Sukhoi plane crash Caused by Pilot Error

The National Commission on Safety Transportation announced that the Russian Sukhoi Superjet 100 that crashed into a mountain in Indonesia seven months did so as a result of pilot error. All 45 Aboard were fatalities.

Indonesian authorities claimed that the plane functioned adequately and there were no malfunctions of safety gear.

The fact that the pilot digressed from the route, and that the (foreign) pilots were not well aware of the mountainous terrain may be behind part of the decision. As a demonstration flight, the pilots were probably encouraged to make dramatic maneuvers at a terrible price.

Thirty-eight seconds prior to the impact, cockpit instruments issued a warning saying “pull up, terrain ahead”. Later the warning “avoid terrain” was issued six times, but the instruments were turned off because the crew assumed there was a problem with the database.

Officials said that “The crew was not aware of the mountainous area surrounding the flight path,”

The Jakarta radar service was incapable of informing flight crews of minimum safe altitudes.

The Russian pilots, while experts, were not intimate with the Indonesian topography.
Six minutes after takeoff, the pilot and co-pilot requested Jakarta ATC for permission to drop from 3,000 meters to 1,800 meters on the scheduled half-hour flight. Six minutes later, the plane struck Mount Salak.

@JetBlue #LoveforNoah Lights the World

Too fast for Sunday delivery of USPS, UPS or Fed Ex.

If you ask Victoria Haller which is the better airline, she’ll tell you it is JetBlue. That’s because she’s the aunt of Noah Pozner, one of the victims of the massacre in Newtown Connecticut; and when a friend tweeted airlines asking if one could get family letters to Connecticut in time for Noah’s funeral so family letters (including that of Victoria’s five year old son) could be buried with his cousin Noah, JetBlue stepped up. Jetblue called back and made it happen. The letters went from Aunt Victoria to JetBlue flight attendant Craig M. to flight attendants Trisha and Heather.

The letters made it from Seattle to Connecticut in time for the 1 p.m. funeral.

All of this was tracked on Twitter (search the hashtag #LoveforNoah), Huffington Post, MSN, UPN, Jspace, etc…

It does the world good to hear of your kindness. When someone takes an extra step, goes beyond, it reminds us that businesses are not made of steel and ink and the bottom line; they are built of human beings. We are all human, and sometimes we are all we have. JetBlue is an example to us all. A simple act of kindness. A light in the darkness. Thank you.

NCAR Beta

Funded from NASA’s Applied Sciences Program, the National Center for Atmospheric Research (NCAR) and several partners developed an 8-hour convective weather forecast based on fuzzy logic and cloud top and moisture level input from the two Geostationary Operational Environmental Satellites (GOES) that cover a large portion of the Atlantic and Pacific oceans.

Storms and turbulence, are noted in deep magenta (closest to the value of 1.0). Visitors to the site can then click on any of the eight hourly forecasts that follow, or play all of the pictures together in movie mode.

Wildlife Feedback Wanted

The FAA is looking for comments regarding minimum acceptable standards for the conduct and preparation of Wildlife Hazard Site Visits, Hazard Assessments and Hazard Management Plans on the following document:

For more information, visit Clarification of Wildlife Hazard Management Requirements for Non-Certificated Federally Obligated Airports in the National Plan of Integrated Airport Systems (NPIAS)

The purpose of this notice is to clarify the FAA’s interpretation of 49 U.S.C. 47107(a) (19) and the corollary Grant Assurance No. 19, relating to airport operations and maintenance. The FAA proposes to require sponsors of federally obligated, non-certificated airports that, after the effective date of this Federal Register Notice, accept a new airport development grant funded under the Airport Improvement Program, or accept a transfer of land under the Surplus Property Act for airport purposes to identify and mitigate wildlife hazards at their airports. These actions will take the form of initial Wildlife Hazard Site Visits (WHSVs) or Wildlife Hazard Assessments (WHAs), depending on the size of the airport, potentially followed by more detailed Wildlife Hazard Management Plans (WHMPs).
The purpose of a WHSV is for the sponsor to identify any immediate hazards and for the FAA to determine whether a more comprehensive WHA is necessary.

For More information, see APHIS below:
