    INDIA’s DGCA Asserts Co-Pilot Assertiveness

    Following the Air India Express tragedy in Mangalore India (and definitely in response to it), the DGCA (India’s Directorate-General of Civil Aviation) has been trying to shut the gate after the horses are out. They’re still pointing their fingers at the pilot.

    The tape of the crash records the co-pilot Capt H S Ahluwalia (*i.e. the pnf) insisting to Commander Capt Zlatko Glusica to initiate a go-around. The Captain (**i.e. the pf) did not, and the plane landed too far down the runway, clipped an antenna, crashed thru the end of the runway, off the edge of the runway plateau. We can only assume the captain ignored his co-pilot, as no go-around was initiated.

    Apparently the pilot was in error and the co-pilot knew it. It must have been obvious to Ahluwalia that they were landing at the wrong angle too far down (the middle) of the runway–but he did not act on his knowledge. At any rate, he was in disagreement with his superior officer.

    Consequently, the DCGA has issued an operations circular highlighting procedural assertiveness for the co-pilot when he knows the pilot is wrong. The Mangalore tragedy is not mentioned by name, but it is certainly the silent subtext.

    Did the crash occur because the co-pilot did not take the controls? What exactly will this circular accomplish? Had this circular been public already, would the pilot and co-pilot have ended up in a wrestling match over controls of the airplane?

    It will be interesting to hear from pilots about what they think the consequences of this directive might be, and if they think a cultural context may play a part.

    I always turn to the experts. Right now, before the full investigation is out, I am hearing a lot of speculative “pilot error” but it is all speculation. But is it all pilot error?

    My experts remind me that “Every incident is linked to a chain of events. Each event alone is not sufficient reason to provoke an accident although when they link together the chances are multiplied and sometimes the accident occurs.” ( I am quoting a pilot.)

    For example, if pilot fatigue were a contributing factor (landing at dawn after a long tiring duty cycle), isn’t the carrier’s policy on pilot fatigue a contributory factor?

    The Boeing 737 landed midway on runway 24, and then overshot it, crashing into a gorge. Initial reports on the civil aviation website showed runway 27 mislabeled as runway 24. The actual crash site was not even pictured, a mistake that certainly casts doubt on the quality of the investigation, and possibly even the quality of the resources available to pilots at the time they were negotiating a landing. What did their map say? Is a pilot alone in error if the resources provided him are in error? I know I am digressing, but the point I want to make is this:

    I don’t have years of flight school, or engineering training but I do have opinion. And in my opinion, pilot error alone does not cause a crash like this.

    What: Air India Express Boeing 737-800 en route from Dubai to Mangalore
    Where: Mangalore airport
    When: 6:00 a.m May 22 2010
    Who: 160 (?) passengers including 23 children, 6 crew= 158 fatalities and 8 survivors
    Why: Air India Express flight 812 attempted touch down was around the middle marker. (Conflicting) reports are that it overshot the runway, and that the pilot intended a go round (tapes reveal the co pilot was urging a go-round) but the plane hit the localizer antenna at the runway’s end, plowed through the perimeter losing part of a wing, and went down 75 feet into a ravine, and broke into pieces and burst into flames. On touchdown, there was a reported “bang” which may have been a tire bursting. A drizzle started after the accident. From 5 to 10 people have been hospitalized, but according to reports there are possibly 3 survivors.

    Survivor Abdul Puttur suffered burns. He was seated near the wing exit and said that the pilot had announced the landing, then there was a thud. He saw huge flames after the plane fell into the ravine and then he jumped out from the back entrance. Another passenger said the pilot made no announcement.

    The Serbian pilot was Zlatko Glusica, first officer was S.S. Ahluwalia.

    According to DGCA rules, Mangalore International Airport is deemed a “critical airfield” which means that “supervised take offs and landings” are prohibited. Only the captain (not the first officer) can pilot take-offs and landings. Mangalore International Airport has a controversial table top runway and which forces pilots to make a precision landing. The airport has no buffer zone, and pilots landing planes must fly precisely or risk hurtling off the edge.

    Pulling A Slater, Round Two

    The saga continues:

    One thing that is coming out is that there’s more to the story, that the flight attendant’s problems started at the beginning of the flight with a couple of women who were fighting over bin space, that a woman (accidentally?) struck Slater on the head with her luggage, that a woman was enraged her checked luggage wasn’t handy.

    The original story:

    It’s being posted as news now that Steven Slater has been granted bail. Whether or not he can afford to pay it is questionable. Maybe one of his new media cubbies will start a fund.

    Of course they have to grant him bail.

    Are they going to lock him in jail and throw away the key for having a melt-down on his job?

    Have the abusive passengers been charged with unruly behavior, or does Jet Blue not concern themselves with the safety and well being of their employees?

  • Pulling A Slater

    By now probably everyone has heard about the Jet Blue flight attendant who flamboyantly quit his job.

    If you haven’t heard, this is the buzz: Steven Slater was at work at the end what must have been a very stressful flight ending at Kennedy.

    A passenger retrieving items from over the seat failed to listen to Slater’s directives, and cussed Slater out. Slater got beaned in the head by this passenger’s falling luggage, failed to get an apology, and cussed out the passenger over the intercom. After blowing off steam, Slater grabbed a beer, evacuated from the plane via the emergency slide, and headed for home. Note that he had the self-restraint not to punch out the rude flyer.

    Slater was arrested at his home and charged with 2nd-and 4th-degree criminal mischief, 1st- and 2nd-degree reckless endangerment and criminal trespass in the 3rd degree.

    Like thousands of others hearing about this story, I have an opinion–and that is that both the passenger and Slater (and possibly the media) need a time out.

    (A little overboard there on the charges, even if Slater is one of those surly attendant types. As a frequent flyer, I’ve come across surly flight attendants who could use charm lessons or another job; I’ve also come across surly passengers who tested flight attendants to their max. The guy should get some slack.)

    Both of them should have had escorts off the plane. But as far as this being a criminal offense on Slater’s part–that’s going off the deep end. Maybe Jet Blue will spring for Slater to have an anger management course–and maybe the passenger should take a class in manners. One wonders just what Slater had to put up with during that flight.

