    Where: 700 kilometers (450 miles) southeast of Moscow near the Pugachov aerodrome
    When: just before noon (0900 GMT) Tuesday.
    Who: 3 people aboard were killed
    Why: The flight data recorders have been recovered and are being used in the investigation.

    Unexplained Helicopter Crash in Grande Cache

    What: Sarvair AviationRobinson R44 (owned and operated by Klint Sarver) en route from Grande Prairie to Grande Cache
    Where: about 50 km before reaching its destination Grande Cache, Alta
    When: 6 p.m. Friday night. (rescue saturday Jan 31) ,
    Who: Passenger and pilot are recovering.
    Why: The crash is under investigation; it is thought that the helicopter flipped and slammed into a mountain. The pilot has a punctured lung and other injuries; and was transported to Grande Prairie. Passenger was uninjured.

    Why: a transmission oil temperature light came on forcing the pilot to make an emergency landing.

    Who: pilot and a female passenger survived the crash

    When: 6:15 p.m. Saturday evening.
    Who: pilot and 2 nurses
    Why: The pilot indicated that a bird strike precipitated the emergency landing

    Siberian Helicopter Crash

     


    What: Mi-2 helicopter hired by Yuganskneftegaz
    Where: Siberian region of Khanty–Mansi
    When: Monday January 12 2009
    Who: Dmitry Podgorodinsky, the copilot Victor Morozov, and two passengers Denis Inotsenko and Sergey Sokolov
    Why: The helicopter was found near the Obninsk oil deposit 50 kilometres away from the city of Nefteyugansk. All those onboard were killed in the crash. Pilot’s documents and samples of fuel have been taken from the scene. The autopsies of the victims and also an examination of the helicopter’s fuel are in progress. The helicopter had been hired to fkt over the company’s area oil wells. The flight was found at 23:40 local time by 3 search helicopter/rescue crews from aviation companies Convers Avia, UTair and Yugra Avia.

    Black Hawk Helicopter Crashes at Texas University; 1 killed, no students hurt

    Where: campus of Texas A&M University near the Corps of Cadets field on the school’s College Station campus, about 100 miles northwest of Houston
    When: Jan-12-09
    Who: 4 Army National Guard and an Army lieutenant assigned to the school’s ROTC unit were the only ones aboard. Three men are in critical condition at College Station Medical Center. One is at St. Joseph Regional Health Center in Bryan
    Why: A witness described that after lift off, the helicopter seemed to lose control, spun then dropped straight down.

    Missing Helicopter Crashed in Siberian Mountains. 7 Dead


    What: Mi-171 helicopter owned by the Russian state gas giant Gazprom
    Where: snow-covered mountain region of Siberia near Chornaya Mountain in the rugged Altai region.
    When: Friday Jan 9 2009
    Who: eleven passengers: Seven people were killed including Russia’s lower house of parliament rep Alexander Kosopkin; Four survivors were being treated in the city of Barnaul
    Why: The passengers had been on a hunting expedition. Under investigation is the possibility that poor quality fuel caused the helicopter’s engines to break down. Initial reports of eight dead were corrected to seven. No indication is made if crew is included in the passenger count.

    Utah Helicopter Crash

    Where: on the outskirts of West Valley City as it was headed to airport No. 2 in West Jordan
    When: 01/09/2009
    Who: pilot Scott Robinson and passenger Anthony Meredith both taken to Intermountain Medical Center
    Why: The helicopter departed from Provo on an IFR training flight to Windover.
    The pilot attempted to fly around the northern end of the Oquirrh mountains and flew into bad weather.

    Brazil Helicopter Crash


    What: AgustaWestland A109C Helicopter en route from the coastal resort town of Parati to Itupeva
    Where: (near Sao Paulo) in Brazil’s northeastern state of Ceara.
    When: Monday 01/05
    Who: Pilot and copilot were hospitalized. One businessman (Gilberto Ramalho) was the only fatality, 5 in all were injured.
    Why: The pilot was about to land at Ramalho’s famiy farm in Itupeva when the helicopter exploded. On board were the pilot, copilot, a business couple, their teenage daughter and her friend. Three passengers got out quickly–the pilot and copilot were severely burned. The security camera at Ramalho’s farm recorded the impact.

    PHI Medical Evacuation Emergency Landing

    Where: emergency landing about 11 miles south of Lafayette–a parking lot at U.S. 231 and Indiana 28 in Romney
    When: Monday afternoon
    Who: A pilot, nurse, paramedic and patient
    Why: The PHI Bell 407 medical helicopter a patient when it was forced to land due to a mechanical problem Monday afternoon. An ambulance carried the patient to St. Elizabeth’s Hospital in Lafayette.

