#MH17 Public Statements

FROM THE FAA:

On the evening of July 17, the Federal Aviation Administration issued a Notice to Airman (NOTAM) prohibiting U.S. flight operations until further notice, in the airspace over eastern Ukraine, due to recent events and the potential for continued hazardous activities. The restricted area includes the entire Simferopol and Dnepropetrovsk flight information regions (FIRs). This action expands a prohibition of U.S. flight operations issued by the FAA in April, over the Crimean region of Ukraine and adjacent areas of the Black Sea and the Sea of Azov. No scheduled U.S. airlines are currently flying routes through this airspace.

The NOTAM reads:

UNTIL FURTHER NOTICE, DUE TO RECENT EVENTS, ALL FLIGHT OPERATIONS BY UNITED STATES (U.S.) OPERATORS WITHIN THE SIMFEROPOL (UKFV) AND DNEPROPETROVSK (UKDV) FIRS ARE PROHIBITED. ICAO Monitoring Loss of Malaysia Airlines Flight MH17
?The International Civil Aviation Organization (ICAO) expresses its deep regrets following the loss of the passengers and crew aboard Malaysia Airlines Flight MH17. ICAO is closely monitoring reports on this tragic incident and is coordinating with all relevant parties.

ICAO recently issued a State letter advising States and their air operators of a potentially unsafe situation arising from the presence of more than one air traffic services provider in the Simferopol Flight Information Region (FIR). The loss of MH17 occurred outside of the Simferopol FIR and ICAO stands ready to support the accident investigation upon request.

Yesterday evening, I was informed of the terrible and deeply shocking news that a Malaysia Airlines jet went down in eastern Ukraine.

Malaysia Airlines has confirmed that the jet was Malaysia Airlines flight 17, which was on a scheduled flight from Amsterdam to Kuala Lumpur.

The flight departed Amsterdam at 12.15pm, local time. It was scheduled to arrive in Kuala Lumpur at 6.10 am, local, Malaysian time.

The aircraft was a Boeing 777-200.

The aircraft’s flight route was declared safe by the International Civil Aviation Organisation.

And International Air Transportation Association has stated that the airspace the aircraft was traversing was not subject to restrictions.

Malaysia Airlines has confirmed that the aircraft did not make a distress call.

The flight was carrying a total number of 295 people – comprising 280 passengers and 15 crew members.

Malaysia Airlines is in the process of notifying the next-of-kin of the passengers and crew. All possible care will be provided to the next-of-kin.

The Government of Malaysia is dispatching a special flight to Kiev, carrying a Special Malaysia Disaster Assistance and Rescue Team, as well as a medical team.

According to information provided by Kiev Air Traffic Control, the location of the plane’s emergency locator beacon is 48 degrees 7 minutes and 23 seconds North; and 38 degrees 31 minutes and 33 seconds East.

The Ukrainian authorities believe that the plane was shot down.

At this early stage, however, Malaysia is unable to verify the cause of this tragedy.

But we must – and we will – find out precisely what happened to this flight.

No stone can be left unturned.

If it transpires that the plane was indeed shot down, we insist that the perpetrators must swiftly be brought to justice.

Emergency operations centres have been established. In the last few hours, Malaysian officials have been in constant contact with their counterparts in Ukraine and elsewhere.

And I will be speaking to a number of world leaders over the coming hours.

I have had several conversations with the Prime Minister of the Netherlands.

I have also spoken to the President of Ukraine. He has pledged that there will be a full, thorough and independent investigation, and Malaysian officials will be invited to take part.

The Ukrainian president also confirmed that his government will negotiate with rebels in the east of the country, in order to establish a humanitarian corridor to the crash site.

Just now, I received a call from President Obama.

He and I both agreed that the investigation must not be hindered in anyway.

An international team must have full access to the crash site.

And no one should interfere with the area, or move any debris, including the black box.

This is a tragic day, in what has already been a tragic year, for Malaysia.

As we work to understand what happened, our thoughts and prayers are with the family and friends of those onboard the flight.

I cannot imagine what they must be going through at this painful time.

The flight’s passengers and crew came from many different countries.

But today, regardless of nationality, we are all united in grief.

Malaysia Airlines confirms it received notification from Ukrainian ATC that it had lost contact with flight MH17 at 1415 (GMT) at 30km from Tamak waypoint, approximately 50km from the Russia-Ukraine border.