    Consider this: Just because you buy a ticket that packs you into a glorified flying sardine can in close quarters with hundreds of other ticket-purchasers, why should you have to have any manners at all? Certainly the ticket buys you the right to cuss out the attendants who ARE THERE TO SEE TO YOUR SAFETY. After all, you’re the only person who matters aboard the plane. Wait. Maybe not. Maybe the ticket assumes that the passenger is going to behave like a civilized human being.

    I hope that whoever decided these ludicrous charges will consider that Slater did not do any physical harm to the rude/unruly passenger (who would have been arrested if Slater’s breakdown had waited a few moments longer.)

    Maybe Slater considers the drama of deploying the emergency slide is worth the cost. I hope so, because that cost is probably going to be coming out of his soon-to-be-unemployed-pocket. It remains to be seen. Anyway, let’s see how long it takes for the term “Pull a Slater” to enter the national vocabulary; he already has a huge fan base, possibly a future with SNL and may end up in a National Lampoon Vacation movie. There’s already a facebook page called ” I Hate The Mf That Called Steven Slater A MF”

    Fake Boeing Disaster at Harare Airport

    Although the head of Zimbabwe’s Civil Aviation Authority David Chawota confirmed with the BBC: “I can confirm that a 767 plane coming from London has had an accident at Harare airport,” the whole thing was a drill with paid actors.

    BBC’s Brian Hungwa reported seeing a large plume of smoke and helicopters over the runway.

    Apparently no one but a chosen few were informed it was a drill. Apparently seeing what the media would do was part of the Zimbabwe’s Civil Aviation Authority’s “exercise.”

    Maybe I’m out of line here, but it seems to me a drill is supposed to test responses in-house.

    The fire drill at a school, for example, involves firemen arriving and evaluating the response time of those endangered.

    But if someone at the school sets off an alarm to see what would happen, or calls 911…they’re criminally liable. Okay, well maybe they were testing emergency services which for the CAA might be considered in house.

    But testing the media? Allowing the news to go out to the media that there was a disaster? As someone who deals with people suffering horrible loss from disasters that are real, this sounds more like a cruel joke than a drill. I wonder what the IATA and ICAO will have to say about this. At the very least, it is unethical.

    Chawota said “This was a usual drill for the period. We do this once every three years. This was for the benefit of our police personnel, security, aviation, medics, hospital — everyone was involved. This exercise helps the general preparedness of our facilities here.”

    India: Civil Aviation Safety Advisory Council

    Air India Airlines did not single-handedly inspire the newly formed Indian separate body to investigate air accidents, but as the committee was formed immediately after the Air India Express crash in Mangalore that claimed 158 lives, it certainly was a major catalyst. It remains to be seen if the new investigatory committee will be as efficient or reliable as the US’s NTSB/FAA which investigates all major air events in the US and some outside of the US (when requested by foreign agencies or when American pilots, citizens, service or goods are involved).

    Certainly the DGCA needs help. We have already pointed out in a prior editorial that the DGCA’s report on the Mangalore crash is overdue. The Times of India quoted the DGCA’s own statutes:

    According to Civil Aviation Requirement (CAR) Section 5 (Air Safety), Series C, Part I issued on 13 October 2006, “preliminary report by the inspector of accidents/inquiry officer should be finalized within 10 days of the accident.

    We have also located a source that claims that * all of Air India Express managerial positions are “part-time.” Does this mean that the company was deliberately set up to have no key personnel who can be targeted as decision-makers? Though why should that matter? Air India Express is the budget branch of a larger company; Air India Express is a wholly owned subsidiary of Air India which is now part of National Aviation Company of India Limited (a merger between Air India and Indian Airlines.) Someone is in control.

    Those who fly India and have high expectations of the new safety committee being effective may be in for a let down while the newly formed body finds its footing. The new, separate Civil Aviation Safety Advisory Council which has been set up to handle air crashes has no experts from Air Traffic Control (ATC) nor expert pilots. Along with the Directorate General of Civil Aviation (DGCA) chief, Dr Nasim Zaidi, the 28 member body includes Aviation CEOs including Air India Chairman Arvind Jadav, Spicejet CEO Sanjay Aggarwal, managing editor of Centre for Asia Pacific Aviation, Kapil Kaul.

    If you take a look at how the NTSB handles an investigation, you will see that a team of experts are engaged and all interested parties, including pilots, are represented, including some you might not expect. Without such input, investigation is a waste of time.

    The NTSB team of investigators handle individual aspects of the individual investigation, allocated by expertise. Each expert is responsible for a specific and sometimes overlapping portion of the investigation. Specialization like this helps develop and apply expertise in each area and prevents key points from being overlooked. Also, bear in mind that the experts involved in investigating each crash will differ, depending on the parties involved; and most of these parties are on site within hours of the crash.

    SomeAreas of investigation/investigators

    • Structures investigators (who also usually takes responsibility for the crash site, ensuring that all the wreckage (e.g. wings, fuselage, and undercarriage) is accounted for and that proper trails of evidence are followed)
    • Engines investigators
    • Systems investigators such as flight controls
    • Airline’s Operations investigators (also studying crews’ training and suitability)
    • Black box investigators
    • Accident flight’s relationship with air traffic controllers
    • Meteorologists who analyze the weather at the time of the crash to determine if it had a bearing on the accident.
    • Metallurgists who analyze the condition of the plane’s metal and the forces on it
    • Sound specialists (for eg, identifying sounds on black box tapes)
    • Fire and Explosions experts
    • Simulation experts who reproduce the events
    • Flight survival experts who analyze the forces the passengers were subjected to. (These are the people who can tell you the safest place to sit on a plane.)
    • Maintenance experts who analyze the plane’s condition–and how it got that way
    • Psychologists may examine procedures to determine if airline practices exist that cause the pilot to make mistakes, like flying different types of planes. (e.g. Different types of cockpit controls make switching planes an unsafe practice.)
    • Manufacturers representatives who study and analyze their product’s performance
    • Pilots (representatives from the pilots’ union) may be attached to many or all groups; just as…
    • Labor Unions such as the International Association of Aircraft Machinists may attach representatives to multiple groups.
    • Cooperating “foreign specialists” in wreckage reconstruction, black box decoding or interpretation. (Countries who develop special expertise in various fields are often called to join the investigation, just as the NTSB is frequently asked to join foreign investigations.)

      Plane manufacturers have their own investigative teams experienced in this type of analysis, and they stand ready to work on any event that needs their expertise. And the investigation is networked so that if and when they need additional specialists, they can tap into their own company resources.

