    The National Transportation Safety Board determined that the
    captain of Colgan Air flight 3407 inappropriately responded
    to the activation of the stick shaker, which led to an
    aerodynamic stall from which the airplane did not recover.
    In a report adopted today in a public Board meeting in
    Washington, additional flight crew failures were noted as
    causal to the accident.

    On February 12, 2009, a Colgan Air, Inc., Bombardier DHC-8-
    400, N200WQ, operating as Continental Connection flight
    3407, was on an instrument approach to Buffalo-Niagara
    International Airport, Buffalo, New York, when it crashed
    into a residence in Clarence Center, New York, about 5
    nautical miles northeast of the airport. The 2 pilots, 2
    flight attendants, and 45 passengers aboard the airplane
    were killed, one person on the ground was killed, and the
    airplane was destroyed by impact forces and a postcrash
    fire. The flight was a 14 Code of Federal Regulations (CFR)
    Part 121 scheduled passenger flight from Newark, New Jersey.
    Night visual meteorological conditions prevailed at the
    time of the accident.

    The report states that, when the stick shaker activated to
    warn the flight crew of an impending aerodynamic stall, the
    captain should have responded correctly to the situation by
    pushing forward on the control column. However, the
    captain inappropriately pulled aft on the control column and
    placed the airplane into an accelerated aerodynamic stall.

    Contributing to the cause of the accident were the
    Crewmembers’ failure to recognize the position of the
    low-speed cue on their flight displays, which indicated that
    the stick shaker was about to activate, and their failure to
    adhere to sterile cockpit procedures. Other contributing
    factors were the captain’s failure to effectively manage the
    flight and Colgan Air’s inadequate procedures for airspeed
    selection and management during approaches in icing
    conditions.

    As a result of this accident investigation, the Safety Board
    issued recommendations to the Federal Aviation
    Administration (FAA) regarding strategies to prevent flight
    crew monitoring failures, pilot professionalism, fatigue,
    remedial training, pilot records, stall training, and
    airspeed selection procedures. Additional recommendations
    address FAA’s oversight and use of safety alerts for
    operators to transmit safety-critical information, flight
    operational quality assurance (FOQA) programs, use of
    personal portable electronic devices on the flight deck, and
    weather information provided to pilots.

    At today’s meeting, the Board announced that two issues that
    had been encountered in the Colgan Air investigation would
    be studied at greater length in proceedings later this year.
    The Board will hold a public forum this Spring exploring
    pilot and air traffic control high standards. This
    accident was one in a series of incidents investigated by
    the Board in recent years – including a mid-air collision
    over the Hudson River that raised questions of air traffic
    control vigilance, and the Northwest Airlines incident last
    year where the airliner overflew its destination airport in
    Minneapolis because the pilots were distracted by non-flying
    activities – that have involved air transportation
    professionals deviating from expected levels of performance.
    In addition, this Fall the Board will hold a public forum
    on code sharing, the practice of airlines marketing their
    services to the public while using other companies to
    actually perform the transportation. For example, this
    accident occurred on a Continental Connection flight,
    although the transportation was provided by Colgan Air.

  • IATA PR: Asia Pacific: Challenges and Opportunities – Intra-Asia Market Eclipses North America as World’s Largest

    Date: 01 February 2010 – Singapore – he International Air Transport Association (IATA) said that in 2009 intra-Asia-Pacific travel had eclipsed the number of travelers in North America as the world’s largest aviation market. Asia-Pacific’s travelers numbered 647 million compared the 638 million who travelled within North America (including domestic markets). By 2013 an additional 217 million travelers are expected to take to the skies within Asia–Pacific.

    “Achieving Asia-Pacific’s tremendous potential is contingent upon short-term efforts to battle the impacts of the economic downturn with cost reductions and efficiency gains. Longer-term Asia-Pacific must also face global challenges including environment, security and liberalization,” said Giovanni Bisignani, IATA’s Director General and CEO at the start of the Singapore Air Show Aviation Leadership Summit.

    The global aviation industry is expected to reduce losses from US$11.0 billion in 2009 to US$5.6 billion in 2010. The loss reduction is being led by Asia-Pacific’s carriers who are expected to see their losses shrink from US$3.4 billion in 2009 to US$700 million in 2010. “Asia-Pacific’s prospects are improving faster than other regions,” said Bisignani.

    Bisignani noted that the Asia-Pacific region is diverse, dynamic and with great potential:

    Diverse: Asia-Pacific is home to two of the world’s top five airlines in terms of profitability. At the same time, the region’s governments provided over US$10 billion in government bailouts to airlines in the first quarter of the year. The region’s two biggest growth markets—India and China—face completely different circumstances. India’s challenge is to reduce costs and improve infrastructure, while China is adjusting to new global trade patterns.

    Dynamic: Over the last decade China replaced Japan as Asia-Pacific’s largest player. Today China’s fleet is 1,400 aircraft compared to Japan’s 540. Its domestic market of 5.7 million weekly seats is more than double Japan’s 2.6 million and China’s 1.4 million weekly international seat market is now slightly larger than Japan’s 1.3 million.

    Potential: In the US, there are three aircraft seats per year for each of the 300 million people who live there. China’s population of 1.3 billion is served by only 0.3 seats per person and India’s 1.1 billion population has only 0.1 seats available per person. “The global air transport industry will triple in size when Asians travel as much as those in the US,” said Bisignani.

    “Asia-Pacific’s diversity, dynamism and potential are a great opportunity. Rapidly developing markets are defining aviation’s future. Is Asia-Pacific prepared for the challenges that this will bring?” said Bisignani. In his opening address, Bisignani highlighted three global issues for Asia-Pacific leadership:

    Environment: The global aviation industry presented the UNFCCC Climate Change talks in Copenhagen with three targets shared by airlines, airports, air navigation service providers and manufacturers. These are: improving fuel efficiency by an average of 1.5% per year to 2020, stabilizing emissions with carbon-neutral growth from 2020 and cutting our emissions in half by 2050 compared to 2005. These targets are backed by a clear strategy based on technology investment, effective operations, efficient infrastructure and economic measures.

