IMPROPER MAINTENANCE LED TO Vegas AIR TOUR HELICOPTER CRASH

What is it that I’ve always said? Maintenance, Maintenance, Maintenance.

Looks like the NTSB Findings agree with me! See their report below about a helicopter crash in December 7, 2011, that occurred in my home away from home, Las Vegas Nevada.

PRELIMINARY REPORT
On December 7 at 4:30 Pacific Standard Time, a Eurocopter AS350-B2, operated by Sundance Helicopters as flight Landmark 57, crashed in mountainous terrain approximately 14 miles east of Las Vegas. The flight, a sightseeing tour, departed Las Vegas McCarran International Airport (LAS) en-route to the Hoover Dam area was operating under visual flight rules. The helicopter impacted in a narrow ravine in mountainous terrain between the cities of Henderson and Lake Mead. The pilot and four passengers were fatally injured.

The National Transportation Safety Board determined today (Jan. 29, 2013) that the probable cause of the Dec. 7, 2011, air tour helicopter crash near Las Vegas, Nev., was inadequate maintenance, including degraded material, improper installation, and inadequate inspections.

“This investigation is a potent reminder that what happens in the maintenance hangar is just as important for safety as what happens in the air,” said NTSB Chairman Deborah A. P. Hersman.

At about 4:30 p.m. Pacific standard time, a Sundance Helicopters Eurocopter AS350, operating as a “Twilight City Tour” sightseeing trip, crashed in mountainous terrain about 14 miles east of Las Vegas, Nev. The helicopter originated from Las Vegas McCarran International Airport at about 4:21 p.m. with a planned route to the Hoover Dam area and then return to the airport. The accident occurred after a critical flight control unit separated from another, rendering the helicopter uncontrollable. After the part separated, the helicopter climbed about 600 feet, turned about 90 degrees to the left, descended about 800 feet, began a left turn, and then descended at a rate of at least 2,500 feet per minute to impact. The pilot and four passengers were killed and the helicopter was destroyed.

The NTSB found that the crash was the result of Sundance Helicopters’ improper reuse of a degraded self-locking nut in the servo control input rod and the improper or non-use of a split pin to secure the degraded nut, in addition to an inadequate post-maintenance inspection.

Contributing to the improper (or lack of) split pin installation was the mechanic’s fatigue and lack of clearly delineated steps to follow on a “work card” or “checklist” The inspector’s fatigue and lack of a work card or checklist clearly laying out the inspection steps to follow contributed to an inadequate post-maintenance inspection. As a result of this investigation the NTSB made, reiterated and reclassified recommendations to the Federal Aviation Administration.
“One of the critical lines of defense to help prevent tragedies like this crash is improved maintenance documentation through clear work cards, or checklists,” Hersman said. “Checklists are not rocket science, but they can have astronomical benefits.”

CHC Heli Returns Safely from North Sea Flight to Aberdeen over Warning Light

What: L2 CHC Scotia Super Puma en route from to the Awilco-owned WilHunter rig in the North Sea

Who: 16 passengers 2 crew
Why: The pilot was flying to the Wilhunter platform when a warning light came on in the plane.

The flight made an emergency return to Aberdeen, with the Scottish Ambulance Service on standby.

The helicopter is being examined for the nature of the problem that caused the warning. No injuries were reported.

Helicopter lost in Baltic Sea, 2 aboard

What: Helitrans Eurocopter AS 350 Ecureuil en route from Lübeck, Germany to Halmstad Sweden

Why: The Helitrans Eurocopter has been missing since December fifth over the Baltic Sea. The emergency beacon located by a German Navy vessel off the coast of Fehmarn island appears to have been a false alarm. Danish air ATC lost radar contact with the helicopter at 13:56 Swedish time, about 10 minutes after takeoff into what should have been a 2 hour flight.

The pilot is a Norwegian employee of Heli Trans. The other has foreign citizenship and is not employed by Heli Trans.

The search operation for the missing Norwegian helicopter was suspended overnight from 10 o’clock p.m. on Dec 5 but rescue centers in Denmark, Norway and Sweden have been involved. Nine search vessels are in the area east of Neustadt in Holstein. See German Link here:No traces of fuel or oil in the water have been found, and the beacon that was located by the German ship appears to have been a lost cell phone. It was not in the expected flight path.

The air search has resumed, with a German Sea King helicopter now searching. Recent snowfall is impeding search more difficult.

The passenger’s name has not been released but the next of kin have been informed.

