    Kenya Crash Investigation in Spotlight

    What: Kenya Police Air Wing Eurocopter AS 350B3e Ecureuil
    Where: Ngong Hills, near Nairobi Kenya
    When: June 10, 2012 8:30 a.m.
    Who: 6 aboard, 6 fatalities
    Why: Kenyan Internal Security Minister George Saitoti was killed in a helicopter crash on June 10, 2012. Clatus MacOwenga had been appointed in the investigation, and the appointment was backdated to January 5 to fill a legal gap for the period when no chief accidents investigator was in office. MacOwenga apparently was running an investigation separate from that of the President Kibaki’s public inquiry.

    Martin Lunani is now the lead investigator, appointed for a period of three years : August 18 to August 17, 2015. The demotion occurred prior to Mr Macowenga submitting a report. Macowenga will now report to Mr Lunani.

    The end of the investigations is scheduled to be October.

    Small Plane Crashes into Alabama Lake after Hitting Power Lines

    Why: The pilot of the plane is reported to have been en route to visit friends when he struck power lines while attempting to land, and crashed into Lake Weiss. There was one fatality reported. His body was recovered by divers.

    The plane was equipped with pontoons.

    Small Plane Crash Takes Lives of Family, Friend

    What: Cirrus SR22 en route from Kansas City to Springfield
    Where: North of Willard, Missouri
    When: Sept 15, 2012 Midnight
    Who: 5 aboard, 5 fatalities
    Why: The Lambert family plane crashed in a pasture just after midnight Saturday in a field near the intersection of State Highway 123 and Farm Road 81. Victims were reported a family and friend, including John M. Lambert, 44, of Springfield; Grayson Lambert, 16; McKinley Lambert, 15; Joshua Lambert, 10; and Robin E. Melton, 46, of Ozark.

    The family was returning from a Kansas City Royals baseball game when their plane crashed in a farmer’s field, sending out what was described as a “sonic boom” and shooting fireballs into the sky. The plane essentially disintegrated on impact.

    The family in whose farm they landed called 911 just after midnight, but no survivors were found.

    The FAA is investigating.

    Small Plane Misses Neighborhood Debris Flying

    Why: The pilot identified as 51-year-old Stephen Annis of Huntsville, crashed behind a subdivision, spiraling down, missing most of the trees and crashing. An ambulance is responded to the crash, which is located in the 15000 block of Treasure Cove in Bullard. In the adjacent neighborhood, witnesses heard the thud, smelled the fumes and saw flying debris. Investigators speculate he was aiming for the lake, and trying to avoid the neighborhood.

    There was no report of a distress call.

    Antonov Impacts in Russia, 4 Survivors

    What: Local carrier FGUPKAP twin-engine Antonov An-28 en route from Petropavlovsk-Kamchatsky Yelizovo to Palana
    Where: Palana village, Russia
    When: Sept 12, 2012 12:28 p.m.
    Who: 12 passengers, 2 crew, ten fatalities
    Why: The plane was scheduled to land in Palana village, but lost contact with ATC and crashed in a cedar forest on a hill 10 kilometers away from the intended landing site. A thirteen year old boy who survived is now comatose. The three other survivors are in critical condition.

    A recovery helicopter landed as close as possible to the wreckage; but rescue personnel had to navigate through rough topography to reach the wounded.

    Originally nine bodies were found, and five injured people, but one child died of injuries.

    FGUPKAP, the carrier is owned by the state. Minister of Health Tatiana Lemeshko Kamchatka participated in a conference regarding the crash, and reported on the status of the 4 surviving passengers. The ministry is sending September rescue personnel and doctors to Palana on September 13.

    Weather conditions, human error or technical malfunction may be the cause. Officials announced that the families of the fatalities will be getting 200,000 rubles in compensation.