Flight MH17 operated on a Boeing 777 departed Amsterdam at 12.15pm (Amsterdam local time) and was estimated to arrive at Kuala Lumpur International Airport at 6.10 am (Malaysia local time) the next day.

The flight was carrying 280 passengers and 15 crew onboard.

I am deeply shocked by the dramatic reports of the air disaster involving Malaysia Airlines flight MH17 from Amsterdam to Kuala Lumpur over Ukrainian territory. Much remains unclear as regards the cause and circumstances of the crash and those on board the aircraft. I have just spoken to the Ukrainian president.

I am now on my way back to the Netherlands to monitor and address the situation from The Hague.

Our thoughts are with those who were on board the aircraft and their family and friends.

FAA Receives Unleaded Fuels Proposals in Safe Fuel Initiative

The Federal Aviation Administration announced today it has received ten replacement fuel proposals from producers Afton Chemical Company, Avgas LLC, Shell, Swift Fuels and a consortium of BP, TOTAL and Hjelmco, for further evaluation in the Piston Aviation Fuels Initiative (PAFI). The industry-government initiative is designed to help the general aviation industry transition to an unleaded aviation gasoline. The FAA will be assessing the viability of the candidate fuels to determine which fuels may be part of the first phase of laboratory testing at the FAA’s William J. Hughes Technical Center.

The goal is to have a new unleaded fuel by 2018.

“We’re committed to getting harmful lead out of general aviation fuel,” said Transportation Secretary Anthony Foxx. “This work will benefit the environment and provide a safe and available fuel for our general aviation community.”

The 167,000 general aviation aircraft in the US that rely on 100 low lead aviation gasoline for safe operation are running on the only remaining transportation fuel in the United States that contains the addition of lead.

Commercial planes have never used leaded gas.

Congress authorized $6 million for the fiscal year 2014 budget to support the PAFI test program at the FAA Technical Center. PAFI was established to facilitate the development and deployment of a new unleaded aviation gasoline with the least impact on the existing piston-engine aircraft fleet.

The FAA asked fuel producers on June 10, 2013 to submit proposals for replacement fuels by July 1, 2014. The goal is to identify, select, and provide fleetwide certification for fuels determined to have the lowest impact on the general aviation fleet.

The FAA will analyze the candidate fuels in terms of their impact on the existing fleet, the production and distribution infrastructure, their impact on the environment, their toxicology and the cost of aircraft operations.

Press Release: FAA Proposes $295,750 Civil Penalty Against Skywest Airlines

The U.S. Department of Transportation’s (DOT) Federal Aviation Administration (FAA) is proposing a $295,750 civil penalty against SkyWest Airlines, Inc. of St. George, Utah, for allegedly violating DOT drug and alcohol testing regulations.

The FAA alleges SkyWest failed to include more than 150 safety?sensitive employees in its random drug testing pool. Further, SkyWest allegedly failed to receive verified negative drug test results for two other employees before hiring one for, and transferring the other to, safety-sensitive positions.

The FAA also alleges SkyWest subjected three employees who were not in safety-sensitive positions to post-accident drug tests that are only applicable to safety-sensitive employees, and improperly cancelled a return-to-duty test because it was not directly observed.

SkyWest is scheduled to have an informal conference with the FAA this month to discuss the matter.

NTSB Denies Petition on 1996 Crash of TWA Flight 800


The National Transportation Safety Board today denied a petition for reconsideration of its findings in the investigation of the 1996 crash of TWA flight 800.

The petition was filed in June 2013 by a group called The TWA 800 Project. Petitioners claimed a “detonation or high-velocity explosion” caused the crash.

“Our investigations are never ‘closed’,” said Acting Chairman Christopher A. Hart. “We always remain open to the presentation of new evidence.”

Before responding to the petition, NTSB staff met with the petitioners’ representatives and listened to an eyewitness who described what he saw on the night of the accident. After a thorough review of all the information provided by the petitioners, the NTSB denied the petition in its entirety because the evidence and analysis presented did not show the original findings were incorrect.

Report on Boeing 787 Ground fire Heathrow July 12, 2013

This Special Bulletin contains information on the progress of the investigation into a ground fire on an unoccupied Boeing 787-8, registration ET-AOP, at London Heathrow Airport on 12 July 2013.

