miles northeast of Peoria airport
    When: 7 p.m
    Who: pilot, 17-year-old Scott Hall
    Why: The plane appeared to clipped a light pole before it skidded across the road.

    Chopper Pilot from Belarus Dies in Congo Crash

    What: Mil Mi-8
    Where: Badondo north of Brazzaville, Congo
    When: Nov 15, 2011
    Who: 4 aboard, 1 fatality
    Why: The military chopper which had been chartered by a mining company crashed during bad weather when it tried to land.

    The pilot, Alexandre Kossiarev, was from Belarus and died in the crash.

    Three passengers were injured.

    Doctors Without Borders-Chartered Helicopter Crashed in Nepal; 4 Killed

    A helicopter crashed on Yamuna Danda hill, near Balefi village, in Nepal, on June 2.

    Authorities said the helicopter, chartered by Doctors Without Borders, was involved in post-earthquake relief activities. It crashed when it was returning to Kathmandu with 4 people aboard.

    All four people, including 3 men and a woman, were killed in the crash. The deceased have not yet been identified.

    The cause of crash is being investigated.

    Hawaii Plane Crash Kills Vet and Son

    What: single engine Piper Cherokee PA32
    Where: Koolau Mountains in East Oahu near the Lanipo Trail on WAIALAEIKI RIDGE, 10 MILES FROM HONOLULU, HI
    When: Jan 11, 2010
    Who: Pilot Dr. Nicolas Polumbo and passenger Timmy Polumbo
    Why: AIRCRAFT CRASHED UNDER UNKNOWN CIRCUMSTANCES, THERE WERE TWO PERSONS ON BOARD, ONE WAS FATALLY INJURED, ONE UNKNOWN INJURIES, WRECKAGE LOCATED ON WAIALAEIKI RIDGE, 10 MILES FROM HONOLULU, HI

    The pilot had been flying to his veterinary clinic and made no mayday call prior to the crash. Visibility may have been a factor as the weather was overcast.

    Crash crews rapelled from the helicopter due to the inaccessibility of the crash site.