    Plane Crashes Kansasville Garage

    What: Beech A36 Bonanza en route from Chicago Executive Airport to Minocqua
    Where: Eagle lake Kansasville, Racine County, WI

    Why: The pilot who had been en route from Chicago Executive Airport to Minocqua crashed into a Kansasville Wisconsin garage but avoided hitting any of the neighborhood houses in Eagle Lake. the plane, a garage, and two cars were destroyed; and two homes sustained damage. Debris and plane parts were scattered over the immediate area.

    Witnesses say that when the plane hit, it shook the neighborhood.

    The FAA and Racine County Sheriff’s Office are investigating.

    Cessna Crash in Rome Kills 2, Burns 30 cars

    What: Cessna 402B en route from Montichiari to Rome
    Where: Ciampino, Rome, Italy
    When: Sept 7, 2012
    Who: 2 fatalities
    Why: In Rome, a Cessna has crashed in a car depot near Ciampino airport. Thirty cars sequestered by police were damaged.

    The two aboard were killed on impact. They had been intending to land at Urbe airport in Rome but impacted at 167 Via Fioranello. Alfredo Segariol and Antonio Salvoldi, 39, Brescia were in the air performing a photographic surveys for the Rossi company. After impact, the Cessna caught fire; and the fire spread to gut thirty cars at the depot. The plume of smoke was visible across the city.

    It is not yet known if the plane had mechanical trouble or ran out of fuel.

    Winch-launched Glider crashes in UK

    Why: An experienced pilot in his forties in his Winch-launched Glider crashed in UK taking off from the Scottish Gliding Centre, Portmoak Airport. The winch runs on a cable that generate speed so the glider can lift off but it was barely off the ground when it plunged into a farm field in Forfar, Angus. The emergency call came at 1:30pm.

    The crash is under investigation.

    Firefighting Helicopter Crashes in Portugal

    Why: While filling a bucket during a firefighting expedition, a Air Media Company (EMA) chopper in Lagoa de Vale de Figueiras, Portugal went down.

    The two aboard were injured. There were no fatalities. The two firefighters have been hospitalized.

    The helicopter had been engaged in fighting the fire in Ourém which has been raging for 24 yours.

    Robinson Chopper Crashes in Valouse

    What: Robinson R-44 en route from Clermont-Ferrand (Puy-de-Dôme) to Le Luc

    Why: The private helicopter was in France over the Drôme when the blades stopped and it fell. One witness said there was no explosion when it broke up in the air.

    When the accident occurred, a father and teenaged son were aboard, headed to Le Luc where the father ran a construction company.

    Twenty firefighters, fifteen police and helicopter EMS responded to the scene as well as two Air Base Orange helicopters.

    Tragic Crash of Free Club Flight

    Where: Katunino, Russia
    When: Sept 1, 2012
    Who: 4 aboard, 4 fatalities
    Why: At 4.45 pm Moscow time, the light Yak-18 crashed in the Russian village of Katunino. There were four aboard the plane at the time of the crash.

    The plane crashed 300 meters from the runway and caught fire. Firefighters, rescue workers, a team of SMP, the police, the prosecutor’s office were on the scene.

    The club holds trials free flights for everyone. This had been one of those free flights. It was the first time 28-year-old businessman Andrei Ershov had ever flown and according to Andrei’s ex-wife, he had been excited over it, and had persuaded his friend Kolodeshnikov Denis to join him.

    Why: After departing from Songshan Airport at 7:25, the plane with two pilots and a photographer (pilot Hsuey Chen-hao, co-pilot Chang Ming-chin and aerial photographer Chien Yu-hsin) was on an aerial photography flight conducting a survey. At 7:44 a.m., the pilot requested permission to switch from instrumental to visual flight. After 9:14 a.m there was no more contact with the plane, although it is supposed to check in every 15 minutes.

    Around 10:15, the National Rescue Command Center informed ATC that an ELT signal had been detected. At the time, visibility was up to 10 kilometers.

    A rescue chopper spotted the wreckage in the mountains at 1600 meters. Weather conditions forced the chopper to return.

    The 20 year old plane had an airworthiness certificate renewed last month.

