    Panel From KLM Plane Falls Onto Highway in Osaka

    A KLM plane dropped a fairing panel on a highway in Osaka, Japan, on September 23rd.

    The incident happened when the Boeing 777-200 plane was performing flight KL-868 from Osaka, Japan, to Amsterdam, Netherlands.

    The panel hit a car on the Keihan National Highway. There were two people in the car at the time; none of them were injured.

    The plane continued to Amsterdam.

    The incident is under investigation.

    LAX Turbulence Injures Flight Attendant

    What: Southwest Airlines Boeing 737-700 en route from San Francisco to Los Angeles
    Where: Los Angeles
    When: Sep 29th 2010
    Who: flight attendant
    Why: On approach to LA, the Boeing encountered turbulence. A flight attendant sustained minor injuries.

    Pilots Ignore ATC Directions, Hear Audio

    On March 11, 2013, an Air Canada Embraer ERJ-190 en route from Edmonton to Toronto,ON was on approach when ATC informed the pilots to abort the landing. Ground radar indicated something moving on the runway. Pilots continued to make the landing, and ignored ATC.

    Mechanics working on a Sunwing Boeing had left a van running and in gear, which subsequently rolled without a driver across the runway. At some point, the van impacted the Sunwing 737.

    While we can’t make assumptions, apparently the pilots saw the van safely flew over it and made a secure landing.

    However, there are a lot of errors here that could have been disaster. We are glad no one was injured. Safety first, everyone!

    • The maintenance crew for failing to secure their vehicle.
    • ATC for not using the call sign, even if it was evident to them who they were speaking to.
    • Pilots for ignoring ATC even if they saw the “threat” because there could have been an additional alert

    That said, of course we are glad no one was injured.

    Oust Paul Morell says USAPA

    Vice President of Safety and Regulatory Compliance Paul Morell needs to be ousted for lack of leadership, and ignoring aviation safety concerns of US Airways pilots, according to the pilot reps of the US Airline Pilots Association. Seventeen points have been cited as examples of his dereliction and lapses in safety protocols.

    An October pilot survey pointed out pilot concerns that:

    • “The push for on-time departures may hinder safety.”
    • “unsatisfactory response” to reported safety issues”
    • little pilot input on the airline safety decisions
    • Safety personnel perceived as “out of touch with the risk of flight operations

    Congonhas Radio Interference

    What: TAM Linhas Aereas Airbus A320-200 en route from Victoria to Sao Paulo Congonhas
    Where: Campinas
    When: May 27th 2009
    Who: 100 passengers
    Why: On approach to Congonhas, the plane’s communication with air traffic control was interrupted. Because of the interference, the plane diverted to Campinas where it made a safe landing.

    Brazil has a history of problems with reception in areas of the Amazon; but in this case, in Sao Paulo, the radio problems caused several flights to be diverted.

    U.S. Marine Corps Planes Collide Mid-Air Off Japan; 1 Dead, 5 Missing

    Two U.S. Marine Corps planes collided mid-air and crashed off the coast of Japan, on December 5th.

    Authorities said the incident happened when the planes were involved in an aerial refueling as part of a routine training.

    At least one Marine was killed in the crash while another was rescued. Five others remain missing.