CHC North Sea Crash Inquiry In Progress

Why: The helicopter that was ditched in the North Sea has been retrieved and taken to Peterhead, Aberdeenshire.

The UK Air Accidents Investigation Branch (AAIB) is investigating the incident in which seventeen passengers and two crew were on the way to the drilling rig when the crew had to ditch the helicopter (controlled ditching) in the North sea.

Summary:

This Special Bulletin contains information about the helicopter’s main gearbox (MGB) lubrication system and the results of an investigation into the indication of a failure of the emergency lubrication system, after it was activated by the crew. It follows publication of two earlier Special Bulletins on this accident, AAIB Special Bulletins S2/2012 and S3/2012.

This was the first occasion that the EC225 LP MGB emergency lubrication system had been operated in-service. One Safety Recommendation is made.

The investigation into the failure of the bevel gear vertical shaft in the MGB continues.

CHC Suspending Super Puma Flights

After yesterday’s Super Puma ditching in the North Sea, CHC helicopters suspended Puma flights due to suspicions of possible flaws in the design of the main gearbox lubrication system.

The concern is a hangover from a 2009 Bond Aviation crash that led to recommendations to look into the Super Puma’s gearbox lube system design.

Drilling rig Flight Ditches in North Sea

What: CHC (for Total) Helicopter Eurocopter EC225 Super Puma en route from Aberdeen to West Phoenix drilling rig

Who: 19 aboard, 0 fatalities
Why: Seventeen passengers and two crew were on the way to the drilling rig when the crew had to ditch the helicopter (controlled ditching) in the North sea.

All 19 aboard were picked up by the Nord Nightingale.

The first official Maritime & Coastguard Agency press release said:

Shetland Coastguard has been coordinating a major search and rescue operation after a helicopter ditched off the coast 15 miles west of Fair Isle this afternoon.There were 19 people on board, all of which have now been accounted for. Three RNLI lifeboats (Kirkwall, Aith and Lerwick) went to the scene, along with the Coastguard Search and Rescue Helicopter from Stornoway, RAF Lossiemouth, and the Bond rescue helicopter from Sumburgh. A vessel in the area at the time – the Nord Nightingale – launched its fast rescue craft. This rescued the 19 people from their life raft and brought them back to the tanker. From here they were flown by the RAF helicopter and Bond helicopter back to Kirkwall.It is believed the helicopter was on its way from Aberdeen to West Phoenix drilling rig.

Lakewood Traffic Interrupted by Chopper Street Landing

What: Los Angeles County Sheriff’s patrol Air-24-American Euro Copter
Where: Lakewood California
When: Oct 21, 2012 8:50 p.m.
Why: The flight crew heard a loud noise from the helicopter and made an emergency street landing at Del Amo Boulevard and Paramount.

After making a safe landing and determining that a bird had struck the fuselage above the pilot canopy, they returned to the helicopter and flew back for repairs.

Lakewood Sheriff Deputies handled traffic until the helicopter left the scene.

Kenya Crash Investigation in Spotlight

What: Kenya Police Air Wing Eurocopter AS 350B3e Ecureuil
Where: Ngong Hills, near Nairobi Kenya

Who: 6 aboard, 6 fatalities
Why: Kenyan Internal Security Minister George Saitoti was killed in a helicopter crash on June 10, 2012. Clatus MacOwenga had been appointed in the investigation, and the appointment was backdated to January 5 to fill a legal gap for the period when no chief accidents investigator was in office. MacOwenga apparently was running an investigation separate from that of the President Kibaki’s public inquiry.

Martin Lunani is now the lead investigator, appointed for a period of three years : August 18 to August 17, 2015. The demotion occurred prior to Mr Macowenga submitting a report. Macowenga will now report to Mr Lunani.

The end of the investigations is scheduled to be October.

Why: An Angolan National Police helicopter crashed killing captain chief-inspector Tiago Casimiro, his assistant, João Adelino João Brandão, and Provincial Electoral Commission employees Lourenço Mussenoho and Maria Augusta João. Investigators have been sent to Lunda Norte Province.

Emergency Landing in Hollywood


What: KTLA EUROCOPTER AS350 helicopter
Where: Hollywood Boulevard and Franklin Avenue.
When: August 28, 2012 1:00 pm
Who: pilot and passenger
Why: When the helicopter started giving off blue and white smoke, the first to see it was the pilot of another helicopter.

KTLA pilot Tim Lynn declared an emergency, then made a forced landing in a parking lot of the senior center at Hollywood Boulevard and Franklin Avenue.