    Utah Compassion Flight Crashes, 2 fatalities

    Who: 2 aboard, 2 fatalities
    Why: Returning to Nephi Municipal Airport, the plan crashed on approach at 4 p.m.. The two aboard died on impact. The impact area was in the area of highway 132. Weather may have been involved, as it was storming at the time. After impact, there was a fire.

    The fatalities were identified as Robert Marion Lamb, 45, of Utah County and Peter John Mrowiec, 58, of Ontario, Canada. Either one could have been flying.

    Robert Lamb, a flight instructor, had been aboard while Peter Morwiec was trying to earn five hours of flight training to fly the plane home. Mrowiec was intending to purchase the plane.

    According to Lamb’s wife who asked why he was going, since he wasn’t getting paid and didn’t know [Mrowiec]. he said, “If I was stranded and couldn’t get home, I would hope someone would help me.”

    Why: NAICOM told seminar attendees that the $70,000 per Dana Plane Crash victim payment is contingent on the death certificate and letter of administration, and 70 per cent of Dana Air Crash Claim settlements have been delayed due to families being unable to submit death certificates and letter of administration.

    58 families have been paid $30,000 (the initial 30%) that is due 30 days after the crash. On verification of the correct claimants, the balance of the claims should be covered by Prestige Assurance, six Nigerian insurance companies and Lloyds and Lloyds Associates.

    Chopper Rolls Head over Heels in India

    Why: On landing, the helicopter crashed and rolled. The pilot and one passenger were extricated from the chopper and rushed to the hospital. One of the passengers was Spiritual guru Asaram Bapu who was scheduled to speak at Godhra Science College.

    Missing Plane found in Borneo

    Why: Four passengers who had been on a chartered Cessna have been found on Borneo Island, in Kutai National Park at the wreckage site in a mountainous area. There were no survivors.

    The remains of Australian Peter John Elliott and three Indonesianshave been flown to a Samarinda hospital.

    The flight left Samarinda to survey a coal mining site. Elliot was a specialist in using geophysics equipment, and general manager of Elliott Geophysics International.

    Lake Tahoe: Piper Accident Kills 2 Couples and Child

    What: Piper PA-32-301T Turbo Saratoga
    Where: off Winnemucca Avenue near Barton Hospital, Barton Meadow, South Lake Tahoe, CA
    When: August 25, 2012, 9:45 p.m.
    Who: 5 aboard, 5 fatalities
    Why: The plane took off from Lake Tahoe Airport. No flight plan was filed. Witnesses heard the plane having engine trouble, and saw the plane turn and crash. A witness said he heard something like “a lawnmower that got something caught in the blade. There were two quick cracking noises and then it was over.”The plane impacted in Barton Meadows within El Dorado County limits, and caught on fire. Other witnesses reported seeing the fireball. The wildfire burned about an acre and was contained by 11:10 p.m.. Debris from the crash remained in the field but the numbers on the wing and tail were not identifiable.

    Those aboard were identified as pilot Francisco de la Mora, his wife Lorena, and daughter, and Harold and Kin Cardwell. The plane was registered in the pilot’s name.

    Cessna Crashes in Canadian Cornfield

    The Transportation Safety Board of Canada (TSB) is sending investigators to the site of a crash involving a Cessna 172 that occurred near Mapleton Township, Ont., Friday night.

    OPP say the plane was found in a cornfield and that the four occupants – three men and one woman were found dead.

    Mapleton Township is located north of Kitchener, Ont.

    Why: A 19 year old woman and 3 twenty year olds were in a rental plane sightseeing when it crashed in the middle of a 50 acre corn field. The plane went off radar about nineteen minutes before emergency crews were notified (8:39).

    Witnesses saw the plane making spirals in the air before it pointed nose down, began sputtering and then the “the engine went dead silent and plummeted down. And within a second or two we heard a really loud bump.”

    The wreckage was located at 10:30 pm. The farmer carved out a path for investigators with his tractor.

    The remains have been taken to a Hamilton hospital.