    “Even without a binding agreement in Copenhagen aviation is united and committed to its targets. The ICAO Assembly in September-October is an opportunity to build government consensus leading to COP-16 in Mexico,” said Bisignani.

    The challenges for Asia include: working through ICAO to accommodate the diverse needs of the region, taking advantage of the tremendous business opportunities in developing sustainable second generation biofuels Biofuels have the potential to reduce aviation’s carbon footprint by up to 80%. “Five airlines have successfully tested biofuels and we expect certification within 2011 at the latest. Aviation biofuel is a US$100 billion plus business opportunity. And I hope that this region will play a key role in its early development,” said Bisignani.

    Security: “We live in a global world—global connectivity and global threats. Governments and industry must protect the connectivity and eliminate the threats. That challenge requires industry and governments to work together for effective and efficient security measures,” said Bisignani.

    “Ten days ago, I saw some hope for a new collaborative approach when the Secretary of the US Department of Homeland Security consulted the airline industry in our Geneva offices,” said Bisignani. IATA presented recommendations to (1) work together, (2) align requirements with the industry’s capability to implement, (3) make passenger collection more efficient, (4) ensure that governments coordinate their requirements across borders and (5) look to develop a new approach to checkpoint screening that combines technology and intelligence so that we look for bad people, not just bad objects.

    Asia-Pacific must define government/industry cooperation on security and find a better way to deal with the cost burden. Currently airlines pay US$5.9 billion a year for security. “These are national security measures. That is a government responsibility, including the bill,” said Bisignani.

    Liberalization: “Asian aviation will not reach its potential if the airlines are constrained to old ways of doing business. Industry is preparing for regional liberalization of market access with the ASEAN target date of 2015. It is important that the target date is met. This is already well-behind the industry leading developments in the US-EU Open Skies agreement. Second stage talks will conclude this year with ownership being the most important issue,” said Bisignani.

    “To move liberalization forward, IATA took the extra-ordinary step of calling governments together with IATA’s Agenda for Freedom. After a year of talks, in November 2009, seven governments, including the US, the European Commission, Singapore and Malaysia signed a multilateral statement of policy principles. These principles preserve a level playing field while addressing liberalization of market access, pricing and ownership.     NTSB ASSISTS GOVERNMENT OF LEBANON IN AVIATION ACCIDENT

    The National Transportation Safety Board will dispatch an aviation investigator to assist the government of Lebanon in its investigation of the crash of an Ethiopian Airlines B- 737-800 (ET-ANB) off the coast of Lebanon.

    At approximately 2:30 a.m. local time on January 25, the aircraft crashed into the Mediterranean Sea shortly after takeoff.

    NTSB Chairman Deborah A.P. Hersman has designated senior investigator Dennis Jones as the U.S. Accredited Representative. His team will include technical advisors from the Federal Aviation Administration and Boeing.

    The investigation is being conducted by the Directorate General of Civil Aviation of the Government of Lebanon, which will release all information on the progress of the investigation.     Mr. Raymond Benjamin,
    to the IATA Global Aviation Security Summit

    (Geneva, 22 January 2010) Giovanni, thank you for this invitation to join Secretary Napolitano and such an impressive gathering of airline CEOs. I very much enjoyed your presentation and the perspective of the Secretary, and I look forward to a stimulating discussion after my brief remarks.

    The attempted sabotage of Northwest Airlines Flight 253 on 25 December is a vivid reminder that security threats transcend national boundaries and can only be properly addressed through a global strategy based on effective international cooperation. As the UN agency responsible for promoting the development of civil aviation worldwide, ICAO facilitates cooperation in all areas of civil aviation, including aviation security – one of our highest priorities.

    Within hours of the 25 December incident which prompted this meeting, I was in contact with high-level officials of the States involved and initiated a series of actions to coordinate and harmonize responses to the terrorist act. We communicated with security authorities in all of our member States to encourage them to conduct risk assessments and implement appropriate screening measures.

    In addition, and at the request of the United States government, a meeting was held on 6 January at ICAO with officials from the Department of Homeland Security (DHS) and the Transportation Security Administration (TSA), headed by the Undersecretary for the National Protection Programme Directorate. The meeting focussed particularly on four areas: collecting passenger information; sharing information between States; requirements for travel document security, and training and technical assistance.

    ICAO provided briefings on each of these subjects, with particular emphasis on work being undertaken in the fields of Advance Passenger Information Systems, the Machine-Readable Travel Documents (MRTD) Programme, the Universal Security Audit Programme (USAP) and associated assistance activities and mechanisms involved in improving compliance with security Standards.

    The US Delegation expressed a strong desire to work with ICAO to address these challenges on a multilateral basis, including through the various initiatives that had been presented to them during the meeting, and by providing assistance to States in coordination with ICAO and other donor States.

    I would like to take this opportunity to commend the United States and all other States concerned for their quick and decisive action to contain the situation. The emergency measures implemented, which included increased gate screening for U.S-bound flights and passenger profiling, were necessary in light of the heightened level of risk. Although these measures have caused costly delays and inconvenience, the attempt to detonate an explosive device on board Northwest Flight 253 clearly justifies greater vigilance in the form of more stringent processes.

    The decision to implement emergency measures is never easy because of the enormous impact on overall operations. But these actions must be taken, to protect the integrity of the system and maintain public confidence in air travel.

    In this connection, I was invited to Washington on 14 January for a meeting, which took place within the framework of bilateral negotiations between authorities of the United States and the European Union, devoted to discussions on the consequences of the events of 25 December. At the outset, it was acknowledged that notwithstanding that the incident was aimed at an American aircraft, it represents a threat to the entire aeronautical community. Three main issues related to passenger safety were discussed:

    First, how long these measures would stay in force. It was recognized that the emergency measures adopted could not be permanent as resources being dedicated to these new processes were being drawn from other essential safety systems.