It follows the publication of Special Bulletin S5/2013 on 18 July 2013. The AAIB are assisted in the investigation by Accredited Representatives from the National Transportation Safety Board (NTSB) (representing the State of Design and Manufacture), the Civil Aviation Authority of Ethiopia (representing the State of Registry and the Operator) and the Transportation Safety Board (TSB) of Canada (representing the State of component manufacture), with technical advisors from the Federal Aviation Administration (FAA), the operator and the aircraft and component manufacturers.

In Special Bulletin S5/2013, the AAIB reported the existence of extensive heat damage in the upper portion of the aircraft’s rear fuselage, particularly in an area coincident with the location of the Emergency Locator Transmitter (ELT). The absence of any other aircraft systems in this area containing stored energy capable of initiating a fire, together with evidence from forensic examination of the ELT, led the investigation to conclude that the fire originated within the ELT battery.

IOSA, which comprehensively evaluates the policies, procedures and correct implementation in all departments including security management, flight operations, organisation and management system, operational control and flight dispatch, cabin operations, aircraft engineering and maintenance, ground handling operations and cargo operations, is an internationally acknowledged evaluation system and is considered to be a benchmark for global air safety management.

IATA mandates all member airlines to acquire and maintain an IOSA registration.

IATA Members Seek Measures to Deter Unruly Air Passenger Behavior

In the recently held 70th Annual General Meeting (AGM) of the International Air Transport Association (IATA), IATA members unanimously passed a resolution focused on effectively deterring and managing the issue of unruly behavior of air passengers.

In the resolution, IATA members prompted government and industry to join hands to develop a balanced set of measures for addressing the problem of unruly air passenger, including disturbing order, committing physical assault or violating crew instructions during the flight.

According to Tony Tyler, the director general of IATA, “This resolution confirms the determination of airlines to defend the rights of their passengers and crew. Everybody on board is entitled to enjoy a journey free from abusive or other unacceptable behaviour. Many airlines have trained both ground staff and cabin crew in procedures, not only to manage incidents of unruly behaviour but also in measures to prevent them. But a robust solution needs alignment among airlines, airports, and governments.”

In 2013 alone, some 8,000 cases of unruly passenger behavior were reported by airlines to IATA.

Alaska Passenger Jet Almost Collided with Cargo Plane Over Fire Island

AlaskaAccording to the National Transportation Safety Board (NTSB), a passenger jet and a cargo plane had a ‘near miss’, after they were recorded to be less than a quarter mile apart over Fire Island, New York, on May 27.

The Alaska Airlines Boeing 737 was about to land at the runway #15 of the Ted Stevens Anchorage International Airport at about 3:08 p.m. when the air traffic controllers asked the pilot to ‘go-around’. The instruction was given to avoid a Sand Point-bound Ace Air Cargo Beechcraft 1900, which was taking off from the airport.

The passenger jet swerved off towards right; the cargo plane also took off and turned in the same direction. Above Fire Island, the aircrafts crossed each other at the same elevation within a quarter mile distance.

The Alaska airlines flight 135 landed without incident and all 143 passengers and 5 crew members remained unharmed.
NTSB has launched an investigation, led by a senior air traffic control specialist, based in Washington D.C. According to NTSB spokesperson Clint Johnson, “He’s going to be reviewing radar data, he’s going to be reviewing the conversations between the two pilots, the two flight crews and air traffic control and also looking at policies and procedures to look and document the circumstances that led up to this incident.”

165 Pilots in India Found Drunk in Pre-Flight Tests, During Last 5 Years

In reply to an RTI application filed by the Time of India, the Directorate General of Civil Aviation (DGCA), India, has revealed that between 2009 and February 2014, 165 pilots throughout the country were grounded after their pre-flight medical tests showed high levels of alcohol in their blood.

Among these, 50 pilots were from Delhi, 47 from Mumbai, 18 from Kolkata, and 17 from Chennai.

The statistics also showed that the number of pilots with higher-than-allowed level of alcohol in their blood has increased every year. In 2009, only 35 such cases were reported while in 2013, 41 pilots were found to have high blood alcohol levels.

The DGCA has not released the names of the airlines where these pilots work. According to official sources “The information pertains to third party, hence details cannot be provided according to the RTI Act.”

The ‘Near Midair Collision’ at the Newark Airport Prompts FAA to Take Action

The April 24’s incident where two passenger planes were noticed to be in a near-collision situation midair at Newark Liberty International Airport, New Jersey, has led to changes in the landing and takeoff protocols of the airport.

