    Pintail Lodge Helicopter Crashes in Louisiana

    What: Pintail Lodge Inc.Bell 206B JetRanger
    Where: Lyons Point, Crowly, LA
    When: Oct 10, 2102
    Who: 1 fatality
    Why: A helicopter out of Texas was flying low over a Crowly Louisiana field when it made contact with a KSIG radio guidewire.

    The helicopter subsequently crashed in the field. Local residents who heard the impact described a thud.

    The pilot was identified as Hugh Clifton Kelley, 66, of Beaumont.

    Azza Air Plane Goes down in Sudan


    What: Azza Air Transport Antonov An-12BP en route from Khartoum to El Fasher
    Where: 25 miles SW of Omdurman Khartoum,Sudan
    When: Oct 7 2012
    Who: 22 aboard, 15 fatalities
    Why: The pilot of a Sudanese military plane informed the airport that he had a problem with one of his engines before the plane crashed in a rural area west of the capital Khartoum. Twenty armed forces personnel were on board. The Antonov military transport plane was carrying military equipment from Khartoum to the capital of North Darfur.

    Initially thirteen people died on the Azza Air jet, and nine were injured.

    The Sudanese Armed Forces (SAF) are investigating.

    Deliberate 727 Crash in Baja California

    Discovery, Dragonfly Film and Television Productions deliberately crashed a $450,000 unmanned 727 in the Sonoran Desert of Baja California, Mexico south of Mexicali, Mexico. The plane was equipped with crash dummies, scientific sensors and experiments. Among other things such as creating a sensational topic for the Curiosity series, the crash is postulated as a study the crashworthiness of the aircraft’s airframe and cabin.

    Pilots parachuted from the plane minutes before it was remote-controlled to a crash.

    The video called “part of a scientific experiment for an unprecedented international television documentary” was captured by 19 cameras inside the plane and 2 on helicopters outside. The film will be presented as an episode of Curiosity.

    Firefighter Huey Crashes, Pilot Walks away

    What: Columbia Basin Helicopters Bell UH1H Huey
    Where: west of Shady Cove
    When: Oct 7, 2012
    Who: pilot
    Why: The firefighting copter began losing pressure after dropping water on a grass fire, then crashed west of Shady Cove in an area of rough terrain. He was on the way to Grants Pass.

    Pilot Cody Seeger walked away.

    A Brim Aviation helicopter located the wreckage and airlifted the pilot out.

    Investigators had to bulldoze a trail for ATV to reach the scene, but the Huey wreckage will probably be airlifted out.

    Pilot and British Tourist Die in Small Plane Crash in Antigua

    What: FlyMontserrat Britten-Norman Islander en route from Antigua to Montserrat Island
    Where: V.C. Bird International Airport
    When: October 7 2012
    Who: pilot and 3 passengers
    Why: The twin-engine Britten-Norman Islander en route from Antigua to Montserrat Island overran a rainy runway at 4 p.m. and crashed.

    Witnesses heard irregularities in the engine before it “plunged into the end of the runway.”

    The pilot was reported as a fatality. A female British tourist died in the crash, and two other passengers are reported to be hospitalized at at Mount St. John’s Medical Centre.

    Texas Helicopter Crash Investigated by FAA

    What: ROBINSON R-44 Astro
    Where: Lake Nocona, Montague county
    When: Sept 30, 2012
    Who: 3 injured
    Why: Three men were injured when the helicopter #8341W flown by Darren Fenoglio crashed into Lake Nocona. Lake Nocona is 10 miles from Nocona Texas.

    The pilot and both passengers, Josh Walterscheid, and Derrick Morse were injured.

    The FAA is investigating, and will be recovering the helicopter.

    On Sunday, lake area residents assisted in recovering the passengers from the lake. Dr. Kabisch in his kayak was the first emergency responder. Toby Howard also assisted with a pontoon, taking the injured to the beach at Boone Park where more emergency responders were assembled.

    Fenoglio, Walterscheid and Morse, were taken to Nocona General Hospital.

