Historically, about two-thirds of all general aviation (GA) accidents that occur in reduced-visibility weather conditions are fatal.1 These accidents typically involve pilot spatial disorientation or controlled flight into terrain.
Even in visual weather conditions, flights at night over areas with limited ground lighting (which provides few visual ground references) can be challenging.

Knives to Fly Planes On April 25

Knives are back and flight attendants aren’t happy.The TSA ban on knives is due to be lifted on April 25. The blade can be no longer than 2.36 inches. The coalition of Flight Attendant Unions lobbied in Washington, D.C., Los Angeles, and Norfolk, Va, Chicago, Denver, Miami, New York LaGuardia, Phoenix, Pittsburgh, Seattle, Philadelphia, San Francisco and Boston against the ban. The arguments are the same as those for and against guns. (If no one carries them, we’re safe.

The family of flight attendant Sara Low, who died on Sept 11, has written an open letter protesting the decision to allow knives back on commercial aircraft, and urging concern for safety and “doing no harm.” Pilots’ union Denounces Five German AIrports
According to Cockpit, Germany’s pilot union, some airports have “serious safety defects.”
The union examined German airports comparing them to a strict set of standards, and came to the conclusion that Weeze, Memmingen, Lübeck, Zweibrücken, and Friedrichshafen failed safety standards on the basis of problems like bad lighting, inadequate taxi routs, etc…
The better situated airports included Berlin Schönefeld, Düsseldorf, Leipzig/Halle, Munich, and Stuttgart.

NTSB PROVIDES UPDATE ON BOEING 787 BATTERY FIRE INVESTIGATION


WASHINGTON – The investigative work into the Jan. 7, 2013, fire aboard a Japan Airlines Boeing 787 at Logan International Airport in Boston, is estimated to be completed by the end of March, the National Transportation Safety Board said today. The analytical and report writing phase of the investigation will follow the completion of the investigative activities. The final report is expected to be presented to the Board at a public meeting in Washington in the fall.Members of the investigative team have been conducting work in the United States, Japan, France, and Taiwan. As the investigation has progressed, the NTSB has been working closely with Boeing, the Federal Aviation Administration, the Japan Transport Safety Bureau, the French BEA, and technical advisors from Japan and France.
Some of the investigative activities include:
– Completed disassembly and documentation of the individual cells of the incident battery.
– Completed examinations of exemplar batteries and battery cells for baseline reference and comparison to the incident battery. These examinations were conducted at NTSB and independent laboratories and included computed tomography scans, non-destructive soft short testing, and destructive evaluation and analysis of the batteries and cells.
– Awarded a contract to Underwriter’s Laboratories to assist the NTSB in defining and performing system-level tests of the Boeing 787 battery and charging system. The testing includes characterization of the thermal and electrochemical properties of the battery and oscillatory testing and is expected to be completed in February.
– Radiographic studies, which included over 200,000 CT scan images, were conducted to examine and document the internal configuration of individual cells from the incident and exemplar batteries.
– Conducted interviews with FAA, Boeing, Thales, and GS Yuasa personnel to review and document key steps, personnel roles and responsibilities, data and information flow, design artifacts, and approvals in the certification process for the battery and charging system.
– Evaluated and documented the process for the battery system safety assessment, including a review of the supporting tests and analysis performed and the safety analysis standards relevant for lithium-ion batteries.
– Conducted on-site survey of battery manufacturing facility in Japan including a review of design, engineering, and production documentation, as well as manufacturing processes, procedures, and training for personnel involved in the manufacture of the battery.
The date of the Board meeting at which the findings of the investigation will be released, including the probable cause of the battery fire, will be announced later in the year.

WASHINGTON – Tomorrow, National Transportation Safety Board Chairman Deborah A.P. Hersman will provide an update on the NTSB’s investigation of the Jan. 7 fire aboard a Japan Airlines (JAL) Boeing 787 at Logan International Airport in Boston.Chairman Hersman will brief credentialed members of the news media on the progress of the investigation, after which she and senior staff will take questions. Those unable to attend in person can call in to a teleconference line.
Reporters will then be invited to view the materials laboratory where the battery is being examined. Photography will be allowed but no “stand-ups” or taped reporting from the lab will be permitted.
For planning purposes, RSVP is required for those planning to either attend in person or call in.

All eight cells had varying degrees of thermal damage. Six of eight cells have been CT scanned and have been disassembled to expose their electrodes. All electrode windings in the battery are in the process of being photo-documented and are undergoing microscopic examination. In the coming days, the remaining two cells will undergo the same examination. Additional information will be provided tomorrow.

Brazil Regulator Proposes Restrictions

Sept 20, 2007SAO PAULO, Brazil-Brazil’s civil aviation regulator proposed restricting flights at the nation’s busiest airport on Thursday, hoping to improve safety at the site of the nation’s deadliest plane crash.
The rules proposed by the Civil Aviation Authority would restrict destinations to a 1,000-kilometer (620-mile) radius of the Congonhas airport and would bar connecting flights from the airport.
The Defense Ministry will now assess the report, and Defense Minister Nelson Jobim announced separately that authorities will decide within one month the precise location for construction of a third runway at Sao Paulo’s Guarulhos airport.
Guarulhos handled mostly international flights before the July 17 crash at Congonhas, but many domestic flights were transferred there after the crash, prompting officials to revive a long dormant plan for the extra runway.As many as 5,000 families living near Guarulhos may have to be relocated to make way for the runway, depending on the location selected, Brazil’s Agencia Estado news service said.
The proposals grow out of a review of operations at Congonhas following the crash in which a TAM airliner ran off the runway and slammed into a building two months ago, killing 199 people.
Officials have not determined a cause, but many experts have said that Congonhas’ main runway is too short and that the airport _ Brazil’s busiest _ handles far too many flights.
The 1,000-kilometer (620-mile) limit would let airlines fly to key destinations such as Brasilia, Rio de Janeiro and cities in southern Brazil, but not to tourist destinations in the northeast.
TAM Linhas Aeras SA and Gol Linhas Aereas Inteligentes _ Brazil’s two largest airlines _ previously used Congonhas as a major hub, but are now reorganizing operations.
The regulators also proposed limiting the number of passengers aboard flights, and restricting takeoffs and landings to 33 per hour at Congonhas, cutting overall passenger capacity at the airport to 4,700 per hour from 5,100 per hour.
Jobim, whose ministry oversees Brazilian civil aviation, earlier announced plans to for an escape area at the end of the runway and limits on operations in wet conditions. Central Wisconsin Airport disaster drill
2011’s three-day disaster training included a simulator aircraft equipped with three dummies, sound effects from an accident scene and smoke, firefighters putting out a fire and evacuating victims.
The FAA mandates drills every three years to prep airport rescue services and first responders for actual emergencies. Saturday’s drill involved Mosinee Fire Department, Marathon County Sheriff’s Department, Red Cross, Aspirus Wausau Hospital, Ministry Saint Clare’s Hospital, Transportation Security Administration, Salvation Army and other local residents.