      One should note that in the case of the Mangalore Crash investigation, the wreckage sat out in the elements for 50+ days before being collected and warehoused. In my opinion, this is carelessness; and furthermore, this carelessness is just one visible aspect demonstrating where the existing procedures are obviously not up to par. While I am sure that experts combed the wreckage, including Boeing, (they always dispatch a team when one of their planes is in a crash,) leaving the wreckage out in the elements is not a best practice for conservation/preservation of evidence. One can only wonder how all other aspects of procedure including those behind closed doors-stand up to Aviation Industry’s internationally expected best practices.

      The industry calls this massive joint investigation the “party system.”

      The parties involved include all parties affected. The more parties involved, the faster the investigation may go.

      Chairmen of each group make their report; and any groups who discover any failures or unusual effects of machinery make note in the final report given to the primary investigator. The investigator invites the individual groups to provide their angle on what caused the crash and how it might be prevented.

      If all the probable causes line up neatly, then it is a simple thing to complete the final report. When theories conflict, the final report takes longer, and tension develops.

      All of this information is supposed to remain confidential until the final report is published.

      In this “party system,” the various opposing parties tend to keep each other “honest”. Unfounded attempts at blame by one party will be met with rebuttal by another–so it is obvious that if there is no pilot representation in this new committee, pilots may end up the “dumping ground.” Let’s say, for example, that a pilot in a crash went in with bad input like an outdated map. Is this committee set up to pursue the investigation to the source of the bad input? A pilot’s association representative would be instrumental in getting to the cause, whether it might be inadequate training, or materials.

      The international community and the people of India are waiting to see how this new committee manages to regulate and oversee all aspects of civil aviation in India, and whether it helps keep India’s DGCA and the safety of India’s aviation systems on track.

    AfavTAM Issues, 3 Years After TAM Airlines Flight 3054

    After nearly three years of sorrow, of missing our loved ones, we still struggle for truth, justice and life!

    The facts: Information extracted from www.globo.com 08/24/2007

    • 7 month before the TAM tragedy, in a meeting with ANAC, Captain Gilberto Schittini, ANAC’s manager of evaluation standards for aircrafts, warned INFRAERO and AIR COMPANIES technicians, TAM’s technicians among them, that “… the three incidents we recently had could be considered as ‘warning signs of more serious incidents, involving the overrunning of the runway…’”, referring to Congonhas Airport, under certain conditions. The same statement was given before the Aerial Blackout Parliamentary Inquiry Committee.

    • On 07/17/2007, TAM flight JJ3054 overran the Congonhas runway, hitting a TAM building, and 199 people lost their lives. Hundreds of families were destroyed and are still struggling for justice. WHAT HAPPENED?

    State and Federal inquiries were already concluded. The state inquiry, as disclosed by the press, identified a number of people from ANAC, INFRAERO, TAM and even AIRBUS to be held accountable.

    Although working from the same facts, the federal inquiry came to a different conclusion, only identifying the pilots as responsible for the tragedy, exasperating us all.

    Now, AFAVITAM awaits and relies on an independent and solid analysis by the Federal Prosecutor’s Office. AFAVITAM and rest of Brazil count on the indictment of the real wrongdoers that are responsible for this foreshadowed tragedy. Time elapsed is a disturbing factor; we don’t have a closure, even after three years. Due to the statute of limitations, time works on behalf of the guilty. We believe that, in order to get the proper measures taken and to PREVENT NEW TRAGEDIES, to compel and awaken society, an exemplary punishment is a valid mechanism.

    We have monthly meetings because we believe we can make a difference. WE LOOK FOR:

    • TRUTH AND JUSTICE;
    • PUNISHMENT FOR THOSE RESPONSIBLE;
    • SOLUTIONS FOR THE PROBLEMS FOUND, so we can avoid another tragedy.

    NO MORE IMPUNITY IN BRAZIL.
    HOW LONG OUR SAFETY WILL HAVE TO WAIT?
    YOU CAN’T PUT A PRICE ON A HUMAN LIFE!!!

    Dos fatos: (Informações extraídas do G1, Globo.com, de 24/08/07)

    ? “O Comandante Gilberto Schittini Gerente de Padrões de avaliação de aeronaves da ANAC, alertou 7 meses antes da tragédia, em reunião em que estavam presentes técnicos da ANAC, INFRAERO E AS COMPANHIAS AÉREAS, dentre elas a TAM, que “ … os três incidentes ocorridos recentemente poderiam ser considerados indícios de que “há um potencial de ocorrências mais graves, com ultrapassagem do final da pista (VARAR A PISTA) …”, referindo-se ao aeroporto de Congonhas, quando ocorridas algumas condições.” Declaração fornecida por este em depoimento, posteriormente, à CPI do apagão aéreo.

    ? O QUE ACONTECEU? No dia 17/07/07 a aeronave da TAM, vôo JJ3054, VAROU a pista do aeroporto de Congonhas, bateu no prédio da TAM e 199 pessoas perderam suas vidas. Centenas de famílias foram destruídas e encontra-se em reconstrução atrás de JUSTIÇA.


    Os inquéritos nas esferas Estadual e Federal já foram concluídos. No primeiro, conforme divulgado pela imprensa, várias pessoas foram apontadas como responsáveis, tanto da ANAC como da INFRAERO e da TAM, tendo referido ainda a empresa AIRBUS. O segundo, em que pese analisar os mesmos fatos, indicou apenas os pilotos mortos como culpados, o que indignou a todos. Agora a AFAVITAM aguarda e confia na análise firme e independente do Ministério Público Federal e espera, junto com a sociedade brasileira, que haja a denuncia e o apontamento dos verdadeiros culpados dessa tragédia anunciada. Estamos preocupados com a demora, porque já se passaram 3 anos e ainda não temos uma definição. Sabemos que o tempo beneficia o culpado, porque pode ocorrer a prescrição da pena. Acreditamos que a punição exemplar dos responsáveis pela tragédia é um mecanismo de coerção e conscientização para que medidas sejam tomadas e novas tragédias NÃO SE REPITAM.

    Nos reunimos todos os meses porque acreditamos que podemos fazer a diferença e
    BUSCAMOS:
    – VERDADE E JUSTIÇA;
    – PUNIÇÃO PARA OS CULPADOS;

    As famílias da TAM, três anos depois.