The Federal Aviation Administration said in recent statement, “The FAA has investigated the recent air traffic incident at Newark and has taken steps to prevent similar incidents from occurring in the future.”

According to the new rules administered by the FAA, the east-west Runway 29 will not be used for arrivals simultaneously with another plane taking off from north-south Runway 4R.

On April 24, a United flight and an ExpressJet flight came at a distance of 200 feet horizontally at the intersection of these two runways. The NTSB’s initial report termed the incident as ‘a near midair collision’.

Two Passenger Planes ‘Nearly Collided’ Midair in New Jersey Says NTSB’s Report

According to a preliminary report released by the NTSB, two passenger planes ‘nearly collided’ mid-air last month near Newark Liberty Airport, New Jersey.

The incident happened on April 24 when a Boeing 737-800 aircraft, registered to United Airlines was about to land at the airport while an Embraer ERJ145 aircraft belonging to ExpressJet flight was taking off. At about 3pm, the distance between the two planes was recorded to be only 200 feet laterally, and 400 feet vertically.

The report revealed that “The local controller recognized that the spacing was insufficient and instructed the B737 to go around. He provided traffic advisories to both the B737 and the ERJ145 pilots and instructed the ERJ145 pilot to maintain visual separation from the B737. The ERJ145 pilot responded that he was going to keep the aircraft’s nose down. The B737 overflew the ERJ145 at the intersection of runways 29/4R”.

There were 47 passengers and 3 crew members aboard the ExpressJet flight while the United flight was flying with 155 passengers and 6 crew members on board.

NTSB Releases Details of Travis Air Show Plane Crash

The NTSB has issued the preliminary investigation report on May 4’s Stearman biplane crash at Travis Air Force Base in Fairfield, California.

The plane crashed when the 77 year old pilot, Edward Andreini, was attempting a ribbon cutting stunt during the Thunder Over Solano air show.

According to the report, “The planned maneuver consisted of a total of three passes…The first two passes were successful, but on the third pass, the airplane was too high, and did not cut the ribbon. The pilot came around for a fourth pass, and rolled the airplane inverted after aligning with the runway. The airplane contacted the runway prior to reaching the ribbon, slid inverted between the ground crew personnel holding the poles, and came to a stop a few hundred feet beyond them.”

The report also revealed that within 50 seconds of coming to rest, the plane’s right side was completely engulfed by flames but the firefighters took 3 to 4 minutes before they arrived at the scene to extinguish the fire.

Indian Airlines to Employ Real-Time Plane Tracking; Orders DGCA

The civil aviation regulator of India has issued a directive, ordering the airlines to use real-time tracking for locating the planes.

In the directive, the Director General Prabhat Kumar has obligated the airlines to track all of their aircrafts with the on-board Aircraft Communications Addressing and Reporting System (ACARS) or Automatic Dependent Surveillance – Broadcast (ADS-B). In areas where these two systems do not have coverage, the airlines will be required to devise other mechanisms to ensure real-time tracking. Operators will also need to ensure that the real-time tracking systems are in working condition before the flight.

The order has been issued in context of the preliminary report released by Malaysian authorities, which revealed that the flight 370 could not be located because the aircraft did not transmit its coordinates in real-time.

According to the statement released by the Directorate General of Civil Aviation (DGCA), “While commercial air transport aircrafts spend considerable amount of time operating over remote areas, there is currently no international requirement for real time tracking of the aircraft. This uncertainty resulted in significant difficulty in locating the aircraft in a timely manner.”

Experimental Plane Crashed in South Carolina; Pilot Remained Stuck in Cockpit for 7 Hours

An experimental plane crashed near Sumter Airport, Sumter County, South Carolina, at about 11:30am on April 27.

The single passenger plane was scheduled to land at Sumter Airport around lunch time but it never showed up. The search and rescue teams were then sent for finding the plane, which finally spotted it along Brewington Road at around 5:30pm.

According to the rescue workers, they found the crashed plane in an upside down position, with the pilot stuck in the cockpit. The rescuers then cut through the wreckage and lifted the pilot, who had remained stuck inside the cockpit for around seven hours. He was then taken to Palmetto Health Richland for medical treatment. His injuries are believed to be non-life threatening.

According to the Sumter County Sheriff Anthony Dennis, “The FAA will come in and do an investigation and determine what caused the crash… At this time, we have no knowledge of what actually caused the crash.”

As per the FAA records, the airplane was an experimental plane, built in 2012.