    Cirrus Crashes in Gary

    What: Gandy Air LLC Cirrus SR22 GTS G3 Turbo en route from Smyrna to Gary
    Where: A mile SE of the Gary Chicago Int. Airport
    When: Oct 3, 2012, 11:00 a.m.
    Who: 2 aboard, 2 fatalities
    Why: A plane that took off from Smyrna, Tenn. General Airport crashed for unknown reasons in a wooded area about 400 feet from the Lighthouse Charter School in Gary. The bodies of the two men aboard were retrieved from the scene of the crash around 5:35 p.m.

    No one reported a distress signal. A witness in Gary heard the plane’s engine “sputter” before the plane crashed. Many of the 650 students of West Gary Lighthouse Charter School heard the plane come down.

    Dassault Mirage Crash in France

    Why: A Mirage 2000-5 fighter crashed in Froideconche killing the Taiwanese pilot.

    The pilot, who was on a training mission, crashed northeast of the town of Luxeuil Froideconche in a wooded area.

    The accident is under investigation.

    CareFlite Crashes in Eastland En Route to Pick Up Patient


    What: CareFlite Agusta 109 medical helicopter
    Where: County Road 309 Eastland, Texas
    When: Oct 1, 2012
    Who: three injured
    Why: A CareFlite medical helicopter crashed in Texas, injuring pilot Scott Wallace, 58, of Fort Worth; medic Teresa Campbell, 53, of Granbury, and Rhett Drahen, 34, of Frisco. The injuries were minor to moderate. All three were conscious and talking after the crash. The crash site was six miles from Eastland Municipal Airport where the injured patient was waiting.

    The pilot was en route to pick up a patient but needed to divert to another airport due to weather. The helicopter crashed three miles south of Eastland.

    Granite Quarry Experimental Ultralight Crash

    What: Experimental Ultralight aircraft en route from Rowan County Airport to High Rock Lake,
    Where: a field close to Shaw’s Mobile Home Park
    When: Sept 30, 2012, 6 p.m.
    Who: pilot
    Why: The ultralight ppeared to stall out shortly before impact. Cecil Dwayne Brown, of West Second Street, in Faith, died in the crash. His remains were recovered within two hours of the accident.

    The plane apparently impacted power lines, causing an immediate outage.

    UK Participating in Sita Air Accident, Nepal

    What: Sita Air Dornier 228-202 en route from Kathmandu-Tribhuvan Airport to Lukla
    Where: 500 Meters from airport
    When: Sept 28, 2012
    Who: 19 fatalities
    Update: Because seven British subjects were killed aboard the Sita Air crash on September 28, the UK Air Accidents Investigation Branch will be joining the accident investigation.

    The British fatalities were named as:

    1. Raymond Eagle, 58, from Macclesfield, Cheshire
    2. Timothy Oakes, 57, from Winwick, near Warrington
    3. Stephen Holding, 60, from Stoke-on-Trent
    4. Vincent Kelly, 50, from Lostock, Bolton
    5. Darren Kelly (brother) 45 from Isle of Whithorn Southern Galloway
    6. Benjamin Ogden, 27, from London
    7. Christopher Davey, 51, from Northampton

    Nepalese authorities have a three month report deadline.

    Cessna Crash in Tyrol, 6 fatalities, 2 survivors


    What: Evram Inc. Cessna 414A Chancellor en route to Valencia
    Where: Ellbögen, Mühltal Austria
    When: Sept 30, 2012
    Who: 8 aboard, 6 fatalities
    Why: A Cessna that crashed in in Tyrol Innsbruck-Land killed six. Two aboard survived the crash. The pilot made an emergency call before losing contact with ATC.

    The passengers were Tyrolean, and took off at 6:50 for Valencia. An area hunter heard the plane before it impacted.Two of those aboard extricated themselves from the plane. One was found burned in the forest, another was ambulatory, there to greet the rescue crew around 7:42. One survivor had minor injuries, and the other required surgery.

