FAA, NTSB Employee’s Planes Collide

Where: southern Fauquier County.

Who: two people aboard the Beechcraft fatalities, one aboard the Piper (injured)
Why: Over Warrenton, Fauquier County, Virginia, two planes collided. The pilot of the piper was hospitalized with injuries; both in the Beechcraft succumbed to their injuries. The Beechcraft was owned by an employee of the NTSB. The Piper is owned by an employee of the FAA. Subsequently, they requested the Canadian NTSB to investigate.

The planes impacted the ground a mile apart by a mile long debris field, one on a family farm. The woodsy secluded area is a challenging site to gather debris due to wildlife like snakes.

Hawker Beechcraft Headed for Bankruptcy

Centerbridge Partners, Angelo Gordon and Capital Research & Management are negotiating bankruptcy with Hawker Beechcraft.

A forbearance agreement expiring in June will probably lead to a chapter 11 which will keep firm in operation.

Centerbridge is the biggest lender. The private equity investment firm manages a $3 billion fund focused on distressed-for-control and buyout investments.

Family Lost in Beechcraft Crash in Minnesota Field

What: Beechcraft 35 Bonanza
Where: Glencoe, MN

Why: The plane crashed on farmland four mines north of Glencoe killing what appears to be a family of three aboard the plane and also three dogs. Conditions were five miles visibility and overcast at 900 feet and the pilot may have been flying by visual flight rules in conditions when VFR flight isn’t allowed. Visibility in the area is known to drop “at the drop of a hat” so the pilot may have flown into worsening conditions inadvertently.

Witnesses say they heard the plane sputtering, and heard a popping noise before impact. Others saw the plane flying low before it hit the ground in foggy drizzly weather leaving a two-hundred yard debris field. The plane may have been flying from St. Cloud.

By the time bystanders arrived on the scene, the plane was missing the tail and a wing, and was engulfed in a “ball of flame.”

The plane had just been sold by a St. Cloud resident to a new owner.

Stuart Dahlberg, 52, his wife Ivelisse Morillo, and his mother, Mae Dahlberg were en route to visit family in Colorado when they were killed in the crash. Stuart Dahlberg was an experienced pilot.

Rio Vista Collision

Why: After the helicopter and plane bumped over Rio Vista, the pilot had to make a hard landing short of Byron Airport, 8 miles south of Rio Vista Municipal Airport. Apparently the plane struck the chopper’s skids. The 1961 six-seat Beechcraft plane involved in the collision is registered to Ronald A. Gawer of Brentwood in Contra Costa County.

When the Solano Fire crews arrived on the scene, they found the pilot with minor injuries, and the passenger was unhurt.

According to the owner of the chopper flight school, the chopper was hit from behind by the Beechcraft.

Beechcraft Baron Crashes in Brazil killing 4

What: Norte Táxi Aéreo Beechcraft 95-B55 Baron en route from Cametá to Baião
Where: Cametá, PA Brazil
When: Feb 16 2012, 10:00 am
Who: 4 aboard, 4 fatalities
Why: The Taxi service plane crashed not long after takeoff, killing the pilot, copilot and two security guards who worked transporting money for a bank. A fire ensued after the crash, and rescuers found the bodies were charred.

The crash occurred a kilometer from the runway. The airport fire department team responded. Seripa (regional research and prevention of Aviation Accidents) is investigating.

Brazil Crash Kills Three, Including Politician

What: Beechcraft A36 Bonanza en route from Pontes e Lacerda to Santa Fé do Sul
Where: Caceres, Brazil
When: 14-JAN-2012, 8 am
Who: 3 aboard, 3 fatalities
Why: A plane piloted by Antonio Carlos Lopes do Amaral (Carlos Lopes Antonio Amaral) 57, former president of the Mato Grosso state legislature crashed in Mato Grosso in torrential rain, killing all three aboard. The crash site was a ranch located 12 miles from downtown Caceres, Site Sao Paulo, San Francisco community. Also killed were Vitório Quarentão and João Batista Paulo do Carmo.

Local witnesses heard the engine sputter or fail three times before it went into a tailspin and fell into a marshy area.