    Second, the issue of discrimination, whereby some States have difficulty in discriminating between measures applied to one set of passengers and not to another.

    The third issue related to the US Transportation Security Administration deciding on the basis of incoming information, the percentage of screening applicable to different passengers. This percentage would vary and be very difficult to establish.

    The meeting concluded with the two parties confirming that they would like to see ICAO adopt a leadership role.

    As we speak, ICAO remains in close contact with its Member States and industry to facilitate and implement a solid response to this latest threat. But we must not forget that aviation is a global system and it requires global solutions. I emphasize that my staff and I are dedicated to ensuring that security threats are addressed in the most effective and efficient manner possible and this means that responses must be implemented with the highest degree of harmonization worldwide. We have a strong record of doing just that.

    The challenge from ICAO’s standpoint is to achieve and maintain an optimum balance between the more stringent security measures required to address emerging threats and the processes intended to facilitate air travel by 2.3 billion passengers per year. This is obviously a tall order. While ICAO’s objective is to ensure air travellers reach their destinations as efficiently as possible, every one of these individuals is subject to screening. The challenge becomes more complicated when you consider that different States may implement different measures. A multilateral approach is more likely to avoid confusing passengers while boosting efforts by States and industry to strengthen the security of the air transport system. This is why collaboration is vital, and concepts such as one-stop security should be encouraged.

    Since this latest serious incident, ICAO has been working to promote international cooperation. Terrorism is a transnational issue, and the attempted sabotage of Flight 253 is reflective of a worldwide problem; as such, it calls for a global response under ICAO’s leadership, with the support of its member States.

    To respond effectively to threats over the long term, it is essential to review existing measures and strategy from time to time, and assess how these need to change to remain effective. In this regard, ICAO is fortunate to have a multinational team of security experts at its disposal. The next meeting of the Aviation Security Panel will be held in late March, and is expected to produce concrete recommendations on how to prevent incidents such as the attempted sabotage of 25 December. In responding to this incident, it will be necessary to consider both procedural and technological solutions to this security gap, including the possible use of imaging technology – commonly referred to as body scanners – as one means of screening passengers, taking into account privacy, data protection and health issues.

    The point is that every time a new type of incident arises, we face the prospect of introducing yet another layer of security. To do so when necessary, but without overburdening the industry, it is important to evaluate existing measures at the same time, and eliminating and revising provisions as required. In developing countermeasures to new threats, they must be practical as well as appropriate for the level of threat. Adapting to each new threat with minimum adverse impact on the air transport system remains a significant challenge.

    I want to assure you that this and other aviation security challenges have ICAO’s full attention, and are being addressed on a proactive basis. Through our panel of security experts, we are striving to identify new ways of anticipating the next threat. In this manner, an increasing risk can be managed by implementing practical countermeasures instead of requiring emergency action.

    The air transport system remains a highly attractive target for terrorist organizations precisely because it has been so successful in promoting global economic growth and has become an essential mode of transport. The volume of operations, and our reliance on an efficient air transport system, means we can expect aviation to remain a target. By remaining proactive, our intention is to ensure that civil aviation’s vulnerabilities are not exploited by terrorists.

    My message today is quite simple. ICAO is keenly aware of the challenges you face as operators and recognizes that whatever security measures are put into place, now and in the future, must not unduly disrupt operations or increase the cost of doing business. Our objective has always been to strike the right balance between protecting the safety of passengers while assuring they transit through airports as quickly and as efficiently as possible. We recognize that the health of the air transport industry, and its ability to support economic and social development around the world, are at stake.

    IATA, US Agree On Aviation Security Cooperation

    Geneva – The International Air Transport Association (IATA) hosted an historic aviation security summit with the US Department of Homeland Security (DHS). IATA Director General and CEO Giovanni Bisignani and DHS Secretary Janet Napolitano spearheaded a new era of industry/government cooperation to improve aviation security around the world.

    “The aviation industry is committed to keeping the global skies safe and secure. We live in a global world with global connectivity and global threats. The challenge is to protect the benefits of aviation connectivity and eliminate the threats. Governments and industry have the same goals but different expertise. Governments understand the threats and the tools needed to mitigate them. Industry has the operational expertise for effective implementation. Working together is the only way forward,” said Bisignani.

    The summit was held at IATA’s headquarters in Geneva and included the Secretary General of the International Civil Aviation Organization (ICAO), top executives from 25 airlines as well as participants from the US Government.

    Bisignani commended the fresh approach of the Obama administration to proactively engage industry. “We applaud Secretary Napolitano’s commitment to engage industry and find workable and effective solutions. A single meeting cannot solve all the security challenges we face but it is a major step in the right direction. We had a lot to teach each other and today is the start of a regular high-level dialogue on this critical issue. This cooperation should become a model for other countries to adopt,” Bisignani said. IATA and DHS agreed to hold a follow-up meeting in the coming weeks.

    During the meeting, IATA and its member airlines made several recommendations including:

    • Institutionalizing government/industry cooperation: This would allow security policies to be written with the benefit of airline operational expertise. IATA encouraged ICAO to create a template for such cooperation to be implemented globally.
    • Implementation: Recognize that prescriptive, one-size-fits-all regulations with numerical targets will not secure a complex global industry. Governments must work with industry to define practical implementation measures for their security targets.
    • Passenger data collection: Make passenger data collection and sharing more efficient: IATA urged DHS to break down internal silos to create a single data collection and sharing program that could serve as a model for implementation by other governments.
    • Harmonization across borders: Governments must talk to each other to ensure that one country’s requirements do not conflict with another country’s laws.
    • Next generation checkpoint: Along with optimizing the capabilities of current screening technology, we must begin to look at future checkpoints that combine technology and intelligence. “We need a checkpoint system that focuses on finding bad people, not just bad objects,” said Bisignani.
    • IATA (International Air Transport Association) represents some 230 airlines comprising 93% of scheduled international air traffic.