    Before the crash, the flight lost contact. After the helicopter rescue was called off due to heavy fog and low lying clouds, rescuers arrived on foot, thirty minutes after the first alarm. The rescue was coordinated by 70 Red Cross, 100 firemen, 20 local police and 12 crime scene investigators (detectives). Offsite, family members gathered in a building near Innsbruck State Police station were taken care of by Red Cross.

    The pilot who was from Saalfelden, Salzburg was killed in the crash. Two of the victims were identified as hoteliers Zellberg and Mayrhofen.

    The remains were taken to Innsburck. The plane was charred, and left a 100 meter trail in the forest. A wing was found on the slope of the forest.

    Piper Crash in Austria

    Why: The 49-year-old pilot from Wiener Neustadt struck trees when flying in Apetion. The pilot landed in a field with parts of the tree stuck in the plane. The pilot was treated on the scene and taken to the hospital with a fractured pelvis.

    Acrobatic Plane Crashes during Indonesian Airshow


    What: Indonesian Aerosport Federation FFA AS/SA 202/18A3 Bravo
    Where: Husein Sastranegara Int. Airport Bandung Indonesia
    When: Sept 29, 2012, 11:40 a.m.
    Who: 2 aboard, 2 fatalities
    Why: During the Bandung Air Show 2012 at Husein Sastranegara Int. Airport, a training aircraft crashed during a maneuver killing the pilot and co-pilot. After completing five maneuvers, the Bravo 202 aircraft was flying low when the pilots crashed, sending up a fireball.

    The pilot was Air Vice Marshall (ret.) Nurman Lubis. The co-pilot was identified as Col. (ret.) Tonny Haryono. Their remains have been taken to Bandung’s Hasan Sadikin Hospital. The men were members of the Indonesian Aero Sport Federation, who owns the plane.

    Lubis was an opthalmologist.

    EVEREST Plane Crash 19 Killed

    What: Sita Air Dornier 228-202 en route from Kathmandu-Tribhuvan Airport to Lukla

    Who: 19 fatalities
    Why: After takeoff, the pilot hit a vulture. The pilot attempted to return to the airport. (The vulture strike is unconfirmed.)

    Villagers could not assist because of the fire. Firefighters had difficulty reaching the area on the banks of Manohara River in Thimi, just a few kilometers east of Kathmandu.

    Four passenger and three crew were from Nepal. Five were Chinese, and seven were British. the remains were taken to the morgue at TU Teaching Hospital, Maharajgunj in Maharajgunj.

    The crew who died in the crash were pilot Bijay Tandukar, co-pilot Takesingh Thapa and air-hostess Ruja Shakya.

    The passengers were identified were Kumar Marshyangdi Magar (Nepal Army), D Rai, Yangchay Tamang and LN Sherpa. One Britain was identified as Eagle Raymond.

    Foz de Iguazu Helicopter Accident

    Who: 4 aboard
    Why: Taking off from the entrance of the National Park of Iguaçu,Foz do Iguaçu in western Paraná, the helicopter had a problem at the helipad and fell from a height of five feet.

    The company that owns the helicopter is a concessionaire of the National Paraque selling air tours over the Iguazu Falls.

    The four aboard the helicopter were wounded. The chopper was described as badly damaged. Three men and one woman were rushed to Hospital Costa Cavalcanti and to City hospital. There was one crewman aboard. The three passengers were tourists.

    Helicopter Down at Mammoth


    What: Asplundh BELL 206B HELICOPTER helicopter crash
    Where: Pole lake road southeast of Mammoth Lakes, near Deer Springs Road
    When: Sept 19, 2012, 12:30
    Who: 2 aboard
    Why: Two Asplundh employees were working on power lines and crashed near the lake causing a small fire. They were taken to Mammoth Airport and ambulanced to Mammoth Hospital. They suffered minor injuries.

    According to the FAA preliminary report, the rotorcraft tail rot0r struck the ground and crashed 12 miles from Mammoth lakes California.

