The monument, named Interrupted Flight, has been designed by Rostov sculptor Anatolly Sknarin. It shows the names of the victims on a vertical stone slab.

Tuesday Poland Remembered

As activists chanted anti-government slogans, on April 10, 2012, Poland remembered the crash that wiped out 96 Russians. Memorials were placed in Warsaw at the Presidential Palace and in Smolensk, Russia, where the crash wiped out dozens of top Polish civilian and military leaders.

Investigations concluded the crash was due to bad weather, inadequate crew training and poor guidance from Russian air traffic controllers. Some say that the disaster was not properly investigated and others say it was a deliberate assassination.

The wreckage remains in Smolensk, and according to reports was only recently covered by a tarp to protect it from the elements.

Jaroslaw Kaczynski, twin brother of the late president and leader of Poland’s Law and Justice party, boycotted the state ceremonies. He believes the disaster was inadequately investigated and that the state findings were in error.

Air France, Providing an Example

Air France

For the last ten years, the victims families of the Air France 4590 Concorde crash have been provided memorials arranged by Air France.

Air France continues that tradition by providing two anniversary memorial services for Air France Flight 447 victims, one in Rio de Janeiro for 100 participants, and one in Paris for 400 participants. Air France is providing transportation and facilities for the memorials and reunions.

The glass monument recalling the victims was placed in Leblon (Rio); it has 228 swallows etched into it, each representing someone who died aboard the flight.

Air India Express

There was also a monument placed in Kenjar, India of six scribed granite slabs remembering 158 people killed in the May 22 Air India Express crash, but it was vandalized in October of 2010. One tragedy compounded by a senseless act of malicious mischief.

Air Blue

Where is the memorial to the 152 victims of Air Blue Flight 202? Airblue management promised that the names of 19 whose bodies were consigned to a communal grave would be scribed on a monument. Where is this monument?

Where is the monument to all the 152 victims, Airblue?

Where will you be holding the anniversary service on July 28?

As the homemade cardboard tribute says “Sympathy is no substitute for action.”

12 Years and 99 Deaths Ago, TAM Airlines flight 402 Was Lost But Not Forgotten

Friday, October 31 2008

It has been 12 years since 99 people died in the Tam Congonhas Fokker-100 accident.

On October 31 1996. TAM Transportes Aéreos Regionais Flight 402 set off for Rio De Janeiro and fell to earth minutes after take-off from Congonhas airport, spreading destruction on two building and seven houses, killing 99 people: 90 passengers, six members of the crew and three people on the street.

Inquiries of TAM and Fokker, (the company who manufactured the airplane) concluded that a malfunctioning relay caused he opening of the right reverser. In December 1997, the Aeronautics Report found TAM guilty and made Fokker responsible for imperfections in the airplane.

According to the Center of Inquiry and Prevention of Air Accidents (Cenipa) Report, during take-off, the reverser of the right engine of the Fokker-100 opened, and without any alarm, was set in motion, catching by surprise a crew in the command cabin that never was trained for this situation.

The report disclosed that on 28 of June 1995, the Fokker sent a letter claiming it was not necessary to train pilots for this type of emergency because it was impossible for the Fokker reverser to open during take-off.

The report says that a system changed altered the probability of imperfection in the system to one in ten million from one in a million.

One another factor helped to knock down the airplane: the contacts of a relay (controlling of electric circuits) that should have set in motion the alarm system. This relay, according to Douglas colonel Axe, head of the Cenipa, was found 13 days after the accident.

The human error that was the consequence of these other two situations: the lack of information on pane, caused by the burnt relay, and the lack of training for the situation by the manufacturer of the aircraft. According to report, the pilots had not followed basic recommendations.

According to colonel Axe, the pilots could have gained time if they had allowed the airplane to go up to stabilize, without “forcing the engine in pane because of the reverse.”

The Cenipa said that the only acknowledgment that the pilots had of the opening of the reverser was the brusque jib of the manual acceleration of the aircraft, then later when the airplane left the soil, the landing gear.

A computer animation showed how the pilots acted in a desperate attempt to discover what was occurring.

The tragedy timeline:

8h26m: The control tower authorizes the airplane to take off.

29 seconds: The airplane runs about 90 kilometers per minute in the track.

55 seconds: The airplane raises in flight. The reverse of right turbine opens and closes for the first time, without the pilot perceiving it. Manete of the one I lock stops backwards and the pilot speeds up the turbine manually.

70 seconds: The reverser opens for the third time and it does not close.

74 seconds: The Fokker stick starts to tremble and the airplane loses height.

79 seconds: The Fokker inclines to the right, the wing striking a three story building . The airplane blows up. Ninety and nine people die.

8h27m.

Helicopter Crashes in Washington park

What: American Eurocopter Dauphin II
Where: Helicopter left its hanger at Andrews Air Force Base. suburban Washington park in a wooded area of Walker Mill Regional Park off of Ritchie Road.
When: Sunday
Who: Killed in the crash besides Bunker were Trooper First Class Mickey Lippy, EMT-B Tonya Mallard of the Waldorf Volunteer Fire Department and Ashley Younger, a 17-year-old who was the second victim from the Waldorf car accident.
Why: The helicopter was transporting car crash victims to the hospital. One eighteen year old crash victim survived both the car crash and the helicopter crash, and was transported to the hospital in critical condition.

On the twenty-five mile trip from the traffic accident to the hospital the flight was diverted due to bad weather. On approach to the runway, the pilot radioed that he was having trouble assessing his surroundings. Under deteriorating weather conditions, the pilot had requested an instrument approach and reported problems “capturing the glide slope”. The glide slope is the part of the instrument landing system that helps keep the aircraft at the proper altitude during the approach. It appears the helicopter first hit a tree at about 80 feet above the ground. The chopper landed on its left side with a tree crashing down on top of the helicopter.

Accident victim Jordan Wells, 18, survived the crash and is at a hospital in critical condition

The law enforcement agency’s helicopter fleet has been grounded until further notice.

Flight 93 Memorial

Families of four victims who died on hijacked U.A Flight 93 in 2001 held a news conference in Pittsburgh.

They are past ready to commence work on the Shanksville memorial. The memorial is to commemorate where the hijacked airliner crashed on Sept. 11– a memorial to the only one of four hijacked planes that the passengers prevented from crashing into a building.

There have been objections that the shape of the planned memorial is reminiscent of the islamic hijackers.