ICAO’s Dangerous Goods Panel Proposes Ban on Lithium Batteries in Passenger Planes

ICAOA recent meeting of the International Civil Aviation Organization’s Dangerous Goods Panel (DGP), held in Montreal, discussed the proposal for banning lithium batteries as cargo in passenger air crafts.

According to the meeting report, the DGP agreed to prohibit lithium batteries in cargo, except those which are packed or contained in some equipment or are kept in the aircraft cabin by the passengers. The panel further proposed Jan. 1, 2015 as the effective date for start of prohibition.

The DGPs decision will be presented for final approval.from the Air Navigation Commission (ANC) in late April,

The report states that “the ANC may adopt the DGP’s decision but has within its prerogative, the option of amending/extending it…..The actual text of the decision, including associated consequential amendments, can be expected towards the end of May.”

When Lady Luck Turns Away

Incidents and Accidents.

Behind every accident, there are many incidents. Accidents may be defined as involving fatalities and incidents as those many smaller events seemingly unconnected from any others. The importance of incidents has gotten little respect but for two obscure references. The industry recognized Heinrich Pyramid says “…. for every accident that causes a major injury, there are 29 accidents that cause minor injuries and 300 accidents that cause no injuries.”

A 2005 Rand Report drawing attention to NTSB databases said “…(there is) poor control of information, part of resolving more complex accidents depends upon a thorough knowledge of prior incidents, the number of major airline incidents the FAA reported in 1997 was ten times the number of major accidents, (there is) neither oversight nor an emphasis on accuracy in the collection and maintenance of NTSB records, as a result, the accuracy of most of the NTSB data sources was rated as “poor” and although the NTSB does examine a significant number of major incidents, only a small portion of the NTSB’s aviation resources are focused on incident events.”

Key Public Databases – NTSB and FAA. Gaps Compromising Safety Assessments.

The NTSB’s most public source of records is the accident/incident database.

It is cited in the FAA Accident/Incident Data System (AIDS), Airworthiness Directives (ADs), risk/analysis studies, and in DOT/GAO Reports to congress.

In my various surveys along major safety issues (uncontained engine explosions, un-commanded rudder movements, shutdowns due to engine main bearing failures, or smoke/fire incidents, the NTSB data contains about 20 % of what is found in SDR data or other counterpart investigative agencies.

Gaps in NTSB data are further compounded by similar gaps in SDR data. From
1992 to 2002 four NTSB Safety Reccommendation Letters and the GAO had complained of such gaps. In 2010 and regarding data on windshield fires, an article said the “FAA said it was aware of 11 cases of fires in the planes over the past 20 years. However, Boeing has said it is aware of 29 incidents involving fire or smoke over the past eight years.’ Source link >

From the House Hearing Electrical Safety. (Ref report 106-112, Thursday, October 5, 2000, Testimony of Alexis M. Stefani, assistant IG for auditing), said; “Third, of most concern to us is the health of this SDR system, itself. While the new rule (coding for wire issues) was intended to improve the data in the system, FAA must also insure that the reports that are provided to it are timely, and follow the guidance. We found, however, that the SDR system is not robust, and over the years, it has suffered from budget cuts with staffing going from twelve full-time to three full-time people. Weakness in this system reduces the reliability and usefulness of the data, and can impact FAA’s ability to do trend analysis.”

650 Records Of “Smoke In The Cockpit” A Lack of Concern.

Gary Stoller at USA Today did a good piece on “smoke in the cockpit”
reports. Of some 650 records, the FAA/NTSB has but a fraction. The story highlights included; (that the) “issue happens roughly four times a month.

Some experts say the problem is under-reported. FAA says there is “no safety benefit” to requiring systems to remove cockpit smoke. Smoke in a plane’s cockpit from electrical or other failures is reported an average of four times each month, a USA TODAY analysis finds.” Further that “In-flight fires left unattended “may lead to catastrophic failure and have resulted in the complete loss of airplanes,” the FAA warned. A flight crew “may have as few as 15-20 minutes to get an aircraft on the ground if the crew allows a hidden fire to progress without intervention.” USA Today

33 records Of Insulation Blankets Fires. – How They Start.

From my catalog of 78 Records of fire from 1983 to 2012 sourced from the NTSB, AAIB (Danish & UK), French BEA, FAA’s SDR databases, and a few media reports of records of accidents and incidents of fire. There is no central repository. There are 33 records where acoustic insulation (blankets) were specifically mentioned are listed. The issues of self-igniting and flammable wire insulations and of flammable blankets were now are co-mingled.

