    Ten Lost in Fretax Taxi Aéreo Crash

    On Mar 12, 2013, a Fretax Taxi Aéreo single engine Embraer EMB-820C Carajá (PA-31) disappeared en route from Belem to Sierra Areão Airport. The flight crashed twenty miles from the airport in Almeirim Brazil near the border with the state of Amapá.

    Seripa (Regional Investigation and Prevention of Aeronautical Accidents) discovered the wreckage after the plane went missing late Tuesday in the vicinity of Monte Dourado. Air Traffic controllers reported the flight missing at 22:00 hours and contacted SALVAERO Service Air Search and Rescue.

    There were ten people on the flight to the Santo Antonio do Jari hydroelectric power plant, including the pilot, and ten employees of Cesbe Engineering. The crew was en route to the Santo Antonio do Jari hydroelectric dam/power plant building project as part of the construction crew.

    A FAB (Força Aérea Brasileira) investigation is underway.

    The aircraft was “fully airworthy and all revisions to date, and crew member license and valid medical examination.

    Beech Crash in Tefe Brazil, no injuries

    On March 2, 2013, a Beechcraft 58 Baron in Tefe, Brazil crashed on takeoff six feet from the end of the runway.

    Three people were aboard the flight. There were no fatalities or injuries but there was damage to the engine and the propellers.

    The investigation was initiated by the Seventh Regional Service and Prevention of Aeronautical Accidents (Seripa VII). The report should be done in three months.

    The pilot said there was a problem in opening the aircraft door during early flight.

    TAM Airbus Birdstrike over São Luis

    On Feb 21, 2013, a TAM Linhas Aéreas Airbus A319 en route from São Luis-Marechal Cunha Machado Airport to Brasilia suffered a bird strike.

    Pilots returned to São Luis Airport where they made a safe landing.

    The plane was taken out of service for repairs and maintenance.

    Passengers reported they were terrified when they heard a huge bang from one of the engines.

    The incident is under investigation by Centro de Investigação e Prevenção de Acidentes Aeronáuticos (Cenipa).

    Names Released in Brazil Crash

    The names of passengers lost in the Brazil Beechcraft crash have been released.

    The fatalities include José Eduardo Ermirio de Moraes, Elizete Aparecida Romangnoli Assunção (Letícia’s mother), Letícia Romangnoli Piveta Assunção José’s fiancee, Pilot Luiz Marcondes Rodrigues Filho, and co-pilot Luciana Aguiar da Costa.

    José and Leticia were engaged to marry in June of this year. José’s great uncle is Brazilian Entrepreneur Antonio Ermirio de Moraes.

    The King Air C-90 crashed in Cândido Mota, in São Paulo.

    ANAC (Agência Nacional de Aviação Civil) is investigating.

    Five lost in Brazil Beechcraft Crash


    At 2:30 pm on Feb 3, 2013, a Beechcraft C90B King Air en route from Maringá to Congonhas was over Cândido Mota, São Paulo when it crashed in Água do Macuco, the Water Macuco, traditional producer of soybeans and cassava.. Residents heard a loud bang as the plane flew close to a neighborhood, circling, then fell into a soybean field. Three women and two men died, including the 58 year old pilot. Fuel outside the tank seemed to indicate to investigators that the pilot had dumped fuel. Just before the crash, the pilot had informed the tower of Curitiba (PR) that would make a forced landing at Assisi State Airport.

    The fatalities included Elizete Piveta and her daughter, Leticia Piveta Assumption, Leticia’s fiance José Eduardo Ermírio de Moraes, pilot Luis Henrique Marcondes, 58 and co-pilot, his wife Luciana Souza Aguiar da Costa.

    Rescue teams from Assisi and Marília assisted at the scene.

    Plane Crashes in Manaus parking lot, Money, Cars up in smoke

    A Tio Taxi Aereo Embraer EMB-810C Seneca II en route from São Paulo Olivenza to Tefé to Manaus-Aeroclube de Flores was flying a cargo of money when the pilot had difficulties and attempted to land on Avenue Nilton Lins, but crashed in the Eletrobrás parking lot with sixty cars of which 20 were destroyed. The cargo went up in smoke.