    Às famílias que agora rememoram a tragédia com o voo JJ 3054 da TAM Linhas Aéreas – o Airbus A320 que se chocou ao pousar em uma noite chuvosa, em uma pista com problemas de pavimentação, em 17 de julho de 2007…

    Às famílias, muitas das quais são amigas minhas…

    Este mês de julho não pode passar sem que relembremos. Assim como cada primavera faz lembrar o inverno, e cada noite faz lembrar o dia, em17 de julho, nós lembramos.

    Espero que, a essa altura, vocês tenham criado algum tipo de tradição que ajude a família a se reunir e lembrar.

    É um privilégio recordar toda a virtude de suas vidas – quer visitando seu túmulo, acendendo uma vela, plantando uma árvore ou realizando atos de bondade em sua memória.

    Vela alguma brilha o suficiente ou por tempo o bastante; arvore alguma se ergue suficientemente alta ou dá sombra o bastante; nenhum ato de caridade é suficientemente generoso. Mas, ainda assim, fazer algo a cada ano que nos ajude a realmente lembrá-los traz em si um ato de vida. Fazê-lo traz a chama da lembrança, entrelaçando o ontem ao presente e ao amanhã.

    Um ritual que, a princípio, pode funcionar como uma muleta emocional, mas tarde dá asas à nossa alma. Um ritual que nos faz lembrar que cada vida é como uma pedra em um monumento, um alicerce que sustenta a todos nós que seguimos em frente.

    Que a perda não seja em vão.

    Lágrimas não nos podem devolver o ser amado, mas espero que o tempo tenha trazido cura e alívio para a dor, deixando apenas a ternura da lembrança.

    Amigos e amigas, celebrem a memória.

  • Tam Familes, 3 Years Later

    To the families who are now remembering the TAM Airlines Flight 3054 (JJ 3054) tragedy–the Airbus A320 which crashed on landing during rain on unfinished pavement in São Paulo on July 17, 2007…

    To the families–many of whom are my friends…

    July can not come and go without our remembering. As each spring recalls the winter, as each night recalls the day, on July 17, we remember.

    I hope that by this year, you have established some kind of family tradition that helps the family come together to remember.

    It is a privilege to recall the goodness of their lives–whether it be visiting their memorial, lighting a candle, planting a tree, or continuing acts of kindness in their memory. No candle burns long enough nor shines bright enough; no tree stands tall enough nor casts enough shade; no act of kindness is quite benevolent enough. And yet, doing something each year to help us actively remember carries on the act of life. Doing so carries the torch of memory, weaving the thread of yesterday into today and tomorrow. A ritual that one time may be a crutch at a later date will give our hearts wings, a ritual that reminds us that each life stands like the stones of a monument, a foundation sustaining all of us who continue.

    Let the loss not be in vain.

    No tears can bring back a loved one, but I hope that time has brought healing and cooled the pain, leaving only the sweetness of memory.

  • One Mangalore Crash Question: The Wreckage

    A plane wreckage is not simply trash. The wreckage is the story of the crash transcribed in twisted metal and broken gears, legible to experts who can read from it details that the layman can not determine. The wreckage is concrete evidence. Because damaged mechanics reflect forces that have acted on the plane, its legal and historical value is in its very tangibility: it is an actual record that can reveal the course of events, as well as being an important factor in determining the culpability of those involved.

    Imagine, if you will, what would happen to paper records left to the mercy of passersby, out in the sun and wind and rain for fifty days. If the paper survives, anything written on it will be degraded by exposure to the elements. If anyone attempts to read it, their task will be 50 times more difficult. This is true whether the record is written on paper–or on crushed metal.

    So why is it that the evidence in the Mangelore crash–the wreckage–was left to the elements for 50 days? Whose responsibility is it to maintain the evidence at its most accurate condition? What can be said about leaving evidence on the scene, in an area where the public has access, and/or allowing it to sit exposed to the elements for 50 days following the event?

    This is not a crash like Air France Flight 447 where the greater mass of the wreckage is lost at sea, and if and when any evidence is found, it will have been inadvertently degraded by exposure that is beyond anyone’s control. In the causal relationship between conduct and result, in the Mangalore crash, who is responsible for maintaining the quality of the evidence so that it maintains its forensic value, especially in a crash within meters of the runway in a known and navigable area? Is there a chain of responsibility? Is that responsibility safeguarded from possible intrusion, alteration or negligence by vested parties? Is safeguarding the forensics the responsibility of the airline or its insurers? Because, while the airline has hardly any experience dealing with such details, its insurers have a long history of dealing with evidence in aviation cases. Is there not an obligation in the vested parties to maintain the forensic value of the evidence?

    Technical Difficulty: Specific Answers imprisoned in “the vague”

    What: Cityjet Avro RJ-85/Air France en route from Edinburgh to Paris
    Where: Birmingham UK
    When: Jul 8th 2010
    Why: While en route, the Air France flight developed “technical problems” and diverted to Birmingham where it made a safe landing. Passengers were provided alternative flights.

    Avro (1963) was taken over by Hawker Siddely, and later merged with British Aircraft Corp and Scottish Aviation (1977) , which was succeeded by British Aerospace( 1999) , and then succeeded by BAE Systems.

    Technical issues? In this day and time, it seems to me that “technical issues” is an inadequate description of a problem. These have all been lumped at one time under “technical issues”:

    A cursory announcement of a problem should be specific.

    Was it a problem with an indicator? an alarm system? a physical problem? a communication problem? loss of control? overheating? leaks? maintenance?

    The only way design issues can be caught ahead of the disaster is if full disclosure of the exact problem is noted publicly, and cross-checked with all other incidents and problem reports. If Air France or Cityjet Avro wants to present details, I would be delighted to present them in this forum here.

    Air France—like all carriers—needs to practice full disclosure. If the cause is unknown, then it is multiplicatively more important to disclose the exact issue so that the problem can recorded, cross checked, and be tracked down. Maybe if there were less secrecy and more open communication in the field of aviation safety, the 575 lost in recent crashes would still be here.

  • Yemenia: A Year Later

    Time flies, does it not? Though at times it seems the year has dragged by day by weary day, minute by weary minute, it hardly seems possible that it has already been a year since June 30, 2009. I debated whether or not to post this, because I wonder if it is not simply a reminder of pain. But then, it is also a chance to remember another life, when days were brighter.