The incident was witnessed by SKY2 pilot Dan Catalano (who taught Lynn to fly heicopters.)

The smoke came from an oil leak. Some newspapers report two aboard the helicopter but the FAA entry only reports the presence of the pilot.

French Alps Chopper Crash Kills 6


What: Eurocopter group Eurocopter AS 532AL Super Puma
Where: La Palud-sur-Verdon, Alpes-de-Haute-Provence
When: July 25, 2012, 13 h 40
Who: 6 aboard, 6 fatalities
Why: In the village of La Palud-sur-Verdon, a Eurocopter group helicopter crashed, killing the six men aboard, including employees of Eurocopter. The newly assembled chipper was en route to the Albanian army when it apparently “caught a EDF power cable.”

Two of those aboard may have been Albanian. Climbers were dispatched to assist with the recovery.

Chopper down in Malaysia, 3 missing


What: Eurocopter EC120 helicopter en route to Nanga Merit Kapit Division
Where: Sarawak, Malaysia
When: July 20, 2012
Who: 4 aboard
Why: A privately owned helicopter with four people aboard went down Sungai Teriso estuary near Sebuyau near a small coastal town in Sarawak Borneo. The German pilot, Rko Steger swam for hours to reach Kampung Tebelu, Sebuyau. Fisherman pulled him from the river estuary. Three others —two men and a woman named Peter Ato Mayau, 53, Siti Khuzaima and Henry Loh—had been aboard the helicopter and are still missing. The two engineers and the quantity surveyor aboard the helicopter and that they were flying to Nanga Merit to look at a school project.

Commission of Inquiry to Begin

Update: The Kenyan Commission of Inquiry established to look into the June 10 helicopter crash in the Kibiku Forest in Kenya is set to hold its first formal session on July 16. The container holding the wreckage will be stored at the Kenya Police Air-Wing facility at the Wilson Airport.

The Kenya Police Airwing Unit Commandant Rodgers Mbithi will be appearing as the first witness.

The pilots involved, Captain Nancy Gituanja and Luke Oyugi, underwent pilot training in the Ukraine in April 2009.

According to commission members, the intent of the commission is “…not to apportion blame, but to prevent occurrence.”

Fatalities included Minister of State for Provincial Administration and Internal Security George Saitoti and his permanent secretary Orwa Ojode, pilots Luke Oyugi and Nancy Gituanja and bodyguards Inspector Joshua Tonkei and Sergeant Thomas Murimi.

Kenya Crash Investigation Begins

Why: The team put together to investigate the June 10 crash has visited the crash site and said they will begin mapping the area. A representative of the victims will be at the team meetings in an effort to promote transparency. Two Kenyan Ministers were aboard the helicopter.

Kenya Crash Hits Kenya Administration Hard

What: Kenya Police Air Wing Eurocopter AS 350B3e Ecureuil

Who: 6 aboard, 6 fatalities
Why: Minister of State for Provincial Administration and Internal Security George Saitoti and his permanent secretary Orwa Ojode, pilots Luke Oyugi and Nancy Gituanja and bodyguards Inspector Joshua Tonkei and Sergeant Thomas Murimi, died in a helicopter crash in Ngong Forest in the outskirts of Nairobi, Kenya. The remains were badly burned. There were no survivors.

The helicopter was en route to a church service.

Kenyan President Mwai Kibaki declared three days of official mourning, and said a full investigation into the crash is planned. “On behalf of the Government and the people of Kenya I send my heartfelt condolences to the family, friends and relatives of the six Kenyans.”


Canadian Helicopter Wrecks at Sleeping Beauty

What: Bailey Helicopters Ltd.Eurocopter AS 350B2 AStar flying out of CYXJ/Fort St John General Airport
Where: Sleeping Beauty Mountain. Terrace BC, Canada
When: June 1, 2012
Who: 3 aboard, 3 fatalities
Why: At nine oclock, ATC lost contact with a training flight flown by 45-year-old Peter Bryant. Two other Bailey Helicopter employees were also aboard.

A search and rescue team including a Canadian Armed Forces Buffalo fixed-wing aircraft and a Cormorant helicopter out of Comox were all deployed to the electronic flight tracking system location and confirmed that the Eurocopter crashed in poor weather (described as “horrible”) into Sleeping Beauty Mountain. The area was undergoing rain and snow, and considered unsafe by an avalanche technician. One rescue searcher hiked to the crash site.