    Institute of Municipal Engineering and Traffic Authority of Manaus and the fire department were on the scene. Regional Service Investigation and Prevention of Aeronautical Accidents (Seripa) expects to provide a report within 30 to sixty days.

    The pilot, Iran Rolim, co-pilot Bill Pacheco and two passengers were hospitalized. One pf the passengers was named as Roberto do Santos. The maintenance engineer of Tio Táxi Aéreo, Amarildo Furtado da Silva provided details to news services.

    Municipal Parks Crew injured, Pilot killed in São Paulo Crash


    On Jan 21, 2013 at 12:45, a Helimarte Bell 206B JetRanger III en route in light rain from West Zone of São Paulo to Campo de Marte Airport with four people aboard flying over a residential area in the neighborhood of Garden Corner, crash landed on two houses in Paul Street Arentino in Jaragua in the north of São Paulo near Highway Taipas.

    A resident saw the plane approaching and saw that the rear propeller stopped. Witnesses in the house say there were electrical lines entangled in the helicopter.

    Civil Defense roped off four properties as unsafe.

    One person aboard did not survive the crash and three others aboard, all crew, were injured but conscious when they were found. One person on the ground was also injured. Some reports say two on the ground were injured. There were four people in one house that was crashed into.

    The sole fatality was the pilot, Stella Melo Marcelo Araujo, who was 29, was single and had no children. The helicopter was an air taxi but the objective was a two hour flyover inspecting, monitoring and surveying regional park areas.

    Ramiro Levy (Depave-1), Bishop Fabian Barbosa (Depave-5) and Idevanir Souza (Communication), all employees of the Municipal Green and Environment were injured. Two crew members were taken to Hospital and Clinics and one to the Hospital São Camilo Emergency Room.

    Five teams were sent to the crash to deal with the injured, the wreckage and the leak in the helicopter. The helicopter’s certificate valid until December 8, 2016 and had passed it’s recent maintenance inspection.

    Helimarte, who operates the helicopter that crashed on Monday in ??Jaragua, north of Sao Paulo, said in a statement that the accident was the first in the fourteen year history of 35 000 flights. Everyone aboard was employed by the City of São Paulo.

    Agricultural Embraer Crashes in Brazil


    On January 5, 2013, a privately owned Embraer EMB Ipanema with only the pilot aboard was making an agricultural pass and spraying crops in Lucas do Rio Verde MT Brazil.

    While cropdusting, the plane collided with a highway sign and the pilot lost control. Engine problems may have also been involved.

    THe pilot’s plane struck a pole then impacted the field of soybeans, instantly killing the pilot, Rubens Fritsch. The plane is a total loss. (See photo)

    His body was recovered by the firefighters who responded to the call at 11:00 a.m. Saturday.

    The married pilot leaves no children. He was buried in Lucas do Rio Verde.

    Crash in Brazil Totals Ultralight


    On January 2, 2013, a Condor MB ultralight was over rural Luziânia Brazil when the engine shut down.

    The ultralight crashed in a forest and was damaged beyond repair.

    Pilot Alberto Rossi Junior, and his son Lucas Ferreira Rossi, survived, and were taken to a Brasilia hospital.

    Military police and the Fire Department respnded to the scene. The paperwork of the pilot and the plane is reported to be up to date.

    Single Engine Plane Crash in Brazil


    On December 22, 2012 18:30, a Van’s RV-7A plane with two aboard was near Tito Teixeira aerodrome in Brazil when it crashed after the pilot lost control.

    The single engine plane blew up as it was falling on Saturday night near the Ituiutaba Airport in Triangulo Mieiro. There was a pilot-instructor aboard and a doctor.

    The engine failed shortly after takeoff and the plane crashed in a pasture 100 meters from the runway.

    The crash is under investigation. This is the third small crash in the area this December.

    Sao Paulo Chopper Training Flight Crash


    Dec 16 2012-An instructor and student of Rangel Escola de Avião Civil were injured during a training flight in a Robinson R-22 when it crashed at 19:30 near the Guarapiranga Dam in Sao Paulo Brazil.

