Experimental Crash in Brazil kills Deputy Mayor


A small plane, an INPAER Explorer#PR-ZAL with three aboard was en route from Campos dos Amarais, Campinas to London but disappeared from radar while talking to ATC in Pirassununga Brazil at about ten on January 23rd. The experimental plane was registered but not certified, and barred from flying over populated areas.

The Brazilian Air Force, Botucatu and São Manuel police forces were searching with a C-105 Amazonas, a H1 and an Eagle Helicopter, and found the remains of the passengers in dense forest, in a rugged area near Monte Alegre.

The Squad of Campo Grande rescue team is on the scene.

Locals who heard the failing engine, a roar and saw a flash of light in the woods notified authorities.

The remains have been identified as:
-Alderman and Deputy Mayor of Suzano (SP) Jessé Almeida,
-Edson Geraldino, identified as owner of the plane.

The identity of the third party, Rubens Geraldino, has not been confirmed by the family. The passengers (uncle and nephew) were from Mogi das Cruzes, SP, Brazil.

Sikorsky Down off Norfolk, Va Beach


A US Navy Sikorsky MH-53E Sea Dragon crashed off a Norfolk Virginia beach, killing one of five aboard. The helicopter was assigned to Helicopter Mine Countermeasures Squadron Fourteen.

Three were injured and hospitalized at at Sentara Norfolk General Hospital. A rescue team is searching for one crewman who is still missing.

Billionaire Lost in French Helicopter Crash

Chinese billionaire Lam Kok purchased a French vineyard, and boarded a helicopter with three others, including his son, the former owner who was flying the helicopter, and a translator, to take an aerial tour of the 65-hectare (160-acre) vineyards and surrounding grounds. Lam Kok’s and James Gregoire’s wives had declined to fly.

Twenty minutes after the flight took off, the helicopter went missing.

The helicopter, flown by James Gregoire, former owner of the vineyard, was missing but a witness came forward and told a mounted policeman that the helicopter had been seen falling into the Dordogne river, and people had been seen struggling in the water.

Emergency helicopters, divers and a hundred foot officers converged on the scene, and were deployed to search. After dark, a helicopter with a searchlight joined the searchers.

Lam Kok, new owner of the vineyard Château de la Riviere was head of the Hong Kong-based Brilliant group which plans to develop a tea and wine tasting center and hotel in Bordeaux.

One of the bodies was recovered around midnight. Rumors are that the body is of Lam Kok’s twelve year old son. The search was halted overnight, but resumed on the morning of the twenty-first. The search has been hampered by strong currents in the river.

Ironically, John Leprince, a former owner of the vineyard, died in a plane crash in 2002, not far from the crash site in the Dordogne. Combined with this crash, this means the last three owners of the vineyard were lost in aviation accidents.

Plane Missing in Clayoquot Sound

A Hutch Air Cessna 421B Golden Eagle en route from Abbotsford International Airport to Tofino BC had one passenger and one pilot aboard when it went missing on December 14, 2013, at 14:25 local time.

The twin engine plane was expected at 2:50 pm. It was over Vargas Island on radar when it disappeared at the densely wooded island in Clayoquot Sound. A search of the west coast of Vacouver island by the Joint Rescue Co-ordination Centre Victoria has been implemented but has been hampered by fog, rain, low cloud cover and darkness over Tofino. Two search and rescue helicopters pulled out Saturday because of no visibility, and are continuing the search today. A crew is also on the island searching on foot.

JRCC Victoria is located at Canadian Forces Base Esquimalt and is responsible for planning, co-ordinating, controlling and conducting aeronautical and maritime search and rescue (SAR) operations within Victoria’s Search and Rescue Region (SRR).

DNA tests will be performed on the 31 bodies so far recovered from the Namibia’s Bwabwata National Park, site of the crash of Mozambique Airlines Flight 470, a Embraer ERJ 190-100 IGW (ERJ-190AR) with 33 aboard. The plane is a total wreck, Written off (damaged beyond repair), burnt wreckage scattered in a low impact crash, possibly the result of bad weather.