    Everyone at our office joins me in sending the families our kindest regards, wishing you what we hope has been a year of healing. You might ask how you could have healing without your loved one who perished this time last year.

    From personal experience, I know that it has not been easy. I believe that when you lose a loved one, through a terrible tragedy like the Yemenia Crash or by any cause at all, our loved one would want us to continue living to the fullest. I’m certain that my grandson, whom we lost many years ago at age 15 in a motorcycle accident, wanted this for my daughter (his mother) and for the rest of the family. We still miss him. We still do a family birthday at the cemetery. Somehow we manage to carry on because he’s still in our lives, in our hearts, every day of every year.

    I know you carry your loved ones in your heart too. Some days their presence feels very close. Those are the times, special times, that I believe they remain with us, out of sight perhaps, but never out of reach.

    They say that living in the hearts we leave behind is not to die. If that is so, then let us all be a miser of our memories, for that is coin we will never spend, so we can always hold our loved ones and their memories close.

    Is Oxygen Hose Length an Issue?

    We talked about the May 16 cockpit fire on a United Boeing 757 (New York to Los Angeles) that made an emergency diversion to Dulles. That event involved an airborne cockpit fire so extreme that the pilot reached for the fire extinguishers to put out flames coming from his windshield—but he was brought up short by the length of the oxygen hose. In the smokey situation, it must have been a special concern to have the oxygen mask rip away from the pilot’s face. I can only assume there is other essential gear stored and unreachable due to the length of the hose, because I haven’t seen anyone talk about solving the hose length issue by moving the extinguishers closer.

    In fact, on some Boeing 757 and 767s the length of the hose attached to the pilot’s oxygen mask and smoke goggles is under scrutiny now by the FAA for being too short to reach the fire extinguishers. Apparently Boeing has resisted lengthening the hose because of the danger of the pilot getting tangled in the hose.

    Since the FAA is looking into the issue of the possibility of the hoses being too short, one can not help but wonder if this problem were a factor in the May 22 – Air India Express Flight 812 Boeing 737-800 crash at Mangalore International Airport, or the Jan 25 Ethiopian Airlines Flight 409 Boeing 737-800 that crashed into the Mediterranean Sea shortly after take-off from Beirut. How do we know that too-short hoses did not contribute to those tragic events?

    Conversely, if the 737 hose length is not too short and allows the pilot to reach where it essential for the pilot to reach, and there is no hose problem on the 737, then why has Boeing not corrected this problem on 757s and 767s?

    Pinnacle Raises Security Bar. Maybe

    The passenger’s luggage tripped security in Tulsa, and so Pinnacle security detained it. However, the passenger who owned the luggage was free to travel to his destination.

    However, this is what is unclear to me:

    The bag flunked, and stayed in Tulsa. The passenger, however flew to Memphis, although his luggage was arrested (explosives detection.) Shouldn’t they have taken him off the flight with his luggage?

    So after he arrived in Memphis, law enforcement interviewed him, and he passed. But they’ll be shipping his luggage to him later. Does something seem strange here?

  • Hudson Crash May Provide AF477 Answers

    The one thing that stands out the most in today’s NTSB statement [about the US Airways Airbus A320 that crash-landed on the Hudson River] is that there is nothing but praise and admiration for the crew, whose quick reflexes, cool heads and complete professionalism saved the lives of everyone aboard.

    The NTSB is NOT so positive regarding Airbus, the manufacturer of the plane involved in that crash. See the NTSB’s Executive Summary

    There was no appropriate checklist for the event in the manual; and some procedures were so precise they could not be carried out.

    Ingesting birds into an engine is rarely so dramatic, mostly because it doesn’t usually happen to both engines. The circumstance allowed experts to examine cause and effect on the plane and a number of surprising things were concluded. Certainly more than I am talking about here.

    Some of the concerns conceivably could have applied to Air France Flight 447…if it had landed intact (although we are assuming it did not.)

    That the cockpit instrumentation did not reflect the status of the engines.
    Did the pilots of AF 447 also not have inaccurate reportage of the status of the engines? We think this is true. How can a pilot in the middle of the Atlantic in the midst of a storm with inoperable speed sensors know what the fly by wire engines are doing of the operating system doesn’t tell him something is wrong? The answer is: he can’t.

    That the rate of descent in the manual is unrealistic (claiming a safe 3.5 feet per second when in reality 13 feet per second caused damage.)
    Although there’s no telling what the rate of descent was for AF447, it appears likely that neither did the pilots. Also, 447 was not guided/glided down by Sully; if AF447 came down intact at all, it probably fell at a high rate of speed with only limited awareness of the pilots when it was too late.

    That in the Hudson event, slides were not flotation devices; wings did not float; and there were no rafts over these wings although the passengers scrambled out there.
    In the Hudson crash, the passengers exited to the wings, expecting flotation resources. Fortunately rescue came quickly before they sank. There were no “wing rafts.” Could AF passengers, had they survived the landing have exited, and been able to access rafts? We think not. Of course this is a moot point as we doubt they survived hitting the water–whether or not the plane was intact.

    Given that the NTSB called the Airbus manual unrealistic and inappropriate, that Airbus’s claim that for the Hudson crash “LaGuardia Runway 13 was technically feasible from an aircraft flight performance point of view” seems equally unrealistic. In fact, Airbus has a lot of nerve to extend a pseudo-panglossian commendation to Captain Sully for appropriately considering the high urbanization of airport surroundings. At this point, any statement from Airbus is highly suspect.
    At this point I consider Airbus itself, “theoretically feasible” but highly unlikely.

  • Air France: Blithe Unconcern, Unconcern or something more?

    At Père Lachaise cemetery in Paris, the memorial dedicated to the victims of flight 447 will have its grand opening. Family members of those lost in the crash are invited.

    Sometimes Air France displays a disturbing lack of propriety, even when they’re doing their public best to appear sensitive and thoughtful.

    Case in point: Generously, Air France provided tickets to the family members of the victims of Flight 447 , the flight from Rio de Janeiro to Paris, that final notorious, fatal flight which disappeared into the Atlantic Ocean on 1 June 2009, killing all 216 passengers and 12 crew members.

    Guess which flight Air France put the grieving families on? Flight AF445 on an Airbus aircraft–it is, in fact, exactly the same flight as the one which killed their family members. (“447” was discontinued; “445” is the current designation for the same flight.)