  • FAA Sends Temporary Air Traffic Control Tower to Haiti

    Additional Assistance Includes Ferry Service and Exemptions that Expand the Pool of Aircraft Available to Help in Relief Efforts

    At the request of the Haitian government, the Department of Transportation’s Federal Aviation Administration (FAA) dispatched a portable, temporary control tower to Haiti today to help assist with aircraft operations at Port-au-Prince International Airport. The tower is being transported aboard a large, chartered cargo aircraft and FAA technicians will install and prepare it for service which will take about 48 hours once it arrives in Port-au-Prince.

    “This tower is a visible representation of the Department’s contribution to our government’s swift and considerable efforts to help Haiti deal with this terrible tragedy,” said DOT Secretary Ray LaHood. “We had great support from the military in arranging for the transport of this critical equipment.”

    “We are all grateful for the incredible effort put in by FAA employees who have worked around the clock in Haiti and the Dominican Republic to help relief supplies and emergency responders arrive quickly and efficiently,” said FAA Administrator Randy Babbitt.

    The portable tower is 44 feet long, 13 feet high and eight feet wide, and weighs about 25,000 pounds. It comes with two diesel-powered generators and supporting fuel tanks, plus tools and other support equipment for installation and maintenance. The FAA uses this tower and others like it to support airports where existing towers are out of service after a disaster, like a major hurricane.

    Controllers providing terminal air traffic control services have worked outside at a folding table, using military radios to handle about 160 flights a day. The airport’s control tower was rendered unusable by the devastating earthquake on January 12. Tower controllers provide service to arriving and departing flights in the immediate area of the airport and serve as ground controllers for movements on the airport surface.

    Besides air traffic employees, the FAA has an airports division team on the ground to inspect and evaluate the physical condition of the runway as it handles a high volume of heavy military transport and cargo aircraft.

    Immediately following the earthquake, the Department granted two exemptions from current aviation restrictions, expanding the pool of available aircraft by making them easier to charter. The first exemption helps streamline the ability of the U.S. Government to procure charter air services on short notice. The second granted authority to a class of carriers that operate large aircraft in private carriage, allowing them to evacuate persons from and bring personnel and supplies to the disaster area. Without this exemption, such conduct could violate the Department’s economic licensing requirement for airlines.

    In addition to the FAA assistance, Federal Transit Administrator Peter Rogoff granted an emergency petition to the principal public ferry service in Puerto Rico to use the Federally-funded vessel the “Cayo Largo” to transport 65 rescuers, rescue vehicles, and medicine to the port of Baraona, Dominican Republic to help in the Haiti relief effort. The ship sailed Monday from San Juan.

    Also over the past week, the Department’s Maritime Administration (MARAD) mobilized six ships to assist with relief efforts in Haiti. Gopher State, Petersburg, Huakai, Alakai, Cornhusker State and Cape May are all either being prepared or currently loading. All are owned or controlled by MARAD, and will be crewed by civilian U.S. merchant mariners.

    Super Bowl Flight Advisory

    FAAST Blast — January 20, 2010

    In anticipation of a large number of aircraft operating to and from the South Florida metropolitan area during the 2010 Pro-Bowl and Super Bowl XLIV weeks, special procedures will be used to enhance safety and minimize air traffic delays. Among the security provisions for this event are TFRs, two-way communications, and discrete transponder requirements.

    Specific procedures for this event may be revised and access to some airports may be restricted. Pilots should be prepared to provide documentation, including personal identification, certificate number,aircraft ownership information, and a contact number. Pilots are also encouraged to check NOTAMs frequently to verify they have the most current information. TFR information is normally disseminated by FDC NOTAM three to five days prior to the event. Once published, text and graphic depictions of restrictions may be found on the following web site: www.tfr.faa.gov. The Super Bowl TFR is scheduled to be in effect on Feb. 7.

    ADS-B Debuts in Gulf of Mexico
    On January 12, 2010, FAA Administrator Randy Babbitt announced that an improved satellite-based system – Automatic Dependent Surveillance-Broadcast (ADS-B) – is now being used to more efficiently and safely separate and manage aircraft flying over the Gulf of Mexico.

    “This is a significant, early step toward NextGen,” Administrator Babbitt said. “We’re delivering on time, a system that’s not only more accurate than radar but comes with significant safety and efficiency benefits. This will save time and money for aircraft operators and passengers and reduce our carbon footprint.”
    Before the installation of ADS-B in the Gulf, controllers were required to maintain a 120-mile separation between aircraft. The new technology shrinks that to only five nautical miles, while increasing safety. The agency also can now provide more direct routes, which reduces fuel costs and improves efficiency.

    This is the nation’s second major installation of ADS-B equipment. Controllers at Louisville International Airport started using the technology last fall. This year, ADS-B is expected to become operational at Philadelphia International Airport, as well as in Juneau, Alaska. It is expected to be available nationwide by 2013.
    Finding and Fighting Fatigue
    “A good laugh and a long sleep are the best cures in the doctor’s book.” This simple Irish proverb offers some profound advice, especially to those involved in the aviation industry. Each year, fatigue is all too often a factor in aircraft-related accidents and incidents. Although airline operator mishaps garner the lion’s share of media attention, GA pilots and mechanics are subject to the same fatigue-related risks as are air carrier personnel.

    According to “Finding and Fighting Fatigue” in the January/February 2010 FAA Aviation News, the solution is amazingly simple, yet often difficult to implement: Get more sleep. In addition to providing tips to help airmen develop better sleep habits, the article also provides a sleep log which helps readers gauge how much sleep they get during a two-week period. The authors offer some sobering facts about fatigue. For instance, did you know that your performance level after being awake for 20 hours is equivalent to that of a legally drunk driver?   • 3 EMS Staff Deployed with Relief Groups

    Two Boston EMS uniformed staff members, EMT Mike Vojak and Paramedic Greg Bond, and Senior EMS Fellow Dr. David Hirsch, all volunteers with disaster medical organizations, were deployed last night to help with emergency response efforts after Tuesday’s earthquake in Haiti and the surrounding area. Each has been asked to report back on the conditions so that information can be relayed to other authorities and supportive assistance can be provided as applicable.