The remains have now been recovered, though it was a hazardous recovery, during which an avalanche occurred.

What: Bond Offshore Helicopters Ltd. Eurocopter EC225LP Super Puma Mk2+
Where: 24 nautical miles off the coast of Aberdeen, North Sea
When: May 10, 2012
Who: 16 passengers, 0 fatalities
Why: The Bond helicopter was en route from Aberdeen Airport to the Maersk Resilient drilling rig when an emergency developed.

Pilots had to make a controlled descent into the North Sea when the gearbox oil pressure tanked. With the use of flotation equipment, the ditched helicopter stayed afloat.

The two crew and twelve passengers were safely recovered by afternoon. A cautionary measure suspending Eurocopter EC225 flights has been issued pending investigation. Examination revealed that the helicopter suffered a crack to the gearbox shaft.

The passengers were employed by Halliburton, Ensco, Brundt and Stag, and Conoco. The Jasmine field is operated by Conoco Phillips.

The rescue was coordinated by the Aberdeen coastguard, who was alerted at 12.15pm, and RNLI lifeboats. The initial call indicated that added that all everyone was on the raft. Nine were flown to Aberdeen Royal Infirmary, and five were taken by life boat.

In George’s Point of View

The choice to suspend the EC225 model until the kinks are worked out is a smart move. Better monitoring of potential cracks to the gearbox shaft may eliminate more “catastrophic failures” like the one that Eurocopter that crashed in 2009. In that event, the operator had put off replacing the main rotor gearbox, resulting in the death of 14 offshoremen and two helicopter crew.

The skill and training of the (May 10, 2012) 100% successful pilots and rescue crew cannot be commended highly enough. Sure, the media is calling this a textbook rescue, but somehow that description feels inadequate to me, leaving out the heroic human element involved. These guys are heroes.

5 Die in Hawaiian Chopper Crash into Mountain

What: Blue Hawaiian Helicopters Eurocopter EC130 B4 helicopter departing from Kahului
Where: Kaunakakai, Molokai island, Hawaii
When: November 9, 2011 at 12:30 pm
Who: pilot Nathan Cline, 30, of Kihei and 4 tourists
Why: While on a 45 minute tour of West Hawaii, the chopper crashed into a mountain. The accident occurred on Wednesday, less than a mile from Kilohana Elementary School, right after a hard rain. At the school, students heard an explosion and saw black smoke when the chopper crashed.

The pilot and four tourists died at the scene. Although the investigation is ongoing, according to at least one published report, the NTSB said that the pilot failed to maintain enough altitude over the terrain amid low-lying clouds.

Two of the passengers were newlyweds, and they came from Pennsylvania and Ontario Canada.

Blue Hawaiian, which conducts 160,000 tours yearly, leased the year old chopper from Nevada Helicopter Leasing LLC.

Four bodies have been recovered. The fifth has been located, but is in a difficult location.

Chopper Makes Field Landing in Indiana

What: Euro Copter EC 130 helicopter en route from Fort Wayne, Indiana to Indianapolis
Where: Noblesville Hanbury Manor Boulevard near the Sagamore Golf Course
When: Nov 3, 2011
Who: pilot and 5 passengers
Why: The pilot made a public statement that he was piloting his chopper and encountered severe weather. He made an emergency landing described as “controlled” in an Indiana corn field.

After landing, when everyone aboard had vacated, the chopper caught fire. There were no injuries.

The incident is under investigation by the NTSB and the FAA.

Journalists Die in Belarus Chopper Crash

What: Belarusian Eurocopter AS 355NP Ecureuil II AS355 NP border patrol helicopter
Where: Lithuanian border about 800 metres from Vileity
When: October 20 2011, 3:45 p.m
Who: 5 fatalities
Why: Three journalists, Vitaly Prochkovsky, 36; Sergei Shamalov, 39; and Yulia Shaternik, 28 died working on a report about border security for Union, a Belarus television and radio company. Pilots Igor Skvortsov and Andrew Pekarsky also died in in the crash. The chopper circled a house, and fell out of the sky. The Twin Squirrel helicopter they were on crashed for an unknown reason. Forester Elena Kirplyuk witnessed the crash. Crash site specialists converged on the scene. An investigation is underway.

