When: Feb 16th 2012
Who: flight crew (cargo flight)
Why: On takeoff, the engine caught fire.

The flight was cancelled. The situation is under investigation. Maintenance is examining the right engine.

CSA First Officer Lands Alone after Captain Succumbs

What: CSA Czech Airlines Avion de Transport Regional ATR-42-500 en route from Warsaw to Prague
Where: Prague
When: Feb 15th 2012
Who: 1 fatality
Why: After a normal flight, on approach to Prague, the first officer informed ATC that the captain was incapacitated. The f.o. landed the plane.

Emergency services responded, but the captain was pronounced dead. He as 58 years old.

PIA Flight Delayed over Sticky Landing Gear

Why: After takeoff, the landing gear would not retract. Pilots returned to Karachi and made a safe landing.

Runway Incursion in Hamburg

What: Air Contractors Avion de Regional Transport ATR-72-200 en route from Paris to Hamburg
Where: Hamburg
When: Feb 8th 2012
Why: The ATR was moving into a holding position short of runway 33 while an Aer Lingus Airbus was taking off from runway 33. Witnesses said that it appeared that the ATR was accelerating. Prompted by an urgent ATC warning, they stopped close to runway 33.

After the Airbus took off, they reported back to ATC that they had seen the ATR.

Who: 25 aboard
Why: Just after takeoff, the plane was still over Iasi when the pilots had to shut down the right engine.

The crew made a safe landing in Iasi, delaying but not postponing the flight.

Bangkok Airways on Ground Emergency

What: Bangkok Airways Avion de Transport Regional ATR-72-500 en route from Koh Samui to Bangkok
Where: Koh Samui
When: Dec 17th 2011
Who: 38 passengers and 4 crew
Why: While taxiing prior to takeoff, the pilot lost control, the plane struck an embankment and slid into a ditch.

Reports say that the pilot attempted to control the plane, but it did not respond. The flight was cancelled. Passengers were provided alternative flights.

Who: 11 passengers
Why: The Flybe flight was en route when the pilots discovered smoke in the cockpit.

Pilots made an emergency landing. A replacement jet was sent to the town Pori.

The source of the smoke was a faulty printer in the cockpit.

Why: The pilot was on the runway for takeoff and rejected takeoff due to birds on the runway, which resulted in a brake fire.

Emergency services put out the fire. Passengers were evacuated, resulting in one injured passenger.

Passengers were booked on alternative flights.

What: Tarom Romanian Air Transport Avion de Transport Regional ATR-42-500 en route from Hungary to Romania
Where: Budapest
When: June 17, 2011
Who: 34 passengers and 4 crew * corrected
Why: After taking off from Budapest, the right engine made abrasive noises, and began smoking. The engines power turbine failed followed by the gearbox. The plane was stopped on taxiway. * corrected

The pilot shut down the engine, and returned to the airport where they made a safe landing. The plane was shut down on the taxiway.

Passengers were provided an alternative flight.

Who: 21 passengers, 4 crew
Why: The pilot made a safe landing in turbulence but the front wheel collapsed (on landing or during taxi). The plane veered on to the grass.

Passengers had to disembark on to the runway via stairs and were ported to the gate.

Why: After takeoff, the plane’s right engine made an explosive noise and began emitting smoke and sparks, and the propeller stopped.

The crew shut down that engine and returned to Sao Paulo. The pilot made a safe landing in Sao Paulo. Passengers were rebooked on another flight. Maintenance confirmed a leak in the right hand engine.

Why: On landing, the plane went off the runway. The runway excursion did not cause delays at the airport. One person was slightly injured. At the time of landing, a tire burst and a landing gear broke, which caused the plane to skid off the runway.

Why: After the flight, on landing, the left main gear collapsed, causing the plane to veer off the runway.

The plane’s left propeller, engine and wing were damaged, possibly due to a hole in the runway.

One passenger was injured.

Emergency Landing in Bangalore

What: Kingfisher Avions de Transport Regional ATR-72-500 en route from Bangalore to Hyderabad
Where: Bangalore airport
When: Jan 16, 2011 8:26 a.m.
Who: 27 passengers, 5 crew
Why: After taking off an hour late due to bad weather, the plane immediately developed a problem in its right engine. A fire warning alerted the pilot.