    Both EMT Vojak and Dr. Hirsch are members of DMAT-1, the Disaster Medical Assistance Team based out of Boston. Vojak has been with Boston EMS since 1982 and Hirsch since 2008. Disaster Medical Assistance Teams are under the jurisdiction of the National Disaster Medical System (NDMS), Federal Emergency Management Agency (FEMA) and Department of Homeland Security (DHS).

    Paramedic Bond, a member of IMSuRT-East, the International Medical Surgical Response Team based out of Boston, has been with Boston EMS since 1997. IMSuRT is a specialized team of highly trained personnel and is equipped to establish a fully capable free-standing field surgical unit anywhere in the world.

    Deployments typically last two weeks with a rotation of staff but depend heavily on the conditions on the ground.

    After production and distribution of the Cycle 1001 databases (effective date 14-Jan-2010),Honeywell was informed by our data supplier, Jeppesen, that the data file delivered to Honeywell contained incorrect Dynamic Magnetic Variations for all terminal and en route waypoint records.For that reason, it is imperative that the incorrect databases NOT be used for arrival, departure, or approach operations.

    Any of the following databases contain incorrect data that must be corrected before use:
    1. Cycle 1001 databases downloaded from Wingman Services prior to January 12th, 2010.
    2. Cycle 1001 databases on programmed media and received prior to January 12th, 2010.

    To fix this issue, a corrected database must be installed prior to the next flight. For all units except the KLN 94, refer to the applicable pilot’s guide for database update instructions. For operators using KLN 94 equipment who downloaded the Cycle 1001 database prior to 16 January 2010, please refer to Service Information Letter “Incorrect Dynamic Magnetic Variation Data on database cycle 1001” PN D201001000010.

    To obtain a corrected database:
    1. For customers with web distribution, download the new data file Cycle 1001 and load the new database in the unit.
    2. For customers with all other types of distribution, return the distribution media (except diskettes) to Wingman Services and a new Cycle 1001 will be sent back to be loaded into the unit.

    Note that if it cannot be determined whether the Cycle 1001 database falls within the categories listed above, or if there is any uncertainty about the correctness of the data, a corrected database should be installed.

  • Alternative Aviation Fuels Efforts Lauded


    January 14 – The Commercial Aviation Alternative Fuel Initiative (CAAFI), a Federal Aviation Administration (FAA) sponsored effort to develop environmentally-sound, sustainable-alternative jet fuels, has received “one of the most coveted and valued honors” by a leading aviation-industry publication. Air Transport World’s Joseph S. Murphy Industry Service Award is given to an individual or an organization that has performed outstanding service benefiting air travel.

    “This recognition is a strong endorsement of CAAFI’s innovative public-private partnership and recognizes the tremendous joint effort leading to significant advancements for alternative jet fuels this past year,” said FAA Acting Assistant Administrator for Policy, Planning and Environment Nancy LoBue. “Environmentally sound alternative jet fuel is a key priority for the FAA and it is rapidly becoming a reality.”

    CAAFI seeks to enhance energy security and environmental sustainability for aviation through alternative jet fuels. The initiative is a coalition of the U.S. commercial aviation community that acts as a focal point for engaging the emerging alternative fuels industry. It enables its diverse stakeholders to build relationships, share and collect needed data, identify resources, and direct research, development and deployment of alternative fuels. The initiative is fostering efforts to ensure critical issues in fuel safety, research and development, environmental impacts, and business and economic viability are addressed.

    CAAFI’s sponsors include the FAA Office of Environment and Energy and three trade associations: Airports Council International of North America (ACI-NA) representing commercial airports, the Aerospace Industries Association (AIA) representing U.S. manufacturers, and the Air Transport Association (ATA) representing U.S. airlines. CAAFI stakeholders are drawn from all elements of the international commercial aviation industry, fuel suppliers, universities, and multiple U.S. government agencies.

    The industry awards program was created in 1974 to recognize excellence. Originally a modest program, over intervening decades it has become of the most valued recognition a company or person in the airline community can receive. CAAFI will receive the award at a ceremony in Singapore on February 1.

    The Aviation Safety Network today released the 2009 airliner accident statistics showing a total of 757 airliner accident fatalities, as a result of 30 fatal multi-engine airliner accidents.
    Over the year 2009 the Aviation Safety Network recorded a total of 30 fatal airliner accidents, resulting in 757 fatalities and 1 ground fatality.The number of fatalities is lower than the ten-year average of 802 fatalities. The number of accident is on average.

    Of those 30 fatal accidents, 11 involved passenger flights. This is the lowest number in over sixty years.

    Five out of 30 accident airplanes were operated by airlines on the E.U. “black list” (as opposed to nine out of 26 in 2007 and three out of 32 in 2008).

    In 2009 Africa was again the most unsafe region: 30% of all fatal airliner accidents happened in Africa, while the continent only accounts for approximately 3 percent of all world aircraft departures.

    ADS-B Satellite-Based Tracking Allows Controllers to Monitor Air Traffic Over Gulf of Mexico

    WASHINGTON — Federal Aviation Administration (FAA) Administrator Randy Babbitt announced today that Houston air traffic controllers are beginning to use an improved satellite-based system – Automatic Dependent Surveillance-Broadcast (ADS-B) — to more efficiently and safely separate and manage aircraft flying over the Gulf of Mexico.

    “Safety is our highest priority at the U.S. Department of Transportation, and this new satellite-based technology will help the FAA improve the safety of flights over the Gulf even as air traffic increases,” said U.S. Transportation Secretary Ray LaHood.

