Air Lingus Attempts and Attempts and Attempts to Land in Barcelona Before Running Out of Fuel

Contact photographer Jens Breuer

What: Aer Lingus Airbus A320-200 en route from Cork to Barcelona
Where: Barcelona
When: Mar 14th 2012
Who: 56 passengers, 6 crew
Why: Pilots on approach to Barcelona aborted their approach at 10,000 feet due to the poor visibility. Barcelona ATC refused the pilots requests for a Category III landing forcing pilots on a holding pattern for thirty minutes, then on flight on a wild goose chase looking for somewhere else to land, until the pilots eventually were about to run out of fuel.

After pilots declared an emergency, Barcelona ATC cleared the landing. Spanish authorities are investigating.

(The wild goose chase? First the denial and holding pattern, then off to Valencia, but before arriving, Barcelona said visibility had cleared, so back to Barcelona, but no, visibility hadn’t cleared. Pilots diverted to Girona, but Girona was congested, so back to Barcelona, which was finally cleared to land on declaring emergency.)

Kananaskis Pilot Dies Waiting for Air Ambulance

What: Kananaskis Heli Tours Bell 206B JetRanger II sightseeing tour from Stoney Nakoda Resort and Casino to Bow Valley
Where: Canmore, Alberta
When: March 30, 2012 10:30 a.m.
Who: 1 fatality, 4 survivors
Why: A sightseeing helicopter crashed near Grotto Mountain near Banff National Park, Canmore, killing pilot Matthew Goodine of Prince George, British Columbia. The crash occurred at 10:30 a.m. Canadian Forces Base Trenton heard the emergency beacon at 12:30 and notified the mounties. EMS did not arrive until 2:45 and the pilot died while waiting to be airlifted to a hospital in Calgary. Rescue efforts on the remote back country mountain slope were hampered by high winds and snow, along with the rugged terrain. Rescue personnel rappelled from the chopper, and the two couples from Britain were airlifted in slings to be flown to hospitals.

It began to snow around 10:30 a.m., and that may have been a factor. Weather will delay the investigation as much as a week due to snowy forecasts. There is no black box.

Family Lost in Beechcraft Crash in Minnesota Field

What: Beechcraft 35 Bonanza
Where: Glencoe, MN
When: March 21, 2012, 11:00 a.m.
Who: 3 fatalities
Why: The plane crashed on farmland four mines north of Glencoe killing what appears to be a family of three aboard the plane and also three dogs. Conditions were five miles visibility and overcast at 900 feet and the pilot may have been flying by visual flight rules in conditions when VFR flight isn’t allowed. Visibility in the area is known to drop “at the drop of a hat” so the pilot may have flown into worsening conditions inadvertently.

Witnesses say they heard the plane sputtering, and heard a popping noise before impact. Others saw the plane flying low before it hit the ground in foggy drizzly weather leaving a two-hundred yard debris field. The plane may have been flying from St. Cloud.

By the time bystanders arrived on the scene, the plane was missing the tail and a wing, and was engulfed in a “ball of flame.”

The plane had just been sold by a St. Cloud resident to a new owner.

Stuart Dahlberg, 52, his wife Ivelisse Morillo, and his mother, Mae Dahlberg were en route to visit family in Colorado when they were killed in the crash. Stuart Dahlberg was an experienced pilot.

What: Norwegian military Hercules C-130-J cargo plane en route from Evenes military base to Kiruna Sweden (military exercise)
Where: Kebnekaise Mountain
When: March 15, 2012
Who: 5 fatalities
Why: The plane disappeared from radar as it crossed a mountain range in northern Sweden. The Norwegian armed forces indicated that the accident may have occurred when pilots flew manually instead of on autopilot. The mission involved tactical flying. Crew members lost in the crash were Lt. Col. Truls Oerpen, 46; Capt. Staale Garberg, 42; Capt. Bjoern Yngvar Haug, 40; Capt. Steinar Utne, 35; and Capt. Siw Robertsen, 45

Guatamalan Rescue Chopper Crashes after Attempted Rescue

What: Guatemalan air force helicopter Bell UH1H
Where: Quebrada Seca, Petén Guatemala
When: Feb 28 2012 10 am
Who: 10 fatalities
Why: After a civilian chopper crashed, the Guatemalan air force helicopter on a regular Tuesday mission went to assist in the rescue but crashed en route. The seven officers and three air force specialists diverted from their mission “to supply some detachments are located in the Laguna del Tigre,” and died in the crash in bad weather in the mountainous area. It appears that when the military chopper reached the scene of the first crash, the rescue had already been handled, so they elected to continue on their mission, and it was on the continuation of the journey to Peten that the accident occurred.

According to eyewitnesses, the helicopter lost power near the village of Quebrada Seca, in the vicinity of the town of San Luis.

