Jet Blue Flight Pins Attendant

On Nov 20, 2013, a JetBlue Embraer ERJ-190 to Boston from Fort Myers diverted to Orlando when an emergency slide emitted a loud bang, and deployed itself in the front galley of the plane.

There were four crew aboard and seventy-three passengers aboard flight B6-1266 who set out for Boston and ended up in Orlando. Passengers were evacuated in Orlando and transferred to a plane that didn’t have an emergency slide in the front galley pinning a flight attendant to the wall.

The flight attendant did not suffocate, but was assisted in freeing his head, apparently.

Associated Plane Crash Was Due to Human Error

The report on the Associated Plane crash is based on a study of the flight data recorder (FDR) which contained 47 hours of data, and 32.5 minutes of cockpit voice recording of internal conversation of the two pilots, radio calls and cockpit noise. Sixteen people were killed when the Embraer 120 plane on a Lagos-Akure trajectory crashed shortly after takeoff outside Lagos airport’s domestic terminal. Four people survived.

The crew had concerns about the plane before they took off; and after they took off, there were more concerns. The crew ignored the plane’s warnings, then the First Officer complained that since the aircraft was not climbing, the Captain should not stall the aircraft. ATC expressed concern of the plane’s orientation.

The flight data shows the characteristics of an aerodynamic stall thirty one seconds before the crash. The failed engine was a Pratt & Whitney Canada model.

This is still the preliminary report, but it does point to facts that seem to show that the Lagos crash was caused by engine failure and human error.

Nigerian Flying Coffins

It is no wonder that Nigerians worry that planes are flying coffins, after the Associated Airlines Embraer crashed on October third. It’s the one that flew the remains of former governor of Ondo State Olusegun Agagu into the ground on October 3, killing 15 people on board. Should I ignore the irony that the passengers of the flight were taking the remains of the governor home when they all joined him in death? It is still too early for the complete analysis of why the plane crashed, but early reports are saying that there was an engine failure that led to the accident.

As the right engine developed problems, the first officer urged Captain Abdulraman Yakubu to abort the flight.

Captain Abdulraman ignored the copilot. He ignored the alerts that the take-off flaps were not activated. The black box indicated the captain was flying.

If he had aborted the flight, the passengers might still be alive. Thirty-one seconds after take-off, the aircraft stalled and nosedived.

But let’s not all jump on the pilot. Because the plane wasn’t airworthy to begin with.
The plane made its last flight on August 30 and had been grounded for about 33 days before the ill-fated flight on October 3, 2013. Someone needs to do their job and, dare I say it again….Maintenance, maintenance, maintenance!

It’s no wonder Nigeria suspended Associated Airlines. “The suspension is to make room for maintenance and operational checks.”

The Director General of NCAA said:
“Associated Airlines operations have been suspended by the NCAA. Let me say for the purpose of clarity, I said their operations, I mean all their operations of Associated airlines have been suspended by the NCAA. I did not say any certification was cancelled but all operations.”

Update on the Associated Jet Crash in Nigeria

On Oct 3, 2013, a twenty-three year old Associated Aviation Embraer EMB-120 was en route from Lagos to Akure to carry the deceased regional governor and family to his funeral in Akure when it crashed, killing 16. Four survivors were hospitalized.

The NCAA said the airline had coverage and the plane’s paperwork and inspections were up to date. The flight data recorder has been recovered.

The son of the late former Governor of Ondo State, Dr. Olusegun Agagu, Feyi, was aboard the plane and survived the crash. The passengers included family members taking the body of Olusegun Agagu, a former aviation minister, to his funeral.

Associated Jet Crash in Nigeria

On Oct 3, 2013, a twenty-three year old Associated Aviation Embraer EMB-120 was en route from Lagos to Akure to carry the deceased regional governor and family to his funeral in Akure.

The Embraer developed engine trouble on take-off. (The failed engine was a Pratt & Whitney.) The plane struck a wooded area, a wall and came to rest on fire, avoiding a residential area. The flight data recorder has been recovered.

The number of people aboard has not been determined. Some sources say the flight had 20 passengers plus a crew of seven; some sources say only twenty were aboard in all. Fifteen victims lost their lives initially, and one died in the hospital.

The four survivors were hospitalized.

Plane crashes and Five Die in Caldas Novas

A Embraer 810D Seneca III plane en route from Ouro Fino to Gurupi was supposed to stop in Caldas Novas to refuel on Sept 20, but it crashed near Nelson Ribeiro Guimaraes airport. Five people——three men and two women——were killed in the crash at about 10:45 a.m. in a semi-urban area.

The plane may have been on approach as its landing gear were down. There was no fire, explosion or leaked fuel at the accident site.

It is expected that the missed fuel stop resulted in the plane crashing due to being out of gas. The forest where the plane fell was about 600 meters from the runway at Nelson Ribeiro Guimarães Airport. The victims were identified as John Constantini, 81, owner and pilot of the plane, Marcia Maria da Silva, 51, Vilmar Alves Passion, 40, Isolina Tumioto, 63, and John the Baptist Constantini, 51. Their remains were taken to Forensic Medical Institute.

The Center for Research and Prevention of Aeronautical Accidents (Cenipa) is investigating.

Small plane crash in Brazil

An Embraer EMB-201A Ipanema was en route from Paracatu MG to Goinia when the pilot had to make a forced landing in a field. The plane was commonly used as a cropduster.

The plane had had a fuel supply problem before but apparently this time the problem was in the fuel pump/power system.

The pilot was uninjured but the plane sustained some damage. There was no fire, but firefighters were on hand, and there was a fuel leak.

Centro de Investigação e Prevenção de Acidentes Aeronáuticos (Cenipa) will be investigating.

Emergency belly landing at Porto Seguro

Contact Carlos A. Morillo Doria

An Embraer EMB-721C Sertanejo (PA-32R-300) en route from Prado to Porto Seguro had a little trouble on landing when the nose gear folded.

The friction from the belly landing was controlled by foam applied by the firefighters who were on standby when the flight landed. Actually, you can see they were there ahead of time, and how they used the time while the pilots were burning off fuel.

Pilots were aware of the inability of the gear to lock, so they burned off fuel before making the landing.

The plane is normally used as an air taxi. The video below shows the plane landing on foam.

No injuries. Good job, Captain!

Pilots Ignore ATC Directions, Hear Audio

On March 11, 2013, an Air Canada Embraer ERJ-190 en route from Edmonton to Toronto,ON was on approach when ATC informed the pilots to abort the landing. Ground radar indicated something moving on the runway. Pilots continued to make the landing, and ignored ATC.

Mechanics working on a Sunwing Boeing had left a van running and in gear, which subsequently rolled without a driver across the runway. At some point, the van impacted the Sunwing 737.

In George’s Point of View

While we can’t make assumptions, apparently the pilots saw the van safely flew over it and made a secure landing.

However, there are a lot of errors here that could have been disaster. We are glad no one was injured. Safety first, everyone!

  • The maintenance crew for failing to secure their vehicle.
  • ATC for not using the call sign, even if it was evident to them who they were speaking to.
  • Pilots for ignoring ATC even if they saw the “threat” because there could have been an additional alert

That said, of course we are glad no one was injured.