    “This is a significant, early step toward NextGen,” Administrator Babbitt said in a press conference at the Houston Air Route Traffic Control Center. “We’re delivering on time, a system that’s not only more accurate than radar but comes with significant safety and efficiency benefits. This will save time and money for aircraft operators and passengers and reduce our carbon footprint.”

    ADS-B, which is one of the technologies at the heart of the transformation to NextGen, brings air traffic control to the Gulf of Mexico, an area that has not had the benefit of radar coverage. Before ADS-B, controllers had to rely on an aircraft’s estimated or reported — not actual — position. Individual helicopters flying under Instrument Flight Rule conditions at low altitudes to and from oil platforms were isolated within 20×20 mile boxes in order to remain safely separated from other helicopters. The complex, manual nature of these operations severely reduced capacity and efficiency for the 5,000 to 9,000 daily helicopter operations in the Gulf of Mexico.

    Aircraft equipped with ADS-B in the region will now know where they are in relation to bad weather and receive flight information including Notice to Airmen and Temporary Flight Restrictions.

    Prior to ADS-B, commercial aircraft flying at high altitudes were kept as much as 120 miles apart to ensure safety. Controllers are now able to safely reduce the separation between ADS-B equipped aircraft to five nautical miles, significantly improving capacity and efficiency. The new technology will also allow the FAA to provide new, more direct routes over the Gulf of Mexico, improving the efficiency of aircraft operations while using less fuel.

    The FAA was able to install ground stations on oil platforms as part of an agreement with the Helicopter Association International, oil and natural gas companies and helicopter operators. A network of ground stations was deployed on oil platforms and the surrounding shoreline, bringing satellite-based surveillance to an area with almost as much daily air traffic as the northeast corridor.

    The Gulf of Mexico is the second key site where ADS-B is being used by controllers to separate aircraft. The new technology is also being used by controllers in Louisville, KY, chosen in part because UPS voluntarily outfitted much of its fleet with ADS-B avionics. Four ground stations give controllers at the Louisville International Airport and the Louisville Terminal Radar Approach Control facility an ADS-B coverage area extending 60 nautical miles around the airport up to 10,000 feet.

    Controllers in Philadelphia will begin using ADS-B in February and the system will become operational in Juneau in April. ADS-B is expected to be available nationwide by 2013.

    The FAA first established an ADS-B prototype in Alaska, outfitting numerous general aviation aircraft with ADS-B avionics. The improved situational awareness for pilots and extended coverage for controllers resulted in a 47 percent drop in the fatal accident rate for equipped aircraft. In South Florida, the installation of eleven ground stations now gives pilots in equipped aircraft free traffic and weather information. Controllers will soon begin using ADS-B in that region to separate aircraft.

    Introduction
    This Special Airworthiness Information Bulletin (SAIB) advises registered owners and operators of
    the transmission of incorrect information from the Honeywell NZ-2000 Primus Epic Flight
    Management System (FMS) software having part number (P/N) VAR9XX, VAR10XX, or
    VAR11XX and installed on Hawker Beechcraft Corporation (Type Certificate Previously Held
    by Raytheon Aircraft Company) Model 4000 airplanes, Gulfstream Aerospace Corporation
    Model G-IV, GIV-X, and GV-SP airplanes, Agusta AW-139 helicopters, and Cessna Aircraft
    Company Model 680 airplanes.

    Background
    During an RNAV approach an airplane programmed with NZ-2000 FMS software (version 5.2), flew
    the WATTS THREE ARRIVAL (RNAV) Standard Terminal Arrival Route (STAR). A different
    runway was selected in the FMS during the STAR, and resulted in the FMS navigating the airplane
    toward the initial STAR waypoint instead of the next sequential STAR waypoint. As a result of this
    incident, Honeywell International Inc. issued Service Information Letter, D200909000044, dated
    October 15, 2009, to provide operators with guidelines for flight plan modifications made on the
    ARRIVAL page, including a destination runway change. This SAIB addresses the Primus Epic FMS
    because its software was a derivative of the NZ-2000 FMS software. At this time, the airworthiness
    concern is not an unsafe condition that would warrant airworthiness directive (AD) action under Title
    14 of the Code of Federal Aviation Regulations (14 CFR) part 39.

    Recommendations
    The FAA recommends that all owners and operators of Model 4000 airplanes, Model G-IV, GIV-X,
    and GV-SP airplanes, Agusta AW-139 helicopters, and Model 680 airplanes with NZ-2000 Primus
    Epic Flight Management System software ensure their flight crews are familiar with the procedures
    outlined in Honeywell Service Information Letter, D200909000044, dated October 15, 2009.

  • Five Reportable Incidents Added To Notification Requirements

    The NTSB is amending its regulations concerning notification
    and reporting requirements regarding aircraft accidents or
    incidents. In particular, the NTSB is adding regulations to require
    operators to report certain incidents to the NTSB. The NTSB is also
    amending existing regulations to provide clarity and ensure that
    the appropriate means for notifying the NTSB of a reportable
    incident is listed correctly in the regulation.

    On October 7, 2008, the NTSB published an NPRM titled
    “Notification and Reporting of Aircraft Accidents or Incidents and
    Overdue Aircraft, and preservation of Aircraft Wreckage, Mail,
    Cargo, and Records” in the Federal Register. This NPRM proposed,
    and the final rule codifies the addition of five reportable
    incidents, the reporting of which the NTSB believes will improve
    aviation safety.

