<p>The aircraft, piloted by one of the first female fighter pilots in the country, was participating in a routine training exercise when it went down.</p>
<p>The pilot Yu Xu, 30, was killed in the crash while her co-pilot was able to eject before the impact using a parachute. He was injured and was taken to hospital.</p>

<p>According to a statement, “The FAA is conducting a routine National Certificate Holder Evaluation of Allegiant Air. We do these on each U.S.-certificated airline every five years. We moved up the date for Allegiant’s evaluation from 2018 to 2016 to ensure that work the carrier is doing to address various internal issues has resulted in the desired improvements. We expect to have the evaluation done by late June.”</p>
<p>The evaluation schedule was moved due to a series of recent incidents. Recently, at least two Allegiant flights made emergency landings due to shortage of fuel. <p>Authorities said the plane went down during a routine testing flight after undergoing maintenance.</p>
<p>There were two people aboard the plane, including the pilot Major Ivy Safatillah, and a technician; both of them were killed in the crash.</p>
<p>Authorities confirmed that two people, including a 47-year-old woman and 27-year-old man, who were present in the house at the time of crash were also killed.</p>

<p>There were four crew members aboard at the time; all of them were killed. They were identified as Sgt. 1st Class Toby A. Childers, 40, of Kansas; Chief Warrant Officer 3 Stephen B. Cooley, 40, of Florida; Sgt.1st Class Jason M. Smith, 35, of Louisiana; and Chief Warrant Officer 2 Michael F. Tharp, 40, of Texas. All of them were assigned to 2nd Battalion, 291st Aviation Regiment, First Army Division West.</p>

<b>When: </b> Dec 6th 2010<br />
<b>Why: </b> On takeoff from Kuwait City, the plane could not retract the landing gear.  According to Al Abdulkarim, the procedure was routine, not an emergency.  The crew returned to the airport, made a safe landing, and the problem was promptly repaired. The problem was apparently in the indicator, not the plane. After the warning light was repaired, the flight took off again within hours.  </p>