Three modes of ignition are seen here: wire shorting/arcing, molten metal sprayed from faulting electrical relays and heating tapes. Most reports lack necessary detail, but seven incidents were seen from wires shorting/arcing.
Some involved only a few wires; one powering coal closet lights. Molten Metal (spatter) comprised another 8. More importantly, within those reports were references to another 19 (but without details) and that the NTSB said; the relays involved were not “substantially different from the receptacles used on other transport-category airplanes.”

Ignition from faulting heating tapes/ribbons was seen in another 4 reports – but there were more. In a November 14, 2002 Letter to the FAA, the Canadian Transport Safety Board (TSB) advised that; “heater ribbons are used extensively in transport category aircraft, including Boeing 707, 727, 737, 747, 757 and 767 series and Boeing (Douglas) DC-9, DC-10, and MD-11 aircraft. ” From a TSB report of such fires on 747s and a 767, four other reports were disclosed. The TSB added; “The standard Boeing 767 incorporates 26 heater ribbons. Between June 1985 and June 2002, operators of Boeing aircraft made a total of 67 reports to Boeing of heater ribbon failures where thermal degradation was evident.” From one Delta MD-88 fire in 1999, the NTSB said; “DAL conducted a fleet wide examination of their MD-88/MD-90 fleet to ascertain the condition of their static port heaters. Eight heaters were found with evidence of thermal damage on their wires and or connectors.” There are 8 ADs, and 24 additional SDRs describing burn marks or fire damage. In 2002, the FAA concluded that “in-flight Fires In Hidden Areas are a risk to aviation safety – most hidden fires are caused by electrical problems – non-compliance with Safety Regulations have been uncovered. Fire safety problems and improvements are in various stages of correction and study” and that “it is impossible to predict the relative risk of serious fires occurring in Hidden Areas or Locations”. Dense, Continuous Smoke in the Cockpit.

In June 2013 a GAO Report to Congress cited but one record of ‘Dense, Continuous Smoke in the Cockpit’ (in 1973). The input came from the NTSB and the FAA. Contrary to that, a Specialist Paper by the Royal Aeronautical Society detailed seven. Only two were in the NTSB databases – but with no mention of ‘continuous smoke.’

In Lady Luck We Trust ? – Those ‘Lucky’ Ground Incidents.
Often heard whenever the safety of our air transportation system is questioned is that we have an enviable safety record due to the industry, the FAA and the NTSB’s efforts. That is true if only actual deaths are counted.

This boiler-plate response comes whenever issues of safety are raised, but something else is left unspoken: its conditional nature. It includes just the U.S. carriers, and is based on the records kept. However, there have been no less than 6 events where fires occurred on the ground and caused significant damage, or loss of the airframes. Fire departments intervened in five.

What if, instead, over 900 lives had been lost over the past 12 years ?
For example:

(1) Aug 8, 2000, AirTran DC-9-32 – fire and blistering of aircraft skin, 63 on board.

(2) Nov 29, 2000, a DC-9-32 by AirTran (97 on board).

(3) Same Day, Nov 29, 2000, a DC-9-82, American Airlines (66 on board) ,
blankets burned, emergency evacuation on taxiways – 97 on board”.

(4) June 28, 2008, ABX 767 freighter burned through the fuselage and was destroyed at the gate, (“The risk of an in-flight fire and the propagation of a fire in those areas is essentially the same whether the airplane is equipped to fly passengers or cargo” says the FAA). Approximate 767 capacity is 190 people.

(5) July 29 2011, Egypt Air 777, fire erupted and burned a cockpit-widow
sized hole through the fuselage. Emergency services put the fire out – 291 passengers were evacuated.

(6) On October 14, 2012, a Corendon Airlines 737-800 had “substantial damage” from fire in the cockpit on the gate. 196 on board were evacuated. Had these fires broken at altitude or during the trans-oceanic crossing, all on board may have been lost.

For the sum each of these fires found in the NTSB’s accident/incident database, over 900 lives were not lost. A more honest assessment and the credit for this remarkable safety record of no fatalities was not the FAA and industry abilities to manage and ‘mitigate risks’ – but rather the kindness of Lady Luck. But what can happen when lady Luck turns away ?