    The hard landing was close to 15796 M’Boi Mirim road in the Garden Angela, in jungle terrain and destroyed the helicopter. Passengers were conscious when the rescue team arrived. Three fire vehicles responded.

    The instructor and the student pilot were admitted to Hospital das Clinicas and Hospital São Luís. The student’s head injuries are supposed to be not serious.

    The Fire Department and “Team Eagle” helicopter and Military Police responded to the scene.

    Tam Airbus Suffers Bird Strike

    Passengers disembarked safely and were provided an alternative flight.

    Tropic Air Táxi Emergency Landing in São Paulo

    Why: The corporate jet sustained substantial damage in a runway excursion. The captain, co-pilot and passenger were all injured on landing three people were injured, one with head trauma, one with chest injuries one hospitalized with lumbar spine injury. The plane landed, bounced and continued down a slope where it crashed at the perimeter fence near Avenida dos Bandeirantes.

    The accident closed Congonhas Airport for an hour afterwards. At the time of the accident, the plane was in good standing with the Annual Maintenance Inspection (AMI) and the Certificate of Airworthiness (CA) was valid until November 2013.

    Chartered Flight Crashes in Paraná Brazil

    Who: 4 aboard, 4 fatalities
    Why: The Táxi Aéreo Hercules Piper left Dourados for the Bacacheri Airport, in Curitiba with the pilot, copilot and two passengers. Táxi Aéreo operates out of Bacacheri Airport. The flight was chartered for Brinks and was carrying money.

    One security personnel named Wilmar survived the crash and was taken to Hospital do Trabalhador where he later died.

    The plane crashed fifteen miles short of its destination.

    Seripa (Serviço Regional de Investigação e Prevenção de Acidentes Aeronáuticos) is investigating. Currently no cause for the accident has been released.

    Why: Two engineers supervising paving works in Confresa lost control of the plane on approach to the airport.

    When: Nov 3, 2012
    Who: Two adults, 1 child, 2 fatalities
    Why: The flight went down in the ocean near Taquarinhas beach. The pilot and another adult aboard were killed. An eleven year old boy who was aboard survived the crash and has been hospitalized. The flight had taken off from Porto Belo, Santa Catarina. The aircraft sank around 10 a.m.

    Fishermen rescued the eleven year old, tossing him a floatation device. He was hospitalized at City Hospital Ruth Cardoso.

    The remains of pilot Julius Mandelli and his adult passenger were retrieved by rescue crews on jet skis and ski boats.

    Gol: US Pilots Retrial Upholds Conviction, No Jailtime

    Joseph Lepore of Bay Shore, New York, and Jan Paladino were flying the Embraer Legacy 600 executive jet that collided with a Boeing 737 operated by Gol Lineas Aereas Intelligentes SA. Lepore and Paladino guided the plane they were flying to safely while the larger jet crashed into the jungle, killing all aboard. The pilots were convicted in 2011 and sentenced to 52 months in prison because according to prosecutors, the aircraft’s anti-collision system was turned off for almost one hour. The pilots deny wrongdoing, and say the TCAS system was never off and suggest the other plane’s TCAS may not have been working. The NTSB blames the collision on the Brazilian ATC. Air force sergeant Jomarcelo Fernandes dos Santos was sentenced two years ago to 14 months in jail for failing to take action when he realized that the Legacy’s anti-collision system was off.

    The case was retried in Brazil Monday and upheld; in fact, the jail time that had last year been commuted to community service was eliminated. In lieu of jail time or community service, now the pilots will only be required scheduled check-ins with Brazilian parole authorities.

    Foz de Iguazu Helicopter Accident

    Why: Taking off from the entrance of the National Park of Iguaçu,Foz do Iguaçu in western Paraná, the helicopter had a problem at the helipad and fell from a height of five feet.

    The company that owns the helicopter is a concessionaire of the National Paraque selling air tours over the Iguazu Falls.

    The four aboard the helicopter were wounded. The chopper was described as badly damaged. Three men and one woman were rushed to Hospital Costa Cavalcanti and to City hospital. There was one crewman aboard. The three passengers were tourists.