    The operator of any civil aircraft, or any public aircraft not
    operated by the Armed Forces or an intelligence agency of the
    United States, or any foreign aircraft shall immediately, and by
    the most expeditious means available, notify the nearest National
    Transportation Safety Board (NTSB) office when an aircraft accident
    or any of the following listed serious incidents occur:

    • Failure of any internal turbine engine component that results
      in the escape of debris other than out the exhaust path.
    • In-flight fire.
    • Aircraft collision in flight.
    • Release of all or a portion of a propeller blade from an
      aircraft, excluding release caused solely by ground contact.
    • A complete loss of information, excluding flickering, from more
      than 50 percent of an aircraft’s cockpit displays known as
      Electronic Flight Instrument System (EFIS) displays, Engine
      Indication and Crew Alerting system (EICAS) displays, Electronic
      Centralized Aircraft Monitor (ECAM) displays, or other displays of
      this type, which generally include a primary flight display (PFD),
      primary navigation display (PND), and other integrated
      displays.
    • Airborne Collision and Avoidance System (ACAS) resolution
      advisories issued either when an aircraft is being operated on an
      instrument flight rules flight plan and compliance with the
      advisory is necessary to avert a substantial risk of collision
      between two or more aircraft, or to an aircraft operating in class
      A airspace.
    • Damage to helicopter tail or main rotor blades, including
      ground damage, that requires major repair or replacement of the
      blade(s).
    • Any event in which an aircraft operated by an air carrier lands
      or departs on a taxiway, incorrect runway, or other area not
      designed as a runway, or experiences a runway incursion that
      requires the operator or the crew of another aircraft or vehicle to
      take immediate corrective
    • action to avoid a collision.

    The final rule was published in the Federal Register January
    7th, and the revisions and additions published in this final rule
    become effective March 8, 2010.

  • U.S. Virgin Applauds DOT Decision

    Virgin America Cleared for Take-Off in 2010

    San Francisco – January 8, 2010 – Virgin America, a privately-held domestic airline, today applauded the Department of Transportation’s (DOT) decision confirming the carrier’s financial fitness and U.S. citizenship status. Earlier today, the DOT issued a ruling that the airline and its proposed new ownership structure will remain fully compliant with U.S. ownership laws. U.S. law requires that domestic airlines remain under the control of U.S. citizens, with no more than 25% of the voting stock being held by foreign citizens. As a matter of policy, the DOT reviews any proposed ownership changes of over 10 percent of the voting stock of any U.S.-domiciled airline.

    “Today is a great day for airline competition, for our traveling guests and for our teammates,” said Virgin America President and CEO David Cush. “We’re extremely pleased with the Department of Transportation’s announcement. With this behind us, we intend to focus on what we do best: injecting new competition into markets as we grow, creating new jobs and delivering an unrivalled guest experience.”

    The proposed new ownership structure will reflect a change among Virgin America’s U.S. investors and also allow the Company’s teammates to participate in the airline’s future via an Employee LLC. Under the new structure, the airline’s voting stock will remain in its previous configuration with 75 percent held by the U.S.-based VAI Partners, LLC and 25 percent held by Virgin Group. However, the U.S. investors comprising VAI Partners will change with the following entities now comprising the VAI stake:

    Cyrus Aviation Investor, LLC – 55.5 percent
    VAI MBO Investors, LLC—27.8 percent
    VX Employee Holdings, LLC—16.7 percent
    VAI Management, LLC—less than 1 percent
    Cyrus Aviation Investor, LLC is an entity related to long-time Virgin America investor, Cyrus Capital. Under the new structure it will substantially increase its investment to become the airline’s principal U.S. investor.

    VAI MBO Investors, LLC is comprised of five individual investors. Four of the investors, Don Carty, Sam Skinner, Cyrus Freidheim, and David Cush, are current Virgin America Board members, and a fifth, Robert Nickell, will assume a Board seat at the closing of this transaction.

    VX Employee Holdings, LLC is an entity established to hold the economic value of 16.7 percent of VAI for distribution to the employees of Virgin America at the time of a qualified liquidity event, such as an initial public offering.

    “Given that our success to date is due to the efforts of this entrepreneurial start-up team, we are pleased that our teammates will now have a stake in the financial success of our airline,” added Cush.

    As part of the proposed transaction, Virgin America will obtain an additional $68.4 million in unsecured debt from its shareholders. This facility will further improve the liquidity of the company, positioning it for additional growth in 2010 and beyond.

    The company will continue to have a U.S. controlled Board of Directors (with seven of nine seats held by U.S. citizens). Although a privately-held company, the airline is announcing these details given DOT’s public ruling earlier today.

    Since its August 2007 launch, the San Francisco-based Virgin America has created more than 1500 new jobs, swept all the major reader-based travel awards, and lowered fares and improved service in the new markets it has entered. The start-up airline’s most recently reported financial results included the announcement of its first quarterly operating profit, a year-over-year revenue increase of 38 percent, record load factors and improved unit costs for the third quarter of 2009.

  • NTSB TO RELEASE FACTUAL REPORTS ON FATAL 2009 HELICOPTER ACCIDENT IN LOUISIANA

    The National Transportation Safety Board will open its public docket Thursday on a January 2009 helicopter accident that killed 8 persons.

    On January 4, 2009, a Sikorsky S-76C++ helicopter (N748P), operated by PHI, Inc., crashed into swampy terrain just north of the Gulf of Mexico coastline near Morgan City, Louisiana. The aircraft was on a Part 135 revenue flight to an oil platform when it crashed about 7 minutes after takeoff. Eight of the nine persons aboard lost their lives.

    The docket will contain factual reports prepared by the investigative team, including reports on aircraft structures, aircraft systems, the transcript of the cockpit voice recorder, and toxicology reports. A comprehensive materials laboratory report on the windscreen structure and a report on the flight data recorder information will be added to the docket in the next several weeks.

  • FAA OKs Four More Portable Oxygen Concentrators

    WASHINGTON — The Federal Aviation Administration (FAA) announced today that passengers needing medical oxygen now have the option of using four additional portable oxygen concentrators while traveling, bringing the total number of approved units to 11.

    The newly authorized devices are DeVilbiss Healthcare’s iGo, International Biophysics Corporation’s LifeChoice, Inogen’s Inogen One G2 and Oxlife’s Independence Oxygen Concentrator.