    The victim association is intending to rebook on Boeing flights.

    It’s difficult to figure out, impartially, what is going on in the heads of Air France management. Are they deliberately ignoring the 400 lb gorilla in the room? (Actually, they’re planning to replace the 400 lb gorilla—the Airbus A330-200— with an even bigger model, the A340. Essentially replacing the 400 lb gorilla with a 500 lb gorilla… but I digress.)

    Meanwhile, the families agonize over the details: would the plane have crashed if the pitot tubes had been replaced with another model? Or if the pitot tubes had been maintained properly? And how can Air France possibly expect them to set foot on a plane–especially the same flight as the one that carried their family members to their deaths?

  • Cost of Aviation

    As I sit here in a foreign country separated from my home by an ocean and a sky full of volcanic ash they are saying may disrupt trans-atlantic aviation for the next six months, and wondering when I will get home, I read the chilling words of a American scrivener remembering the recent flight that wiped out the Polish “politocracy.”

    Michael White: “An airplane is a hollow bullet filled with humans lives, shot from a gun we call an airport. Should it impact with the ground other than according to the dictates of precise technology, its content perishes.”

    Those of us who fly so often take for granted that flight is a right, but every so often, Nature reminds us we are not birds, we are Icarus, daring to fly too close to the sun. We are creatures of the earth. We have no wings beyond those our imagination have wrought.

    Who among us can say it is worth the cost, especially those people I work with every day who have lost their nearest and dearest.

    And yet, how we have flown.

  • The Next Chapter

    When has an aviation disaster so changed an entire nation?

    The world is asking “What next for Poland?”

    No answers now, only questions. Only time will tell.

    The speaker of the lower house of parliament, Bronislaw Komorowski will get the ball rolling on presidential elections that will come early.

    Piotr Wiesiolek, a Central Bank deputy governor will temporarily assume the governorship.

    No crystal ball is needed to know that the next hierarchy will sort itself out after the crash of Polish politics. Whether the cream will rise to the top or mediocrity stand-in until a new equilibrium surfaces, the dynamics of the Polish political machines will adapt. That in itself is nothing new. Everything changes; it is the way of the world.

    It is not possible now or even fair to say that Poland will heal. Healing is not always an accurate word. What is meant, perhaps, is scab over, or scar, bearing in mind that how a scar heals determines if it remains a weakness, or becomes stronger than ever. There will be a time in which to convalesce. It is too soon, possibly even a cruelty– to speak of recovery now. The country needs time. Poland will endure as anyone does who has suffered tragedy, but eventually it will recoup its losses. Now is not the time to speak of the buoyancy of the human spirit, but to walk through the stages of mourning, and for other countries—without intrusion—like siblings, lend their strength where they can. Words can mean little now, but the free world shares in the mourning of the Polish people.

    In the sixties, Dr. Kubler-Ross postulated five stages of grief: denial and isolation, anger, bargaining, depression and acceptance; but I prefer the more recent model of seven stages which adds at the end, reconstruction and hope.

    Poland will eventually be all right. Not without scars, of course but however dim it seems now, there will be a light at the end of the tunnel.

  • Ethiopia Flight 409: Dubious Claims Circulate the Blognet

    There’s a general consensus among the people who deal with terrorists that terrorists like to take credit for disasters, even when they’re not responsible for them. Ownership of violence is part of the whole extremist “psych-you-out “ ideology. To claim responsibility for an act one did not do to is to “claim” one for the malevolent gipper, to make one appear more powerful, especially when actually helpless, and/or to appease whomever one is confessing to with false collateral by churning out false confessions.

    But consider this: if I say I am Santa Claus or the Easter Bunny, it does not mean I am Santa Claus or the Easter Bunny. Legally, it’s going to take more than tall talking and word of mouth to establish that claim in court. It will take heavy duty concrete proof to make that claim stand up in a court of law. The head of the Engineering Department at the BEA (France’s version of the NTSB, the Investigation and Analysis Bureau) Christophe Ménez said in an interview … that evidence shows that last month’s Ethiopian plane crash was not a result of an explosion. “The fuselage and other heavy parts, such as the engine, all sank together, and the plane’s wreckage was in close proximity, which indicates that the airliner did not explode. ”

    The investigation of Ethiopia Airlines Flight 409 so far does not develop a case for terrorism having anything to do with causing the crash. But even if it were true (and we don’t think it is), the primary liability rests with the operator, Ethiopia Airlines. This is not a legal opinion, just my own opinion.

    Now let’s speculate. If it were a terrorist act, it might derail a law suit against component manufacturers in the United States, but an experienced lawyer would tell us that if an act of terror downed the plane, it would not budge legal claims against Ethiopia Airlines. Under the various treaties that govern air travel, the operator has the primary responsibility for that innocent passenger who paid good money to get on that plane.

    The big news services are not the source of this claim; it looks like it is coming from a blog network, so it appears to be a rumor rather than fact, or a rehash or repost of an earlier theory. There is no plausible evidence of sabotage.

    But we are still hoping for more evidence, and for the families, more remains, to be discovered by the Odyssey Explorer in the ongoing marine search for definitive evidence.

    The terrorism theory has already been postulated and discarded once so I don’t give much credence to it. But more than anything, I regret seeing these things crop up to stress the victims families even more. They’ve already been through too much; and the investigation is going to take a long time. They have a long way to go.

  • Ethiopian Airlines: Victims’ Families Endure

    There’s always a lot of finger-pointing going on, so it really isn’t unexpected that Ethiopia accused Lebanon of misleading and hampering the investigation. We should remember that the ICAO could not be there if the investigation did not live up to the highest standard.

    It is also important to remember that any speculative conclusions, or “leaks” or preliminary investigation results are just that: speculation or investigation, or preliminary. It takes months and sometimes years to get to the cause of a crash, and this one is no exception. Eventually, no stone will be unturned.

    Remember that all of the parties involved will be conducting their own investigations–not only the governments, but also the plane manufacturers, the component manufacturers, (any party who thinks they may be sued), as well as attorneys who are looking out for the welfare of their clients.

    Many research teams are engaged in ferreting out, determining and studying the facts of the crash.

    It is a shame that after all their grief, the families have to endure finger-pointing and speculative theories masquerading as truth, smokescreens which are put out by reporters or politicians for political purposes.