The hundred-seat plane was only 1 / 3 full.

  • Only one body was found intact according to Theo Shilongo, investigation leader.
  • The pilot was in Botswanan airspace when the pilot notified ATC of problems, though it took Mozambiqan authorities 3 hours to inform Nambia that the plane was off radar.
  • The search was called off due to bad weather, but the next day locals called in reporting the crash.
  • The wreckage will be transported to Rundu for investigation.
  • The remains have been transported to Windhoek.

Nashville: Cessna Crash


On October 29, 2013, a Windsor Flying Club Cessna 172R Skyhawk crash landed at Nashville International Airport, killing the pilot. There was dense fog early Tuesday morning when the crash occurred, and the airport was closed. The crash occurred some time between 2:00 a.m. (the final sweep of the runways) and its discovery at 8:45 a.m. by a taxiing jet pilot.

The plane is one of three belonging to Canada’s Windsor Flying Club school fleet.

National Transportation Safety Board spokesman Peter Knudson said that the plane sat on the runway for hours before it was discovered Tuesday morning. The crash apparently occurred Monday night. Metro Police, Metro Fire and Metro OEM, the medical examiner, and an accident investigator from National Transportation Safety Board responded belatedly. The accident occurred on Runway 2, which was closed on discovery of the wreckage. Operations at the rest of the airport continued as usual.

The FAA is on the scene. The pilot was in his mid forties, was night-rated but not instrument-rated (i.e. he was not cleared to fly in low visibility like the dense fog that blanketed Nashville.) His name has not been released. He rented the plane from Monday 8.pm. to Tuesday noon.

The investigation has not yet determined if the pilot made the required contact with ATC. The plane as no flight data recorder but it does have gps and current instrumentation.

Nashville Airport Statement

As reported yesterday by Metropolitan Nashville Airport Authority (MNAA), a small single-engine Cessna-172 crashed at Nashville International Airport on Tuesday, October 29, 2013. The Airport Emergency Plan requires MNAA to notify the National Transportation Safety Board in the event of an accident and the NTSB begins an investigation to determine the probable cause of the accident and other facts related to it. This investigation is now underway and the NTSB will issue a preliminary report in 10 days and a final report in 9 – 12 months.

Importantly, the time of the crash has not been determined and reports to the contrary are not factual. These facts will be determined as part of the NTSB investigation.

Following is a statement from MNAA:

“The MNAA’s top priority is the safety and security of all operations at the airport. As an FAA certificated airport, MNAA is required to comply with the FAA’s safety standards. MNAA has long maintained a strong record of safety and follows stringent federal safety regulations. We are inspected annually by FAA safety inspectors to ensure compliance. We uphold these regulations every day, including yesterday. We have inspectors and crash, fire and rescue personnel on duty 24 hours per day, 365 days per year to respond in the event of any emergency.

“MNAA is working collaboratively with the FAA and NTSB as they investigate the accident and determine the facts. It would be premature to comment about any facts involving this accident until the NTSB completes its investigation. All of us at the airport express heartfelt condolences to the family of the pilot involved in this accident.”

Yellow Wings Crash in Kenya


On July 25, 2013, a Yellow Wings Air Services Ltd Cessna U206F Stationair Cessna type 206, registration number 5Y BUG en route from Nanyuk to Wilson Airport disappaeared from radar around 5 pm in the Aberdares Range, Nairobi Kenya.

The plane went missing Thursday, was found Saturday. Bodies were said to be recovered Sunday.

The three missing swiss, a family of father, mother and daughter, were found beside the wreckage by a Kenya Wildlife Services helicopter.

The plane struck the side of a mountain in the Aberdares mountains.

On Saturday, not to be confuse with this accident, a plane with one occupant crashed in Tsavo National Park.

