    Plane Fails Takeoff in Congo. Propellors fly off the plane

    On a United Nations MONUSCO flight departing Lubumbashi for Kalemie/Goma, during takeoff, a propellor flew off the plane. THe plane veered off the runway, damaging the undercarrage on non-paved ground.

    A Radio Okapi report said:
    Un avion de la Monusco a raté son décollage ce lundi 29 juillet matin à l’aéroport de la Lwano à Lubumbashi. Tous les passagers et membres d’équipage sont sains et saufs.
    Selon des sources aéronautiques de la Monusco, cette situation a été causée par une panne technique. Les hélices se sont détachées et le moteur endommagé. Cet appareil de type Antonov était encore immobilisé dans la soirée à 200 mètres de la piste.

    A MONUSCO plane missed take-off Monday morning at July 29 at the Lubumbashi Lwano Airport. All passengers and crew are safe.

    According to aviation sources MONUSCO, this situation was caused by a technical failure. The propellers and the engine were damaged. This Antonov plane is still stuck 200 meters from the runway.

    Mir Aero Eurocopter Crashes in Romania, 5 of 6 are Fatalities

    On July 29. 2013, a Mir Aero Eurocopter EC 130 (AS 350 B4) with 6 aboard was en route.

    It crashed in Taureni Lake, Mures County, Romania. Four bodies were found initially; the fifth, Sorin Terbea was found by divers. Sorin Terbea is a wealthy businessman. The woman, Terbea’s company’s marketing director who was engaged to Terbea was rescued by witnesses at the lake. She is reported to be the only survivor. Two German citizens and their child were also aboard.

    The Romanian Police forensic team was sent to Mures to coordinate investigation launched after the tragedy.

    Spicejet Emergency Landing at Thoothukudi

    On July 29, 2013, a Spicejet de Havilland Dash 8-200 with 49 aboard was en route from Chennai to Thoothukudi (AKA Tuticorin). After a safe flight, the plane had a tail strike on landing at Thoothukudi Airport. The damaged plane was grounded pending examination.

    Passengers returning to Chennai on the next scheduled flight were provided alternative transportation.

    Passengers described a bounce, and then the tail making a grating sound as it landed on the runway.

    There are no reports of passenger injury.

    Pudukottai police and airport security officials responded to the scene.

    5 Fatalities in Noxen, PA Helicopter Crash During Thunderstorm

    A Robinson R66 #N646AG en route from Lehighton PA to Endecott NY with five aboard was en route when it disappeared from radar during conditions of severe thunderstorms and heavy fog. The last communication to ATC was a report around 10 pm Saturday that he was “losing altitude and would try to return to a nearby airfield.”

    The fragmented wreckage was located at 1:50 p.m. in a heavily wooded area. There was no fire, and no witness.

    Emergency response vehicles and personnel traveled an access road on the Mehoopany Wind Farm, a BP Wind Energy location, to the top of South Mountain. The unofficial guess at the cause of the crash is that the helicopter was caught in a thunderstorm.

    Yellow Wings Crash in Kenya


    On July 25, 2013, a Yellow Wings Air Services Ltd Cessna U206F Stationair Cessna type 206, registration number 5Y BUG en route from Nanyuk to Wilson Airport disappaeared from radar around 5 pm in the Aberdares Range, Nairobi Kenya.

    The plane went missing Thursday, was found Saturday. Bodies were said to be recovered Sunday.

    The three missing swiss, a family of father, mother and daughter, were found beside the wreckage by a Kenya Wildlife Services helicopter.

    The plane struck the side of a mountain in the Aberdares mountains.

    On Saturday, not to be confuse with this accident, a plane with one occupant crashed in Tsavo National Park.

    AirBlue Flight 202 Remembered

    This day, July 28 is carved in history as the day 146 passengers and six crew boarded an Air Blue plane to Islamabad Pakistan. They arrived in heavy rain going the wrong direction and flew into a hillside. They arrived, but they went up in a cloud of blue fire and black smoke. They arrived, but the families who were waiting for them in 2010 will be waiting forever.

    Airblue Flight 202 was a scheduled domestic passenger flight which crashed on 28 July 2010. We can remember the crash, and write it on our calendars to remember the event again next year, and the year after that. Our remembrance, unlike all the kings horses and all the kings men, will not put together those who were lost; but it will give us an opportunity to also recall the report of the crash. That report is notorious now; it was missing transcripts, black box recordings, and evidence. It was repudiated by the Peshawar High Court, a landmark pronouncement in itself. The lack of transparency, the lack of scholarship and truth has consequences in future Airblue and Pakistan flights. This is because, in order to fix the problems, problems must be reviewed, analyzed, taken apart, studied, examined in the clear light of day. Problems are opportunities for correction. The examination of such flaws is the only way that future tragedies can be prevented.

    It is probably the saddest thing of all that the problems on this flight could have been prevented with adequate CRM (crew resource management) training. Simply put, CRM is methodology designed to improve efficiency. A crew well-trained in CRM knows how to step up and handle issues when the pilot in control has lost touch, as it appears happened on Flight 202.

    We can only hope that Air Blue and other Pakistani air carriers are now undergoing adequate CRM training that will give flight crews procedures that will enable them to survive.

    To the families who lost loved ones and friends in this tragedy, let us all again make our condolences. We can only hope that time will ease your loss.

    NTSB Issues Investigative Update into Southwest Flight 345 Accident

    The National Transportation Safety Board today released factual information from the July 22 accident involving a Southwest Airlines Boeing 737-700 landing at New York’s LaGuardia Airport. The airplane’s front landing gear collapsed on landing.

    • Evidence from video and other sources is consistent with the nose-gear making contact with the runway before the main landing gear.

    • The flight data recorder on the airplane recorded 1,000 parameters and contained approximately 27 hours of recorded data, including the entire flight from Nashville to New York.

    • The cockpit voice recorder contains a two-hour recording of excellent quality that captures the entire flight from Nashville to New York and the accident landing sequence.

    • Flaps were set from 30 to 40 degrees about 56 seconds prior to touchdown.

    • Altitude was about 32 feet, airspeed was about 134 knots, and pitch attitude was about 2 degrees nose-up approximately 4 seconds prior touchdown.

    • At touchdown, the airspeed was approximately 133 knots and the aircraft was pitched down approximately 3 degrees.

    • After touchdown, the aircraft came to a stop within approximately 19 seconds.

    • A cockpit voice recorder group will convene tomorrow at NTSB laboratories in Washington to transcribe the relevant portion of the accident flight.

    Southwest Airlines Diverts due to Flaps Flops

    On July 23, 2013, a Southwest Airlines Boeing 737-300 en route from Kansas City to Indianapolis developed a problem with the flaps.

    Pilots diverted back to Kansas City and made an emergency landing. (See flight path below)

    None of the 54 aboard suffered injury.

    The flight eventually took off and arrived in Indianapolis at 1 a.m. with no indication if this was the same or a different jet.

    Southwest Airlines Boeing Nosegear Collapses in NY


    On July 22, 2013, a Southwest Airlines Boeing 737-700 en route from Nashville,TN to La Guardia was making a landing when the nose gear collapsed.

    Five passengers and 3 crew were injured on the slides and hospitalized.

    There were 144 passengers and 6 crew aboard.

    A passenger described landing as “a bang and a bounce and then just a slam on the brakes and then it was a skidding”

    The airline confirmed a nose gear collapse, 5 passengers and 3 cabin crew reported injuries and were taken care of by local responders.

    Indigo Flight Loses Baby


    An IndiGo flight covering the 1329 miles from Mumbai to Chennai had 150 passengers aboard when one of them, an infant, suffered a medical episode.

    The seven month old was flying to Chennai for medical treatment for three holes in his heart, but an hour into the flight, suffered distress.

    Efforts to sustain the baby were made, including defibrillation and CPR.

    Pilots returned to Mumbai and made a safe landing, but on return to Mumbai, the infant was declared dead by a doctor on the ground.

    Helicopter Chops Passengers on Fishing Expedition


    On July 21, 2013, a Eurocopter crashed in Murmansk in the Kola Peninsula.

    Two British tourists who were traveling for a salmon fishing trip on the River Rynda and a Russian translator had just exited the helicopter. It was lifting off when it crash landed, striking them by the rotors when the helicopter attempted to lift off and tilted on its side, 150 kilometers from Murmansk to the tourist camp on the river Rynda.

    Four people were injured but only the pilot has survived.

    The Britons, Diamond Rupert and Mark Robinson were born in 1944, and the translator Alexander Tushnikov was born in 1960.

    Emergency services flew in from the Emergencies Ministry.

    Kazakhstan Training Crash

    A Kazakhstan Air Force Aero L-39C Albatros was on a training mission when it crashed in Akhtobe Kazakhstan.

    A teacher and a student pilot were aboard. “Methodologist of the-inspector of the security service of flights of the Military Institute lieutenant colonel aviation Colonel Vissarionov Evgeny Nikolaevich and Military Institute Cadet Massalimov Arman Evgeniyuly died on impact.”

    The crash was witnessed by passengers who had just arrived from Almaty.

    The plane crashed into the hanger.

    A criminal investigation has been opened under Article 392 of the Criminal Code of the Republic of Kazakhstan “Violation of the rules of safety and training.”