    These investigations take time. There are no fast and easy answers. Look at the Air France Flight 447 crash of 2009. We’re still hearing all sorts of theories, but even in that earlier crash, the investigation is still going on; we’re still looking for wreckage and black boxes.

    And so, the families weather the storm, continuing to bide their time, enduring their grief, and waiting—patiently or not—for facts, for answers, for the truth to show itself.

  • Airworthiness Directive: Life

    In an aviation disaster, what happened is never a true mystery. A true mystery is something that baffles understanding and cannot be explained. We know there’s an explanation. We know something tangible happened. There is a tangible explanation. We just don’t know what it is. What happened. Or in what sequence. Or if one or two of the events in the sequence didn’t happen, could the disaster have been prevented.

    Most aviation disasters live in a realm of tragic mystery. We are always walking around, picking up the pieces of the puzzle. In fact, there are usually few pieces of evidence to pick up–and the ones we do find crumble in our hands, or they’re inexplicable or perplexing. So when we put the puzzle pieces together, they may fit a multiplicity of ways. There may be huge gaps. Or there may be no way at all to fit together the disparate elements. Oh, sure, we come up with a thousand theories–perhaps a different theory for each time we consider, each time we try to take another perspective, each time we speak to another expert. These experts agree; those experts disagree. Perplexing, an enigma, a riddle the answer of which remains forever out of reach. The alleged mystery—the answered question— remains, all the more tantalizing because we know there is an answer.

    As troubling as it is for those of us who look for the answer to “how” it happened, families struggle with fathomless questions a thousand times harder to deal with: why my loved one? Of all the thousands of trips, why this particular flight, why this particular plane, why this January, why this Monday?

    Families play the questions back in their heads, rewind their lives and replay the events as if somehow in the thinking of it, they could reach back through time, stop their loved one from boarding that plane, or otherwise change a single factor–make the pilot leave his phone behind. Make the mechanic give that lug nut an extra turn. Bless that plane designer—or the parts designer—with something extra so that on that long ago day when he came up that design, it was flawless. The thing that if it had been done just a little differently, would have kept the tragedy from ever happening. That magic something that could undo what has been done with such conclusive finality. As if the moment could be rewound as in a movie, and come out differently. These are the questions which are beyond inscrutable, profound questions heavy with the weight of life, of love wrenched away before its time.

    The answer may be elusive.

    But there’s always rigid hard reality behind a crash. There’s always an actual sequence of events that instigated the tragic event.

    So any time I see an airline being proactive, I think of lives that will be saved.

    Alaska Airlines begun inspecting their 14 Boeing 737 jetliners because of the emergency airworthiness directive issued Friday. Severe vibration occurred in some European flights was due to “lug failure on an elevator control mechanism. The elevator controls the plane’s pitch.”

    There are mysteries hanging about 737s such as ITEK & PERM and recently Ethiopia. In the first two it was determined that it was human error.

    I have always believed that pilot error alone will not bring down a plane.

    This vibration opens a whole new window of possibility of answers to our unanswerable questions. In some of these past crashes where we have not found the answers, is it possible these tragic flights encountered turbulence which was exacerbated by this vibration? Could this flaw have been instrumental in the Aeroflot-Nord B735 crash at Perm on Sep 14th 2008? or to Itek Air B732 at Bishkek on Aug 24th 2008? Or more recently, to Ethiopian Airlines B738 in the Mediterranean Sea on Jan 25th 2010?

    So I am glad to hear that Alaska Airlines begun inspecting their 14 Boeing 737 jetliners. I am glad to hear that Alaska Airlines is committed to safety. They should be commended for wasting no time in ordering an inspection.

    If only all carriers were so diligent, there might be fewer grieving families. And that wouldn’t be a bad thing, would it?

  • Virgin America: Trapped in Plane

    Never heard of SWK? It’s Stewart International Airport, 90 miles N of New York. It’s the infamous airport where this Saturday 126 passengers on a Virgin America flight shared a nightmare for four to six hours on the tarmac, completed with rationed potato chips (4) rationed water (every 1/2 hour.)

    On the bright side, the flight did not crash.

    Diverted from NYC by weather, the 5 1/2 hour flight turned into a runway waiting room. Not all of the passengers were allowed off the plane into the airport; the flight attendants and airport personnel were getting testy. Then Jet Blue provided bus transportation to the stranded…er…trapped…er…Virgin America passengers for the 90 mile trip to JFK.

    Passengers amused themselves by elaborating on their ordeal online. Much to the benefit of the travel-weary passengers, Virgin America’s CEO David Cush was one of the people who heard the online tale of woe, and is (we hear) refunding the cost of the trip plus $100 to the 126 passengers.

    Cheaper than a lawsuit, I guess. I wonder if he’ll be getting them to sign anything.

  • The New Rage: Unruly Door Openers

    What is going on with all of these unruly passengers attempting to open the door?
    It is aerodynamically impossible to open these doors most of the time, but it is still a spectacularly stupid thing to attempt.

    I hope it is not a trend. IF it is, it’s a bad one.

    Humans are amazing inventors, capable of envisioning a “possible” for every “impossible,” creating incredibly complex feats of engineering, in the name of Hughes and Learjet and McDonnell Douglas and Lockheed and a hundred others whose names won’t be forgotten. Even amazing inventors can engineer lemons, or fail to see every possible contingency. But the possibility of error goes beyond human error. Humans are not responsible for the fact that rust exists. Do you blame Einstein for the fact of relativity? It’s the dichotomy of reality: for every up there’s a down. Everything creates its own opposite. “Fixed” allows the possibility of “broken” to exist.

    Thinking logically along those lines…we can speculate and plan, study and test, polish and revise, but does that make our planning and scheduling invincible? Even if it were perfect (and I’m not saying it is, because I am speaking of things in general), things fall apart. The nature of reality is change. Change happens. Plan for contingencies 1-3000 and the one that will happen is contingency # 3001.

    We have to be careful sometimes when we start talking about human error. Unfortunately the container where the human error begins is life itself, because in theory EVERY thing can be blamed on human error. The patient died, blame the doctor. Nevermind that the patient was 112 and suffered every illness known to man.

    In aviation, gunnysacking the humans happens too. Maybe they didn’t cause the lightning but they flew through it. We humans are pretty amazing creatures but we don’t control nature. We can plan and schedule all we want, but if you are going to fly during rain, you’re going to encounter thunderstorms. So let us say that the storm is responsible for the event. Do we blame the pilot for flying through the storm? Do we blame the arrogance of the plane manufacturer who claims the plane is invincible and can fly through anything?