    Federal Officials Investigating Louisiana Crash


    What: Sikorsky S-76C helicopter, operated by PHI Inc
    Where: Louisiana marshland – Bayou Penchant area about 100 miles from New Orleans in southwestern Terrebonne Parish
    When: Sunday Jan. 4, 2009
    Who: eight fatalities, one survivor. The helicoptor was carrying two pilots and seven passengers The dead were identified as Allen Boudreaux, of Ama, La.; Andrew Moricio and Ezequiel Cantu of Morgan City, La.; Randy Tarpley of Jonesville, La.; Jorey A. Rivero, of Bridge City, La.; Charles W. Nelson of Pensacola, Fla.; Thomas E. Ballenger of Eufaula, Ala. and Vyarl W. Martin of Hurst, Texas. The injured survivor is Steven Yeltin was medevacted; he is listed as being in critical condition and is awaiting to be transferred to Oschner Medical Center from the Leonard J. Chabert Medical Center in Houma. Mike Moreno, CEO of the Moreno Group along with Dynamic Industries, employed five of the victims.
    Why: The helicopter is believed to have crashed about seven minutes after it took off at 3 p.m, A distress signal was received just after 3. The NTSB investigation is well underway and the black box has already been recovered.

    Wind, Storm Crashes Helicopter in Santa Clarita


    What: Swanson Aviation Group Helicopter hired by Southern California Edison to string electrical line
    Where: Santa Clarita area north of Los Angeles behind Lombardi Ranch on Bouquet Canyon Road.
    Who: 1 fatality. The pilot sustained minor injuries.It is unclear if Curtis Cauthen of Ashfork, CA.–the mechanic on the ground– worked for the helicopter company or the electrical company.) Curtis was caught by the rotor blades and sustained fatal injuries.
    Why: The helicopter was hoving above-ground when a gust of wind wind caused the helicopter to spiral.

    Sabine Helicopter Crash update


    What: Helicopter owned by leasing company Rotorcraft en route to an unmanned oil rig platform 18-miles off-shore
    Where: Gulf of Mexico, about two miles below Sabine Pass at the border of Texas and Louisiana
    When: Thursday, Dec. 11, 2008
    Who: 5 on board. The pilot of the helicopter was identified as Joe Laugelle
    Why: Cause is under investigation; however there was a weather advisory in effect at the time of the crash
    One of the victims of the crash –Jacob Matt–has washed ashore near the McFaddin Wildlife Refuge. One victim John Larry Conn, was buried Sunday. James Cody Smalts who had just moved in with his fiance, Monica Lopez was buried Monday. One victim –Nick Hebert from Basile, Louisiana–hasn’t been recovered.

    At the time of the crash, air temp was 36 degrees; water temp 62 degrees. Wind was from the west at 23 knots, and there was a weather advisory. The body of John Laguelle is one of those recovered. Two other crew members whose bodies were recovered were wearing personal floatation devices. A search is ongoing for two others. All four passengers were crew members for a contract customer of Rotorcraft. The pilot also worked part-time as a helicopter pilot for a television station in Boston.

  • Helicopter flips on Tohopekaliga lake


    What: helicopter spraying herbicide on Lake Tohopekaliga Florida
    Where: near Southport Park near the southern end of the 18,810-acre lake.
    When: around 10:30 a.m.
    Who: only the pilot was aboard. He was uninjured.
    Why: The helicopter flew too low, touched the water and flipped into the lake.

    South Florida Water Management District says no fuel or herbicide spilled into the lake

    Where: football field of Alondra Intermediate School in Paramount

    Why: After the pilot reported a problem with the engine drive belt, he made a successful emergency landing.

    Casualties Released in Helicopter Crash


    What: 2007 Robinson R44 Helicopter en route from Saint Anne des Plains, Que. to the pilot’s cottage site at Lac Simon
    Where: northeast of Ottawa, in Lac Simon in western Quebec
    When: 6 p.m. Wednesday
    Who: Leo Blanchet, 63, and pilot Serge Louiseize, 49. Two survivors swam to shore in 1 C water. One is Mario Louis-Seize (the pilot’s brother); the other a company employee.
    Why: The pilot’s brother said as his brother was trying to land, the helicopter’s rotor hit the water, bringing the aircraft to a standstill.

    Witnesses saw it fall from the sky into the lake “6 p.m. Wednesday, entering the lake about 50 metres from shore, where the water was about 12 metres deep.”

    Police divers, local firefighters and a Canadian Forces were searching for the missing men whose bodies were recovered Thursday Morning. The survivors are recovering from hypothermia in a Buckingham hospital.