Thompson Falls Crash Under Investigation

A helicopter that went down in the Prospect Creek drainage outside of Thompson Falls ten miles up Prospect Creek road on July 27, 2013, suffered one fatality and two injured who were transported to the hospital.

Two Bear Air 2 assisted in finding the downed helicopter.

Search and Rescue, Forest Service and fire personnel responded to the scene. An air evacuation helicopter call was cancelled.

Peru training crash


A Peruvian National Police Bell UH-1H with three aboard was en route from Tangier Island airport 8:05 p.m. when it crashed over an area of coca cultivation during night training.

The wreckage was located near Tingo Maria Peru on La Moyuna Mountain.

Base control indicated that before communication broke off at 8:00 pm, the helicopter was in distress.

The helicopter worked the area of ??Leoncio Prado and Alto Huallaga (Tingo María) under drug control in cooperation with the United States.

The three crew, two officers and a sergeant of the Peruvian police members were retrieved and taken to the air base in Tingo Maria. Search teams located the helicopter five minutes from a local police base.

June 30, Yemenia Flight 626

We remember on June 2009 when 152 people died aboard a Yemenia Airlines Airbus A310-300.

The years pass and the headlines fade, but the families will never forget the loss of their loved ones, and neither should we.

The pilot was about to land on an airstrip in the Comores Islands when ATC lost contact.

The plane and all its passengers crashed into the ocean.

12-year old Bahia Bakari survived the crash, rescued after thirteen hours in the ocean, clinging to debris. She was and is a miracle.

The final report on Flight 626 concluded the crash was due to the crew’s inappropriate flight control inputs that led to an aerodynamic stall.

Crews inappropriate flight control inputs means pilot error. In this case, the pilot error was exacerbated by a nineteen year old plane in dubious condition, bad weather conditions (winds gusting to 64 km/h 40 mph; 35 kn) and a primitive airport.

Crashes like Yemenia should function like red lights. The only way they make sense is if when they happen, we stop and consider those who were lost, and investigate what went wrong. On behalf of safety, the aviation industry should mandate to use every atom of what is discovered to prevent future accidents. The industry in general, and the airlines in specific should make changes that will help prevent future accidents.

Pilot error means airlines like Yemenia are still having problems training pilots. Still problems in the cockpit. Sadly, conditions still exist for more accidents like this in Yemenia’s future.

Netherlands Helicopter Crash at Sea

On May 8, 2013, Cessna 172P was on a photo flight over the North Sea when it crashed at sea in foggy weather near the town of Egmond, Netherlands. Around 9:00 pm, the pilot had sent a distress signal.

A military helicopter and an offshore helicopter responded to the alarm. There was dense fog with visibility of 50 meters.

The pilot and passenger were rescued from water at 10 degrees Celsius. The pilot is a 71-year-old man from the North Holland Sint Maartensvlotbrug, and was unconscious when he was picked up and transported to hospital in Den Helder. The pilot later died of hypothermia.

The 48 year old passenger was picked up from Bussum by boat and admitted to an Amsterdam hospital.

Helicopter Goes Missing in Siberia


On May 6, 2013, 9 people aboard an Angara Airlines Mil Mi-8T were lost during a rescue operation in Siberia.

Fragments of missing Mi-8 helicopter found in Irkutsk

The helicopter was participating in a flood rescue and was carrying 1.9 tons of explosives for blasting ice dams in the river.

Missing are three crew, five officials and the emergency chief. Stanislav Omelyanchik.

Air France Flight 447


Air France 447 went down over the Atlantic in 2009.

The fly-by-wire A330 incorporates technology that prevents the airplane from entering a stall, but during a complete loss of airspeed information, however, the system reverted to manual control.

The final report said said the pilots were “completely surprised” by technical problems experienced at high altitude and engaged in increasingly de-structured actions until suffering “the total loss of cognitive control of the situation.”

CEO of the Flight Safety Foundation has been recorded said pilots generally manually manipulating the controls for only three minutes:one minute and 30 seconds each for take-off and landing.

