FAA Proposes $547,500 Civil Penalty Against Hawaiian Airlines

The U.S. Department of Transportation’s Federal Aviation Administration (FAA) is proposing a $547,500 Civil Penalty against Hawaiian Airlines, Inc. for operating a Boeing 767-300 that was not in compliance with Federal Aviation Regulations.

The FAA alleges Hawaiian operated the aircraft thousands of times when it was not in compliance with a July 2000 Airworthiness Directive (AD) that required inspections of certain engine thrust reverser components. The purpose of the AD was to prevent a portion of the thrust reverser from coming off in flight, which could cause a rapid decompression of the aircraft.

The AD required initial and repetitive inspections of the components to detect damage and wear, and corrective actions if necessary. It required replacement of the components with new and improved parts within four years of the AD taking effect.

During a July 2012 inspection, the FAA discovered that some of Hawaiian’s records erroneously showed the AD did not apply to one of its Boeing 767 aircraft. The FAA alleges Hawaiian operated the aircraft more than 5,000 times – mostly on passenger carrying flights – between July 2004 and July 2012 when it was out of compliance with the AD. The FAA further alleges Hawaiian operated the aircraft on 14 passenger flights after the agency alerted the carrier that some of its records erroneously indicated that the AD did not apply to the aircraft.

Additionally, the FAA alleges Hawaiian failed to keep required records of the status of the AD for the aircraft in question.

Hawaiian has requested an informal conference with the FAA to discuss the matter.

Hawaiian Airlines Oil Light Cuts Flight Short

If you’re one of those drivers who ignores that oil light on your car’s dashboard when it goes off, then be glad you’re not a pilot. Pilots have the same light, and it can’t be ignored.

It’s a good thing you’re not a pilot, then.

258 passengers aboard a 10:30 a.m. Hawaiian Airlines A330 to Los Angeles were relieved to be back on the ground after the plane they were on developed engine problems and had to return to Honolulu.

Fifteen minutes into the flight, the oil light went on. Pilots shut down the affected engine.

Passengers caught the alternative flight at 11:15 a.m.

Hawaiian Airlines Appoints Susan Donofrio as Senior Director of Investor Relations

HONOLULU, Feb. 7, 2011 — Hawaiian Airlines (Nasdaq: HA) today announced the appointment of Susan Donofrio as the company’s Senior Director of Investor Relations, effective immediately. Ms. Donofrio will be responsible for interacting with Hawaiian Airlines’ global investor. She will report to Peter Ingram, Chief Financial Officer, and will be located in New Jersey.

.
Prior to joining Hawaiian Airlines, Ms. Donofrio was a Director level Analyst at Cathay Financial where she was responsible for the Aviation, Aerospace and Trucking industries. She also has held similar roles at Fulcrum Global Partners (Soliel Securities), as well as Deutsche Bank. Ms. Donofrio was recognized by the Wall Street Journal as a top airline analyst for stock picking for 2005-2007. She was also ranked #1 in stock picking according to Forbes in 2007.

Donofrio received her MBA in Finance at The Lubin School of Business, Pace University. She received her BA from Rutgers University.

Hawaii Flight Diverts to Sea Tac

What: Hawaiian Airlines Boeing 767-300 en route from Portland to Kahului Hawaii
Where: Sea-Tac, Seattle
When: Dec 28, 2010
Why: After takeoff from Portland, the crew discovered a problem with the trim. Protocol dictated diverting to Seattle. The plane made a safe landing around 11:00 a.m. after a couple of hours in the air.

No injuries were reported.

The NTSB is investigating.

Hawaiian Airlines Flight Encounters Turbulence

What: Hawaiian Airlines Boeing 767-300 en route from Pago Pago American Samoa to Honolulu,HI
Where: Pago Pago
When: Dec 2nd 2010
Who: 173 passengers 12 crew
Why: While en route, the flight encountered unanticipated turbulence. The sky was clear but storms were reported in the area.

Three flight attendants and one passenger were injured; one flight attendant broke her leg.

The flight continued to Honolulu and made an uneventful landing.

When: Jun 3rd 2010
Who: 117 people on board,
Why: After takeoff, smoke was seen coming from the plane’s engine, and the crew observed irregular indications. The flight returned to Honolulu for maintenance. A replacement jet was provided.

Why: While en route, the flight developed a hydraulic leak. The crew decided to divert to San Francisco where the leak was repaired after a safe landing.