    Pilots are human. We have the full range of the spectrum, from the sublime, Sully, to the ridiculous (Colgan Air’s Marvin) Renslow. The problem that a hero like Sully presents to the rest of us is that he creates the philosophy that well trained pilots should be able to solve any emergency.

    But take for example Air France 447… could the pilots have been able to cope with flight speed problems caused by the performance of flawed parts?

    Is the failure to recover from an unexpected event (wind shear, potential conflicts with other planes, broken planes) pilot error? If we put human beings in an easy chair of an automated job in some point-and-click fly-by-wire captain seat, when the crisis happens, the built-in redundancies in flight systems fail and the pilot can’t snap out of humanness to turn into Superman when the wire breaks, is that pilot error?

    Our pilots now have to deal with cost-cutting industry practices causing fatigue and poor training.

    Even if the ultimate safety device in the airplane is the well-trained pilot, guess what? Pilots are human. Humans are capable of mistakes. They are capable of mistakes when operating planes.

    Let us not forget that pilots fly planes that were crafted by humans, maintained by humans, orchestrated by human ATC, landing on airports designed by humans.

    There is no one single answer. I have seen it a hundred times, and each case is unique unto itself, with one generality that can apply.

    Human error occurs at all levels, and rarely alone.

  • 6 Years of Unsupervised Supervision=65000 Flights of Chaos

    They say about real estate that the three most important things are location, location, location. And that makes sense, because the key factor about real estate is its lack of portability. Whether you’re in Calabasas or Timbuktu, where you are ultimately is where you are. The fact that it is where it is is what makes it what it is. Intrinsic value, and all that.

    One could argue what the most important thing about travel is. You could make a strong argument for getting there, being there, or smelling the roses along the way. If we narrow the question to the single most important thing about air travel, the most important thing is probably getting there in one piece–mostly because it seems that getting there safely and completely without incident is an ever more dubious proposition, a perception which is being emphasized now more than ever due to NASA’s Aviation Safety Reporting System. When record keeping changes, it is difficult to say if some underlying factor has changed, or if the reporting is the only change. At any given time, it looks like the odds of getting there in one piece–on a plane that is 100% airworthy–are starting to look slim. And that is important whether the a plane is landing at LAX, (third busiest US Airport)

    NASA’s Aviation Safety Reporting System allows crew members to phone in anonymous reports, a whistle-blower’s hotline as it were. So there’s a record of blunders, errors, faults, and slip-ups, like improper repairs, substandard repairs, unqualified mechanics and maintenance deficits that might otherwise have slipped by or gotten swept under the rug.

    Honestly, what I see on these reports makes me seriously anxious. I’ll be facing more white-knuckled flights, just knowing that in the past six years, 65,000 flights have taken off in less than airworthy condition in conditions of deferred maintenance, or sporting less than stellar repairs farmed out to contract shops. 65,000 flights is a lot of flights, even if it is spread out over 6 years and 63 million flights. And do I believe that ALL of the other planes were in perfect running order? No.

    Some of the mechanical problems that down planes can be linked back to tiny repairs, or perhaps I should say involving small but very important parts. If a plane can go down because a single screw is loose, or the threads are worn, then acute attention to maintenance is exponentially more important. I wouldn’t set foot in a golf cart in the condition some of these planes are in. But as a commercial passenger, I would never know if repairs on my flight were deferred. I would not know if a plane were outsourced to a sub-standard repair facility by mechanics without repair manuals, proper tools or correct parts. Outsourcing repairs to other countries not only removes shops from proper supervision, it also opens up the possibility of security risk. But I digress from what is my primary concern right now.

    A sense of proportion and common sense should be applied here. Lights over a passenger seat are not as crucial as a screw assembly on the tail that provides pitch control.

    I’m just saying:

    We need more qualified supervised aviation repair facilities on U.S. soil.
    We need airlines to stop deferring repairs.
    We need airlines to maintain high standards.
    We need the FAA and the NTSB to do their jobs.

    Hazards of Winter: Another Ground Crew Death

    Why: After de-icing, the airplane’s wing hit the de-icing hydraulic lift, struck the vehicle’s operator and knocked the lift on the assistant who died on the scene.

    De-icing isn’t something we normally think about. Not for planes. Maybe those of us who have wintered in Chicago or New York have considered it. Those of us who have had to get up extra early to scrape the outer skin of ice and snow off of the car, even those of us who have had the forethought to stack a layer of cardboard over the windshield to limit the ice buildup, who turn on the heat inside so the commute will be tolerable. But frankly, scraping the windshield with the ice scraper, or knocking snow off headlights–with a car, it is usually not that hazardous–albeit sometimes slippery. Obviously de-icing a plane is significantly more dangerous. After all, the instructions on the back of an ice scraper fit on the ice scraper.

    The aircraft ground anti-icing operations manual available from ICAO runs 37 pages and cost $25. So there are 37 pages worth of established procedures to follow.

    News of a death makes me remember the ground crew who is working out there de-icing the plane while the passengers sit inside, all warm and cosy. It’s cold out there; and dangerous. More dangerous than we think about, but maybe we should.

    It’s only by learning from our mistakes, in aviation, and in every other industry that we progress. It’s too late for these two ground crewmen, one in Calgary, one in Thailand. But it’s not too late for us to learn from our mistakes–if mistakes there were.

    It has been just over a month since another de-Icing crew member in Calgarydied on the job. In that case, it was a Servisair ground worker who fell out of a cherry picker.

    In this latest instance, we haven’t really heard much yet. We know that the death occurred in Thailand, and that it was snowing heavily. The investigation will reveal the details of what happened. Who was in the wrong place at the wrong time. If the operator or crewman weren’t following guidelines–or the flight crew. The investigation will reveal who acted carelessly, or if it was simply an accident, and the plane just following the path of least resistance.

    I respect winter.

    And I respect ground crews. They brave the elements, just to make plane travel just a little bit safer. When you think about it, it’s a little bit heroic, and maybe a little bit sad how the ground crew works on plane after plane. It’s a little bit like a pastry chef who makes delicacy after delicacy, but never gets to eat them himself. The ground crew stays behind when the plane soars away.