“We are moving towards automated operations where the pilot isn’t even permitted to fly. That means the first time in your career you will ever feel what an aircraft feels like at 35,000 feet is when it’s handed to you broken.”

Ten Lost in Fretax Taxi Aéreo Crash

On Mar 12, 2013, a Fretax Taxi Aéreo single engine Embraer EMB-820C Carajá (PA-31) disappeared en route from Belem to Sierra Areão Airport. The flight crashed twenty miles from the airport in Almeirim Brazil near the border with the state of Amapá.

Seripa (Regional Investigation and Prevention of Aeronautical Accidents) discovered the wreckage after the plane went missing late Tuesday in the vicinity of Monte Dourado. Air Traffic controllers reported the flight missing at 22:00 hours and contacted SALVAERO Service Air Search and Rescue.

There were ten people on the flight to the Santo Antonio do Jari hydroelectric power plant, including the pilot, and ten employees of Cesbe Engineering. The crew was en route to the Santo Antonio do Jari hydroelectric dam/power plant building project as part of the construction crew.

A FAB (Força Aérea Brasileira) investigation is underway.

Russian Chopper Crew Lost in Peacekeeping Mission

UN Russian Mi-8 helicopter crashed at an altitude of 2,700 meters in the Democratic Republic of Congo killing four.

The UTair helicopter was returning to Bukavu when it crashed 35 miles from its destination.

The four crew members were Russian. The crew was en route between Shabunda and Bukavu when it hit bad weather.

The crash occurred in mountainous terrain that is inaccessible, but it has been located. Forty rescuers are en route on foot. The helicopter was part of peacemaking mission MONUSCO.

Kenn Borek Air Update

The flight that went missing on January 23, 2013, the Kenn Borek Air de Havilland DHC-6 Twin Otter en route from the South Pole to Terra Nova Bay has been found.

Wreckage of the plane was found after it impacted terrain half way between South Pole and McMurdo Station.

The two helicopters that reached the crash site on Jan 26th could not land. They surveyed the area.

Rescue Coordination Centre New Zealand said that
The site of the crash is at a height of 3,900 metres (13,000 feet) at the northern end of the Queen Alexandra Range. The aircraft wreckage is on a very steep slope, close to the summit of Mount Elizabeth. It appears to have made a direct impact that was not survivable.

PRESS RELEASE – KENN BOREK AIR LTD. – JANUARY 25, 2013

Friday evening Calgary time, a C130 Hercules aircraft of the New York Air National Guard made visual contact with the overdue aircraft in Antarctica. The sighting was confirmed approximately thirty minutes later by a Kenn Borek Air Ltd. Twin Otter aircraft deployed in a search and rescue (SAR) role. The crew of the SAR Twin Otter reports that the overdue aircraft impacted a steep snow and ice covered mountain slope. No signs of activity are evident in the area surrounding the site, and it appears that the impact was not survivable.

Due to the terrain and ongoing weather conditions, the SAR Twin Otter was unable to land near the site. Subject to favourable weather conditions, helicopter crews and mountain rescue personnel will attempt to access the accident site Saturday morning Calgary local time.

Missing Coyote Found, Pilot Safe

On January 10, 2013, a homebuilt Rans S4 Coyote went missing. A search was underway for the plane which is not equipped for night flight. Only the pilot, ERIC P HEINZ was aboard.

The missing plane was located when the pilot’s campsite was discovered, at 9:30 after it had crashed in Lake Pleasant Arizona.

The experimental plane sustained damage. The pilot survived.

French Pilot Crashes in Vienne

Why: The pilot of a SkyRanger failed to return from a 2 hour flight, triggering a Sater Bravo land-air rescue.

A search went on till 2:00 a.m. The next day his remains were discovered near an electrical cable in a wooded area, ten miles east of the airport. The plane was severely damaged.