Business Jet India and SpanAir’s Planes Grounded for Violating Safety Norms

The Directorate General of Civil Aviation (DGCA), India, grounded two business jets belonging to Business Jet India Private Limited and Span Air for violating safety norms during its safety audit of non-scheduled operators (NSOPs).

During the inspection of the Cessna aircraft belonging to Business Jets India, the aviation regulator found that the crew on-board were not properly trained in safety and emergency procedures. The log books and the operation manuals were outdated and the lavatory had unnecessary goods which is strictly prohibited. The Business Jets India Private Limited operates three Hawker 850 XP aircrafts in addition to the Cessna Citation CJ2 aircraft in and out of India. The company is a subsidiary of Singapore-based BJETS Private Limited. Tata group also has a stake in it.

Moreover, the aviation regulator also found some significant deficiencies in the Hawker 900XP aircraft registered VT-BKL with SpanAir. It was observed that the crew were not properly trained in safety and emergency procedures, the life vest had passed its expiry date and the operations manual and the route guides were outdated. SpanAir has four more aircrafts –a seven-seater Beech Super King Air B-200; a six-seater Premier 1A, Bell 407GX and Bell 420 helicopters.

FAA Proposes $547,500 Civil Penalty Against Hawaiian Airlines

The U.S. Department of Transportation’s Federal Aviation Administration (FAA) is proposing a $547,500 Civil Penalty against Hawaiian Airlines, Inc. for operating a Boeing 767-300 that was not in compliance with Federal Aviation Regulations.

The FAA alleges Hawaiian operated the aircraft thousands of times when it was not in compliance with a July 2000 Airworthiness Directive (AD) that required inspections of certain engine thrust reverser components. The purpose of the AD was to prevent a portion of the thrust reverser from coming off in flight, which could cause a rapid decompression of the aircraft.

The AD required initial and repetitive inspections of the components to detect damage and wear, and corrective actions if necessary. It required replacement of the components with new and improved parts within four years of the AD taking effect.

During a July 2012 inspection, the FAA discovered that some of Hawaiian’s records erroneously showed the AD did not apply to one of its Boeing 767 aircraft. The FAA alleges Hawaiian operated the aircraft more than 5,000 times – mostly on passenger carrying flights – between July 2004 and July 2012 when it was out of compliance with the AD. The FAA further alleges Hawaiian operated the aircraft on 14 passenger flights after the agency alerted the carrier that some of its records erroneously indicated that the AD did not apply to the aircraft.

Additionally, the FAA alleges Hawaiian failed to keep required records of the status of the AD for the aircraft in question.

Hawaiian has requested an informal conference with the FAA to discuss the matter.

FAA Announces the Republic of the Philippines’ Aviation Safety Rating

The U.S. Department of Transportation’s Federal Aviation Administration (FAA) today announced that the Republic of the Philippines complies with international safety standards set by the International Civil Aviation Organization (ICAO) and has been granted a Category 1 rating.

The country previously held a Category 1 rating until January 2008, when it was downgraded to a Category 2. A Category 2 rating means a country either lacks laws or regulations necessary to oversee air carriers in accordance with minimum international standards, or that its civil aviation authority – equivalent to the FAA for aviation safety matters – is deficient in one or more areas, such as technical expertise, trained personnel, record keeping or inspection procedures.

The return to Category 1 status is based on a March 2014 FAA review of the Civil Aviation Authority of the Philippines. A Category 1 rating means the country’s civil aviation authority complies with ICAO standards. With the International Aviation Safety Assessment (IASA) Category 1 rating, the Republic of the Philippines’ air carriers can add flights and service to the United States and carry the code of U.S. carriers.

As part of the FAA’s IASA program, the agency assesses the civil aviation authorities of all countries with air carriers that have applied to fly to the United States, currently conduct operations to the United States or participate in code sharing arrangements with U.S. partner airlines and makes that information available to the public. The assessments determine whether or not foreign civil aviation authorities are meeting ICAO safety standards, not FAA regulations.

In order to maintain a Category 1 rating, a country must adhere to the safety standards of ICAO, the United Nations’ technical agency for aviation that establishes international standards and recommended practices for aircraft operations and maintenance.

Press Release: FAA Installs Equipment for NextGen Aircraft Tracking System


The U.S. Department of Transportation’s Federal Aviation Administration today announced the completion of a nationwide infrastructure upgrade that will enable air traffic controllers to track aircraft with greater accuracy and reliability, while giving pilots more information in the cockpit. This upgrade is a key improvement in the Next Generation Air Transportation System.

