What: Germanwings Airbus A319-132 Where: Cologne Bonn German When: Oct 26, 2012 Who: 128 aboard Why: On takeoff from the airport, the flight suffered a bird strike in one engine.
Pilots shut down the affected engine, and diverted to the airport where they made a safe landing.
What: Caribbean Airlines ATR 72 en route from Trinidad/Tobago to Caracas Where: Piarco International Airport Trinidad and Tobago When: October 25, 2012 Who: Four crew, 48 passengers Why: On takeoff, a panel on the plane became detached. One source identified the detached part as an engine panel. Another source called it an “exterior panel.” A third source indicated there was debris on the runway.
The pilot dealt with an engine fire indication, and returned to the airport to make a safe landing with emergency services on standby. Passengers disembarked and the panel was examined.
Passengers were provided an alternative flight which left at 10:00 a.m.
What: Arik Air Airbus A340-542 en route from London to Lagos Where: Lagos Murtala Muhammed International Airport When: Oct 26, 2012 Who: 1 fatality Why: The body of a man was discovered in the wheel well of an Arik Air Airbus after a flight from London to Lagos. The body had no identification. There is no indication how long the body remained there, nor where it came from.
According to officials the body was discovered “during a check on the aircraft panel as it was being prepared for another flight and that the undercarriage compartment of the Airbus A340-500 is big enough to accommodate a person, besides the space for the tires.”
Security at Lagos Murtala Muhammed International Airport is reputed to be lax, and authorities have apparently been ineffective in eliminating illicit activities.
What: Kensington Capital LLC Piper PA-24-250 Comanche en route from Arizona to Schlemeyer Field Airport Where: Odessa When: Oct 26 Who: 3 aboard Why: When the pilot realized he didn’t have enough fuel for the Piper PA-24-250 Comanche to make it to Schlemeyer Field, he dodged power lines and made an emergency landing on 87th Street between Harvard Ave. and Rainbow Dr.
The pilot called for a fuel delivery, and was subsequently permitted to take off for the airport. There were no injuries.
What: Spirit Airlines Airbus A319-100 en route from Cartagena, Colombia to Fort Lauderdale Where: Palm Beach International Airport When: Oct 25, 2012 Who: 54 aboard Why: As pilots were flying from Cartagena, Colombia to Fort Lauderdale, the plane developed problems and the pilots had to divert the flight. According to one source, one of the engines developed problems and was shut down. (Reports vary on the exact nature of the issue behind the diversion.)
Pilots diverted to Palm Beach International Airport where they made a safe landing.
What: Herman Hassinger Architects Beechcraft A36 Bonanza en route from Nashua to Laconia Where: Hooksett, New Hampshire When: Oct 25, 2012 1:30 pm Who: 2 aboard, 2 fatalities Why: According to reports, the single-engine Beechcraft hit a light pole alongside I-93 northbound.
The two aboard, Husband and wife Herman and Doris Hassinger, were on their way to a board of trustees meeting at a New Hampton boarding school, and died when the plane crashed on to the shoulder and guard rail after glancing off the pole.
Herman Hassinger was ejected into the woods about 25 yards from the plane.
The plane sustained heavy front-end damage. The debris included parts fallen from the light pole and about six feet of the plane’s wing.
The Bureau of Aeronautics, FAA and the National Transportation Safety Board will be investigating.
Two miles off shore of Waikiki, a US Coast Guard MH65 Dolphin was hit by a laser pointer. The pilot had to make an emergency landing at Barbers Point Air Station in Hawaii. A flight mechanic and rescue swimmer were aboard the helicopter when it had to divert back to the base.
The crew was sent to have their eyes checked and are temporarily off-duty.
It is a federal crime to aim a laser pointer at an aircraft.
What: Sriwijaya Air Boeing 737-400 en route from Surabaya to Jakarta Where: Juanda International Airport When: October 24, 2012 6:30 a.m. Who: 115 passengers Why: The Sriwijaya flight returned to Juanda International Airport after a ten minute flight after the plane developed a problem with its hydraulics. No one was injured.
Repairs were made to the plane.
Passengers reboarded and took off for Jakarta at 1 pm that same day.
What: PIA Pakistan to Manchester Where: Allama Iqbal International Airport Pakistan When: October 24, 2012 Who: 203 aboard (183 economy class passengers, 16 business class and four infants) Why: After passengers had boarded for the 11:45 a.m. flight to Manchester, and the plane was about to taxi for take off, a catering van drove onto the tarmac and into the parked aircraft, damaging the door of the plane.