    Mono County Sheriff’s Department, Mammoth Lakes Fire Department, Long Valley Fire Department, Cal Fire, U.S. Forest Service, and Mono County Paramedics responded to the scene.

    Student Pilot Crashes in Ukraine

    What: Ukraine Ministry of Defense Aero Vodochody L-39 Albatross
    Where: Chuhuiv, Kharkiv Oblast, Ukraine
    When: Sept 22, 2012
    Who: 1 fatality
    Why: In Chuguev, a combat trainer aircraft L-39 crashed on the runway. The student pilot who was flying the plane when it crashed died at the scene.

    The pilot was a third year student and had completed 71 flights.

    The incident is under investigation. Use of L-39 planes has been suspended pending the investigation.

    Chopper Strikes Syrian Airlines Passenger Jet

    What: Syrian Arab Airlines
    Where: Duma east of Damascus
    When: Sept 20, 2012
    Who: 200 aboard, no injuries
    Why: The Syrian military helicopter that crashed near Damascus struck the tail of a Syrian Arab Airlines plane.

    200 aboard were uninjured. Pilots managed to land the plane at Damascus.

    A release stated that “The control tower at Damascus airport confirmed that the plane landed safely at Damascus airport and all 200 passengers are in good health.”

    The Mil helicopter was damaged beyond repair. No one aboard survived.

    Helicopter Crash in S. Africa injures Three

    Why: The helicopter crashed for unknown reasons in a field near Rand Airport. The pilot was airlifted to Milpark Hospital with chest injuries. A ER24 Advanced Life Support paramedic escorted a woman with a broken leg to Union hospital in the SAPS medevac; and a man was driven by ambulance to Union hospital.

    One Fatality in Mexican Lake while Tourism Video Filming Underway, No Academy Actors Involved.

    When: Sept 18, 2012
    Who: 5 aboard, 1 fatality
    Why: The Helicopter was filming a promotional tourism video when it crashed in Lake Zirahuen. Four aboard were injured. There was one fatality.

    The aircraft became entangled with a tension wire, used as ziplining.

    Despite the rumor, no members of the cast of “The Academy’ television show were aboard. The rumor appeared apparently after tweets immediately following the accident.

    The injured were reported as Enrique Barragán, Carlos Vela, Pablo Gabriel Angulo and Luis Fernando Gutiérrez. Daniel Martinez was initially reported as missing, then later as a fatality.

    THe helicopter is owned by the State of Michoacan.

    Plane Landing During Storm Hydroplanes Across Street Into Woods

    Why: The Beechcraft hydroplaned while landing during a downpour at Herbert Smart Airport, overran the runway, went over an embankment, crossed Oculgee East Blvd and stopped in the trees thirty yards from the runway. The businessman and quarterback who were aboard the plane were not hurt. They had flown in on a business trip on the Dewberry Air Corporation Jet, on a plane registered in Dover Delaware.

    The pilot was pinned in the plane and had to be cut out was not seriously injured.

    Forestville Plane Crash

    Where: North of Forestville, Quebec
    When: Sept 18, 2012 1 pm
    Who: 2 fatalities
    Why: The floatplane carrying two men crashed in an inaccessible area near a North Shore lake, Lake Petit Argent killing one man in his twenties and one in his fifties.

    After they were recovered by Canadian Forces Rescue, they were rushed to a Baie-Comeau hospital. The cause of the crash is unknown. The victims names have not been released.

    Microlight Crash in German Corn Field

    Where: Niederschöna Germany
    When: Sept 17, 2012 at 8:30
    Who: 2 fatalities
    Why: The 582 Storch microlight engaged on a round trip from Mohom crashed in a Halsbrücke field and broke into pieces on impact.

    The two aboard the plane died in the crash. The 22 year old pilot has been identified but the name has not been released. The passenger has not been identified.

    The Niederschöna fire department were first responders. The Federal Bureau of Aircraft Accidents Investigation of Brunswick is investigating.