“This upgrade is an important step in laying the foundation for the NextGen system, which provides controllers a much more precise view of the airspace, gives pilots much more awareness and information, and as a result strengthens the safety and efficiency of our system,” said U.S. Transportation Secretary Anthony Foxx. “This state-of-the-art satellite system is already providing controllers with visibility in places not previously covered by radar.”

The nationwide installation of the Automatic Dependent Surveillance-Broadcast (ADS-B) radio network supports a satellite-based surveillance system that tracks aircraft with the help of GPS. This provides more accurate aircraft location information than the current radar system.

NextGen refers to a set of initiatives being implemented by the FAA in collaboration with the aviation community to ensure that the United States has the safest, most efficient airspace possible for decades to come. In addition to ADS-B, NextGen improvements are already delivering benefits that include more efficient air traffic procedures that save time and fuel and reduce emissions.

“The installation of this radio network clears the way for air traffic controllers to begin using ADS-B to separate equipped aircraft nationwide,” FAA Administrator Michael Huerta said. “It will also provide pilots flying aircraft equipped with the proper avionics with traffic information, weather data and other flight information.”

Of the 230 air traffic facilities across the country, 100 are currently using this system to separate traffic. It is expected to be connected and operating at all 230 facilities by 2019. All aircraft operating in controlled airspace must be equipped with ADS-B Out avionics that broadcast the plane’s location, by Jan. 1, 2020.

With the upgraded surveillance and broadcast system and aircraft equipped with ADS-B Out transponders, aircraft positions on controller screens update almost continuously, compared to every 4.7 seconds or longer with radar.
ADS-B also enables more accurate tracking of airplanes and airport vehicles on runways and taxiways, increasing safety and efficiency. The new system significantly improves surveillance capability in areas with geographic challenges like mountains or over water. Airplanes equipped with ADS-B In, which is not currently mandated, will give pilots information through cockpit displays about location in relation to other aircraft, bad weather and terrain, and temporary flight restrictions.

In addition to the operational benefits of ADS-B, each one of the 634 ground stations installed by Exelis of McLean, Va., is substantially smaller than a radar installation – resulting in less impact to the environment and less cost to maintain.

ICAO Declares Nepal as 45% Non-Compliant

CAANInternational Civil Aviation Organization (ICAO) has declared Nepal as 7% percent more non-compliant than the global average of 38%.

After an on-site audit during the Universal Safety Oversight Audit Programme , ICAO has given a non-compliance rating of 45% to Nepal.

The audit report states that this high rating is due to the negligence of the Civil Aviation Authority of Nepal [CAAN] in strengthening the mechanism of aviation safety oversight.

CAAN’s failure has also put Nepal in the bad books of the UN aviation watch dog.

Engine Fault Caused Palatka Plane Crash: NTSB

PalatkaThe National Transportation Safety Board has released the preliminary report on Palatka plane crash incident that happened on March 21.

According to the report, the pilot was preparing to land when he saw other planes passing by. So he altered the plane’s position in order to provide sufficient space between his and other planes heading to the runway. However, when he finally attempted to go around and screwed the throttle in, the engine stopped working.

The Cessna LC41-550FG was carrying two persons: the 73 year old pilot Richard Carrara and 71 year old Malcolm Clevenstine. Clevenstine died March 22, while the pilot has survived with serious injuries.

MCDonnell Douglas MD 11 Recommendations

    From the NTSB to the FAA to an MD-11 near you:

    • Work with Boeing to (1) assess the effectiveness of flare cueing systems to assist MD-11 pilots in making timely and appropriate inputs during the landing flare
    • (2) provide a formal report on the findings of the assessment, and
    • (3) if the assessment shows that flare cueing systems could be useful to MD-11 pilots, provide copies of the report to all US operators of MD-11 airplanes and encourage them to install such a system on these airplanes.
    • Work with Boeing to (4) assess methods for providing weight-on-wheels cueing to MD-11 pilots to enhance pilot awareness of bounced landings and facilitate proper pilot reaction and effective control inputs when bounced landings occur,
    • (5) provide a formal report on the findings of the assessment, and
    • (6) if the assessment shows that the weight-on-wheels cueing methods could be useful to MD-11 pilots, provide copies of the report to all US operators of MD-11 airplanes and encourage them to provide a means for weight-on-wheels cueing for these airplanes. (A-14-005)