Six Missing in Australia

Why: Six people who were aboard a vintage plane were returning to Caboolture after spending the day raising money for charity by hosting joy flights at the Monto Fly-In in central Queensland have been missing in Australia since Monday:

The AMSA (Australian Maritime Safety Authority) received a distress call from Mr Porter saying he was having trouble “fixing his position in heavy cloudcover.” That was the last contact with the plane.

3 Canberra search and rescue co-ordinators flew in assisting AMSA.

Sixteen helicopters and two planes are participating in the search.

Cessna Crash in Tyrol, 6 fatalities, 2 survivors

Why: A Cessna that crashed in in Tyrol Innsbruck-Land killed six. Two aboard survived the crash. The pilot made an emergency call before losing contact with ATC.

The passengers were Tyrolean, and took off at 6:50 for Valencia. An area hunter heard the plane before it impacted.Two of those aboard extricated themselves from the plane. One was found burned in the forest, another was ambulatory, there to greet the rescue crew around 7:42. One survivor had minor injuries, and the other required surgery.

Before the crash, the flight lost contact. After the helicopter rescue was called off due to heavy fog and low lying clouds, rescuers arrived on foot, thirty minutes after the first alarm. The rescue was coordinated by 70 Red Cross, 100 firemen, 20 local police and 12 crime scene investigators (detectives). Offsite, family members gathered in a building near Innsbruck State Police station were taken care of by Red Cross.

The pilot who was from Saalfelden, Salzburg was killed in the crash. Two of the victims were identified as hoteliers Zellberg and Mayrhofen.

The remains were taken to Innsburck. The plane was charred, and left a 100 meter trail in the forest. A wing was found on the slope of the forest.

Antonov Impacts in Russia, 4 Survivors

What: Local carrier FGUPKAP twin-engine Antonov An-28 en route from Petropavlovsk-Kamchatsky Yelizovo to Palana
Where: Palana village, Russia

Who: 12 passengers, 2 crew, ten fatalities
Why: The plane was scheduled to land in Palana village, but lost contact with ATC and crashed in a cedar forest on a hill 10 kilometers away from the intended landing site. A thirteen year old boy who survived is now comatose. The three other survivors are in critical condition.

A recovery helicopter landed as close as possible to the wreckage; but rescue personnel had to navigate through rough topography to reach the wounded.

Originally nine bodies were found, and five injured people, but one child died of injuries.

FGUPKAP, the carrier is owned by the state. Minister of Health Tatiana Lemeshko Kamchatka participated in a conference regarding the crash, and reported on the status of the 4 surviving passengers. The ministry is sending September rescue personnel and doctors to Palana on September 13.

Weather conditions, human error or technical malfunction may be the cause. Officials announced that the families of the fatalities will be getting 200,000 rubles in compensation.

Why: After departing from Songshan Airport at 7:25, the plane with two pilots and a photographer (pilot Hsuey Chen-hao, co-pilot Chang Ming-chin and aerial photographer Chien Yu-hsin) was on an aerial photography flight conducting a survey. At 7:44 a.m., the pilot requested permission to switch from instrumental to visual flight. After 9:14 a.m there was no more contact with the plane, although it is supposed to check in every 15 minutes.

Around 10:15, the National Rescue Command Center informed ATC that an ELT signal had been detected. At the time, visibility was up to 10 kilometers.

A rescue chopper spotted the wreckage in the mountains at 1600 meters. Weather conditions forced the chopper to return.

The 20 year old plane had an airworthiness certificate renewed last month.

On 28 July 2010 after taking off from Karachi at 0241 UTC (0741 PST)on a domestic flight to Islamabad, Airblue Flight 202 (AP-BJB), an Airbus A321, executed a circling approach for RWY-12 at Islamabad, crashed into the Margalla Hills nine and a half miles north-east of Islamabad apparently due to bad weather. One hundred and forty-six people were aboard, and six crew. One hundred forty-six passengers and all six crew members died. The plane was a total loss. It is the first fatal accident involving an Airbus A321 and Pakistan’s worst air disaster.
