What: Feeder Airlines Fokker 50 en route from Juba to Wau
Where: Wau
When: Mar 29th 2012
Who: 50 aboard
Why: THe runway has been in the process of being repaired for the past several months. The pilots were apparently not advised regarding the necessity to touch down at midpoint due to construction. The first attempt at landing was aborted; on the second, after touching down, the plane left the pavement, and stopped on the ground with a nose and main gear collapse. At least five were hospitalized.

Passengers evacuated with some casualties, including a broken leg.

We sincerely hope ATC and the airport will accept responsibility for any inaction on their part in this matter.

Both stairs and pilot ended up on the tarmac, the first officer ending up with a broken arm and leg. The plane was in its parking bay. The pilot departed for the hospital, presumably by ambulance, and the plane departed for Bali, with only a 90 minute delay.

I don’t think I’ve ever heard of this happening!

DGCA Gulf Air Landing Examined

What: Gulf Air Airbus A320-200 en route from Bahrain to Kochi, India
Where: Kochi
When: Aug 28th 2011, 3.55 am
Who: 137 passengers and 6 crew, 8 injuries
Why: In examining this August incident, after doing some runway testing, the DGCA is blaming the accident on a slick runway.

However defects have been found in the plane’s front wheel. And the statement of the DCGA’s opinion also mentions pilot error. So they have released a statement combining pilot error, plane flaws and a bad runway as being behind the runway excursion.

Can a pilot land on a slick runway? What exactly was the pilot’s error in this situation? Does the entire runway have the identical friction coefficient or just the area tested? Runway friction is also going to be affected by the condition of the tires; what was the condition of the tire before it blew? Inquiring minds want to know.

Airport testing at Cochin airport revealed airport friction level.

A C K Nair, director of the Cochin airport made the following defensive statement:

“As per the International Civil Aviation Organisation’s (ICAO) norms the friction coefficients of the runways should not go below 0.34, while the value was 0.61 to 0.86 till recently. On the day of the mishap the average value was hovering between 0.52 and 0.84. The value at the spot where the ill-fated aircraft touched the runway was 0.75 to 0.85. We did another test on September 3 and the value was above 0.5.’

Hard Landing in Ottawa, Roughing it on United Airlines

What: Trans States Airlines Embraer ERJ-145/United Airlines en route from Chicago to Ottawa
Where: Ottawa International Airport
When: Sep. 04 2011
Who: 44 passengers, 3 crew
Why: On landing in Ottawa, the plane slid, and veered off the side of the runway, with collapsed main gear struts.

On Apr 28th 2011, an Embraer ERJ-145 landed in Moscow with collapsed main gear.

Last year, another plane with the same airline also skidded off the runway into soft ground.

Sunday’s passengers describe coming down from a dark cloud to land in rain.

The landing is under investigation.

In George’s Point of View

I do not know if this runway is incorrectly graded for drainage, or if the ERJ-145 has a special problem with main gear struts or struts in hard landings.

I do know I fly 100,000 miles on United in order to keep my status as a 1K member.

I do not know if the maintenance of the aircraft is as terrible as the typical maintenance of United plane interiors. If so, then we have huge problems. The refurbished planes of two years ago already look worn out. The galleys are grimy looking the toilets are terrible.

I could say bad things about the food, the lav hot water is not hot enough to meet standards required by most states for food workers, (where I have never seen Waterless Hand Sanitizers and for sure, NEVER have seen a food preparer or server wear gloves.)

Is United maintenance absolutely out of control? Maybe this stretches to other airlines, economizing on staff and maintenance due to the times.

I fly Delta, American, Virgin, they have some of the same issues, but, I see these problems in the United flights, no matter which direction I am headed.

Who knows yet if this incident is runway or plane, pilot training or just an old plane that needs better maintenance. But whatever truths the investigation reveals, the fact remains that no one steps up to the plate until something bad happens.

On landing, the plane veered off the runway but came to a stop safely in the rough off the runway. In fact, it is the condition of the runway that appears to have caused the problem–assuming the plane ran over a pothole or bump which caused the hydraulic problem and forced the left engine shutdown.

GOL Inspects Flight for Airworthiness

What: GOL Transportes Aereos Boeing 737-700 en route from Bauru to Sao Paulo Brazil
Where: Bauru
When: Feb 2 2010
Who: 82 on board
Why: Prior to take-off, the engine presented an automated warning sufficient to deter the pilots from taking off. Details about the warning have not been made public although they were adequate for Gol to begin an investigation. While the original plane went into maintenance, a replacement plane was provided within four hours.

George’s Point of View

While we of course have no idea what the warning was, nor how important an issue it represented, we commend GOL for taking proactive action and responding to warning systems as they were intended to be. Hopefully maintenance will become as diligent as the crew that actually has to be in the plane aboveground.