    “This is our latest step toward making air travel more accessible for people who need medical oxygen,” said FAA Administrator Randy Babbitt. “We want these passengers to have as many options as possible while ensuring all of our safety guidelines are being met.”

    Portable oxygen concentrators are small, portable devices that separate oxygen from nitrogen and other gases in the air and provide oxygen to users at greater than 90 percent concentration. They do not use compressed or liquid oxygen, which the government classifies as hazardous material.

    The Department of Transportation requires that U.S. carriers allow passengers to use portable oxygen concentrators approved by the FAA during all phases of a flight – including taxiing, takeoff and landing – if the unit displays a manufacturer’s label that indicates it meets FAA requirements for portable medical electronic devices, is properly stowed, and meets certain other applicable safety-related conditions.

    For example, passengers must ensure the unit is in good working order and must be able to respond to the unit’s warning alarms. They must protect extra batteries in carry-on baggage from short circuits and physical damage. Carriers also must let passengers operate these FAA-approved concentrators while moving about the cabin whenever the captain turns off the “Fasten Seat Belt” sign.

    Carriers can legally refuse to allow inflight use of an FAA-approved portable oxygen concentrator that does not have a manufacturer’s label indicating that the device complies with FAA requirements. However, the Department encourages airlines to voluntarily allow the inflight use of such devices. Inflight use of an FAA-approved portable oxygen concentrator, whether labeled or not, poses no safety danger so long as carriage of the device is in accordance with instructions provided by the FAA.

  • IATA Comment on New US Security Procedures

    Geneva – IATA understands the need for government-mandated emergency security measures as a result of the attempted downing of Northwest Flight 253 on 25 December 2009. Security is a government responsibility. Emergency measures should be revised as information is gathered in the investigation.

    The 3 January announcement by the US Transportation Security Administration (TSA) to replace the original emergency requirement for 100 percent pat-down screening of all US-bound passengers with threat based and random screening is a welcome step in the right direction.

    The TSA announcement is in line with requests made by IATA in a letter to Janet Napolitano, Secretary of the Department of Homeland Security (DHS) on 29 December. “It is critical that DHS partner with industry to identify the most effective and efficient ways to address this challenge going forward. Clearly, the air transport system cannot support 100 percent pat-down searches over the long term. However, a smaller percentage of intensive pat downs accompanied by trace detection technology may reduce delays and achieve near-term security requirements as we focus on longer-term technology solutions to address the ongoing threat,” said Giovanni Bisignani, IATA’s Director General and CEO.

    Long-term, sustainable aviation security must be globally harmonized, risk-based and have efficient processes for passengers.

    One lesson already learned from this incident is the importance of combining screening procedures with intelligence. Following the new TSA announcement, IATA calls on DHS and TSA to work with their international counterparts to look at a next generation checkpoint. This should give screeners access to effective intelligence to deliver proportional screening measures based on intelligence driven risk assessments.

    IATA continues to urge the US and other governments to coordinate security requirements and to work closely with industry for their effective implementation.

    Airworthiness Directives: McDonnell Douglas Corporation Model MD-11 and MD-11F Airplanes

    SUMMARY: FAA is adopting a new airworthiness directive (AD) for certain
    Model MD-11 and MD-11F airplanes. This AD requires a one-time
    inspection to determine if wires touch the upper surface of the center
    upper auxiliary fuel tank and marking the location, if necessary; a
    one-time inspection of all wire bundles above the center upper
    auxiliary fuel tank for splices and damage; a one-time inspection for
    damage to the fuel vapor barrier seal and upper surface of the center
    upper auxiliary fuel tank; and corrective actions, if necessary. This
    AD also requires installation of nonmetallic barrier/shield sleeving,
    new clamps, new attaching hardware, and a new extruded channel. This AD
    results from fuel system reviews conducted by the manufacturer. We are
    issuing this AD to reduce the potential of ignition sources inside fuel
    tanks, which, in combination with flammable fuel vapors, could result
    in fuel tank explosions and consequent loss of the airplane.

  • Secretary Napolitano Dispatches Senior Department Officials to Review Security Procedures With International Airport Leaders

    Secretary Napolitano to travel in the coming weeks to build on these efforts

    Secretary of Homeland Security Janet Napolitano today announced that she is dispatching Deputy Secretary Jane Holl Lute, Assistant Secretary for Policy David Heyman and other senior Department officials on a broad international outreach effort to meet with leaders from major international airports in Africa, Asia, Europe, the Middle East and South America to review security procedures and technology being used to screen passengers on flights bound for the United States.

    “As part of the ongoing review to determine exactly what went wrong leading up to Friday’s attempted terrorist attack, we are looking not only at our own processes, but also beyond our borders to ensure effective aviation security measures are in place for U.S-bound flights that originate at international airports,” said Secretary Napolitano. “Because I am fully committed to making whatever changes are necessary to protect the safety of the traveling public, I am sending Deputy Secretary Lute and Assistant Secretary Heyman to work with our international partners on ways to collectively bolster our tactics for defeating terrorists wherever they may seek to launch an attack, and I will follow up on these efforts with ministerial-level meetings within the next few weeks.”

    Deputy Secretary Lute and Assistant Secretary Heyman will first travel to Europe , departing on Monday. While there, they will brief European authorities on the findings of President Obama’s aviation security review and then report back to Secretary Napolitano on their discussions on enhancing international security measures.

    Following the attempted attack on Christmas Day, the Transportation Security Administration (TSA) issued a directive for additional security measures to be implemented for last point of departure international flights to the United States , such as increased gate pat-downs and bag searches. At the direction of the flight crew, passengers may also be asked to follow additional instructions, such as stowing personal items, turning off electronic equipment and remaining seated during certain portions of the flight.

    Other security measures implemented have included the deployment of additional law enforcement at airports, air marshals, and explosives detection canine teams. TSA will continue to work with airline and law enforcement authorities, as well as federal, state, local and international partners to put additional security measures in place to ensure that aviation security remains strong.
