Forty Apartments Damaged, No Fatalities Reported, Virginia Beach Hornet

What: US Navy – VFA-106 $29 million McDonnell Douglas F/A-18D Hornet twin-engine supersonic fighter plane en route from Naval Air Station Oceana
Where: Virginia Beach
When: April 6 2012, 12:00 pm
Who: 40 apartments damaged, no fatalities, 5 injures
Why: Going 170 mph, a student pilot was in the front seat of the two-seat Hornet and the experienced instructor was in the rear when the plane experienced a “catastrophic mechanical malfunction.” Pilots dumped fuel just before it dived into a Virginia Beach apartment complex in the Hampton Roads area (Birdneck Road, Virginia Beach, near NAS Oceana). No residents of the Mayfair Mews complex were killed in the crash, and the pilots ejected before impact. The plane experienced “catastrophic engine system failure” right after takeoff. The crash site is two miles from the base and a mile from the ocean.

The fuel and the carbon fibers from the burning composite were a special toxic concern for first responders.

There were seven injuries on the ground in addition to the two pilots, but no fatalities have been reported. Witnesses on the scene saw smoke billowing from the building. Bystanders found the young pilot still in his parachute, and cut him free before escorting him farther from the fire. Six buildings were damaged, but now the residents, mostly elderly, are homeless.

The trainee in the front had at least 1 1/2 years of intensive training before boarding the F/A-18D, which is used in training and in combat. The jet was not carrying live ordnance,

Within 24 hours of the crash, sources confirm no one was killed in the crash. The mayor, and the us fleet forces commander agree that it is miraculous that there were no fatalities.

A fund has been established for those made homeless by the fire.

Guatamalan Rescue Chopper Crashes after Attempted Rescue

What: Guatemalan air force helicopter Bell UH1H
Where: Quebrada Seca, Petén Guatemala
When: Feb 28 2012 10 am
Who: 10 fatalities

The seven officers and three air force specialists diverted from their mission “to supply some detachments are located in the Laguna del Tigre,” and died in the crash in bad weather in the mountainous area. It appears that when the military chopper reached the scene of the first crash, the rescue had already been handled, so they elected to continue on their mission, and it was on the continuation of the journey to Peten that the accident occurred.

According to eyewitnesses, the helicopter lost power near the village of Quebrada Seca, in the vicinity of the town of San Luis.

Three were crew—two lieutenants and a specialist—and the seven passengers were a flight lieutenant, two second lieutenants of aviation, a second lieutenant of infantry, two specialists and an aére-military policeman. (José Everardo Portillo Salazar, Manuel Ovalle Garcia and Armando Barrientos Mynor, the lieutenants Jose Alejandro Rodriguez and Flores Be Zechariah, Amilcar specialist Humberto Leon and Manuel Escobar Axpuac, Emerson Reyes and Paolo Rivera Martinez Galindo Elbu Stuart, One person was unidentified.)

Pilot Lieutenant Everaldo Portillo had more than 700 flight hours.

Three injured survivors of the original crash were taken to del Hospital Roosevelt in Morales, Izabal. The survivors of the first crash were listed as Carlos Larios Aguilar, 48, the pilot of the ship John Paul Moran, 52, and mechanic Jose Gomez, 28. The first chopper was #TG-SAN, also out of Petén.

Ground Resonance Rattles Brazilian Chopper to Bits

What: Corpo de Bombeiros Militar do Estado do Pará Helibras HB 350BA Esquilo
Where: Benevides Brazil
When: Feb 22, 2012
Who: 4 aboard, no fatalities
Why: The rescue chopper vibrated itself apart after landing.

“Ground resonance” occurs when a sitting helicopter’s rotors become unbalanced while spinning. A resident expert says this can happen because of a hard landing; the best way to get it to stop is to pull collective and break contact with the ground. (At the time, that would feel counter intuitive, I would think!)

Helicopter Crashes in a Street in Mexico

What: Gobierno del Estado de Mexico Agusta A109S Grand
Where: Peréz Valenzuela (street), Del Carmen district, in Coyacán, Mexico
When: Oct 21, 2011
Who: 4 aboard, 2 fatalities
Why: If you imagine going about your normal daily life, you do not expect a chopper to fall in the street, crush your cars and slam into your house.

A chopper owned by the government of Mexico fell in the streets of Coyoacán, between Guillermo Pérez Valenzuela and Avenida Universidad, landing on two vehicles and striking the wall of a house. Because of a fuel spill, the area was cordoned off.

The chopper is an Agusta A-109-S Grand, XC-EDM

Initial coverage was confusing and indicated no fatalities, then three fatalities, then two fatalities with identities unknown.

Eventually two people were confirmed to be deceased, and two as survivors, but the identities took some time to sort out. Two names were released: Juan Estevan Rodriguez (Ruiz Rodriguez) and Julius Caesar Maturano Gomez were reported to be the fatalities—that is, one of the pilots, and the assistant of the secretary of Metropolitan development. We can assume this is correct because of reports of officials attending the funerals in #7 Rue insurgentes in Sapotian neighborhood, Santa Maria Cuevas, Zumpango.

Fernando Alberto Garcia Cuevas (the government official) and the surviving pilot were rushed to Hospital Ángeles Pedregal by emergency responders. Cuevas suffered bleeding of a kidney, which is being addressed by catheter embolization. The surviving pilot, Juan Manuel Ramírez Rivera will be undergoing surgery for broken vertebra in his chest, when he is stabilized.

The pilots attempted to land and save the lives of their passengers and we acknowledge their heroic effort.

Our sources have one photo of the chopper in question:

There was significant confusion in the reportage, so that based on reported coverage, all I can say is this:

There may have been a flaw in the rotor of the aircraft, though it is not clear if it was something that the pilots had been informed of prior to the flight, or if there was a failure during the flight. There was also a statement published that the chopper fell out of the sky, and was seen plummeting, and that the rotor stopped because the hasp detached (or did not detach) from the main rotor. I am sure the truth come out in the investigation; it is too soon to know what happened, but if any parties in the know want to comment or make corrections, we welcome the input.

As always, condolences to the families of Rodriguez and Gomez, and best wishes to the recovery of the injured.