The wreckage shattered, and it appears that DNA testing will have to be used.

Amaral has sister in Caceres, businesswoman Maridalva Vignard Amaral, owns a radio station. The family said Amaral’s body and his friend Victor will be transferred to Santa Fe do Sul via plane and taken to counter Pontes e Lacerda.

A team from the Department of Civil Aviation will be arriving in Caceres to start investigating the cause of the accident. Amaral leaves a wife and three children. John was single.

The plane is registered to Air Martins School of Civil Aviation.

Aviation Technology Innovators Philippine Crash Investigation Will Take Years

What: Aviation Technology Innovators Beechcraft 65-80 Queen Air en route from Manila to McGuire Field, San Jose, Mindoro.

Why: The first steps after a crash are those of the investigators whose job it is to examine the wreckage and determine the facts. This information will eventually be turned into an interim and a final report, which will stand as a record. Simultaneously, the records will be examined. Not only will the pilots’ recorded histories be searched and the level of their skill examined, but also the maintenance and ownership of Beechcraft 65-80 Queen Air RP-C824

Captains Timoteo Aldo and Jessie Kim Lustica died in the crash three minutes after takeoff.

Here is a big question: Where is Captain Fidel Hembrador who leased ATI for the repair, maintenance and hangar parking services for the plane?

My first thought takes me to wondering about the condition of the plane. (Maintenance! Maintenance! Maintenance!) My second is to wonder about the pilots’ experience. My third is a strong suggestion that people refrain from rebuilding so close to the airport. However, such an edict would mean it is the government’s responsibility to see that 600 displaced people get generous government assistance in the form of grants or compensation, providing the means for displaced families to establish domiciles elsewhere.

Below is the DEC 12 release from the Department of Transportation and Communications of the Philippines:

Secretary of Transportation and Communications Mar Roxas met with CAAP [Civil Aviation Authority of the Philippines] officials today, and ordered a thorough investigation of the light aircraft incident that left 14 people dead on Saturday in Barangay Don Bosco, Paranaque City.

The officials discussed the liability of the owner of the light plane and take into effect a speedy, seven-day timeline for the submission of the investigation report.

CAAP director general Ramon Gutierrez reported that he constituted the Aircraft Accident Investigation Inquiry Board (AAIIB) headed by Captain Amado Soliven, Jr. to lead the panel. The panel will investigate why and how the Beechcraft Queen Air (RPC-834) eight-seater light plane plunged into the shanty town in Paranaque City, exploding on impact, and causing a fire that gutted a 2,000-square-meter section of the slum area.

Gutierrez also told the DOTC secretary the ill-fated aircraft submitted an official flight plan to the CAAP hours before the flight under the registry of Aviation Technologies Innovators, Inc. [ATI], a repair station/maintenance and hangar provider.

But CAAP investigators revealed that the plane had a certificate of registration under one Captain Fidel Hembrador, a private operator, who reportedly leased ATI for the repair, maintenance and hangar parking services for the said plane.

Hembrador has yet to surface following the incident last Saturday.

The CAAP reported that it had retrieved and secured the engines of the plane. CAAP investigators, with the assistance of the authorized repair station of the engine manufacturer (LYCOMING), are currently breaking down the engines to determine if a mechanical malfunction had occurred.

The CAAP had commandeered the aircraft record and logbooks, as well as summary records of the training experience of the crew.

“I have tasked the fact-finding panel to come up with a report and recommendations within the week,” said Secretary Roxas. “We will make sure that all facts of this unfortunate accident are investigated, the victims are duly compensated, and make sure that similar accidents do not happen in the future.”

Roxas added that the general aviation services is one sector that the DOTC intends to relocate to another site. “The objective is to decongest NAIA from small aircraft operations, so as not to comingle with commercial airline operations.”

The CAAP had recommended two potential transfer sites, Fernando Air Base in Lipa City, Batangas and Sangley Point in Cavite.

Both military facilities are military-owned. Roxas said the DOTC and the Department of National Defense will come up with a MOA that will govern the transfer, as well as the relocation of residents who will be displaced by the move.

BeechCraft Queen Air Crashes Philippine School, Neighborhood Inferno, 600 Homeless

Why: After taking off from Ninoy Aquino International Airport, the pilot reported trouble to ATC, and was trying to return to the airport. The pilots, Timoteo Aldo and Jessie Kim Lustica, had been fetching a cargo of fish. A witness described the plane “twirling” in the air before impact.

The Beechcraft crashed into Felixberto Serrano Elementary school and exploded, killing the 3 aboard and 11 on the ground in Better Living Subdivision, Parañaque City. The specific location was Taiwan St., Barangay Don Bosco. School was not in session, but there had just been a Christmas party there. The F. Serrano Elementary School burned down; school for 2000 will be suspended on Monday as plans are made to send students to alternative schools.

Victims included children playing at the playground.

Twenty injured were taken to Parañaque Doctors Hospital and Parañaque Community Hospital along the South Luzon Expressway. The Philippine Red Cross was on the scene, reporting a number of burn victims.

50 domiciles on the subdivision’s easement were damaged. Residents were not called “squatters” but were described as informal settlers. Seventy families were evacuated–a total of 600 people evacuated.

A covered court inside the subdivision is being provided as temporary shelter for evacuees.

Maricel Rodriguez Garado, Ronald Sabidario were two adults reported as lost in the crash in addition to the two pilots.

Northern Thunderbird Air Crash in Vancouver BC

What: Northern Thunderbird Air Beechcraft 100 King Air en route from Vancouver International Airport to Kelowna International Airport
Where: Russ Baker Way, near Vancouver International Airport, Canada
When: Oct 27, 2011 4 pm
Who: 9 occupants, 2 fatalities
Why: Reporting difficulty to ATC after a warning light came on in the cabin fifteen minutes after takeoff, the pilot, Luc Fortin attempted to return to the airport. The plane crash landed short of the runway. On impact, the fuselage separated from the tail section by 10 yards and was on fire beside the road. Fortin died of his injuries, and the co-pilot died later with burns on 80% of his body. There were 9 people aboard, and all of them were hospitalized. The plane had hit the ground and skidded, it’s wing striking a car. Two on the ground (in the car) were injured by debris.

Fortin, a 14,000-hour pilot, died at Vancouver General Hospital, and leaves a wife Dagne and sixteen month old daughter Katelyn.

After weeks in intensive care, co-pilot, Matt Robic, 26, died at Vancouver General Hospital on November 16, 2011 with 80% burns. He leaves his wife Natalie Redfearn.

On landing on Russ Baker Way (street), the plane burst into flames. Passersby on the street pulled over and rushed to the burning plane to pull those aboard from the flames. Initially both pilots were alive and were rushed to the hospital in critical condition. Read the accounts of some of those involved by following the “read more” link below.

The flight had been chartered as a private rental by an executive group with business in Kelowna.

Jeremy Kerr, John Redmond, Haim Peri, Shawn Nagurny, Lonney Lee and half a dozen other good samaritans had been driving by (or worked in the immediate area) and pulled over to run into the burning plane and rescue those aboard. Thanks to their efforts, seven people survived. Carolyn Cross, Lorelei Sobolik, Cameron Sobolik, Tony (Troy?) Zanatta, Kelly Jablonski, Jeff McCord were among the survivors.

Entrepreneur, Dies Piloting Plane, 2 Sons, Woman Passenger, 4 dead

Why: In the night, as rain was falling, the Beechcraft Bonanza en route for Campinas in Sao Paulo never made it to its destination, but crashed on farmland near Marabá Paulista, leaving a five foot crater. Four people died in the crash: entrepreneur Ângelo Rafaeli D’elia, 51, who was piloting the aircraft, the children Bruna D’Elia, 18 and Rafael Ângelo, 12 years, and Syhlvia Katheiny Torreta.

Reports are that the pilot was lost, and that someone aboard the plane was sick. They asked for assistance from London’s control tower.

Locals reported hearing an explosion when the plane impacted the ground.