    Solemont Crash May be Lightning Strike, 4 Swiss Killed

    When: August 24, 2012
    Who: 4 aboard, 4 fatalities
    Why: Eye witnesses say that the Pilatus was en route Friday night when it was struck by lightning. Forty minutes after the plane had sent out a distress call, emergency services found the wreck. Debris from the plane is scattered over a wide area.

    Those on the plane included the pilot, aged 57, a woman passenger aged 20, and two men aged 36 and 51. All the remains have not been found and identified, although family members have been informed.

    The accident is under investigation, and no specific cause has been released. Heavy rain was falling at the time of the crash, and there was low visibility. The four aboard the plane were from Switzerland. Late Friday, the Minister of Economy and Finance and managing director of the city of Montbéliard, Pierre Moscovici, expressed his “emotion” and “deep sympathy” towards victims.

    Final Report on Helicopter Crash

    Why: The pilot had picked his passenger to take him to Sydney Adventist Hospital apparently without checking weather reports which resulted in the Bell helicopter crashing in a cliff. Inclement weather and hazardous conditions in Lane Cove National Park led to the decision to call off the recovery operation on July 22, 2012, the same inclement conditions which caused the accident. Businessman Bruce Campbell, 65, and freelance pilot Colin Greenwood died in the crash at the foot of 15m cliff in Lane Cove National Park.

    Balloon Crashes Due to Wind Shear, Pilot lost License

    Who: 30 passengers, 2 crew, 4 fatalities, 28 injured
    Why: A hot air balloon with 32 aboard caught fire and crashed into a tree, burning four beyond recognition. At least six children were injured. Then people at the scene were reported resuscitated; eight in the crash were in critical condition. One passenger was quoted as saying “we were landing, but I think the speed was too high for landing so we hit the ground, bounced off once and hit it again. We held on, but five of us fell out“. Twenty-one were admitted in the hospital Ljubljana, and seven to other hospitals. The injured included one brit and three Italians.

    When the balloon hit the ground and and bounced back in the air, some passengers jumped or fell out, scattering the injured across four locations and making it more difficult for rescuers. Nine teams with ambulances and twelve emergency medical service teams responded as well as 79 firefighters, 20 fire fighting vehicles and three helicopters.

    The balloon takes off and lands in uncontrolled airspace and does not need air traffic control clearance.

    Prime Minister Janez Janša, Minister for Health Tomaz Gantar, Minister of Defense Aleš Hojs and Interior Minister Vinko Gorenak expressed condolences.

    The cause is being attributed to ground level wind shear. The pilot needed to retake a health exam to renew his license in June.

    Five are unidentified.

    The on board extinguishers apparently had cracked cylinders.

    Mombasa Air Safari Crash in Kenya Preserve

    Who: 11 passengers, four fatalities, seven injured
    Why: The Mombasa Air Safari passenger plane crashed on takeoff at the Ngerende Airstrip. Narok district police chief Peterson Maelo said a middle-aged German man, an elderly German woman and two Kenyan pilots died from their injuries and “Those injured have been flown to Nairobi for treatment.” The ambulance service was Flying Doctors Service.

    Five German tourists, four Americans and two nationals from Czech Republic were aboard the flight.

    Cessna Crashes in Village Kindergarten yard

    Where: Annino, Russia
    When: August 22, 2012
    Who: 2 aboard, 2 fatalities
    Why: The light aircraft crashed and fell not far from St. Petersberg. The two pilots, 1st pilot Victor Ignatiev and 2nd pilot Alexei Syrchin died in the crash near the village of Annino.

    The pilots were taking the plane out just prior to a routine certification test. The pilots were attempted to land because they experienced engine problems, but they did not reach the runway and the plane impacted in the yard of a village kindergarten. No children were in the yard at the time of the crash.

    The plane ignited on impact. One pilot was ejected, and one is locked in the cockpit.

    42 people and 20 vehicles were involved in the rescue/recovery effort.

    The plane had undergone repairs in Poland in 2007.
