A Small Point about the NTSB and Investigations

We’d like to remind people that NTSB investigations take a year or more.

We post news as it comes. The media being what it is, we hope everyone that reads our posts realizes that tertiary sources gleaned through the media are speculative at best. This speculation, published or not, does not compare in merit to NTSB investigations that take a year or more, and are examined with a fine-toothed comb in order to determine causes and consequences.

While the inexperience of the Asiana Crash on this particular plane at this particular airport may be a contributory cause, there may also be underlying causes that were behind autopilot being switched off, or the speed decreasing enough to cause a stick-shaker alert.

See a Boeing 777 stick shaker test below:

The NTSB has an extensive description of the investigative processes here:

    http://www.ntsb.gov/investigations/process.html

Obviously, the brief summation of news teams and even witnesses can not compare to the depth and breadth of the official investigation.

    Asiana Interviews

Some stories of the Asiana Crash in San Francisco

Currently there are 51 hospitalized, 8 in critical condition, one child. Five fire attendants are being treated for burns.

Federal investigators talked to the pilots why they shut down the autopilot 82 seconds before landing. This was the first time the pilot had landed a 777 at this airport. When the fire started in the middle of the plane, there were passengers still in the back that had to walk through the smoke to exit, passing people trapped in their seats. Flight attendants heroically got passengers out of the plane, and tried to put out the fire.

One passenger Eugene Ra describes the instants before the crash, looking out the window knowing they were too low. THe plane’s impact was powerful enough that it snapped the plane, and the spines of at least two of the passengers. The cabin filled with smoke and fear. After the plane stopped, there was silence.

The plane flew in too low and too slow, initiating a stick shaker (stall) warning, and struck the sea wall, severing art of the tail.

    * A cautionary note: The official investigation of the cause of the crash will take a year or more. No matter what news releases or speculations come about before the official investigation is just speculation. We do not know, for example, if some part or software in the plane malfunctioned, leading the pilots to respond as they did.

Breaking News: Asiana Airlines Crash in San Francisco

On July 6 2013, an Asiana Airlines Boeing 777 Flight 214 en route from Taipei made a hard landing on runway 28 at San Francisco international airport.

Rescue services—firefighters and ambulances—responded to the scene.

Passengers exited via air slides. On the video below, it appears the tail is missing. Other video shows the top of the aircraft is charred.

The Asiana Airlines #HL7742 landed hard, slid and/or cartwheeled, and came to rest with landing gear not visible. Debris–apparently the landing gear–is scattered down the runway. Smoke appeared to be coming from the side of the plane opposite from where passengers were exiting via slides. After the plane was on the ground, witnesses saw parts break off and a ball of fire. The attitude of the plane seemed normal but an apparent tailstrike may have damaged systems and limited flight control.

Air traffic to San Francisco international airport has been suspended.

Weather at the time of the accident (11:28 LT / 18:28 Z) was repored as:
    KSFO 061756Z 21006KT 10SM FEW016 18/10 A2982 RMK AO2 SLP097 T01780100 10183 20128 51005

    Commercial Plane Comparison

    June 17. 2013
While the upbeat “video below” compares basic features, the real world of aviation is focusing on a comparison of the problems noted.

    • Airbus (350) had a “blockage in the drilling station and the knock-on effect hampering wing production” and problems with suppliers of A350 structural components. A350 also face the looming challenge of obtaining certification
    • Boeing (787) which already graduated to the market, has demonstrated issues with 787’s lithium-ion batteries which use 10 times more power during initial start-up than other Boeings. In-flight monitoring made a demonstrable difference in reliability. A recent report estimates that 29 flights had been “saved” because of the 787’s extensive monitoring system

    Plane Crash Investigation Reveals Cause

On April 29, we reported the National Air Cargo flight that crashed when it’s load shifted and the plane stalled. Federal investigators and Boeing experts went to Afghanistan to to assist in the investigation.

On Jun 2nd 2013, the Ministry of Transport and Civil Aviation of Afghanistan investigation announced officially that it was the weight of three armored vehicles and two mine sweepers 80 tons––of shifting cargo that caused the plane to crash.

The wiring in the back of the plane was cut by impact with the cargo, leaving the pilots helpless to control the plane.

    There were no survivors.

As a result of the April Crash, on May 17, 2013 Boeing issued the following SAFO:

    Teenager Sues Masturbation Airlines

Seventeen year old Monica Amestoy was flying home from a debate in New York—a six hour flight—and witnessed an unwanted exhibition: a man across the aisle was masturbating. She pointed out the incident to Flight attendants.

After nothing was done to stop the man, seventeen year old Monica Amestoy returned home to California to file a lawsuit against United Airlines.

    The incident happened last year.

Although Monica complained to a flight attendant, United failed to safeguard Monica. Subsequently, United took no action and are surely guilty of neglecting the welfare of their passengers.

On landing in Los Angeles, UA failed to immediately report the criminal activity. Monica reported the incident to her father and he contacted law enforcement. After the investigation, the FBI prosecuted and convicted the perpetrator.

Gloria Allred filed the suit in Los Angeles county superior court.

BA Airways emergency landing with Engine fire

A British Airways Airbus A319-100 was en route from London to Oslo when it developed an engine problem.

Passengers heard a loud explosion, and saw smoke from the right engine. Apparently the engine doors blew off.

During takeoff, the casing of the left engine ‘came away’ and the right engine exploded when the plane made a quick return, landing in London.

Good thing that Emergency services were on standby to extinguish the engine fire.

Passengers and crew made an emergency evacuation via emergency slides.

Emergency belly landing at Porto Seguro

    Contact Carlos A. Morillo Doria

An Embraer EMB-721C Sertanejo (PA-32R-300) en route from Prado to Porto Seguro had a little trouble on landing when the nose gear folded.

The friction from the belly landing was controlled by foam applied by the firefighters who were on standby when the flight landed. Actually, you can see they were there ahead of time, and how they used the time while the pilots were burning off fuel.

Pilots were aware of the inability of the gear to lock, so they burned off fuel before making the landing.

The plane is normally used as an air taxi. The video below shows the plane landing on foam.

    No injuries. Good job, Captain!