Sightseeing Flight atKatmandu ends in Fatal Buddha Air Crash, 19 lost

What: Buddha Air Beech 1900D performing flight U4-103 from Kathmandu to Kathmandu

Why: On the flight were 2 Americans,3 Europeans, 10 Indians, 1 Japanese, and 3 Nepalese crew. The plane encountered foggy, rainy conditions on approach to Katmandu after a sightseeing flight. On approach, the flight lost contact at 7:31 local time, struck the roof of a house, and crashed into Kot Dada Hill.

Problems with rescue efforts were compounded by heavy rain. Only one individual was retrieved alive by rescue personnel, but that person died.

The black box has been recovered. A three person investigative team has been commissioned by the Nepalese government.

Student Pilot in Palo Alto Lesson Scare

What: single engine Beechcraft Musketeer on student flight
Where: East Palo Alto’s Ravenswood Open Space Preserve

Why: While practicing touch and go maneuvers at Palo Alto Airport, the plane’s engine failed.

When the engine failed, the plane went down in the Ravenswood Open Space Preserve. A plane crashed there in Feb of 2010, which shook the neighborhood, but many locals were unaware of when this incident happened. This was an emergency landing, although the plane went off course after glancing against a tree.

Both Instructor Jassen Todorov and student pilot David Shoemaker survived.

Why: While en route, the plane was struck by lightning.

The right winglet began smoldering after being struck. The flight crew returned to Wellington where they made a safe landing.

Aero Resources Flight Crashes in North Carolina, 4 die in crash


What: Aero Resources Corp. of Hazard Beechcraft 58 Baron en route from Georgia to Kentucky
Where: Unaka, about 125 miles west of Asheville, NC
When: May 25, 2011
Who: 4 fatalities: pilot Mat Shuey, Tiffany Maggard, Kassie Robinson and Miranda Morgan
Why: Pilot Mat Shuey was flying at 9,000 feet, above the area peaks when he reported an emergency over North Carolina when it crashed north of Lake Hiwassee near the Tennessee border, and started a brush fire.

The wreckage was spotted on the afternoon of the 25th but the site was not immediately accessible. The pilot and three female passengers died in the crash. The four bodies were recovered by rescue crews on Thursday.

Tiffany Maggard, Kassie Robinson and Miranda Morgan, who had all graduated from a Knott County Kentucky high school, had been visiting a friend in Alabama. The pilot’s name was not released.

Eagle Airways Beech Diverts to Gisborne

Why: Ten minutes into the flight, while flying over Opotiki, the pilots shut down an engine because of readings they were getting; and they announced to passengers they were going to make an emergency landing.

The pilots then returned to the airport. Everyone aboard clapped when the nineteen seat plane landed safely.

The engine of the Eagle Airways Beech was examined and no problems were found.

Beech Crashes House in Miami Gardens

What: Beech 18 (N18R)
Where: near Opa-Locka Airport, Miami Gardens Florida
When: May 2, 2010

Why: Right after takeoff, the pilot of the twin engine Beechcraft 18 cargo plane that was carrying household goods to the Bahamas radioed he was going down, before he clipped trees, crashed through a fence and 2 cars and into a residential yard. Neighbors were at home at the time of the crash, and witnessed it.

More than 90 Miami-Dade firefighters were on the scene. Although we saw reports that the fire only caused smoke damage to the exterior of the home, and that it did not strike the building, it is clear in the video that the house was breached by the plane, and there were several areas of the house that were on fire. The family is staying elsewhere and no one in the house was injured.

The pilot who was killed in the crash was 64 year old Harry Trauffer, who regularly flew clothing, bedding and auto parts to the Bahamas.

Family Tragically Lost in Kansas Cornfield

Why: A family of four missed the approach to Philip Billard Municipal Airport. Witnesses to the crash say they heard the engine sputter.

The plane was flying too low as it made a go-around to make a second landing pass. The plane was a Beechcraft built in Kansas.

The young Scott city couple and their daughters were making an Easter trip when they crashed. The victims names were released: Richard Dylan Spencer, 35; Amy Beth Spencer, 34; and their daughters, Chase Spencer, 7, and Ansley Spencer, 5.

All of these aviation crashes that involve deaths are terrible; but somehow it hits home sharper when it is a young couple with their two small children.

Rest in peace Spencers.