Three were crew—two lieutenants and a specialist—and the seven passengers were a flight lieutenant, two second lieutenants of aviation, a second lieutenant of infantry, two specialists and an aére-military policeman. Pilot Lieutenant Everaldo Portillo had more than 700 flight hours.

The first chopper apparently hit power lines which apparently helped break the fall. It crashed in the El Guarumo in Semox village on Guatemala’s Caribbean coast ( Livingston, Izabal). Three injured survivors of the original crash were taken to del Hospital Roosevelt in Morales, Izabal. The survivors of the first crash were listed as Carlos Larios Aguilar, 48, the pilot of the ship John Paul Moran, 52, and mechanic Jose Gomez, 28. The first chopper was #TG-SAN, also out of Petén.

Flight 409: Ethiopia Airlines Rejects Official Report

Click to view full size photo at Airliners.net
Contact photographer Alastair T. Gardiner

The report from Lebanon on the January 2010 crash of ET409 has been rejected by Ethiopia Airlines. It is not surprising that they would reject it as the report blames Ethiopian airlines; specifically they cite the probable cause of the crash as being “the flight crew’s mismanagement” and “a failure in basic piloting skills,” and that the pilot flew 188 hours in 51 days with minimum rest.

The crew ate during their Beirut stop, and complained they could not sleep afterwards.

Ethiopian Airlines claims the government withheld information, and claim the plane exploded.

No sign of fire or explosion were detected in the wreckage

What: PNG Airlines de Havilland Canada DHC-8-102
Where: 20 km (12.5 mls) S of Madang Airport (MAG) (Papua New Guinea)
When: Oct 13, 2011
Who: 4 crew, 28 passengers, 4 survivors
Why: About 5.15pm, while en route from Lae to Madang, the flight went down in dense forest in bad weather. The pilots managed to send a distress call. Four survivors were pulled from the wreckage and taken to Yamad clinic. The survivors include an 64 year old Australian pilot and a New Zealand pilot and an Asian passenger who was sitting in the seventh seat, who crawled through a crack in the fuselage, and who suffered burns on 27% of his body (including back and arms). Survivors are being transported by boat to Modilon hospital in Madang.

First responders were initially blocked when they could not cross the Guabe river, then a boat suffered engine problems, then the third attempt made it to the scene of the crash. A violent storm and dense forest hampered the rescue efforts, which were facilitated by chopper.

Australia engaged four army Blackhawk helicopters to assist in the rescue. Rescuers on the Gogol river were impeded due to heavy rains. It took four boats to reach the crash site.

PNG quarantined Laes Nadzab airport and grounded the Dash 8 fleet.

Witnesses saw the plane crash on the Guabe River bank and on impact, burst into flame. Most of the passengers were local Papuan parents of Madang’s Divine Word University students. According to a resort manager who went to pick them up from the airport, six of the plane’s occupants — one passenger and five crew members – had reservations to stay at Madang Resort.

PNG Accident Investigation Commission’s spokesman Sid O’Toole talked to the media.

PNG Airlines released the following:

POMSOX/MEDIA RELEASE – PORT MORESBY 10:30 PM THURSDAY, 13th OCTOBER 2011 – AIRLINES PNG ANNOUNCEMENT

Chile Turns to Psychic Leonor del Carmen Díaz

What: Grupo 10, Fuerza Aerea de Chile CASA 212 Aviocar en route from Santiago to isla Juan Fernandez
Where: near the Juan Fernandez islands
When: Sept 2, 2011 4:58 p.m.
Who: 21 aboard, 21 fatalities
Why: The Chilean air force plane on a relief flight attempted to land, but the landing was aborted twice as pilots fought hard winds. On the third attempt, the flight disappeared from radar, as witnesses saw it veer left. It was lost at sea.

In addition to the usual sources, Defence Minister Andres Allamand of Chile told the world Chile has turned to a psychic who was on one of the search boats as it looked for 17 bodies missing off of the island believed to have been the setting for the novel Robinson Crusoe by 18th century British author Daniel Defoe, the main island of the Juan Fernandez archipelago.

The local psychic who assisted was Leonor del Carmen Díaz. She drew a map of her “vision” of the accident that revealed to her body of the “plane on the ocean floor with victims’ bodies still strapped in their seat belts inside it.”

WE believe in whatever it takes to find the bodies, and if a Psychic works by all means.

The instrument approach to the airport requires overflying North and then turning back. (See the airport chart below.)

The last contact was reporting the flight was “in the procedure turn” but the flight did not land on the runway, and went missing in a mountainous area.

Rescuers reached the crash site in the afternoon of Sept 8 and confirmed that there were survivors.

The wreckage of the plane was located 30km north-east of the city of Trinidad, in the Amazon jungle province of Beni. Chacon reported on thursday that a helicopter crew had seen the plane “completely destroyed” in a difficult-to-reach jungle region.

Six bodies were recoverable immediately when the wreckage was found. Two bodies trapped in the wreckage required rescue teams and equipment.