Passengers disembarked and had to wait at the airport for hours until 7:30 when the replacement plane for PK 709 left for Manchester.
What: Piper PA-28 Cherokee#N55620 en route from Delaware, Ohio to Creve Coeur airport. Where: Creve Coeur Lake, Maryland Heights When: Oct 24, 2012 Who: 2 aboard Why: While attempting a landing on the shore, the piper that crashed into Creve Coeur Lake had two occupants, pilot Russell Hazelton, 78 who died at DePaul Health Center in Bridgeton and passenger Suzanne Hazelton, 69 who was on life support. The plane flipped on to its back during the attempted landing.
Lake Saint Louis Fire Protection District dive team, police and rescue services were on hand for the recovery.
WASHINGTON – The Federal Aviation Administration (FAA) is proposing a $354,500 civil penalty against US Airways, Inc., of Phoenix, for operating a Boeing 757 airliner on 916 revenue flights when it was not in compliance with Federal Aviation Regulations.
The FAA alleges US Airways removed and replaced a leaking engine fuel pump on the aircraft on Aug. 3, 2010, and US Airways failed to carry out FAA-required tests and inspections before returning the aircraft to revenue service. The noncompliant flights took place between Aug. 3 and Dec 3, 2010.
US Airways, Inc., has 30 days from the receipt of the FAA’s enforcement letter to respond to the agency.

Wings Over Holland Diamond DA40D Diamond Star TDI Where: Zeeasterweg, Dronten Netherlands When: October 22, 2012 4:35pm Who: 2 aboard, 2 injured Why: A Diamond DA40D Diamond Star TDI (PH-TDE) and a General Avia F-22BV Pinguino (D-EDMJ) crashed following a mid-air collision. The accident happened during an air-to-air photo flight
Those aboard the Pinguino flight were airlifted by medical evacuation Lifeliner 1 and Lifeliner 4.
The two aboard the Diamond Star did not survive.
Rescue efforts were hampered by the wet ground which trapped afire truck that responded to the scene.
What: Skyranger Where: Vienne France When: Oct 23, 2012 3:00 pm Who: 1 fatality Why: The pilot of a SkyRanger failed to return from a 2 hour flight, triggering a Sater Bravo land-air rescue.
A search went on till 2:00 a.m. The next day his remains were discovered near an electrical cable in a wooded area, ten miles east of the airport. The plane was severely damaged.
What: Emirates Airbus A330-200 em route from Lusaka to Dubai Where: Lusaka, Zambia When: Oct 21, 2012 Why: After takeoff, the flight developed a technical problem.
The pilot returned to the airport to make a safe landing at Kenneth Kaunda International Airport. The plane continued to the gate where passengers disembarked. Emirates representatives have indicated that earlier reports that the plane crash landed were incorrect; and that passengers were accommodated in hotels and have now been re-booked on alternative flights with Emirates or other airlines. Passengers reported hearing a bang in an engine, a bumpy thirty minute flight back to the airport and on disembarking, seeing a hole in the engine.
What: Transavia Boeing 737-BK2 En route from Sharm el-Sheikh to Amsterdam Where: Primošten Croatia When: Oct 20, 2012 Who: 192 aboard Why: While the flight was en route over Croatia, smoke and the odor of burning plastic developed in the cabin. The pilot diverted to Zagreb-Pleso airport and made a safe landing although two passengers complained of breathing problems. Emergency Medical Services at the airport treated them.
Apparently there was no one available certified on Boeing, so an expert from Amsterdam was called in.
While passengers spent the night at a hotel, a replacement plane was flown in.
What: Globe Swift en route from Spadaro Airport Where: Moriches inlet, Fire Island When: Oct 20, 2012 3 p.m. Who: 2 aboard Why: A Globe Swift plane that took off from Spadaro Airport had two aboard.
The plane was witnessed nosediving into Moriches inlet where it sank twenty to thirty feet deep, ending up upside down on a rock where divers took hours trying to retrieve the occupants.
Fishermen were the first on the scene, and tried to haul the plane up with a rope.Medics were on standby waiting for the divers to retrieval of the passengers, but the occupants were underwater for hours and presumed deceased.
What: TACA airlines Airbus A321-200 en route from El Salvador to San José Costa Rica Where: Costa Rica When: October 18, 2012 Who: 146 passengers, 7 crew Why: On touchdown, three tires burst on the wet runway, resulting in an emergency landing. The plane skidded and spun to the right and when it came to a stop, it was perpendicular to the end of the runway.
There were no injuries reported when the plane landed at 4.pm. Burst tires can often occur as the result of a hard or too fast landing.
