    NTSB Issues 9 New Safety Recommendations

    NTSB Issues Nine New Safety Recommendations as a Result of Its Investigation of the 8/26/2011 Crash of a Eurocopter AS350 B2 Near Mosby, Missouri

    May 6, 2013 The National Transportation Safety Board Issues the Following Recommendations to the Following Organizations:

    • Prohibit flight crewmembers in 14 Code of Federal Regulations Part 135 and 91 subpart K operations from using a portable electronic device for nonoperational use while at their duty station on the flight deck while the aircraft is being operated. (A-13-007)
    • Require all 14 Code of Federal Regulations Part 121, 135, and 91 subpart K operators to incorporate into their initial and recurrent pilot training programs information on the detrimental effects that distraction due to the nonoperational use of portable electronic devices can have on performance of safety-critical ground and flight operations. (A-13-008)
    • Require all 14 Code of Federal Regulations Part 121, 135, and 91 subpart K operators to review their respective general operations manuals to ensure that procedures are in place that prohibit the nonoperational use of portable electronic devices by operational personnel while in flight and during safety-critical preparatory and planning activities on the ground in advance of flight.     • Inform pilots of helicopters with low inertia rotor systems about the circumstances of this accident, particularly emphasizing the findings of the simulator flight evaluations, and advise them of the importance of simultaneously applying aft cyclic and down collective to achieve a successful autorotation entry at cruise airspeeds.     • Revise the Helicopter Flying Handbook to include a discussion of the entry phase of autorotations that explains the factors affecting rotor rpm decay and informs pilots that immediate and simultaneous control inputs may be required to enter an autorotation. (A-13-011)
    • Require the installation of a crash-resistant flight recorder system on all newly manufactured turbine-powered, nonexperimental, nonrestricted-category aircraft that are not equipped with a flight data recorder and a cockpit voice recorder and are operating under 14 Code of Federal Regulations Parts 91, 121, or 135. The crash-resistant flight recorder system should record cockpit audio and images with a view of the cockpit environment to include as much of the outside view as possible, and parametric data per aircraft and system installation, all as specified in Technical Standard Order C197, “Information Collection and Monitoring Systems.” (A-13-012)
    • Require all existing turbine-powered, nonexperimental, nonrestricted-category aircraft that are not equipped with a flight data recorder or cockpit voice recorder and are operating under 14 Code of Federal Regulations Parts 91, 121, or 135 to be retrofitted with a crash-resistant flight recorder system. The crash-resistant flight recorder system should record cockpit audio and images with a view of the cockpit environment to include as much of the outside view as possible, and parametric data per aircraft and system installation, all as specified in Technical Standard Order C197, “Information Collection and Monitoring Systems.” (A-13-013)

      To Air Methods Corporation:

    • Expand your policy on portable electronic devices to prohibit their nonoperational use during safety-critical ground activities, such as flight planning and preflight inspection, as well as in flight. (A-13-014)
    • Revise company procedures so that pilots are no longer solely responsible for nonroutine operational decisions but are required to consult with the Air Methods Operational Control Center for approval to accept or continue a mission when confronted with elevated risk situations, such as fuel-related issues and unplanned deviations.   • AFA-CWA Files Interference Charges Against Delta Air Lines Management

    WASHINGTON, Nov. 23, 2010 /PRNewswire-USNewswire/ — The Association of Flight Attendants-CWA (AFA-CWA) today filed formal interference charges with the National Mediation Board (NMB) against Delta Air Lines management alleging unlawful conduct during the recent flight attendant representation election.

    Delta management compromised the secrecy of the ballot by urging flight attendants to vote on company-controlled work computers that could track whether they clicked on the NMB’s internet voting site. In addition, hundreds of Delta/Northwest flight attendants have reported coercive and unfair methods used by management to influence the results of the recent union election.

    Feedback includes reports of the company’s anti-AFA literature, excessive supervisor surveillance and meddling, and repeated supervisor phone calls to flight attendant homes telling them to vote.
    “Delta and Northwest flight attendants deserve the opportunity to freely participate in an election without being intimidated by management and heavy-handed efforts to keep them from gaining a voice,” said Patricia Friend, AFA-CWA International President. “Delta management launched the largest anti-union campaign in history and spared no expense in attempting to destroy the collective bargaining rights that Northwest flight attendants have worked to maintain for over 60 years. We now call on the NMB to conduct an exhaustive investigation of these charges and hold Delta executives accountable for their actions.”

    If the NMB finds sufficient evidence that election interference occurred, it can order a new election that will allow flight attendants to decide representation in a free and fair manner. On November 3, the NMB counted 9,216 votes in favor of union representation for Delta flight attendants and 9,544 votes against.

  • NASA HOSTS VIRGINIA SCHOLARS DURING THREE WEEKLONG ACADEMIES

    HAMPTON, Va. – After spending six months learning and communicating via the web, more than 140 rising seniors from across Virginia will finish an intensive distance-learning program by attending weeklong summer academies onsite at NASA Langley Research Center.