Hawker Goes Off Runway with Flat

Why: On takeoff, the plane blew a tire. The plane veered off the left of the runway. Neither of the two pilots nor the passenger was injured. There was no damage to the plane except for the blown tire. Emergency crews were on hand.

Morning Takeoff after Layover Ends in Fatality

Why: Encountering bad weather while en route, Stanley diverted to Dequeen Airport. He made a safe landing, and spent the night.

In the morning after taking off, while circling, the pilot lost control and crashed off of the east end of the 5,000-foot De Queen runway in a ditch. He died on the scene.

Boeing Fuselage Fails

What: Southwest Airlines Boeing en route from Phoenix to Sacramento
Where: Yuma Marine Corps Air Station, Arizona
When: April 1, 2011
Who: 118 passengers
Why: While en route, after what sounded like an explosion, a three foot hole opened up in the roof, straight through the fuselage. After rapid decompression, the pilots diverted to Yuma Marine Corps air Station. There were no injuries among the passengers but one fight attendant was hurt. Some passengers passed out when their oxygen wasn’t working.

Subsequent to the initial event, more planes were found with similar issues. (More to come)

Australian Beechcraft Crashes; No injuries

Why: On takeoff, the pilot crashed into a shed just beyond the end of the runway. The pilot survived. The plane appears to have developed problems on takeoff, did not get far off the ground, and its wing struck a shed across the road. Although the plane is a write-off, the pilot walked away.

Who: 2 aboard
Why: A corporate jet scheduled eventually to fly to Rome aborted its landing and banked into a transformer in St. Moritz, bursting into flames. The German pilot and first officer did not survive the crash.

2 Die in Colorado Plane Crash

What: Beech 60 Duke en route from Brownwood Regional Airport, Texas to Colorado
Where: 3 1/2 miles North of Edwards, Colorado
When: Dec 15, 2010, 4:30
Who: 2 aboard, pilot Barton Harris and Jerry Hoggatt
Why: While en route, the pilot lost radar contact with the Eagle County and Denver airports while making a final approach to the Eagle County Regional Airport in Eagle. Pilot, Barton Harris, 67, of Brownwood, Texas, and Jerry Hoggatt, 73, of Pelham, Alabama, were flying to ski in Colorado.

The wreckage was found by a National Guard OS58 helicopter crew 3.5 miles north of Edwards in steep terrain at an elevation of approximately 10,800 feet.

The plane is registered to Barton Harris of Brownwood, Texas.

Eagle County Airport Rescue and Fire Fighting, Eagle County Ambulance District, Eagle County Coroner’s Office, Eagle County Sheriff’s Office, Eagle River Fire Protection District, Garfield Search and Rescue, Greater Eagle Fire Protection District, the Salvation Army, Summit County Search and Rescue, Vail Mountain Rescue, Vail Police Department and Western Eagle County Search and Rescue assisted in the recovery.

Harris had spent 12 years as a Navy aviator. Harris was co-founder of W.T. Harris Company, a service firm providing warehousing, transportation, assembly work and wooden pallets, crates and other products.

Monarch Cargo Plane Lost in Nassau

What: Monarch Aircraft Group (fort Lauderdale Florida) twin-engine Beechcraft 18 cargo plane en route from Fort Lauderdale to Nassau
Where: Nassau
When: December 14, 2010
Who: 2 passengers aboard
Why: The cargo plane was en route to Nassau’s Lynden Pindling International Airport when it vanished from radar.

It crashed in the Atlantic, north of Nassau. A man’s body has been recovered. A U.S. Coast guard helicopter and 3 Royal Bahamas vessels are searching for the missing passenger. Neither has been identified.

Australia’s CAA Investigation Points to Baggage Handlers


The April 9 emergency landing of Eagle Air/Air New Zealand Beech 1900D en route from Auckland to Whangarei, attributed to an open rear cargo door, has been blamed on untrained baggage handlers who did not lock cargo doors.

The hatch flew open on takeoff. See the link to a photograph of the open door posted on the NZ Herald

The doors tend to be left unlocked in case late bags come in; but that is not a good safety practice because a broken cargo door can smash the tail of the plane or worse.