What: Wideroe de Havilland Canada DHC-8-103 Dash 8 en route from Florø to Bergen Where: Bergen-Flesland Airport, Norway When: Oct 18, 2012, 7:30 a.m. Who: 17 aboard Why: The pilot was en route to Bergen when smoke in the cockpit forced him to land immediately; but he called off the mayday because he felt in control of the situation.
Before landing, the pilot informed passengers.
The crew stayed aboard until passengers were evacuated. The evacuation was described as undramatic.
What: jet2.com Boeing 757-200 en route from Tenerife to Manchester Where: Faro When: Oct 13 2012 Why: The Tenerife to Manchester flight was en route when the plane developed a problem with the hydraulics.
Pilots diverted the flight to Faro in Algarve, Portugal where they made a safe landing.
A replacement jet was provided to complete the flight.
What: Jet2 737 en route from Glasgow to Alicante Where: Glasgow Airport When: October 19, 2012 7.40am. Who: 189 passengers Why: On the takeoff run, the pilot “hit the brakes” and according to passengers “the plane started shuddering and there were lights flashing and smoke and the crew started shouting ‘Get out, get out’.”
Another passenger told BBC “We were careering up the runway and smoke started belting out of all the air vents with a smouldering smell.Then the brakes went on and the emergency chutes went out and we were all evacuated.I have a nine-week-old daughter and a four-year-old daughter. I had my nine-week-old attached to my chest as I went down the emergency chute and my wife had our four-year-old and was in tears. it was like something out of a movie. There was panic, people started running and I shouted ‘slow down’, and then the pilot shouted ‘get out, get out'”
Passengers disembarked via emergency chutes; ten passengers climbed out on a wing where there was no chute but only a few of them jumped.
Six people were taken to the Royal Alexandra Hospital in Paisley; fourteen were treated at the scene. Injuries from the chutes are not uncommon, especially if they throw themselves down in a panic; and injuries from jumping off a wing could also occur.
This incident occurred a couple of hours prior to the Thomas Cook flight that diverted to Dublin Ireland
What: Thomas Cook Boeing 767-300 en route from Manchester to Tenerife Where: Dublin When: Oct 19th 2012 9.40am Who: 325 aboard Why: After taking off from Manchester, the crew reported smoke in the cockpit.
THe pilot diverted to Dublin and made a safe landing with a dozen emergency services units including fire and ambulance on standby.
Passengers disembarked and waited in the terminal for a replacement flight to Tenerife
What: Helicopter Where: Bucks County, Tinicum Township When: October 17 2012 Who: pilot Why: Pilot Cpl. Douglas Ward Brigham died in a crash when his helicopter went down in the woods and caught fire shortly after takeoff. Residents of the area heard an explosion. The helicopter impacted in woods off River Road in Erwinna, across the Delaware River from Frenchtown, N.J.Brigham had been flying for 25 years. He was a licensed pilot, instructor and volunteer rescuer. He had taken off from the private helipad at his house.
Tinicum Township Department of Emergency Management, and Patient Airlift Services responded to the scene.
What: Polish Air Force Tupolev TU-154M Where: Smolensk When: Apr 10th 2010 Who: 88 passengers and 8 crew Why: The flight that killed 96 people including President Kaczynski has come to news attention as Poland protests Russia’s publication of grisly images of the crash showing mangled bodies, bodies in body bags, etc… Several websites were involved.
Russia’s federal Investigative Committee is looking into the leaked photo incident.
Russian investigators blame Poland for the crash; Polish invistigators blame Russian ground controllers for allowing the plane to land in heavy fog.
This comes after prior problems of misidentified passengers who were buried by the wrong families. Other agencies are looking into the problems of misidentification.
What: Globus Boeing 737 en route from Chita to Moscow Where: Perm, Russia When: Oct 17, 2012 10:52 Who: 7 crew, 100 passengers Why: After the right engine shut down, pilots decided to divert to Perm.
The Globus flight was made on behalf of S7.
Pilots made a safe landing in Perm with emergency services standing by. 107 were aboard. No injuries were reported.
Passengers were provided an alternative flight to Moscow.
What: Aero Contractors flight en route from Lagos to Abuja Where: Lagos Nigeria When: Oct 16, 2012 8 pm Who: not released Why: After take-off, one of the engines quit.
The pilot was in the air less than twelve minutes, and returned to Lagos to make a asafe landing at the airport. The problem was corrected as not an engine failure but an apu failure.
Repairs were made to the plane which is now apparently operational.