The pilot reported to ATC and returned to make a safe landing at Bangalore airport after ten minutes aloft. Anther plane is being scheduled for the passengers.

Moscow Runway Excursion

When: Dec 27th 2010
Who: 52 passengers
Why: On landing at Vnukovo Airport (Moscow), the plane skidded and came to rest off the runway stuck in snow. There were no injuries reported.

Why: After landing, while taxiing back to the gate, the plane skidded off the runway, off of the pavement.

Passengers disembarked directly on to the runway via mobile stairs and were bussed to the terminal. The airport closed while the plane was moved.

Why: After takeoff, the crew reported smoke in the cockpit. They returned to make a landing (termed “no emergency landing” by New Zealand officials) in Wellington, with emergency services (euphemistically called airport services) on standby.

First Air Lands Safely at Iqaluit with One Engine Down

Cuban Flight Crashes; All aboard are Lost

What: Aerocaribbean ATR-72-500 en route from Santiago to Havana
Where: Guasimal, Sancti Spiritus Cuba
When: Nov 4 2010
Who: with 61 passengers and 7 crew
Why: The last flight to leave Santiago before the airport closed due to tropical storm Tomas disappeared from radio contact at 5:42 pm while over Guasimal not far from the Zaza Reservoir. The crew made an emergency call at 5:42. 40 cubans were aboard and 28 non-cubans (9 Argentinian, 7 Mexican, 3 Dutch, 1 French, 2 German, 2 Austrian, 1 Italian, 1 Spanish, 1 Venezuelan, and 1 Japanese).

A hospital worker said “The plane made several abrupt movements before crashing to the ground.” Rescuers hiked in through thick vegetation (using bulldozers to get through the vegetation to find the plane shattered and in flames and no survivors.

Tropical storm Tomas developed into a hurricane. Cuba declared a state of alert.

The ongoing investigation has not ruled that weather is the definitive factor because the storm had not reached Cuba at the time of the crash.

Cuban state airline Cubana de Aviacion owns Aerocaribbean. The flight normally flies twice a week from Port-au-Prince, Haiti, to Santiago de Cuba to Havana

Venezuela Crash: 17 Dead

Who: 51 aboard, 15 fatalities, 36 passengers and crew survived
Why: After takeoff while 10 miles from Manuel Piar International Airport, the crew experienced “control problems” requiring a return to the airport. Between the Caribbean island of Margarita and Puerto Ordaz, the plane crashed. 36 survivors were recovered from the wreckage and taken to hospitals and the remains of 2 were recovered.

As of Sept 18, the number of dead has risen to 17. Conviasa is suspending flights for two weeks for safety assessments.

The pilot was Conviasa Ramiro Cardenas.

The plane crashed in the Sidor Steel Mill junkyard/storage area amid heaps and barrels of unused steel.

While attempting an emergency landing, the plane became entangled with electrical wires and crashed.

Current reports indicate 23 survivors. We will be updating as we hear news.

Conviasa Crash in Venezuela, Survivors Hospitalized

Who: 43 passengers and 4 crew
Why: After takeoff while 10 miles from Manuel Piar International Airport, the crew experienced “control problems” requiring a return to the airport. Between the Caribbean island of Margarita and Puerto Ordaz, the plane crashed. 30 survivors were recovered from the wreckage and taken to hospitals and the remains of 2 were recovered. However the numbers that have been released indicate 23 survivors, (implying 5 additional fatalities.) Also, there may have been 51 aboard, instead of the number 47 released earlier, possibly because a count did not accurately include the crew.

The pilot was Conviasa Ramiro Cardenas.

When: Sept 4 2010
Who: Captain Bhawana Pant, copilot S Rawal, two flight attendants and 4 passengers
Why: The crew asked permission for an emergency landing after receiving an on board false indication. (The autopilot auto-initiated, and presented spontaneous error code.)

Nine minutes after taking off, the Buddah Air flight returned to Bhairahawa after the flight developed technical problems. The cause of the problem is under investigation.

Emergency Landing in Mexico

Who: 38 passengers
Why: 25 minutes into the flight, a windshield cracked. The plane returned and made a safe landing in Saltillo. Passengers were not informed of what was happening until they had already returned to the terminal.

The plane was turned over to maintenance where it took twelve hours to replace the damaged glass. The crack was not due to a bird strike, but was considered “material fatigue.” Passengers waited at the airport for a replacement plane.