The Fire Department responded with four vehicles and the Military Police (MP) sent a chopper. The two crew members aboard suffered no injuries.

    Why: On a training flight, two Embraers collided. This plane crashed in a sugar cane field. Four people died in the crash on the edge of the Bandeirantes Highway.

    Azul Araken de Oliveira Salamene, pilot Cassiano Ricardo Callegaris, flight dispatcher Voninho Souza Aguiar, and Vandeir Fábio de Lima.

    Seripa (Serviços Regionais de Investigação e Prevenção de Acidentes Aeronáuticos) is investigating.

    A witness, Ricaro Paulo Silveira de Souza, said he saw a wing falling. Another witness, Julio Cesar Prestes, said he saw two planes falling from the sky.

    Both planes were taking off from the same airport and going to the same destination but left five minutes apart. On impact, there was an explosion.

    Four burned remains were recovered.

    2012: Tam Remembered

    It is again mid July. Inevitably, in mid July, my thoughts turn toward Tam 3054. On July 17 2007, five years ago, a Tam Airbus failed its landing at São Paulo’s Congonhas airport and stole a hundred and ninety-nine lives. The casualties always number greater than those who died. Each of the 199 touched had families; and each family exponentially touches countless others. I came to know each of the families, and I loved them. I learned from them, especially from their strength in the face of adversity. What I think of too, is how that accident changed me, how I learned from the families left behind to pick up the pieces and move on.

    On this day, I want to remember these families, their grace, and their lost loved ones. We cannot forget that in their passing, a hole was left in the world.

    I hope that the families—my well remembered friends—have overflowed the holes in their lives with something else. Five years of life. I hope they have taken the opportunity to fill to capacity these last five years with a life well lived. What does that mean? A life lived to the fullest. It’s the good and bad and everything in between. I hope that this well-lived life has lit their dark spaces with light, and packed the emptiness with new memories, each to be revisited, like the pages of a beloved book.

    We are all each a harp of many strings, each string struck by memories that play one and many notes. The threads of our lives stretch from the past to future, distant anchors never seen and never known. Each day brings us a chorus of new, and distinct notes. Let us lose all regret, and only joy in the harmony of our histories and our tomorrows. It is the wholeness of these notes that is the music of our lives.

    I hope that in filling the hole with new life, that my friends can now painlessly cast the net into their past. I hope they have learned to embrace the roses in December. Memories are for us to cherish, each one a forever jewel in itself. Memories are a constant gift of joy that is never lost; and as we can remember with poignant joy the roses of summer in December, we can remember our lost loved ones in July. I do not smile because I have forgotten. I smile because I remember.

    Brazil Crash Kills Crime Scene re-enactors and murder suspect

    Why: An AgustaWestland AW119 MkII Koala Enhanced helicopter was participating in a reconstruction of a murder on a farm in Goiás, and crashed. The eight people aboard included The accident victims are the delegates Jorge Moreira, Osvalmir Carrasco (pilot), Vinicius Baptist, Bruno Carneiro (copilot), the superintendent of the Judicial Police, Antonio Goncalves, experts and Marcel de Paula de Paula Fabiano and suspected murderer, Ali Ahmad Souza.

    The helicopter had been inspected Monday.

    The Ministry of Justice and the National Civil Aviation Agency was bitufued, The Secretary of Public Security of Goiás, John Furtado will be supervising rescuers and the investigation.

    Eyewitness accounts disagree. One witness says the aircraft was spinning on its axis and falling, another says it exploded in midair.

    Air France 447 and Airbus Design Issue: Possible Design Flaw in Control Stick

    Stall warning sounded 75 times after the pitot tubes froze over.

    Could the crash have been averted if the stick had communicated to the more experienced co-pilot what the less experienced pilot was doing? We agree that if the stick had been designed to communicate that the angle was wrong, the error would have been caught before the situation became critical.

    We happen to agree with journalist, Nick Ross, who said in the video below that “All commercial airliners are remarkably safe,” but that the design could be made safer.

    Should the stick provide pilot feedback? We believe amending the Airbus design to provide feedback would make the airbus a safer vehicle. What is your opinion?