    Before Crash, Asiana Airlines Told to Revamp


    Asiana Flight 214 clipped a sea wall with its tail as it came in to land at the US airport and skidded out of control before catching fire at San Francisco’s international airport on July 6, killing three people and injuring dozens. Two months prior to the accident, South Korea’s transport ministry had requested Asiana Airlines Inc. to review safety measures.

    On April 30, the Korean government told Asiana to hire more pilots, engineers and cabin crew, to centralize its safety management system and to set up oversight for aircraft maintenance.

    Had Asiana complied with this directive? Did Asiana have adequate training for new pilots? Was the PIC aware of the situation of the SF airport? – South Korea’s Asiana Airlines will be subject of a three-week government investigation. These are some of the questions that the investigation should be covering.

    Japan Airlines Flight Returns to Boston


    A Japan Airlines 787 was en route from Boston to Tokyo when a maintenance indicator flashed early in the flight.

    Pilots returned to Boston where they made a safe landing at 6:15 p.m. The problem is being reported as a maintenance issue.

    Flightradar24 website shows that Japan Airlines flight 7 was over Hudson Bay in Canada when it turned around.

    Colombia Emergency Landing

    On July 15, 2013, an Aero Expreso del Pacífico Cessna 402B Utiliner with eleven aboard was en route from Gerardo Tobar López Airport to Pizarro Airport when it developed mechanical problems. Runway conditions and weather conditions may have contributed to the problem.

    Pilots made an emergency landing in Pizarro, Bajo Baudo, Chocò, Colombia.

    The two pilots and nine passengers were uninjured.

    The nose of the plane and the right wing sustained damage. Colombian Marine Brigade No. 2 troops removed the plane from the crash site.

    NTSB ISSUES INVESTIGATIVE UPDATE ON CRASH OF ASIANA FLIGHT 214

    The NTSB investigation into the crash of Asiana flight 214 pivots this week from the on-scene phase in San Francisco, with the NTSB closing its on-scene command post today and investigative groups wrapping up their work at the airport.

    The investigative team completed the examination of the airplane wreckage and runway. The wreckage will still be available for further examination if necessary at its secure storage location at San Francisco International Airport.

    The Airplane Systems, Structures, Powerplants, Airplane Performance, and Air Traffic Control investigative groups have completed their on-scene work and have left San Francisco. The Flight Data Recorder and Cockpit Voice Recorder groups completed their work in Washington last week.

    The Flight Crew Operations group completed several witness interviews over the weekend. The Survival Factors/Airport group will be completing their interviews of the first responders today.

    The next phase of the investigation will include additional interviews, examination of the evacuation slides and other airplane components, and more in-depth analysis of the airplane’s performance.

    Gazpromavia Mil Crash Kills 7


    On July 14, 2013, a Gazpromavia Mil Mi-8 helicopter took off, made a hard landing at Igolsko-Talovoye oil field and rolled over on its side. Fourteen workers and three crew were aboard.


    There were seven fatalities on the scene.

    The condition of the other seven aboard has not been released.

    In 2010 there was a fire in a helicopter at Igolsko-Talovoye oil field.

    Rans Crash in Leicestershire

    On July 14, 2013, a privately owned Rans S-6 en route from an airfield near Tywcross Zoo to Stoke Golding airfield crashed in Fenn Lane, Fenny Drayton, Leicestershire with two aboard.

    The Rans plane crashed in a field in Leicestershire. Witnesses heard a bang from the aircraft, the engine quit. The plane glided along, banked right and left, then crashed.

    The plane was located at Fenny Drayton at about 18:45 BST on Sunday. The man and woman aboard were confirmed dead at the scene, found by emergency responders.

    The Air Accident Investigations Branch is investigating.

    The fatalities were informally identified as Bob and Olive.

    MIG 21 plane crash in Rajasthan

    On July 15, 2013, at about 0930 hours, a vintage MiG-21 Bison fighter aircraft of IAF crashed on Monday while landing at Uttarlai Airbase in western Rajasthan. The pilot who was flying the plane died in the crash.

    The Indian Air Force fighter crashed while landing at Uttarlai Air Base, Rajasthan. The pilot had been on a routine training flight. The pilot failed to eject from the plane.

    A Court of Inquiry will be investigating.

    Utair Boeing Skids off Runway


    On July 15, 2013, a UTair Boeing 737-524 en route from Moscow to Kiev ran off the runway on landing. It was misting at the time, leading to a slippery runway but was described as coming to a quick stop at 30-40 meters on level ground.

    Firefighters took five minutes to arrive and ambulances five minutes after that but there was no fire and no injury reported.

    None of the 77 passengers or crew sustained any injury. Passengers disembarked evacuated from the plane with fixed ladders rather than inflatable emergency ladders on to the soft ground, and were transported to the gate.