Lifenet Medical Evacuation Chopper Down in Missouri, Four Fatalities

What: Air Methods Corp Eurocopter AS-350 en route from a hospital in St. Joseph, MO
Where: a field a mile north of Midwest National Air Center. Missouri
When: April 26, 2011, 7 pm
Who: 3 crew and one patient
Why: The St. Joseph Missouri medical chopper was on the way to Liberty Kansas about to refuel at Midwest National Air Center in Mosby when it went down in a Clay County field. There is no indication what went wrong that caused the crash and skies were reported to be clear. The wreckage is reported to be mangled. According to the video below the wreck might have been a failed landing, hitting the skids, then flipping.

There were no survivors. The crew lost in the crash were Randy Bever, Flight Nurse, 47, of Savannah, Mo., Chris Frakes, Flight Paramedic, 36, of Savannah, Mo. and James Freudenberg, Pilot, 34, of Rapid City, Sd.

The name of the 58-year-old female passenger has not yet been released.

The helicopter was certified in 2005, good until 2013. Key Equipment Finance Inc. out of Boulder, CO.holds the lien and it was being operated by LifeNet, a subsidiary of Air Methods Inc. of Englewood, CO.

The NTSB is investigating.

Names Released in Construction Chopper Crash in Norwegian Mountainscape

What: Airlift AS Eurocopter AS 350B3 Ecureuil
Where: between Dalamot and Busete in Hardanger, Hordalan, Norway
When: July 24, 2011, 21.30
Who: 5 fatalities
Why: About to transporting building materials into the mountains during heavy fog, the Airlift to Dagbladet helicopter crashed and caught on fire. The witnesses were part of a crew that had flown in earlier. A crisis team from Eidfjord and Ullensvang is on the scene.

The Airlift company had a fleet of 17 helicopters.

Police and fire departments, two air ambulances and at least two Sea King helicopters were engaged after emergency services were called. Three of the deceased were found quickly, and the other two took longer to find. The pilot was also killed in the crash.

The crash occurred in a remote location far (5 hours walk) from a road. It was so remote that the individual who reported the crash had to travel to a location where there was phone service coverage.

The state agency Havarikommisjonen is investigating.

India: Crashed Helicopter found, Minister Dorjee Khandu’s body recovered.

What: Pawan Hans Helicopters Eurocopter AS 350B3 Ecureuil en route from Tawang to Itanagar

Who: 5 fatalities, including Dorjee Khandu
Why: The helicoptor taking Dorjee Khandu from a retreat disappeared en route. Two MI-17 and two Cheetah helicopters were engaged in the search.

The helicopter wreckage was found 5 days after it went missing. Dorjee Khandu, Mr Khandu’s security officer Yeshi Choddak, Yeshi Lamu, and crew members JS Babbar and KS Malick all died in the crash. The wreckage was at 4900 meters, near Jung Falls in the Tawang district. The search was hampered by bad weather.

The chopper that crashed was 4 months old, according to Chief Minister Khandu’s adviser Kiren Rijiju. However, the maintenance department of Pawan Hans has a bad reputation.

4 Lost in Chopper Crash in Austrian Lake

Where: Lake Achensee, Austria
When: March 31, 2011
Who: 4 crew members, 4 fatalities
Why: During a border inspection flight, the helicopter was flying very low, and tipped the surface lightly, the helicopter shot up, then fell. The helicopter had a crew of 3 Tyroleans and one Swiss. The air rescue and mountain guide, Stephen L. (38) was found floating; Pilot Mark P. (41), Mr Herbert F. (53) from the state police, and a Swiss civil servant (43) were not found until evening. They appeared to have been trapped in the cockpit. The pilot had 2,500 flight hours,and the helicopter was new.

Fire and water emergency personnel responded, and (3 rescue helicopters and 43 divers) dive teams are on site using sonar imaging. They are looking for the black box so they can learn the details of the crash.

In 2009 in Syria, two people died in an EC 135 helicopter.

Squirrel Crash Kills Aussie Pilot

Why: While flying from the Eastern Highlands centre of Goroka to Manus for mining contractors, Australian Sunshine Coast helicopter pilot Lloyd Lester suffered engine failure in a Eurocopter AS350 P2-PHL Ecureuil, and crashed into the Bismarck sea en route to Manus Island. Eyewitnesses report indicate that the engine lost power, started up and lost power again.

Pacific Helicopters owner, Mal Smith (who is also Eastern Highlands Province governor) said some of the wreckage was found, but not the engine or Mr Lester. He was hopeful he would be found alive, but three helicopters and two planes searched an area 24 kilometres to the west and 70 kilometres to south of Baluan Island and Mr Lester was not found. Nearby islands are being searched in case he made it to shore.

Lester is from Papua New Guinea