    The Virginia Aerospace Science and Technology Scholars (VASTS) program is an interactive, on-line science, technology, engineering and mathematics learning experience that offers selected students workforce experience as well as college credit. This year is the program’s third year and because of its success a third weeklong academy was added to the schedule this year to allow more students to participate. The academies will be held June 26-July 2, July 10-16 and July 24-30.

    While at NASA, scholars will use what they learned on-line by designing and implementing plans for a human journey to Mars. They will also tour various facilities, build and test robots and talk with NASA scientists, engineers and technologists.

    The Virginia Aerospace Science and Technology Scholars program is modeled after a distance-learning program designed at NASA’s Johnson Space Center in Texas. The Virginia program is managed by the Virginia Space Grant Consortium.

    Airworthiness Directives: McDonnell Douglas Corporation Model MD-11 and MD-11F Airplanes

    SUMMARY: FAA is adopting a new airworthiness directive (AD) for certain
    Model MD-11 and MD-11F airplanes. This AD requires a one-time
    inspection to determine if wires touch the upper surface of the center
    upper auxiliary fuel tank and marking the location, if necessary; a
    one-time inspection of all wire bundles above the center upper
    auxiliary fuel tank for splices and damage; a one-time inspection for
    damage to the fuel vapor barrier seal and upper surface of the center
    upper auxiliary fuel tank; and corrective actions, if necessary. This
    AD also requires installation of nonmetallic barrier/shield sleeving,
    new clamps, new attaching hardware, and a new extruded channel. This AD
    results from fuel system reviews conducted by the manufacturer. We are
    issuing this AD to reduce the potential of ignition sources inside fuel
    tanks, which, in combination with flammable fuel vapors, could result
    in fuel tank explosions and consequent loss of the airplane.

    Boeing Fabricará 3 Satélites para el Gobierno de México

    EL SEGUNDO, California, 20 de diciembre de 2010 — Boeing [NYSE: BA] anunció hoy la adjudicación de un contrato por aproximadamente US$1,000 millones de parte del gobierno mexicano para la entrega de un sistema integral de comunicaciones satelitales. El sistema, denominado MEXSAT, consta de tres satélites, dos estaciones terrenas, los sistemas de operaciones de red correspondientes y terminales de usuario de referencia. MEXSAT proveerá comunicaciones seguras para las necesidades de seguridad nacional de México, así como una cobertura mejorada para las telecomunicaciones civiles.

    “MEXSAT es la cuarta generación de satélites que Boeing ha suministrado a México para comunicaciones satelitales gubernamentales y civiles,” dijo Craig Cooning, director general ejecutivo (CEO) de Boeing Satellite Systems International. “MEXSAT ha sido desarrollado con base en 13 años de experiencia de parte de Boeing en el diseño y la fabricación de avanzados sistemas de satélites geoestacionarios para servicios móviles.”

    Conforme a los términos del contrato, Boeing entregará un sistema llave en mano completo que se compone de dos satélites geoestacionarios para servicios móviles (o “GEO-Mobile”) Boeing 702HP denominados MEXSAT 1 y MEXSAT 2, así como un satélite para banda C extendida y banda Ku, llamado MEXSAT 3, que proveerá servicios satelitales fijos (FSS) desde una órbita geosíncrona. El primer lanzamiento programado es el de MEXSAT 3, a finales de 2012.

    Cada satélite Boeing suministrará 14 kilowatts de potencia mediante alas de arreglos de cinco paneles solares dotadas de celdas fotovoltaicas de arseniuro de galio de triple unión de alta eficiencia. Cada una de las naves transportará un reflector de banda L de 22 metros para servicios satelitales móviles (MSS), complementado por una antena de banda Ku de 2 metros.
    Boeing subcontratará la fabricación del satélite MEXSAT 3 y de un centro de operaciones a su empresa asociada Orbital Sciences Corporation [NYSE: ORB]. MEXSAT 3, un satélite Orbital Star 2.4, brindará cobertura total al territorio mexicano y sus aguas patrimoniales, y transmitirá comunicaciones civiles a fin de coadyuvar al desarrollo socioeconómico del país.

    Asimismo, Boeing desarrollará dos estaciones terrenas equipadas con avanzada tecnología de flexibilidad de conformación del haz para dirigir haces puntuales de usuario móvil a dependencias gubernamentales que operan en México y sus aguas patrimoniales, tanto en el litoral del Pacífico como en el Golfo de México.

    Con anterioridad, Boeing suministró cinco satélites para el gobierno mexicano: los satélites Morelos 1 y Morelos 2, ambos puestos en órbita en 1985; los satélites Solidaridad 1 y Solidaridad 2, lanzados en 1993 y 1994; y el Satmex 5, lanzado en 1998. Solidaridad 2, que ya ha rebasado la vida útil estipulada en el contrato, aún continúa en servicio, y se espera que el satélite Satmex 5 deje de operar en 2012.
