Plane crashes and Five Die in Caldas Novas

A Embraer 810D Seneca III plane en route from Ouro Fino to Gurupi was supposed to stop in Caldas Novas to refuel on Sept 20, but it crashed near Nelson Ribeiro Guimaraes airport. Five people——three men and two women——were killed in the crash at about 10:45 a.m. in a semi-urban area.

The plane may have been on approach as its landing gear were down. There was no fire, explosion or leaked fuel at the accident site.

It is expected that the missed fuel stop resulted in the plane crashing due to being out of gas. The forest where the plane fell was about 600 meters from the runway at Nelson Ribeiro Guimarães Airport. The victims were identified as John Constantini, 81, owner and pilot of the plane, Marcia Maria da Silva, 51, Vilmar Alves Passion, 40, Isolina Tumioto, 63, and John the Baptist Constantini, 51. Their remains were taken to Forensic Medical Institute.

The Center for Research and Prevention of Aeronautical Accidents (Cenipa) is investigating.

Dubai -Toronto Flight, First Ever A380 Diverts to and Lands in Ottowa

What: Emirates airbus A380 en route from Dubai to Toronto
Where: Ottowa
When: June 1, 2012
Who: 530+ passengers
Why: After taking off from Dubai, the pilot encountered problems. In fact, pilots circled Toronto before they diverted to Ottowa, where they made a safe landing with firefighters and paramedic emergency services on standby. The plane, the first A380 ever to land at Ottawa International Airport, touched down at 5 pm.

Pilots diverted to Ottowa because Ottawa International Airport has a 10,000-foot runway. An A380 requires at least 9,875 feet.

According to Global Toronto, the jet nearly ran out of fuel after being rerouted due to torrential rains in Toronto.

The flight eventually made it to Toronto at 9:00 pm.

Air Lingus Attempts and Attempts and Attempts to Land in Barcelona Before Running Out of Fuel

What: Aer Lingus Airbus A320-200 en route from Cork to Barcelona
Where: Barcelona
When: Mar 14th 2012
Who: 56 passengers, 6 crew
Why: Pilots on approach to Barcelona aborted their approach at 10,000 feet due to the poor visibility. Barcelona ATC refused the pilots requests for a Category III landing forcing pilots on a holding pattern for thirty minutes, then on flight on a wild goose chase looking for somewhere else to land, until the pilots eventually were about to run out of fuel.

After pilots declared an emergency, Barcelona ATC cleared the landing. Spanish authorities are investigating.

(The wild goose chase? First the denial and holding pattern, then off to Valencia, but before arriving, Barcelona said visibility had cleared, so back to Barcelona, but no, visibility hadn’t cleared. Pilots diverted to Girona, but Girona was congested, so back to Barcelona, which was finally cleared to land on declaring emergency.)

Passengers Get the Raw Deal in Kolkata

What: Mahan Air en route from Phuket to Tehran
Where: Kolkata airport
When: September 21, 2011
Who: 305 passengers (plus another 176 passengers from another flight)
Why: Mahan Air ran out of gas in the wrong place and the wrong time. The IOC allows the airline a certain amount of credit, when they pay a security deposit, but they were a month overdue.

The flight landed in Kolkata, the crew expecting to be able to refuel, and they were grounded. (Indian Oil Corporation.) So the airport lounge filled up with passengers from one…no, make that two Mahan Air flights when a second Mahan flight showed up. There was only room for 280, and the passengers were stuck until morning. Some sources say all 305 were stuck in the airplane, not in an overcrowded lounge for thirteen hours. Anyway, was this the experience they bought when they paid for their ticket? Apparently.

Spectrum Flies Students While in “EM” Status-School board Unaware

Transport Canada’s Enhanced Monitoring System http://www.tc.gc.ca/eng/civilaviation/opssvs/managementservices-referencecentre-documents-sur-sur-002-1028.htm is an aviaition surveillance program, the entry into which is based on an airline’s failure to meet minimum requirements. A score of 3, 4, 5 or a score of 2 where there are minor or moderate findings and there is no evidence of systemic failures, will return the certificate holder to routine monitoring. A score of 1 will result in the issuance of a Notice of Suspension.

A plane which made an emergency landing (termed a miracle landing, and the pilot heavily lauded) while carrying three Grade 9 geography students from White Oaks Secondary School was under enhanced monitoring at the time; and since the emergency, has been suspended.

The plane made an emergency landing with the students in a field. For twenty years, Spectrum Airways has had an agreement with the Halton District School Board to provide scenic flights for Grade 9 geography students. The Cessna Skyhawk 172 had run out of fuel, and the pilot landed with no damage or injuries, landing in a field behind Corpus Christi Catholic Secondary School.

Burlington Airpark, Spectrum Airways and Kovachik Aircraft Maintenance are responsible for maintenance.

What: Ontario Limited-Spectrum Airways Cessna 172M Skyhawk en route from Burlington Airport

Congo: Filair Crash kills 21

What: Filair Let 410 en route from Semendwa to Bandundu
Where: Bandundu Congo
When: August 25 2010
Who: 21 aboard (18 passengers 3 crew), 20 fatalities
Why: The preliminary information says that the plane was unable to land, and ran out of fuel. Landing short of the runway, the plane struck a building, killing two children inside.

The Let is a Czeck turboprop.

The Filair is based in Kinshasa. It’s fleet is reputed to be poorly maintained and is on the nofly list. The plane’s owner Danny Philemotte was piloting and died in the crash.

19 bodies were recovered and two survivors. One survivor died in the hospital. One survivor is hospitalized and in critical condition.

A survivor said that the crew had been told to land on a “reserve strip” beside the main runway. The passengers panicked, running to one side and unbalancing the plane, and the pilot lost control. Apparently fuel was not the issue (150 liters liters recovered from the crash.)

Legacy Crash Murieta CA

Pictured: A Lancair Legacy 2000
Click to view full size photo at Airliners.net
Contact photographer Connector

What: Poulin L/Cullen D Lancair Legacy
Where: Murrieta, CA
When: May 30 2008
Who: 2 aboard, both fatalities
Why: The Lancair Legacy was heading south to the runway, made a hard right turn and nosed into the ground with one “loud engine rev” before the engine ” died out.” The propeller was not under power when it impacted terrain. The wings, wreckage, vegetation, and ground did not indicate signs of a fuel spill. It appeared that the airplane had run out of fuel and stalled on approach, causing it to enter a spin prior to impacting terrain in a near vertical attitude. Cited as cause was the pilot’s inadequate preflight planning, which resulted in the loss of engine power.
