    Helicopter Crashes into Pearl Harbor; 1 Critically Injured

    A Bell 206 helicopter crashed and sunk near the Pearl Harbor Visitor Center in Honolulu, Hawaii, on February 18.

    The private helicopter went down while it was carrying five people, including the pilot and a family of four visiting from Canada.

    All five occupants of the aircraft survived the crash. However, one of them – a 16-year-old boy- was trapped underwater and had to be extricated from his seat. He was taken to hospital in critical condition.

    Authorities believe the helicopter belongs to tour operator Genesis Aviation.

    The FAA and the NTSB are investigating the cause of crash.

    Tsunami Closes Airports

    Sendai Airport in Northern Japan was inundated by a tsunami and remained shut down.
    Narita, which focuses on long international flights, was partially re-opened today.
    Haneda, Tokyo’s other airport, closed for much of the day

    Flights incoming to Tokyo were cancelled due to the quake and tsunami, and some diverted to U.S. Air Force’s Yokota Air

    EVALUATION

    A TSUNAMI HAS BEEN GENERATED THAT COULD CAUSE DAMAGE ALONG
    COASTLINES OF ALL ISLANDS IN THE STATE OF HAWAII. URGENT ACTION
    SHOULD BE TAKEN TO PROTECT LIVES AND PROPERTY.

    A TSUNAMI IS A SERIES OF LONG OCEAN WAVES. EACH INDIVIDUAL WAVE
    CREST CAN LAST 5 TO 15 MINUTES OR MORE AND EXTENSIVELY FLOOD
    COASTAL AREAS. THE DANGER CAN CONTINUE FOR MANY HOURS AFTER THE
    INITIAL WAVE AS SUBSEQUENT WAVES ARRIVE. TSUNAMI WAVE HEIGHTS
    CANNOT BE PREDICTED AND THE FIRST WAVE MAY NOT BE THE LARGEST.
    TSUNAMI WAVES EFFICIENTLY WRAP AROUND ISLANDS. ALL SHORES ARE AT
    RISK NO MATTER WHICH DIRECTION THEY FACE. THE TROUGH OF A TSUNAMI
    WAVE MAY TEMPORARILY EXPOSE THE SEAFLOOR BUT THE AREA WILL

    IATA Statement:Press release

    Swine Influenza

    Geneva – The International Air Transport Association (IATA) is monitoring events concerning the recent cases of ‘swine influenza’ in Mexico and the United States. On 25 April 2009 the World Health Organization (WHO) convened a meeting of its Emergency Committee, and subsequently determined that the situation represented a ‘public health emergency of international concern’ under the terms of the International Health Regulation.

    The World Health Organization has released a statement.

    WHO is not recommending any travel or trade restrictions.

    IATA goes on to say:
    “At the present time, IATA recommends that airlines review their preparedness plans for public health emergency and consider how they may be implemented in the event that the current situation becomes more widespread. No specific additional measures are currently advised. Recommendations will be reviewed in light of WHO evaluation of the evolving situation”

    The Cost of Cheap Air Travel

    Hindsight is teaching Brazil some hard lessons. Some uncomfortable things are coming to light, like how the warning signs have been ignored. There’s a long trail of complaints about Brazilian aviation from the people in the system–(air traffic controllers, pilots, airport employees, etc.) that urgent major improvements were needed went ignored.

    Airline safety relies on government-provided infrastructure. To put it simply, you can not skimp on airports. Things like long runways, grooved surfaces, overruns, planned SAFE approaches. Brazil dragging its feet is what killed nearly 200 people, no matter what the investigation says.

    Airports need standards, and standards need to be enforced. Would the accident have happened on a longer runway that was properly surfaced? The pilots did NOT skid off the end of the runway. When they couldn’t stop, the pilots decided t get airborne again.

    The mechanics of landing

    When landing, a jet gets the main wheels onto the runway, which takes up 1000 or more feet of runway.

    The pilot deploys the speed brake lever, then pulls the throttles into reverse thrust. This takes up runway at the rate of 200 feet per second.

    So what happens when the jet brakes and the pilot realises it is too slick to stop? The pilot must get the engines back max power–was there adequate time and space for the Airbus to do this? Obviously not.

    Are we following in Brazil’s footsteps?

    The FAA is asking Congress to pawn off federal funding responsibilities for a national system through “user fees” (as our current FAA administrator has asked Congress to do).

    Our amazing safety record comes from not skimping, not searching for cost-cutting measures. Will we continue to set the world safety standard or not? Only time will tell.

    SkyWest Airlines Plane Returns to Tucson International Airport

    A United airlines flight had to return and make an emergency landing at Tucson International Airport in Arizona, on February 7th.

    The plane, operated by SkyWest Airlines, took off for Los Angeles, California, but had to return shortly afterwards after the cabin filled with smoke.

    The plane landed uneventfully. All 73 passengers and 4 crew members were safely evacuated.

    The FAA is investigating.

    Press Release – FAA Proposes Comprehensive New Helicopter Safety Rules

    WASHINGTON–The U.S. Department of Transportation’s Federal Aviation Administration (FAA) today proposed broad new rules for helicopter operators, including air ambulances, which, if finalized, would require stricter flight rules and procedures, improved communications and training, and additional on-board safety equipment.

    “This is a significant proposal that will improve the safety of many helicopter flights in the United States,” said Transportation Secretary Ray LaHood. “The FAA’s initiatives have helped the helicopter industry make progress on many safety issues, but it’s time to take steps towards mandating these major safety improvements.”

    Under the proposed rules, operators would use the latest on-board technology and equipment to avoid terrain and obstacles. The proposal also contains provisions which, if finalized, would require operators to use enhanced procedures for flying in challenging weather, at night, and when landing in remote locations.

    “We can prevent accidents by preparing pilots and equipping helicopters for all of the unique flying conditions they encounter,” said FAA Administrator Randy Babbitt. “These new rules are designed to protect passengers, patients, medical personnel, and pilots.”

    The FAA document includes new proposals covering a variety of helicopter operators.

    The proposed rules would require air ambulance operators to:

    • Equip with Helicopter Terrain Awareness and Warning Systems (HTAWS).
    • The proposal seeks comments on requirements for light-weight aircraft recording systems (LARS).
    • Conduct operations under Part 135, including flight crew time limitation and rest requirements, when medical personnel are on board.
    • Establish operations control centers if they are certificate holders with 10 or more helicopter air ambulances.
    • Institute pre-flight risk-analysis programs.
    • Conduct safety briefings for medical personnel.
    • Amend their operational requirements to include Visual Flight Rules (VFR) weather minimums, Instrument Flight Rules (IFR) operations at airports/heliports without weather reporting, procedures for VFR approaches, and VFR flight planning.
    • Ensure their pilots in command hold an instrument rating.

    Under the proposal, all commercial helicopter operators would be required to:

    • Revise IFR alternate airport weather minimums.
    • Demonstrate competency in recovery from inadvertent instrument meteorological conditions.
    • Equip their helicopters with radio altimeters.
    • Change the definition of “extended over-water operation” and require additional equipment for these operations.

    The proposed rules would require all Part 135 aircraft, i.e. helicopter and fixed wing on-demand operators, to:

    • Prepare a load manifest.
    • Transmit a copy of load manifest documentation to their base of operations, in lieu of preparing a duplicate copy.
    • Specify requirements for retaining a copy of the load manifest in the event that the documentation is destroyed in an aircraft accident.

    In addition, the proposal would require Part 91 general aviation helicopter operators to revise the VFR weather minimums.

    Since August 2004, the FAA has promoted initiatives to reduce risk for helicopter air ambulance operations. While accidents did decline in 2005 and 2006, 2008 proved to be the deadliest year on record with six accidents that claimed 24 lives. Overall, from 1992 through 2009, 135 helicopter air ambulance accidents claimed 126 lives. From 1994 through 2008, there were also 75 commercial helicopter accidents (excluding air ambulances) that resulted in 88 fatalities.

    The estimated cost of the proposal in present value for the air ambulance industry is $136 million with a total benefit of $160 million over 10 years. The cost for other commercial operators is $89 million with a total benefit of $115 million over 10 years.
